One Clinical Case Report of Lung Cancer Patient with Depression and Anxiety Disorder Improved by Korean Traditional Medical Treatment and Breathing Meditation (한방치료와 호흡명상으로 호전된 폐암 환자의 불안 및 우울 증상 치험 1례)

  • This study was case report to show the good effects of korean traditional medical treatment and breathing meditation. The methods were applied to lung cancer patients. We treated one patient who scored 27 point at Beck's depression inventory(BDI) and 28 point at Hamilton rating scale for depression(HRSD) and 22 point at Beck's anxiety inventory(BAI) and 20 point at Anxiety status inventory(ASI) by korean traditional medical treatment and breathing meditation. After treatment, the clinical symptoms were improved and the score of BDI, HRSD, BAI, ASI were decreased. According to this study korean traditional medical treatment is effective for the cure of depression and anxiety disorder and breathing meditation forifies the korean traditional medical treatment.

The Clinical Analysis on 19 Cases of Anxiety Disorder (불안장애환자(不安障碍患者) 19례(例)에 대(對)한 임상적(臨床的) 고찰(考察))

  • A clinical analysis was carried out for 19 patients who was treated in Dept. of Neuropsychiatry of Taejon Oriental Medicine Hospital from I aug. 1997 to 31 Jul. 1998 and was ruled out as anxiety disorder. The results were summarized as follows. 1. In the distribution of the sex, ratio between female and male was 1:2.8, distribution of the age was even each age, and the primary motive of the onset were psychological trauma, question of money, discord of a household. 2. As for the duration in disease, the duration from a week to a month had a majority of all, and the most duration of the addmission-treatment was a week. 3. The most common symptom were palpitation ·anxiety, headache·dizziness, chest discomfort, the cases of generalized anxiety disorder and atypical anxiety disorder was a large number. 4. In the aromatherapy of absorptive method, the sort of many used essential oil were Lavenda and Rosewood, in the massage, that were Peppermint and Rosemary. 5. The most used recipes were BUNSIMKIYEM(分心氣飮) HACHULBOSIMTANG(夏朮補心湯), CHUNGGANSOYOSAN(淸肝逍遙散), BANHABAEKCHUCHUNMATANG(半夏白朮天麻湯), CUIBIONDAMTANG(歸脾溫膽湯) etc. 6. In the judgement of efficacy, 9 cases showed good efficacy, 9 cases showed no change, 1 case showed excellent efficacy.

Double-blind Comparative Trial of Fluoxetine and Amitriptyline in Major Depression (주요우울증에서 Fluoxetine과 Amitriptyline의 치료효과에 대한 이중맹검법 비교연구)

  • The efficacy of fluoxetine was evaluated in 32 patients with major depression in double-blind amitriptyline-controlled clinical trials. Patients were randomly assigned to 6 weeks of treatment with 20mg/day of fluoxetine and 25-100mg/day of amitriptyline. We used the Hamilton rating scale for depression(HAM-D) and the Clinical Global Improvement(CGI) to evaluate the improvement of depression. In addition, we also used the Covi Anxiety Scale and the Anxiety/Somatization subscale of HAM-D to investigate the relieving effect of anxiety. The improvement by fluoxetine in mean total score of HAM-D and CGI was comparable to amitriptyline. Fluoxetine also reduced anxiety significantly, but there was no difference between fluoxetine and amitriptyline in relieving anxiety symptoms. Fluoxetine showed considerably less adverse effects, especially anticholinergic effect and weight gain than amitriptyline. In conclusion, 20mg/day of fluoxetine was sufficiently effective in the treatment of depressive patients and was better tolerated than amitriptyline.

Effects of Supportive Nursing Care Program on Hip Function, Anxiety and Uncertainty for Patients with Total Hip Arthroplasty (인공 고관절 수술 환자에서 지지간호중재가 고관절 기능, 불안 및 불확실성에 미치는 효과)

  • Purpose: This study was to evaluate the effect of supportive nursing care program for patients with total hip arthroplasty. Methods: Forty-two patients with total hip arthroplasty were enrolled in this study from September, 20, 2011 to January, 3, 2012. The participants were assigned to one of two groups: Twenty-six subjects in the experimental group were provided with supportive nursing care program which comprised of 6 nurse visits pre and post operation and 4 follow-up phone calls after discharge. Another 26 subjects received conventional nursing care program as a control group. Hip function, anxiety, and uncertainty were evaluated before the intervention, and 5 weeks after completion of the intervention. The analysis included descriptive statistics, Chi-square test, and t-test by SPSS 18.0. Results: Hip function was significantly increased in the experimental group (t=-3.31, p<.002). Anxiety and uncertainty group was significantly lower in the experimental group (t=7.12, p<.001 and t=4.66, p<.001). Conclusion: The results of this study indicate that the supportive care intervention for patients receiving total hip arthroplasty could be utilized as a nursing intervention to improve hip function and to reduce anxiety and uncertainty of patients receiving total hip arthroplasty.

The Influence of Death Anxiety and Terminal Care Stress on Job Satisfaction of New Nurses (신규간호사의 죽음불안, 임종간호 스트레스가 직무만족도에 미치는 영향)

  • Purpose: This study aimed to investigate the correlation between death anxiety, terminal care stress, and job satisfaction of new nurses, as well as to identify factors affecting job satisfaction using descriptive correlations. Methods: This study included 143 new nurses who had 3 to 12 months of experience in terminal care. Data were collected from January to February 2018, and were analyzed using descriptive statistics, t-test, ANOVA, Scheffe? test, Pearson's correlation coefficient, and hierarchical regression analysis. Results: There was a negative correlation between job satisfaction and terminal care stress (r=-.170, p=.043), while death anxiety and terminal care stress were positively correlated (r=.284, p=.001). The following findings demonstrated a significant effect on job satisfaction: lesser the clinical career experience, the lower the job load causing death anxiety and terminal care stress, and the higher the job satisfaction. Furthermore, the explanatory power of these factors was 15.1%. Conclusion: To assist new nurses within three months of joining in clinical adaptation, it is necessary to provide them with appropriate knowledge regarding terminal care through training, and with counseling opportunities for the psychological burdens they experience while caring for dying patients.

The Influence of Care Stress for Older Patients, Self-perceptions of Aging, Aging Anxiety on Retirement Preparation in Nurses (간호사의 노인간호 스트레스, 노화인식, 노화불안이 노후준비에 미치는 영향)

  • Purpose: The purpose of this study was to explore the influence of care stress for older patients, self-perceptions of aging, and anxiety about aging on preparation for retirement in clinical nurses. Methods: This was a cross-sectional descriptive study in which participants were 222 nurses who had at least six months of work experience and were involved in caring for older patients in the last six months at the work site. Data were collected from August to September, 2020 using an online survey questionnaire. Data were analyzed using the IBM SPSS/WIN 25.0 program. Results: The mean level of preparation for retirement in the participants was moderate (3.21 out of 5 points). Among the sub-domains of preparation for retirement, financial preparation had the lowest score. Self-perceptions of aging and aging anxiety were significant predictors for retirement preparation in nurses, accounting for 16% of the variable's total variance. Conclusion: A greater level of preparation for retirement was associated with a positive perception of aging and a decreased level of aging anxiety in the clinical nurses. Further research should focus on exploration of specific determinants of financial preparation for retirement and development of intervention strategies for improving preparation for retirement in the nursing workforce.

Physiological Markers in Anxiety and Depression (생리적 지표를 이용한 불안과 우울 측정에 대한 고찰)

  • Purpose: Anxiety and depression are the concepts that commonly used as an emotional approach in the nursing researches and most of researches have been used questionnaires as a marker of anxiety and depression. There were few researches using physiological markers in measuring anxiety and depression. Methods: Journals published between 1950 and 2007 that include depression and anxiety measuring through physiological markers were reviewed. Results: As in the case of the hypothlamus-pituitary-adrenal axis system, it appeared that cortisol, epinephrine, and norepinephrine belonged to the category of hormones which were responsive to anxiety and depression. Plasma norepinephrine was a marker of the anxiety whereas plasma cortisol was a marker of the depression. The anxiety and depression were correlated with immune and taste, but it considered as an outcome variable not a physiological marker. Conclusion: Catecholamine and cortisol reflect anxiety and depression state. Our findings suggest that further researches are needed to distinguish between markers and outcomes of depression and anxiety using physiological markers.

Effect of Acupressure on Preoperative Cesarean Section Anxiety

  • Anxiety is a common preoperative problem in cesarean section candidates. Nonpharmacologic anxiety control has been demonstrated to be more suitable in pregnant women. The current study was a randomized, single-blind clinical trial which evaluated the effect of acupressure on preoperative C-section anxiety. In this study, 60 patients facing surgery were randomly divided into two groups of 30 patients each. Those patients in the intervention group received simultaneous acupressure at the Yintang and HE-7 acupoints for 5 minutes before surgery, and patients in the control group received intervention at a sham acupoint. The anxiety level of patients was preoperatively assessed twice using the Spielberger's State-Trait Anxiety Inventory. The mean anxiety scores of the two groups were shown to be insignificantly different before the intervention (p = 0.859), whereas a significant difference between the mean anxiety scores of the two groups was observed after the intervention (p = 0.001), suggesting that acupressure reduced the anxiety of patients before surgery.

Clinical Symptoms and the Duration of Illness in Patients with Obsessive-Compulsive Disorder (강박증 이환기간과 임상 양상)

  • Objectives : There is a paucity of data on the long-term course of obsessive-compulsive disorder (OCD) and chronological relationship between OC symptoms and their related symptoms such as anxiety and depression. The purpose of this study was to investigate the longitudinal course of OC symptoms as well as anxiety and depression which are believed to be associated with OC symptoms. Methods : Data for 155 patients with OCD who completed general evaluation for OCD were used. Forty four were excluded to minimize the effect of the different age of onset on the clinical course. One hundred eleven patients finally participated in the analysis. Cross-sectional correlations between each symptom as well as between such symptoms and the duration of illness were analyzed. Further correlation analysis was done within two groups that were divided by 7 years of the duration of illness. Results : There were significant correlations not only between the severity of OC symptoms and anxiety but also between anxiety and depressive symptom, regardless of the duration of illness. These correlations between such symptoms were also found within patients with the duration of illness below 7 years, whereas these were not within the group with the duration of illness above 8 years. Conclusion : Patients with OCD in this study shows the moderate to severe level of OC symptoms irrespective of the duration of illness. Our finding also suggests that the OC symptoms, especially obsessions are closely related to anxiety and depressive symptoms and these relationships might be pronounced in relatively early phase of the OCD after onset.

The Effect of the Pulsatilla 30C as Homeopathy for Ophthalmic Diseases with Concomitant Separation Anxiety (동종요법을 이용한 개의 분리불안과 안과질환 치료 4례)

  • Homeopathic Pulsatilla 30C was evaluated in 4 clinical cases of ophthalmic complications with concomitant separation anxiety in dogs. Separation anxiety in dogs is excessive anxiety regarding to the separation from owners and is one of the most common behavioral problems in dog. In addition, ocular diseases is sometimes related with abnormal behaviors. Holistic approach is good alternative method to treat dogs with physical problem associated behavior problem. Homeopathy is one of the complementary and alternative medicines and bases on healing of patients themselves which includes mind healing, not the disease itself. Among the material medical remedy of homeopathy, the Pulsatilla is used as homeopathic remedy for ocular problems, earache, cough, cold, and anxiety from lost attention. These case studies described the successful treatment of ocular diseases and separation anxiety by application of Pulsatilla 30C as homeopathy in 4 dogs. During the homeopathy, these dogs did not receive any other treatments, and were performed the physical exam to check the ocular problems. These dogs were completely cured of physical and behavior problem within 10 days. On following up, any clinical signs were not observed at one month after the last therapy. In addition, as from the treatment the anxiety problem of patients has been much better improved. According to these case studies, we know that an application of homeopathic remedy may be useful therapeutic choice to treat both problems of physical and behavior.
