소세포 폐암의 치료성적에 대한 고찰 (A Clinical Therapeutic Results on Small Cell Lung Cancer)

  • 연구배경: 소세포 폐암은 비소세포 폐암에 비해 병의 진행경과 및 치료에 대한 반응이 상이하게 나타나는데 저자들은 지난 7년간(1986~1992) 본원에서 진단된 소세포 폐암환자들을 대상으로, 그 치료성적에 대해 고찰해 보고자 하였다. 방법: 조직학적 및 세포진 검사상 폐암으로 진단받은 806명 중 소세포 폐암으로 밝혀진 153명의 환자들을 대상으로 하였으며, 병력일지 및 우편물 또는 전화를 통하여 102명의 환자에 대한 생사확인과 사망일지 등을 확인하였고 임상기록을 중심으로 후향적인 방법으로 분석하였다. 결과: 1) 전체 암환자 수는 8,050명 이었으며, 그 중 폐암환자는 806명(10.0%) 이었고 소세포 폐암환자는 153명으로 전체 폐암환자의 19.0%를 차지했으며, 남녀의 비는 129명: 24명(84.3% : 15.7%) 이었다. 2) 소세포 폐암환자의 연령분포는 60대가 60명(39.2%), 50대가 53명(34.6%)으로 3/4 정도의 환자가 50~60대 사이에 발생했다. 3) 다양한 증상 및 증세를 나타냈으며, 반 수 이상의 환자에서 기침, 객담, 호흡곤란 및 흉통이 있었고 진단방법으로는 128명(83.5%)이 기관지 내시경 및 생검으로 진단 되었으며 ECOG에 의한 수행상태는 $H_2$가 59.5%를 차지하였고, 그 다음은 $H_1$이 30.7%를 차지하였다. 병기 및 위치의 분류는 대개가 limited stage(105명, 68.7%)와 central type(134명, 87.6%) 이었다. 4) 화학요법과 병합요법(항암 및 방사선 치료)을 시행한 군의 치료에 대한 반응을 비교해 본 바 병합요법을 시행한 군이 완전관해 및 부분관해율이 63.4%을 보였으며 평균 생존기간은 3주기 이상의 화학요법으로 시행한 환자중 limited stage의 경우 5.3개월 extensive stage는 4.6개월을 보여 양군간에 유이한 차이가 없었으나, limited stage 에서 시행한 병합요법 환자의 경우 15개월 1일의 평균 생존기간을 나타내어 limited stage시의 화학요법과 유이한 차이를 보였다. 5) 치료의 유무와 상관없이 시행한 각 병기간 환자의 중앙 생존기간은 limited stage가 10.9개월 extensive stage가 4.8개월을 보여 양군간의 유이한 차이를 보였다. 결론: 폐암은 현재 국내에서도 점차 증가하는 추세에 있는 악성 종양중의 하나로 본원에서의 최근 7년간 전체 암환자 순위 중 특히 남자의 경우 위암 다음으로 많았으며, 그 중 소세포 폐암의 치료성적에 대해 후향적인 방법으로 고찰하여 보고하는 바이며, 특히 limited stage 시 항암 및 방사선치료가 항암단일요법보다 더 생존율이 높은것으로 나타났는데, 향후 더 정확한 임상고찰을 위해서는 전향적인 연구가 필요하리라 생각된다.

대형 위암의 임상병리학적 분석 및 예후 (Clinicopathologic Features and Prognostic Factors for Patients with Large Gastric Tumors)

  • 목적: 수술 전 측정이 가능한 종양의 크기는 위암의 중요한 예후 인자로 생각되고 있으며 크기가 큰 위암은 주위 장기 침범, 광범위 림프절 전이, 원격 전이 등을 동반한 진행성 위암으로 예후가 불량하다고 알려져 있다. 본 연구에서는 종양의 크기를 기준으로 하여 대형위암의 임상 병리학적 분석과 생존율에 영향을 주는 인자들을 분석하였다. 대상 및 방법: 1983년 9월부터 2001년 12월까지 고려대학교 외과학교실에서 위암으로 진단되어 절제술을 시행받은 2,260명을 대상으로 후향적 연구를 시행하였다. 대상군을 장경 8 cm를 기준으로 대형위암 335 (14.8%))예 소형위암 1,925예(85.2%)의 두 군으로 나누어 분석하였다. 대형위암의 임상 병리학적 특성을 분석하기 위하여 두 군 간의 임상 병리학적 인자들을 다변량 분석과 단변량 분석을 시행하였으며 생존은 COx proportional hazards model을 사용하였다. P<0.05인 경우를 통계학적으로 유의한 것으로 판정하였다. 결과: 종양의 위치(P<0.001), 수술 방법(P<0.001), 근치도(P<0.001), 위벽침윤도(P<0.001), 림프절 전이(P<0.001), 분화도(P<0.001) 그리고 타장기의 합병절제율(P<0.001)이 두 군간의 통계학적 차이를 보였다. 대형 위암의 생존율을 Cox proportional hazards model로 분석한 결과 림프절 전이(P<0.001), 근치도(P<0.001), 위벽 침윤도(P=0.010), 수술 방법(P=0.018)과 나이(P=0.033)가 통계적으로 유의한 독립적인 위험인자였다. 결론: 대형위암은 소형위암에 비해 진행된 소견을 보였다. 대형 위암에서 근치절제는 생존율을 향상시키는 매우 중요한 인자로 근치적 절제를 위해 노력이 필요하며 근치적 수술 가능성에 대한 좀더 정확한 수술 전 병기결정에 주의를 기울여야 할 것이다.

간호학 교과과정 개선을 위한 조사 연구 (A Study on improvement of curriculum in Nursing)

  • This Study involved the development of a survey form and the collection of data in an effort-to provide information which can be used in the improvement of nursing curricula. The data examined were the kinds courses currently being taught in the curricula of nursing education institutions throughout Korea, credits required for course completion, and year in-which courses are taken. For the purposes of this study, curricula were classified into college, nursing school and vocational school categories. Courses were directed into the 3 major categories of general education courses, supporting science courses and professional education course, and further subdirector as. follows: 1) General education (following the classification of Philip H. phoenix): a) Symbolics, b) Empirics, c) Aesthetics. 4) Synthetics, e) Ethics, f) Synoptic. 2) Supporting science: a) physical science, b) biological science, c) social science, d) behavioral science, e) Health science, f) Educations 3) Professional Education; a) basic courses, b) courses in each of the respective fields of nursing. Ⅰ. General Education aimed at developing the individual as a person and as a member of society is relatively strong in college curricula compared with the other two. a) Courses included in the category of symbolics included Korean language, English, German. Chines. Mathematics. Statics: Economics and Computer most college curricula included 20 credits. of courses in this sub-category, while nursing schools required 12 credits and vocational school 10 units. English ordinarily receives particularly heavy emphasis. b) Research methodology, Domestic affair and women & courtney was included under the category of empirics in the college curricula, nursing and vocational school do not offer this at all. c) Courses classified under aesthetics were physical education, drill, music, recreation and fine arts. Most college curricula had 4 credits in these areas, nursing school provided for 2 credits, and most vocational schools offered 10 units. d) Synoptic included leadership, interpersonal relationship, and communications, Most schools did not offer courses of this nature. e) The category of ethics included citizenship. 2 credits are provided in college curricula, while vocational schools require 4 units. Nursing schools do not offer these courses. f) Courses included under synoptic were Korean history, cultural history, philosophy, Logics, and religion. Most college curricular 5 credits in these areas, nursing schools 4 credits. and vocational schools 2 units. g) Only physical education was given every Year in college curricula and only English was given in nursing schools and vocational schools in every of the curriculum. Most of the other courses were given during the first year of the curriculum. Ⅱ. Supporting science courses are fundamental to the practice and application of nursing theory. a) Physical science course include physics, chemistry and natural science. most colleges and nursing schools provided for 2 credits of physical science courses in their curricula, while most vocational schools did not offer t me. b) Courses included under biological science were anatomy, physiologic, biology and biochemistry. Most college curricula provided for 15 credits of biological science, nursing schools for the most part provided for 11 credits, and most vocational schools provided for 8 units. c) Courses included under social science were sociology and anthropology. Most colleges provided for 1 credit in courses of this category, which most nursing schools provided for 2 creates Most vocational school did not provide courses of this type. d) Courses included under behavioral science were general and clinical psychology, developmental psychology. mental hygiene and guidance. Most schools did not provide for these courses. e) Courses included under health science included pharmacy and pharmacology, microbiology, pathology, nutrition and dietetics, parasitology, and Chinese medicine. Most college curricula provided for 11 credits, while most nursing schools provide for 12 credits, most part provided 20 units of medical courses. f) Courses included under education included educational psychology, principles of education, philosophy of education, history of education, social education, educational evaluation, educational curricula, class management, guidance techniques and school & community. Host college softer 3 credits in courses in this category, while nursing schools provide 8 credits and vocational schools provide for 6 units, 50% of the colleges prepare these students to qualify as regular teachers of the second level, while 91% of the nursing schools and 60% of the vocational schools prepare their of the vocational schools prepare their students to qualify as school nurse. g) The majority of colleges start supporting science courses in the first year and complete them by the second year. Nursing schools and vocational schools usually complete them in the first year. Ⅲ. Professional Education courses are designed to develop professional nursing knowledge, attitudes and skills in the students. a) Basic courses include social nursing, nursing ethics, history of nursing professional control, nursing administration, social medicine, social welfare, introductory nursing, advanced nursing, medical regulations, efficient nursing, nursing english and basic nursing, College curricula devoted 13 credits to these subjects, nursing schools 14 credits, and vocational schools 26 units indicating a severe difference in the scope of education provided. b) There was noticeable tendency for the colleges to take a unified approach to the branches of nursing. 60% of the schools had courses in public health nursing, 80% in pediatric nursing, 60% in obstetric nursing, 90% in psychiatric nursing and 80% in medical-surgical nursing. The greatest number of schools provided 48 crudites in all of these fields combined. in most of the nursing schools, 52 credits were provided for courses divided according to disease. in the vocational schools, unified courses are provided in public health nursing, child nursing, maternal nursing, psychiatric nursing and adult nursing. In addition, one unit is provided for one hour a week of practice. The total number of units provided in the greatest number of vocational schools is thus Ⅲ units double the number provided in nursing schools and colleges. c) In th leges, the second year is devoted mainly to basic nursing courses, while the third and fourth years are used for advanced nursing courses. In nursing schools and vocational schools, the first year deals primarily with basic nursing and the second and third years are used to cover advanced nursing courses. The study yielded the following conclusions. 1. Instructional goals should be established for each courses in line with the idea of nursing, and curriculum improvements should be made accordingly. 2. Course that fall under the synthetics category should be strengthened and ways should be sought to develop the ability to cooperate with those who work for human welfare and health. 3. The ability to solve problems on the basis of scientific principles and knowledge and understanding of man society should be fostered through a strengthening of courses dealing with physical sciences, social sciences and behavioral sciences and redistribution of courses emphasizing biological and health sciences. 4. There should be more balanced curricula with less emphasis on courses in the major There is a need to establish courses necessary for the individual nurse by doing away with courses centered around specific diseases and combining them in unified courses. In addition it is possible to develop skill in dealing with people by using the social setting in comprehensive training. The most efficient ratio of the study experience should be studied to provide more effective, interesting education Elective course should be initiated to insure a man flexible, responsive educational program. 5. The curriculum stipulated in the education law should be examined.

카테고리 연관 규칙 마이닝을 활용한 추천 정확도 향상 기법 (A Study on the Improvement of Recommendation Accuracy by Using Category Association Rule Mining)

  • 인터넷이라는 가상 공간을 활용함으로써 물리적 공간의 제약을 갖는 오프라인 쇼핑의 한계를 넘어선 온라인 쇼핑은 다양한 기호를 가진 소비자를 만족시킬 수 있는 수많은 상품을 진열할 수 있게 되었다. 그러나, 이는 역설적으로 소비자가 구매의사결정 과정에서 너무 많은 대안을 비교 평가해야 하는 어려움을 겪게 함으로써 오히려 상품 선택을 방해하는 원인이 되기도 한다. 이런 부작용을 해소하기 위한 노력으로서, 연관 상품 추천은 수많은 상품을 다루는 온라인 상거래에서 소비자의 구매의사결정 과정 중 정보탐색 및 대안평가에 소요되는 시간과 노력을 줄여주고 이탈을 방지하며 판매자의 매출 증대에 기여할 수 있다. 연관 상품 추천에 사용되는 연관 규칙 마이닝 기법은 통계적 방법을 통해 주문과 같은 거래 데이터로부터 서로 연관성 높은 상품을 효과적으로 발견할 수 있다. 하지만, 이 기법은 거래 건수를 기반으로 하므로, 잠재적으로 판매 가능성이 높을지라도 충분한 거래 건수가 확보되지 못한 상품은 추천 목록에서 누락될 수 있다. 이렇게 추천 시 제외된 상품은 소비자에게 구매될 수 있는 충분한 기회를 확보하지 못할 수 있으며, 또 다시 다른 상품에 비해 상대적으로 낮은 추천 기회를 얻는 악순환을 겪을 수도 있다. 본 연구는 구매의사결정이 결국 상품이 지닌 속성에 대한 사용자의 평가를 기반으로 한다는 점에 착안하여, 추천 시 상품의 속성을 반영하면 소비자가 특정 상품을 선택할 확률을 좀더 정확하게 예측할 수 있다는 점을 추천 시스템에 반영하기 위한 목적으로 수행되었다. 즉, 어떤 상품 페이지를 방문한 소비자는 그 상품이 지닌 속성들에 어느 정도 관심을 보인 것이며 추천 시스템은 이런 속성들을 기반으로 연관성을 지닌 상품을 더 정교하게 찾을 수 있다는 것이다. 상품의 주요 속성의 하나로서, 카테고리는 두 상품 간에 아직 드러나지 않은 잠재적인 연관성을 찾기에 적합한 대상이 될 수 있다고 판단하였다. 본 연구는 연관 상품 추천에 상품 간의 연관성뿐만 아니라 카테고리 간의 연관성을 추가로 반영함으로써 추천의 정확도를 높일 수 있는 예측모형을 개발하였고, 온라인 쇼핑몰로부터 수집된 주문 데이터를 활용하여 이루어진 실험은 기존 모형에 비해 추천 성능이 개선됨을 보였다. 실무적인 관점에서 볼 때, 본 연구는 소비자의 구매 만족도를 향상시키고 판매자의 매출을 증가시키는 데에 기여할 수 있을 것으로 기대된다.

마켓 인사이트를 위한 상품 리뷰의 다차원 분석 방안 (Multi-Dimensional Analysis Method of Product Reviews for Market Insight)

  • 인터넷의 발달로, 소비자들은 이커머스에서 손쉽게 상품 정보를 확인한다. 이때 활용되는 상품 리뷰는 사용자 경험을 토대로 작성되어 구매의사결정의 효율성을 높일 뿐만 아니라 상품 개발에 도움을 주기도 한다. 하지만, 방대한 양의 상품 리뷰에서 관심있는 평가차원의 세부내용을 파악하는 데에는 많은 시간과 노력이 소비된다. 예를 들어, 노트북을 구매하려는 소비자들은 성능, 무게, 디자인과 같은 평가차원에 대해 각 차원별로 비교 상품의 평가를 확인하고자 한다. 따라서 본 논문에서는 상품 리뷰에서 다차원 상품평가 점수를 자동적으로 생성하는 방안을 제안하고자 한다. 본 연구에서 제시하는 방안은 크게 2단계로 구성된다. 사전준비 단계와 개별상품평가 단계로, 대분류 상품군 리뷰를 토대로 사전에 생성된 차원분류모델과 감성분석모델이 개별상품의 리뷰를 분석하게 된다. 차원분류모델은 워드임베딩과 연관분석을 결합함으로써 기존 연구에서 차원과 단어들의 관련성을 찾기 위한 워드임베딩 방식이 문장 내 단어의 위치만을 본다는 한계를 보완한다. 감성분석모델은 정확한 극성 판단을 위해 구(phrase) 단위로 긍부정이 태깅된 학습데이터를 구성하여 CNN 모델을 생성한다. 이를 통해, 개별상품평가 단계에서는 구 단위의 리뷰에 준비된 모델들을 적용하고 평가차원별로 종합함으로써 다차원 평가점수를 얻을 수 있다. 본 논문의 실험에서는 대분류 상품군 리뷰 약 260,000건으로 평가모델을 구성하고, S사와 L사의 노트북 리뷰 각 1,011건과 1,062건을 실험데이터로 활용한다. 차원분류모델은 구로 분해한 개별상품 리뷰를 6개 평가차원으로 분류했고, 기존 워드임베딩 방식보다 연관분석을 결합한 모델의 정확도가 13.7% 증가했음을 볼 수 있었다. 감성분석모델은 문장보다 구 단위로 학습한 모델이 평가차원을 면밀히 분석함으로써 29.4% 더 높은 정확도를 보임을 확인했다. 본 연구를 통해 판매자, 소비자 모두가 상품의 다차원적 비교가 가능하다는 점에서 구매 및 상품 개발에 효율적인 의사결정을 기대할 수 있다.

성폭력 피해여성의 경험에 관한 연구 (A Grounded theory Approach on the Experience of Sexual Abuse Victims)

  • This studies designed to work out a theoretical framework on the experience of sexual abuse from the perspective of grounded theory in an effort to provide more practical and efficient nursing intervention for female victims. The subcategories identified were "sexual abuse", "threatening", "absent mindness", "embarrassment", "horripilation", "dizziness", "wondrousness", "filthiness", "sexual curiosity", "violence level", "victim's age", "neighbors response", "victims personality", "common experience", "sexual abuse information", "family relations", "level of familiarity", "hiding", "suppression", "self-torture", "self-protection", "avoidance", "asking aid", "withdrawal", "hatred", "confusion", "dodging, "remmant", and "pursuing". The 29 subcategories given above were further integrated into 16 categories such as "victimizedness", "being astounded", "filthiness", "degree", "developmental stage", "response pattern", "personality", "rarity", "information availability", "family support", "cover-up", "escaping", "informing", "negative internalization", and "positive pursuit of change". The core categories linked to all the other categories turned out to be "being taken aback" and "filthiness" incorporating the relevant subcategories. A total of 23 theoretical hypothesis emerged in the process of analyzing data. 1. the grater sexual curiosity, the weaker the senses of being taken aback and filthiness. 2. The weaker sexual curiosity, the stronger the senses of being taken aback and filthiness. 3. The stronger the level of violence, The more violent the senses of being taken aback and filthiness. 4. The lower the level of violence, the weaker the senses of being taken aback and filthiness. 5. The younger the victims, the stronger the senses of being taken aback and filthiness. 6. The older the victims, The weaker the senses of being taken aback and filthiness. 7. 'Escaping' will transpire regardless of the given circumstances. 8. The weaker the senses of being taken aback and filthiness, the more probable 'informing' and 'escaping' transpire. 9. The stronger the senses of being taken aback and filthiness, the more probable 'informing' and 'escaping' transpire. 10. The more protective the response from 'informing' and 'escaping' transpire around, the more likely the response to being taken aback' and 'filthiness' will be 'informing' and 'escaping'. 11. The following four hypotheses were conformed in the process of data analysis. 1) In case the level of violence is strong but 'being taken aback' and 'filthiness' in weak because of strong sexual curiosity and also if information concerning sexual abuse is not readily available and the frequency is low, negative internationalization marked by 'covering-up' and 'escaping' will take place despite the fact the subject is open-minded, the family is cohesive and the abuser is unfamiliar. 2) In case the level of violence is weak but 'being taken aback' and 'filthiness' is weak combined with weak sexual curiosity and also if information concerning sexual abuse is readily available and the response from around is protective and the frequency is high, the subject will pursue positive changes to 'being taken aback' and 'filthiness', further aided by the fact that the subject is open-minded, the family is cohesive and the abuser is unfamiliar. 3) In case the level of violence is strong and 'being taken abuse' and 'filthiness' is strong because of weak sexual curiosity and also if information concerning sexual abuse is reading available and the response from around is readily available and the response from around is protective and the frequency is low, the subject will persue positive changes marked by 'informing' and 'escaping' despite the fact that the family cohesion is weak and the abuser is familiar. 4) In case the level of violence is strong and 'being taken aback' and 'filthiness' is strong because of weak sexual curiosity and also if information concerning sexual abuse is not readily available and the response from around is respelling and the frequency is low negative internalization like 'covering-up' and 'escaping' will take place, further aggravated by the fact that the subject's personality is closed, family cohesion is weak, and subject is familiar. On the basis of the above finding, it is recommended that nursing intervention should focus on promoting the milieu conductive to the victims pursuing positive changes along with the adequate aids from protection facilities as well as from the people around them.

  • 이 글은 1979년 MBC의 6·25 특집극 <최후의 증인>을 분석하여 1970년대에 추리와 전쟁이 어떤 상관관계를 맺고 있는지를 분석하려 한다. 전쟁의 기억을 전유하려는 국가의 기획과 이를 새로운 방식으로 전유하려는 제작주체의 의도, 그리고 시청자들이 브라운관을 통해 드라마를 수용하는 맥락에 대한 종합적인 이해를 통해 텔레비전의 시대로 전진하는 1970년대의 굴절을 특집극에서 찾으려 한다. 이를 위해 소설 『최후의 증인』과 1979년 방송된 6·25 특집극 <최후의 증인>을 주요 텍스트로 삼아 김성종의 추리가 텔레비전드라마로 각색되는 양상을 살펴보려 했다. 본고가 분석대상으로 삼은 6·25 특집극 <최후의 증인>은 예술성과 목적성을 동시에 갖춘 수준 높은 특집극을 제작하기 위한 기획의 산물이었다. 하지만 텔레비전 시청이 여가의 중요한 수단으로 자리 잡게 되면서 텔레비전드라마 시청의 핵심적인 이유가 되면서 시청자의 즐거움을 위해 고안된 자극적이고 선정적인 서사들이 정권의 입장에서는 장애물로 인식되었다. 주지하다시피 특집극은 일일극 위주의 텔레비전드라마 시장에 예술성과 목적성을 함께 충족시키기 위해 고안된 예술형식이다. 제작주체가 추진하던 예술적인 드라마와 시청자들이 요구하던 즐거운 드라마의 충돌은 역동적인 소통의 장을 형성한다. 이 과정에서 특집극은 본래 기획을 포괄하는 동시에 시청자들의 즐거움을 충족시키기 위해 구성된 새로움은 국민총화의 의도를 아득히 벗어나기도 했다. 추리과정을 시각적으로 형상화하는 방식으로 고안된 수사드라마는 시대적 리얼리티를 담보하는 새로운 가능성이 되기도 했지만 본래의 의도를 상실한 저속한 문화의 표본으로 지적되었다는 점에서 역설적인 존재와도 같았다. 그렇게 보자면 한국 대중예술에서 유독 힘을 쓰지 못했다고 여겨졌던 추리물은 1970년대에 이르러 한국 텔레비전드라마와 조우하며 수용자들에게 적지 않은 영향력을 발휘했다는 사실을 기억할 필요가 있다. 모든 장르물이 그러하겠지만 장르를 구성하는 다양한 요소들은 이식과 수용의 과정을 거친 형태로 수용자와 대면하게 된다. 주지하다시피 한국에서 추리는 반공, 수사와 결합을 거듭하면서 자신의 고유한 양식을 형성해냈다. 이러한 결합의 양상은 <최후의 증인>의 각색 과정에서 선택과 배제된 요소들을 통해 자연스럽게 장르적 정체성을 형성하는 방식에서 확인할 수 있다. 결론적으로 특집극 <최후의 증인>은 1970년대 내내 반공과 수사를 통해 형성된 텔레비전드라마의 추리가 범죄로 이동해가는 경향을 드러낸 중간적인 텍스트라고 볼 수 있다. 결국 이 글이 도달한 결론은 추리라는 특정한 장르가 텔레비전드라마와 조우할 때 공적 체제에 대한 신뢰감을 드러내는 한편 즐거움에 대한 정치적 해석을 우회할 수 있는 가능성을 끊임없이 탐색한다는 사실이다. 전쟁의 기억은 사회와 체제의 어두운 이면에 새겨진 불온한 상상을 중화시켜줄 알리바이에 가깝다. 결과적으로 유신정권의 말기에 방송된 <최후의 증인>은 특집극의 논리가 텔레비전의 매체적 특성과 조우한 기이한 결과물인 셈이다. 텍스트에 새겨진 흔적을 쫓아 사회적 담론에 대응하여 대결과 우회를 거듭하며 소통을 시도하던 시청자들의 장르적 욕망은 이제 다시 복원될 필요가 있다.

가맹본부의 리더십 행동유형과 가맹사업자의 관계결속에 관한 실증적 연구 - 가맹사업자의 자기효능감의 조절효과를 중심으로 - (An Empirical Study in Relationship between Franchisor's Leadership Behavior Style and Commitment by Focusing Moderating Effect of Franchisee's Self-efficacy)

  • 본 연구는 가맹사업자의 자기효능감에 주목하여 정부가 예비가맹사업자들을 보호하기 위해 가맹본부에 다양한 규제와 정책을 사용하는 것이 최선의 방법이 아니라는 것에 관심을 두고 있다. 본 연구에서는 경로-목표이론(path-goal theory)에서 제시한 가맹본부의 리더십 행동 유형과 가맹사업자의 관계결속의 영향관계에 있어서 가맹사업자의 특성으로 자기효능감의 조절효과를 규명하고, 실증 분석한 결과 다음과 같은 연구의 시사점을 발견할 수 있었다. 첫째, 가맹본부의 리더십 행동유형이 관계결속에 긍정적 효과를 가져 온다는 사실이 확인됨으로써 가맹본부는 가맹사업자에게 맞는 리더십 행동유형을 적용할 수 있도록 하여야 한다. 둘째, 가맹사업자의 자기효능감이 관계결속에 긍정적 효과가 있을 뿐만 아니라, 리더십 행동유형과 관계결속 사이에 상당한 조절효과가 있기 때문에 가맹본부는 가맹사업자들의 개인차(individual difference) 관리가 필요하다. 셋째, 정부는 가맹본부를 규제할 것만이 아니라 가맹본부가 가맹사업자들의 특성을 확실하게 파악하고 기업의 목표달성을 위한 정당한 통제가 가능하도록 제도적 지원을 해야 할 것이다.

가족계획과 모자보건 통합을 위한 조산원의 투입효과 분석 -서산지역의 개입연구 평가보고- (An Intervention Study on Integration of Family Planning and Maternal/Infant Care Services in Rural Korea)

  • This project was a service-cum-research effort with a quasi-experimental study design to examine the health benefits of an integrated Family Planning (FP)/Maternal & Child health (MCH) Service approach that provides crucial factors missing in the present on-going programs. The specific objectives were: 1) To test the effectiveness of trained nurse/midwives (MW) assigned as change agents in the Health Sub-Center (HSC) to bring about the changes in the eight FP/MCH indicators, namely; (i)FP/MCH contacts between field workers and their clients (ii) the use of effective FP methods, (iii) the inter-birth interval and/or open interval, (iv) prenatal care by medically qualified personnel, (v) medically supervised deliveries, (vi) the rate of induced abortion, (vii) maternal and infant morbidity, and (viii) preinatal & infant mortality. 2) To measure the integrative linkage (contacts) between MW & HSC workers and between HSC and clients. 3) To examine the organizational or administrative factors influencing integrative linkage between health workers. Study design; The above objectives called for quasi-experimental design setting up a study and control area with and without a midwife. An active intervention program (FP/MCH minimum 'package' program) was conducted for a 2 year period from June 1982-July 1984 in Seosan County and 'before and after' surveys were conducted to measure the change. Service input; This study was undertaken by the Soonchunhyang University in collaboration with WHO. After a baseline survery in 1981, trained nurses/midwives were introduced into two health sub-centers in a rural setting (Seosan county) for a 2 year period from 1982 to 1984. A major service input was the establishment of midwifery services in the existing health delivery system with emphasis on nurse/midwife's role as the link between health workers (nurse aids) and village health workers, and the referral of risk patients to the private physician (OBGY specialist). An evaluation survey was made in August 1984 to assess the effectiveness of this alternative integrated approach in the study areas in comparison with the control area which had normal government services. Method of evaluation; a. In this study, the primary objective was first to examine to what extent the FP/MCH package program brought about changes in the pre-determined eight indicators (outcome and impact measures) and the following relationship was first analyzed; b. Nevertheless, this project did not automatically accept the assumption that if two or more activities were integrated, the results would automatically be better than a non-integrated or categorical program. There is a need to assess the 'integration process' itself within the package program. The process of integration was measured in terms of interactive linkages, or the quantity & quality of contacts between workers & clients and among workers. Intergrative linkages were hypothesized to be influenced by organizational factors at the HSC clinic level including HSC goals, sltrurture, authority, leadership style, resources, and personal characteristics of HSC staff. The extent or degree of integration, as measured by the intensity of integrative linkages, was in turn presumed to influence programme performance. Thus as indicated diagrammatically below, organizational factors constituted the independent variables, integration as the intervening variable and programme performance with respect to family planning and health services as the dependent variable: Concerning organizational factors, however, due to the limited number of HSCs (2 in the study area and 3 in the control area), they were studied by participatory observation of an anthropologist who was independent of the project. In this observation, we examined whether the assumed integration process actually occurred or not. If not, what were the constraints in producing an effective integration process. Summary of Findings; A) Program effects and impact 1. Effects on FP use: During this 2 year action period, FP acceptance increased from 58% in 1981 to 78% in 1984 in both the study and control areas. This increase in both areas was mainly due to the new family planning campaign driven by the Government for the same study period. Therefore, there was no increment of FP acceptance rate due to additional input of MW to the on-going FP program. But in the study area, quality aspects of FP were somewhat improved, having a better continuation rate of IUDs & pills and more use of effective Contraceptive methods in comparison with the control area. 2. Effects of use of MCH services: Between the study and control areas, however, there was a significant difference in maternal and child health care. For example, the coverage of prenatal care was increased from 53% for 1981 birth cohort to 75% for 1984 birth cohort in the study area. In the control area, the same increased from 41% (1981) to 65% (1984). It is noteworthy that almost two thirds of the recent birth cohort received prenatal care even in the control area, indicating that there is a growing demand of MCH care as the size of family norm becomes smaller 3. There has been a substantive increase in delivery care by medical professions in the study area, with an annual increase rate of 10% due to midwives input in the study areas. The project had about two times greater effect on postnatal care (68% vs. 33%) at delivery care(45.2% vs. 26.1%). 4. The study area had better reproductive efficiency (wanted pregancies with FP practice & healthy live births survived by one year old) than the control area, especially among women under 30 (14.1% vs. 9.6%). The proportion of women who preferred the 1st trimester for their first prenatal care rose significantly in the study area as compared to the control area (24% vs 13%). B) Effects on Interactive Linkage 1. This project made a contribution in making several useful steps in the direction of service integration, namely; i) The health workers have become familiar with procedures on how to work together with each other (especially with a midwife) in carrying out their work in FP/MCH and, ii) The health workers have gotten a feeling of the usefulness of family health records (statistical integration) in identifying targets in their own work and their usefulness in caring for family health. 2. On the other hand, because of a lack of required organizational factors, complete linkage was not obtained as the project intended. i) In regards to the government health worker's activities in terms of home visiting there was not much difference between the study & control areas though the MW did more home visiting than Government health workers. ii) In assessing the service performance of MW & health workers, the midwives balanced their workload between 40% FP, 40% MCH & 20% other activities (mainly immunization). However, $85{\sim}90%$ of the services provided by the health workers were other than FP/MCH, mainly for immunizations such as the encephalitis campaign. In the control area, a similar pattern was observed. Over 75% of their service was other than FP/MCH. Therefore, the pattern shows the health workers are a long way from becoming multipurpose workers even though the government is pushing in this direction. 3. Villagers were much more likely to visit the health sub-center clinic in the study area than in the control area (58% vs.31%) and for more combined care (45% vs.23%). C) Organization factors (admistrative integrative issues) 1. When MW (new workers with higher qualification) were introduced to HSC, it was noted that there were conflicts between the existing HSC workers (Nurse aids with less qualification than MW) and the MW for the beginning period of the project. The cause of the conflict was studied by an anthropologist and it was pointed out that these functional integration problems stemmed from the structural inadequacies of the health subcenter organization as indicated below; i) There is still no general consensus about the objectives and goals of the project between the project staff and the existing health workers. ii) There is no formal linkage between the responsibility of each member's job in the health sub-center. iii) There is still little chance for midwives to play a catalytic role or to establish communicative networks between workers in order to link various knowledge and skills to provide better FP/MCH services in the health sub-center. 2. Based on the above findings the project recommended to the County Chief (who has power to control the administrative staff and the technical staff in his county) the following ; i) In order to solve the conflicts between the individual roles and functions in performing health care activities, there must be goals agreed upon by both. ii) The health sub·center must function as an autonomous organization to undertake the integration health project. In order to do that, it is necessary to support administrative considerations, and to establish a communication system for supervision and to control of the health sub-centers. iii) The administrative organization, tentatively, must be organized to bind the health worker's midwive's and director's jobs by an organic relationship in order to achieve the integrative system under the leadership of health sub-center director. After submitting this observation report, there has been better understanding from frequent meetings & communication between HW/MW in FP/MCH work as the program developed. Lessons learned from the Seosan Project (on issues of FP/MCH integration in Korea); 1) A majority or about 80% of the couples are now practicing FP. As indicated by the study, there is a growing demand from clients for the health system to provide more MCH services than FP in order to maintain the achieved small size of family through FP practice. It is fortunate to see that the government is now formulating a MCH policy for the year 2,000 and revising MCH laws and regulations to emphasize more MCH care for achieving a small size family through family planning practice. 2) Goal consensus in FP/MCH shouBd be made among the health workers It administrators, especially to emphasize the need of care of 'wanted' child. But there is a long way to go to realize the 'real' integration of FP into MCH in Korea, unless there is a structural integration FP/MCH because a categorical FP is still first priority to reduce the rate of population growth for economic reasons but not yet for health/welfare reasons in practice. 3) There should be more financial allocation: (i) a midwife should be made available to help to promote the MCH program and coordinate services, (in) there should be a health sub·center director who can provide leadership training for managing the integrated program. There is a need for 'organizational support', if the decision of integration is made to obtain benefit from both FP & MCH. In other words, costs should be paid equally to both FP/MCH. The integration slogan itself, without the commitment of paying such costs, is powerless to advocate it. 4) Need of management training for middle level health personnel is more acute as the Government has already constructed 90 MCH centers attached to the County Health Center but without adequate manpower, facilities, and guidelines for integrating the work of both FP and MCH. 5) The local government still considers these MCH centers only as delivery centers to take care only of those visiting maternity cases. The MCH center should be a center for the managment of all pregnancies occurring in the community and the promotion of FP with a systematic and effective linkage of resources available in the county such as i.e. Village Health Worker, Community Health Practitioner, Health Sub-center Physicians & Health workers, Doctors and Midwives in MCH center, OBGY Specialists in clinics & hospitals as practiced by the Seosan project at primary health care level.
