병원의 지적자본이 진료 및 재무 성과에 미치는 영향 (The Effect of Intellectual Capital on the Clinical and Financial Performance of Hospitals)

  • Objectives : This study applied the research model of Bontis (1998), which has been used to investigate everything from business corporations to hospitals in Korea, examined its generalizability and attempted to verify which factors of the intellectual capital affects organizational performance, represented by indicators for the clinical and financial performance of hospitals. Methods : Primary data for the study were collected from 91 general hospitals through self-administered questionnaires, and secondary data on clinical and financial performance of hospitals were collected form existing statistics. Collected data were analyzed with SPSS 11.5 (ver.) and AMOS 18.0 (ver.). Results : It was found that only human capital and customer capital had indirect effects on hospital performance. It was also revealed that structural capital had a direct effect on clinical performance, and an indirect effect on financial performance. Conclusions : This result means that there is a need to individually consider both the clinical performance and financial performance when measuring the organizational performance of a hospital, and it also shows that positive improvement in the clinical performance can contribute to the improvement of the financial performance through this process.

간호대학생의 임상실습교육에 대한 자기효능감, 전문직 자아개념 및 임상실습만족도 (Self-Efficacy, Self-Concept of Professional Nursing, and Clinical Practice Satisfaction to Clinical Practice Education in Nursing Students)

  • The purpose of this study was to identify self-efficacy and self-concept of professional nursing which might influence clinical practice satisfaction in nursing students. The participants were 278 nursing students in U city. Date were collected from November 29 to December 17, 2010. The collected data was analyzed with the PASW 18.0 statistics program. Self-efficacy had significant positive correlation with self-concept of professional nursing(r=.595, p<.001) and self-concept of professional nursing had significant positive correlation with clinical practice satisfaction(r=.396, p<.001). In multiple regression analysis, self-concept of professional nursing is most influential factor explained 21% of variance of clinical practice satisfaction. The results indicate that to develop a more reasonable and effective curriculum and teaching program for enhancement of students' self-concept of professional nursing.

  • The purpose of this study was to find out the general characteristics of school health teachers, the status of school health resources and the degrees of self-confident performance for the 124 school health teachers in Pusan City. Data was collected by means of questionaires from Aug. 1986 to Mar. 1987. The data were analyzed by use of percentage, mean, standard deviation, t-test, ANOVA and Pearson-correlation coefficient. The results of this study were as follows: 1 . General Characteristics of School Health Teachers (SHT) 1) The average of age of the SHT was 32.8 years old and 39.5% of them were from 30 to 34 years old. 2) The average for school nursing experience of the SHT was 7.9 years and 37.9% of them were from 5 to 9 years. 3) The 45.2% of them have not the clinical experience. 4) The 74.2% of them were graduated from the 3 years college of nursing. 2. Status of school health resources and nursing activities. 1) The average of student number was 2497.3 and class number was 45.2. 2) The average of school health budget was 1039000 won and 27.7% of school health budget expended on examination cost. 3) Only 29.0% of all schools have organization for school health. 4) The 84.7% of all schools have health clinic separately and 69.1% of schools have less than $33m^2$ sized. 5) The average of clinic visitor number was 2111.8 for 1 year. 6) Major problem was on digestive system. And other problems were skin, respiratory, musculo-skeletal system and dental problem. 7) The number of literal message was 14.4 times for 1 year. 3. The degree of the school health teachers' self-confidence. 디지털 병원의 CDSS구현을 위한 CPG 개발 (Developing CPG for Implementation of CDSS in Digital Hospitals)

  • Purpose: The purpose of this study is to propose Clinical Practice Guideline(CPG) model and Clinical Index(CI) for implementing CDSS in digital hospitals. Methods: This study uses EMR data at department of family practice in A hospital; 636 patients, 570 diseases (based on ICD 10-CM criteria), and 37,000 data related with labs and treatments. This study focuses on disease J342 which is the most high rate of incidence. Results: Using the suggested model, this study calculates frequency matrix and probability matrix to find out the correlation of diseases and labs. This study indicates the lab sets of Disease (J342) as CI for CPG. Conclusion: This study suggests CPG model including Lab-based, Disease-Based and Case-based modules. Through 6 level cased-based CPG model, especially, this study develops Clinical Index(CI) such as the Incidence Rate, Lab Rate, Disease Lab Rate, Disease confirmed by Lab.

복강경 위절제환자와 임상적 건강관리와의 관련된 요인 (Factors Related between Clinical Health Management and Laparoscopy-Aided Gastrectomy Patients)

  • 본 연구는 복강경 위절제환자의 임상적 건강관리에 영향을 미치는 요인을 규명하고자 시도하였다. 자료수집은 2012년 1월 16일부터 2월 16일까지 수도권 지역의 종합병원 외과에 내원한 환자 201명을 설문 및 면접조사를 하였다. 연구결과로는 첫째, 수행성과 효율성은 유의한 양의 상관관계를 나타내었다(r=0.24, p<.01). 둘째, 선정된 변수에 의해 임상적 건강관리의 설명력은 42.7%이었다. 셋째, 다중회귀 분석결과 효율성이 0.25로 건강관리 실행에 가장 영향력이 있는 변수였으며 유의한 것으로 나타났다(p=.02). 이러한 결과를 토대로 복강경 위절제술환자의 임상적 건강관리정도를 높이기 위해서는 수행성, 자기성취감 및 지각된 장애 등 개인 특성을 포함하는 포괄적인 교육 프로그램을 개발하여 적용할 필요가 있다.

The Intelligent Clinical Laboratory as a Tool to Increase Cancer Care Management Productivity

  • Studies of the causes of cancer, early detection, prevention or treatment need accurate, comprehensive, and timely cancer data. The clinical laboratory provides important cancer information needed for physicians which influence clinical decisions regarding treatment, diagnosis and patient monitoring. Poor communication between health care providers and clinical laboratory personnel can lead to medical errors and wrong decisions in providing cancer care. Because of the key impact of laboratory information on cancer diagnosis and treatment the quality of the tests, lab reports, and appropriate lab management are very important. A laboratory information management system (LIMS) can have an important role in diagnosis, fast and effective access to cancer data, decrease redundancy and costs, and facilitate the integration and collection of data from different types of instruments and systems. In spite of significant advantages LIMS is limited by factors such as problems in adaption to new instruments that may change existing work processes. Applications of intelligent software simultaneously with existing information systems, in addition to remove these restrictions, have important benefits including adding additional non-laboratory-generated information to the reports, facilitating decision making, and improving quality and productivity of cancer care services. Laboratory systems must have flexibility to change and have the capability to develop and benefit from intelligent devices. Intelligent laboratory information management systems need to benefit from informatics tools and latest technologies like open sources. The aim of this commentary is to survey application, opportunities and necessity of intelligent clinical laboratory as a tool to increase cancer care management productivity.

RAND 방법으로 합의한 임상진료지침의 정의와 질 평가 기준 (Consensus on definition and quality standard of clinical practice guideline using RAND method)

  • Background : Clinical practice guidelines are an increasingly familiar part of clinical practice. Moreover, rigorously developed evidence based guidelines has been widely used. However, in Korea, some of published documents as clinical practice guidelines have shown considerable disparity in structure, contents and quality. This is mainly because there is no consensus on the definition and quality standard of clinical practice guidelines. The purpose of this study was to draw consensus on the definition and the quality standard about clinical practice guidelines. Method : We developed a questionnaire about the definition of clinical practice guidelines with inclusion criteria(23 items) and the quality standard(30 items). We selected 9 experts who had prior experience in developing and implementing guidelines. Rating methods for appropriateness of items were adopted from the RAND method. Consensus was drawn in three rounds. Results : Of the 47 items agreed, 40 items were determined to be appropriate. Clinical practice guidelines were defined as "scientifically and systematically developed statements to assist practitioners and patients on making decisions about appropriate health care for specific clinical circumstances." Narrative reviews, systematic reviews or health technology assessment without recommendations, translation of foreign guidelines, guidelines for patients only and training manuals were not considered as clinical practice guidelines. For the quality standard of clinical practice guidelines, 27 items were deemed necessary. Conclusions : The consensus on the definition with inclusion criteria and the quality standard of clinical practice guidelines carries an important meaning as the first attempt to draw a general agreement in our society. The unique achievement of the consensus reflects the current status of clinical practice guidelines that there has been a high tendency to adapt foreign guidelines. We hope efforts of this kind will continue to bring improvement in clinical practice guidelines.

치위생과 학생의 관찰-수행실습 빈도와 만족도 (Satisfaction and the frequency of observation and clinical practice in dental hygiene students)

  • The frequency of clinical observation and clinical practice and satisfaction among dental hygiene students were investigated during the clinical practice process. The results were analyzed to come up with a more effective clinical practice program and provide basic data that would aid in actual operation of clinical practice. The tool used in this study was a survey composed of 11 areas with a total of 55 questions. It was distributed to 471 graduating dental hygiene students at 5 different universities. The Cronbach's Alpha coefficient was .975. The data were analyzed using SPSS 10.0 for Windows. Significance was determined at ${\alpha}$=0.05. The results were as follows. 1. The results of analyzing the frequency of clinical observation according to the different clinical areas showed that the score was the highest in the area of basic diagnosis, followed by prosthodontics, operative dentistry, hospital service management, oral surgery, oral radiology, periodontics, preventive dentistry, orthodontics, pediatric dentistry, and oral medicine. 2. The results of analyzing the frequency of clinical performance according to different clinical areas showed that the score was the highest in the area of basic diagnosis, followed by operative dentistry, oral surgery, prosthodontics, oral radiology, pediatric dentistry, orthodontics, oral medicine, preventive dentistry, endodontics and hospital service management 3. The area that showed most significance difference between the frequency of clinical observation and the frequency of clinical performance was the area of hospital service management, followed by prosthodontics, periodontics, preventive dentistry, oral radiology, oral medicine/oral surgery, orthodontics, operative dentistry, and pediatric dentistry. 4. When satisfaction in dental hygiene students was analysed according to different clinical areas, they were most satisfied practicing in basic diagnosis, followed by operative dentistry, prosthodontics, oral surgery, oral radiology, pediatric dentistry, orthodontics, preventive dentistry, hospital service management, periodontics, and oral medicine.

의료기관 종사자의 진료정보 보호행위분석: 건강심리이론관점을 중심으로 (Clinical Information Protection Behavior in a Medical Institution : Based on Health Psychology Theories)

  • Purpose: This research aims to find out clinical information protection behavior within a medical institution in mandatory circumstance based on health psychology theories Methods: This research has developed the survey based on the variables from ealth psychology theories; and conducted the survey during the whole month in April 2013. In the end, 256 samples have been used for this research's analysis. Results: First of all, Empirical results has proved that perceived benefits, self-efficacy, and cues to action have an positive influence on clinical information protection behavior. Perceived barriers has an negative influence. Finally, it has proven from the research that perceived severity and perceived susceptibility do not have an impact on clinical inf ormation protection behavior Conclusion: These findings provide an enriched understanding about medical institution workers information protection behavior on patient's clinical information.

학교병원 유무에 따른 보건-의료계열 대학생의 임상실습 현황 및 만족도 조사 (A Study on Satisfaction Investigation and Current Status of Clinical Training Program for Healthcare-Affiliated Students Depends on University Hospital Availability)

  • The purpose of this study was to identify clinical practice (clinical training education) and satisfaction depend on university hospital availability according to a survey with 850 healthcare-affiliated students. The collected data were analyzed with the SPSS17.0 program. The result was shown statistically significant difference depends on gender, major departments and satisfaction of college life. In addition, there were significant differences in satisfaction of clinical training education in the environmental aspects of practice depending on whether affiliated hospital or not. There were no significant correlation found between degree of satisfaction of clinical training education and the need for affiliated hospital (r = .204, p <.01). As a result, Attached to the university hospital can be helpful to healthcare-affiliated students on environmental and educational aspects of clinical training education. Therefore, the effective and practical training programs using university-affiliated hospitals are surely needed to develop and to apply the active support.
