Objectives: Many symptoms of cold and heat patterns are related to the thermoregulation of the body. Thus, we aimed to study the association of cold and heat patterns with anthropometry/body composition. Methods: The cold and heat patterns of 2000 individuals aged 30-55 years were evaluated using a self-administered questionnaire. Results: Among the anthropometric and body composition variables, body mass index (-0.37, 0.39) and fat mass index (-0.35, 0.38) had the highest correlation coefficients with the cold and heat pattern scores after adjustment for age and sex in the cold-heat group, while the correlation coefficients were relatively lower in the non-cold-heat group. In the cold-heat group, the most parsimonious model for the cold pattern with the variables selected by the best subset method and Lasso included sex, body mass index, waist-hip ratio, and extracellular water/total body water (adjusted R2 = 0.324), and the model for heat pattern additionally included age (adjusted R2 = 0.292). Conclusions: The variables related to obesity and water balance were the most useful for predicting cold and heat patterns. Further studies are required to improve the performance of prediction models.
Heat and cold often produce a similar clinical effect but different physiologic mechanisms. The purpose of this study was to compare the effects of local heat and cold therapy on joint pain, discomfort and, ROM of the arthritic knee joint. Thirty female subjects took Ice bag and hot bag with random assignment of initial therapy Subjects rated the degree of Joint pain and discomfort before and after each therapy, and then ROM was measured. The results of the study were as follows : 1. The hot bag group showed significantly lower joint pain & discomfort score than the ice bag group. ROM was increased in the hot bag group compared with subjects in the ice bag group. But there is not significant differences. 2. The ice bag group showed significantly lower joint pain score after than before therapy. But there are no significant differences in discomfort score & ROM between after & before therapy. 3. The hot bag group showed significantly lower joint pain, discomfort score after than before therapy. ROM was significantly increased after than before therapy in hot bag group. Therefore hot bag was significantly more effective than the ice bag in relieving the arthritic joint symptoms. A further study is necessary to determine the effect of local heat and cold therapy including the related variables such as preference for heat or cold.
Objectives : An accurate judgment of prognosis when treating diseases is crucial. While the 『Shanghanlun(傷寒論)』 deals with the prognosis of the Three Yin Diseases with great importance, full-scale studies have been lacking. This paper aims to study the Three Yin Diseases with a focus on prognosis analysis. Methods : Among the Three Yin Disease verses, those that could provide clues to prognosis were selected and analysed. Conclusions & Results : When Yang pulse patterns such as long(長脈)·floating(浮脈)·rapid(數脈) pulses and Yang symptoms such as fever, vexing heat, mild perspiration, thirst, warmth in hands and feet are present in Yin disease, it could be taken as signs of Yang Qi restoration. In these situations, Yin Cold pattern such as diarrhea and reversal cold disappear and the prognosis is positive. However, despite Yang pulse patterns and symptoms, there are cases where diarrhea happens as a result of cold dampness being eliminated due to Yang Qi restoration. Also, when Yang Qi starts communicating smoothly after its restoration in the Three Yin Diseases, perspiration can happen. When diarrhea and reversal cold, which are patterns of Yin Cold get worse, with pulse patterns such as unfelt(脈不至)·replete(實脈)·fulminating(脈暴出) pulses, false heat symptoms such as fever and hot flashes happen, accompanied with Yang Qi depleted symptoms such as inability to lie down due to agitation, continuous perspiration, sore throat, dyspnea, and exaggerated breathing happen. When fast pulse, fever, and perspiration are present due to depression and stagnation of ministerial fire, symptoms such as bloody stool with pus, purulent abscess, sore throat, and inability to lie down due to agitation show, which signal negative prognosis. In bad cases of Reverting Yin Disease, there is continuous diarrhea and bloody stool with pus, which can be due to either Kidney Yang deficiency or depression and stagnation of ministerial fire. It could also be caused by excessive heat.
Objectives : We presented the results of reliability study in advance, and analyzed agreement between Korean medicine doctors(KMDs)' diagnosis and cold-heat pattern identification questionnaire(CHPI)'s diagnosis. Methods : This survey was conducted from May 16 to 17, 2015. The subjects were 93 adults living in rural society. Diagnosis of CHPI was performed by 2 KMDs who have clinical experience more than 5 years. The KMDs' diagnosis was set as a reference index, and then we compare 23 items(cold pattern 11 items and heat pattern 12 items) of CHPI questionnaire and 15 items(cold pattern 8 items and heat pattern 7 items) that were brief form of it. We had cut-off value by standard of KMDs' diagnosis using receiver operating characteristic-curve(ROC-curve), with which we calculated agreement including kappa value. Correlation analysis between CHPI evaluation score by KMDs and by the questionnaire was fulfilled as well. Results : Agreement about 11 and 8 cold pattern items showed 87.1% together, and the value of kappa each recorded 0.742 and 0.741. Agreement about 12 and 7 heat pattern items suggested 81.7% and 78.5%, and the value of kappa showed 0.634 and 0.570. Correlation coefficients were 0.803 of 11 items and 0.761 of 8 items about cold pattern. In addition, correlation coefficients were 0.789 of 12 items and 0.767 of 7 items about heat pattern. The significant probability (p-value) was under 0.001. Conclusions : We have developed CHPI questionnaire involving reliability and agreement based on usual symptoms, and hope additional complements so that Korean medicine diagnostics and Korean preventive medicine would be improved.

Objectives: Cold extremities have been suggested to correlate with sleep disturbances. This study aims to explore the relationship between thermal sensations in body, encompassing both cold and heat sensations, with sleep quality and insomnia. Methods: Self-administered questionnaires were utilized to assess thermal sensations in body, sleep quality and symptoms of insomnia in middle-aged women. A multiple logistic regression analysis was performed to ascertain the association between thermal sensations in body and both sleep quality and insomnia symptoms. Results: Among 899 participants, 255 (28.4%) were categorized in the cold sensation group, 95 (10.6%) in the heat sensation group, 70 (7.8%) in the group with both cold and heat sensations, and 479 (53.3%) in the no-sensation group. Pittsburgh Sleep Quality Index and Insomnia Severity Index were notably higher in the group experiencing both sensations when compared to the no-sensation group. After adjustments for covariates, the odds ratios for poor sleep quality, moderate/severe insomnia, and long sleep latency were significantly elevated in the group with both sensations when compared to the no-sensation group. The odds ratios for poor sleep quality in the cold sensation group and for moderate/severe insomnia and low sleep efficiency in the heat sensation group were significantly higher when compared to the no-sensation group. Conclusions: The risk for sleep disturbances varied depending on the presence of thermal sensations in body, with the greatest risk observed for low sleep quality and insomnia in individuals experiencing both cold and heat sensations.
Objectives The purpose of this study is to report a case of improving various symptoms such as polyarthritis, dyspnea, fatigue, heat sensation. In addition, the promising explanation on the perspective of the heat and cold syndrome could be elicited. Methods Dokhwaljihwang-tang was prescribed for an outpatient for 3 weeks without any other therapy such as acupuncture, moxibustion and Western medicine. To evaluate the results of this treatment, polyarthritis was assessed by Visual Analogue Scale(VAS). The patient's general condition such as digestion and defecation discomfort is assessed by the progress notes. Results&Conclusions Most of the various symptoms have improved by about 80 percent. It is more important to observe defecation, urine, cold symptoms and condition of hands and feet synthetically.

By the way, Taiyin is classified into 'One who had had the Cold Disease Primarily and the Febrile', Taiyang has the nature of 'No Cold, No Heat' that is not tendency to The Cold or Heat sydromes, according to promote the respiration of air and fluid of the Liver and Lung. In the viewpoint of a few symptoms(constipation, diarrhea, sweat, urine, vomiting), we are known that cold and heat connception of the each symptome is fixed, according to nature of cold and heat in each type. This study was performed to develop cold-heat and deficiency-excess pattern identification for dementia, as well as for standard Korean medicine diagnosis and treatment. Five experts comprised of 4 neuropsychiatrists of Korean medicine and 1 statistician to develop cold-heat and deficiency-excess pattern identification for dementia. We searched studies about pattern identification and selected 507 articles using Oasis search terms provided by the KIOM. As a result, 10 pattern identification research study were recruited. Moreover, we analyzed neuropsychological assessments for dementia that evaluate Behavioral and Psychological Symptoms of Dementia (BPSD) and cognitive function using experts conferences and we selected neuropsychological instruments using pattern identification. Six cold patterns, six heat patterns, ten deficiency patterns, and four excess patterns were identified according to the cold-heat and deficiency-excess pattern identification of dementia. We selected the Caregiver-Administered Neuropsychiatric Inventory and the Korean Mini-Mental State Examination as neuropsychological assessments of dementia, which examine behavioral symptoms and cognitive function, suspectively. We formed positive and negative correlation between Korean medicine pattern identification and neuropsychological assessments for dementia. We developed and suggested a forecast module of pattern identification for dementia. But, it is necessary to perform additional clinical trials to verify its validity and accuracy.

Methods: First, read and study the medical records on metrostaxis(崩漏) of that made a profound study by Yi-Da-Gan(易大艮) and cold syndrome with pesudo-heat(眞寒假熱) of that made a profound study by Yu-Chang(喩昌). The next, write a paper on results and conclusions. Results and Conclusions: First, Yi-Da-Gan(易大艮) insist that must control the Qi under the blood disease conditions, taking the case of metrostaxis(崩漏). Secondly, we must study more on estimating the changing condition of Qi and the blood as time goes by, also study on the pulse and pulse condition in the four seasons(四時脈). Thirdly, Yu-Chang(喩昌) insist that be more careful in differentiation of symptoms and signs, taking the case of cold syndrome with pesudo-heat(眞寒假熱). Fourthly, Yu-Chang(喩昌) give an example that in condition of cold syndrome with pesudo-heat(眞寒假熱), sometimes, the pulse and pulse condition can be strong.

1. Objectives Among many symptoms that the climacteric patients complain of, the major subjective symptom is both heat in the upper part and cold in the lower part of the body(e.g foot, knee, and leg). We use DITI as a method to prove the symptoms of climacteric patients such as both heat in the upper part and cold in the lower part of the body, diagnose these symptoms, and follow up the progress of the clinical treatments. 2. Subjects and Methods We compare the subcutaneous heat of the climacteric patients with the symptom of heat in the upper part and cold in the lower part of the body with young women without any specific disease. The age distribution of the experimental group is 28 patients in 40's, and 22 in 50's. The mean age of the experimental group is $48.5{\pm}4.25$. The age distribution of the control group is 17 patients in teen, 26 in 20's, 7 in 30's. The mean age of the control group is $23.31{\pm}6.45$. We measure the subcutaneous heat on the cheeks, upper arms, palms, thighs, knees, dorsum pedis, lower back, hip, upper abdomen, lower abdomen of both groups and analyse the results. 3. Statistical methods All the results are statiscally analysed using student T-test of Microsoft Exel program. Statistically significant value by the analysis of variance procedure is P<0.05. 4. Results 1. The subcutaneous heat of the whole body of the experimental group shows hypothermia compared with control group, and the hypothermic pattern gets severe on the periphery. ${\Delta}T$ between the right and left side of the body in experimental group is larger than that of control group. 2. ${\Delta}T$ between the cheeks and the hypogastric in the experimental group is significantly larger than that of the control group, which shows cold face. 3. ${\Delta}T$ between the upper arms and the palms in the experimental group is significantly larger than that of the control group, which shows cold hand. 4. ${\Delta}T$ between the thighs and the knees in the experimental group is significantly larger than that of the control group, which shows cold knee. 5. ${\Delta}T$ between the thighs and the feet in the experimental group is significantly larger than that of the control group, which shows cold foot. 6. ${\Delta}T$ between the hip and the lower back in the experimental group is significantly larger than that of the control group.
