A Comparison of the Shoulder Stability Muscle Activity according to the Shoulder Flexion Angles in Open and Closed Kinetic Chain Exercise (열린사슬운동과 닫힌사슬운동에서 어깨 굽힘 각도에 따른 어깨안정화 근육의 근활성도 비교)

  • Purpose: The purpose of this study was to investigate any visible difference in shoulder stability muscle activity along the bending angle($70^{\circ}$, $90^{\circ}$, $110^{\circ}$) to perform the open kinetic chain(OKC) and closed kinetic chain(CKC) exercise. Method: This study was performed on normal 20 males subjects. Exercise was carried out in two ways OKC and CKC. Shoulder stability muscles are examined activity in the bending angles. In the OKC and CKC exercise to subjects with stretched out waist straightened, and than push forward the shoulder. Muscular activation was examined using the WEMG-8. Result: The results of this study were as follows. 1. Only in the trapezius muscle, it showed specific interaction between exercise methods and angles. When it comes to muscle activity, there was no significant difference in all muscles except for the anterior fibers of the deltoid muscle during both CKC and OKC exercise. 2. The average value of maximum muscle activity was significantly increased with increasing order of angles. But in CKC exercise, the muscle activity of infraspinatus tended to decrease with increasing angles. Conclusion: To summarize the research results, the muscle activity in upper fibers of the trapezius, anterior fibers of the deltoid, and serratus anterior was significantly different depending on the angle of the shoulder in both CKC and OKC exercise.

The Effect of Sling Exercise Therapy with Vibration Balls on Upper Limb Muscle Activity for Paraplegia-Spinal Cord Injury

  • Objective: The purpose of the present study is to investigate the effect of a muscle activity by applying the complex exercise method of sling in accordance with the provision of various vibration intensities for paraplegia-spinal cord injury. Method: The subjects of the study were 15 men in their 40s and 50s with lower limb disabilities and low potential risk, who were randomly divided into a sling exercise group (SG n=4), a sling with low frequency vibration group (SLVG n=4), a sling with mid-frequency vibration group (SMVG n=4), and a sling with high frequency group (SHVG n=4) in accordance with the provision of slings and vibration stimuli. The vibratory intensity provided was divided into low frequency (30 Hz), mid-frequency (50 Hz), and high frequency (70 Hz). The anterior deltoid (AD), the posterior deltoid (PD), the pectoralis major (PM), the upper trapezius (UT), the latissimus dorsi (LD), and the multifidus (MF) were measured to compare and analyze muscle activity. Results: The closed kinetic chain (CKC) exercise to the shoulder joint showed higher muscle activity in most muscles for the SMVG, and statistically significant differences in the anterior deltoid (AD), the pectoralis major (PM), and the multifidus (MF) in particular. Conclusion: The intermediate frequency (50 Hz) string vibration was the effective vibration stimuli for Closed kinetic chain (CKC) exercises.

Effects of EMG-Biofeedback based Closed Kinetic Chain Exercise on Quadriceps Muscle Activity and Dynamic Balance in Patellofemoral Pain Syndrome (근전도 생체되먹임 기반 닫힌사슬운동이 무릎넙다리통증증후군의 넙다리네갈래근 근활성도와 동적 균형에 미치는 영향)

  • Purpose: The purpose of this study was to determine the effects of electromyography (EMG)-biofeedback based closed kinetic chain exercise (CKCE) on quadriceps muscle activity and dynamic balance ability in patellofemoral pain syndrome (PFPS). Methods: Thirty subjects with PFPS were included and they were divided into EMG-biofeedback using CKCE (Group I) and squat exercise using CKCE (Group II), each group consisted of 15 patients. Group I and Group II was performed by the patients for three times a week, for six weeks. sEMG was used to measure quadriceps muscle activity and star excursion balance test (SEBT) was used to measure dynamic balance ability. Results: According to the results of the comparisons between the groups, after intervention, quadriceps muscle activity and dynamic balance ability were significantly higher in Group I than in the Group II. Conclusion: Findings of this study suggest that EMG-biofeedback using CKCE that provides real-time biofeedback information on muscle contraction may have a beneficial effect on selective muscle strength of vastus medialis oblique muscle and dynamic balance ability in PFPS.

  • Dysfunction of the anterior and dorsal muscles of the trunk have been studied in relation to low back pain of many years. Many muscles of the trunk are capable of contributing to the stabilization and protection of the lumbar spine, recent evidence has suggested that transversus abdominis may be critically involved and has been the focus of rehabilitation. The delay in onset of contraction of trunk muscles associated with movement of the upper or lower limb in patients with low back pain indicates a significant deficit in the automatic motor command for control of disturbance to the spine. The function of transversus abdominis has been largely ignored in the evaluation of spinal stabilization and protection. The most essential stabilizing muscles for the lumbar column are the transversus abdominis and the multifidus. Sling exercise therapy(SET) concept consists of a system of diagnosis and treatment. The system of diagnosis involves testing the muscle's tolerance through progressive loading in open and close kinetic chains. The SET system contains elements such as relaxation, increasing the range of movement, traction, training the stabilizing musculature, sensorimotor exercises, training in open and close kinetic chains, dynamic training of the mobilizing musculature, cardiovasc+ular exercises, group exercise, personal exercise at home. Sensorimotor training is an essential element of the SET concept. The emphasis is on closed kinetic chain exercises on an unstable surface, thereby achieving optimum stimulation of the sensorimotor apparatus.

  • Cervical pain is a rapid increase that is owing to a flexion-extension whiplash injury, unappropriated posture, chronical repetition injury from abdominal position of head and neck, excessive repeating work, chronical deficiency of excercise. Because of that is bring about muscle unbalance, tightness of cervical extensor muscle, weakness of cervical deep flexor muscles, instability of cervical region and reduction of proprioceptive sensor. Recent the role of muscle is more emphasized for preservation of sine stabilization. And cognition of integrated muscular system, importance for the operation and relation is increased to maintain stability of the motor system and pertinent function. Therefore we are going to introduce the sling exercise and stabilization exercise method for advanced efficient of cervical and upper limb and for the muscle strengthening to importance cervical stabilization through neurological program as control the reaction of cervical stabilization. This review article wishes to introduce about neuromuscular activation, "Neurac" technique that can speak as step developed more concept based on application principle etc, of basic sling exercises effort's change by suspension point's change that is concept, stability exercises principle, open kinetic chain exercises and closed kinetic chain exercises. Arranged about Plateau potential's characteristic and working mechanism and Neurac technique's application method that can say as Neurac technique's neuro-physiologic base, and presented treatment method of lumbar part disease and cervical part, shoulder joint part disease to application example. Because plateau potential forward player that induce using Neurac technique in part muscles that act role that keep spine surrounding stability in this article keeps muscle's contraction continuously, between global and local muscles, presented several study findings that can cause affirmative change in insufficient muscle adjustment state such as imbalance of muscles' action order. Expect that case studies that use this Neurac technique here after consist continuously.

Effect of lower extremity resistance exercise on gait performance in a patient with systemic lupus erythematosus with cerebral infarction and lower extremity vasculitis: a case study

  • Objective: This study was conducted to investigate the effects of resistant exercise on the gait performance of a patient with systemic lupus erythematosus (SLE) patient. Design: A case study. Methods: A 30-year-old male adult who had been diagnosed with systemic lupus erythmatosus (SLE) in April 2013, right middle cerebral artery infarction, and with left hemiplegia agreed to participate in this case study. Patient was unable to walk due to being affected with adynamia. Due to developing necrotizing vasculitis on the left lower extremity, patient underwent a myotomy on the left thigh. The patient was trained with a progressive resistant exercise program for 8 weeks. An intensity of 15 RM was used for the resistant exercises and the resistance level was increased progressively in order to improve the muscle power of the patient. Methods used to increase resistance included changing positions, providing mechanical resistance instead of manual resistance, transitioning from open kinetic chain to closed kinetic chain exercises, and changing the colors of the theraband to those with increase level of resistance. Outcome measures included the 5-repetition sit-to- stand test (5RSST), Timed Up & Go (TUG), and 10-meter walk test (10MWT). In addition, the GAITRite was used to assess the spatio-temporal gait variables, including gait speed, cadence, stride length of the left side, and double limb support pre and post-intervention. Results: The patient was able to perform sit-to- stand after two weeks of performing the resistant exercises. The patient was able to walk after 4 weeks, and the patient's overall gait performance had improved after 8 weeks. All of the variables had improved after each week. Conclusions: The results of this case study may be used to enhance future efforts to objectively evaluate resistant exercises during gait performance in persons affected by SLE.

Effect of Local Vibration on Triceps Surae Flexibility Compared to Static Stretching

  • Purpose: This study examined the effects of local vibration using a massage gun on the triceps surae flexibility by measuring the ankle dorsiflexion range of motion (ROM) compared to static stretching in healthy adults. Methods: Twenty healthy subjects were instructed in this study. They were allocated randomly to two groups: local vibration (LV) and static stretching (SS). The ankle dorsiflexion ROM was measured before and after the intervention in supine (open kinetic chain, OKC) and standing (closed kinetic chain, CKC). The LV group received local vibration using a massage gun for five minutes on their triceps surae, and the SS group stood on the Q board with a dorsiflexed ankle for five minutes. The ROM between pre- and post-intervention and the change in ROM between two groups were analyzed. A paired t-test was used to compare the ROM between pre- and post-intervention, while an independent t-test was used to compare the change in ROM between the two groups. Results: Both groups showed a significant difference between pre- and post-intervention in the position of both the OKC and CKC. The change in ROM, however, was not significantly different between the two groups. Conclusion: The application of local vibration using a massage gun for five minutes on the triceps surae could improve its flexibility as much as the application of static stretching. A massage gun for the application of a local vibration stimulus may be effective, simple, portable, and comfortable.

Evidence-Based Physical Therapy for Anterior Cruciate Ligament Injury: Literature Review

  • Most athletes with anterior cruciate ligament (ACL) ruptures undergo a surgical ACL reconstruction (ACLR) and rehabilitation. On the other hand, controversy still exists because neither a reconstruction nor rehabilitation have been proven to be superior in the management of ACL injury. This study reviewed the success rates of interventions to provide recommendations for the optimal management after an ACL injury. One of the most important considerations after an ACL injury is the timing and type of intervention. At the early stages, which involve the loss of volume and strength of quadriceps femoral muscle, weight bearing (closed kinetic chain) exercises with pain management followed by high velocity resistance exercises in an open kinetic chain environment are recommended to improve the quadriceps function. After that, it is important to apply intensive isokinetic exercise with a lower extension rate. In this case, it is important to apply overload to the muscles and to simultaneously lead the co-contraction of the hamstrings. Standards are essential because the timing and type of interventions are crucial to prevent re-injury and complications, such as osteoarthritis, as well as to confirm the successful outcome of the treatment. Different interventions recommended for ACL damage have yet to reach consensus. Further studies will be needed to observe the effects of the intervention through multidisciplinary approaches.

Immediate Effects of Joint Mobilization Techniques on Clinical Measures in Individuals with CAI

  • Objective: Epidemiological research shows that 47 to 73% of athletes suffer from recurrent ankle sprains. Joint mobilization techniques (JMT) implemented in correcting may be beneficial in the management of ankle injuries. The purpose of this study is to examine the immediate JM on ankle complex as clinical measures in individuals with chronic ankle instability (CAI) through intervention. Method: Thirteen subjects with CAI (8 males and 5 females) participated in this study. Each subject tried total four alignments (Navicular drop test: NDT, Standing rearfoot angle: SRA, Tibia torsion: TT, and dorsiflexion range of motion: DFROM). The participants were performed the 10 meter shuttle run after JMT for post-task. Finally, it was tried to compare between pre-post tasks after shuttle run. Results: SRA and DFROM after intervention showed significant differences. SRA (p=.026), and DFROM (p=.034). Conclusion: We concluded that the JMT has resulted in improvement in SRA, DFROM. Increased DFROM and varus shapes of foot would be closed kinetic chain, indicating that reduce the risk factors of ankle sprain. Future study needs to be conducted in order to measure the effects of prolonged intervention of JMT.
