Clinical Study on the Effects of Auricular Acupuncture Treatment in Allergic Rhinitis Patients (알레르기 비염환자에 대한 이침 치료 증례보고)

  • Objective : The purpose of this study was investigated the effect of auricular Acupuncture treatment in Allergic rhinitis patients. Methods : This study was carried out on the 16 allergic rhinitis patients who had been treated at Cheongdogun public health center for The Good-Breath program. The Good-Breath program included auricular acupuncture was performed 2 times a week during 4 weeks. Auricular acupuncture points used were M101(肺), M13(副腎), M16(內鼻), M22(內分泌), M33(額), M55(神門). Patients nasal symptoms and QOL were assessed before and after 4 weeks treatment by the Severity Scoring Systems of Rhinitis(SSSR) and Quality of Life Questionnaire for Korean Rhinitisian(QLQKR). Results : 1. Total Severity Scoring Systems of Rhinitis(SSSR) Scores were significantly improved. but Each items was no significant difference. 2. Among items of QLQKR, Itching worry about western medicine worry about change of environment Role limitation-physical were significantly improved. and Total Quality of Life Questionnaire for Korean Rhinitisian(QLQKR) Scores were significantly improved.

  • Allergic rhinitis characterized by stuffy nose and watery discharge is, in Oriental medicine, referred to as Bisak(鼻塞) or Bigu. This condition occurs either exogenously or endogenously, and from my clinical experience, most patients who visit my clinic have endogenous causes. The nose in Oriental medicine is closely related to the Lungs, Heart, Large Intestines, Skin, Spleen and Stomach. Therefore, the wholistic approach to allergic rhinitis is much more efficient in treating it than the mere focus-oriented treatment.

  • Allergic diseases, including allergic rhinitis, asthma, and atopic dermatitis, are common heterogeneous diseases that encompass diverse phenotypes and different pathogeneses. Phenotype studies of allergic diseases can facilitate the identification of risk factors and their underlying pathophysiology, resulting in the application of more effective treatment, selection of better treatment responses, and prediction of prognosis for each phenotype. In the early phase of phenotype studies in allergic diseases, artificial classifications were usually performed based on clinical features, such as triggering factors or the presence of atopy, which can result in the biased classification of phenotypes and limit the characterization of heterogeneous allergic diseases. Subsequent phenotype studies have suggested more diverse phenotypes for each allergic disease using relatively unbiased statistical methods, such as cluster analysis or latent class analysis. The classifications of phenotypes in allergic diseases may overlap or be unstable over time due to their complex interactions with genetic and encountered environmental factors during the illness, which may affect the disease course and pathophysiology. In this review, diverse phenotype classifications of allergic diseases, including atopic dermatitis, asthma, and wheezing in children, allergic rhinitis, and atopy, are described. The review also discusses the applications of the results obtained from phenotype studies performed in other countries to Korean children. Consideration of changes in the characteristics of each phenotype over time in an individual's lifespan is needed in future studies.

A Study on Correlation between ARIA, TNSS and Nasal Endoscopy as Instruments of Evaluation for Allergic Rhinitis (비내시경 소견의 알레르기비염 분류 도구와 중증도 평가도구의 상관성 연구)

  • Objectives : We performed a clinical study to investigate characteristics of pattern identifications using nasal endoscopy for allergic rhinitis (AR). Methods : We assessed 32 patients with allergic rhinitis using nasal endoscopy, classifications of allergic rhinitis and it's impact on asthma (ARIA) and total nasal symptom score (TNSS). Results : The watery rhinorrhea score of nasal endoscopy was significantly high in 'persistent and moderate/severe' group (P<0.05). Patients were classified as rhinorrhea group and nasal obstruction group according to nasal endoscopy. TNSS and rhinorrhea score was significantly high in rhinorrhea group (P<0.05). Conclusions : The result may provide that the watery rhinorrhea of nasal endoscopy is useful as 'Cold' and 'Deficiency' pattern identification diagnostic tool.

  • Objectives : Kamibojungikgi-tang is one of the most frequently used medical treatment for the allergic rhinitis. The main symptoms of the allergic rhinitis are nasal obstruction, watery rhinorrhea, and sneezing. In this study, we investigated the medical effect of the Kamibojungikgi -tang with intradermal acupuncture treatment on the allergic rhinitis in 10-11 years old children. Methods : For 8 weeks, we administered Kamibojungikgi-tang to 84 patients and treated them with intradermal acupuncture. We checked their serum IgE and examined 10 symptoms of the allergic rhinitis (nasal obstruction, watery rhiorrhea, sneezing, nasal pruritus, hyposmia, quality of life, color of nasal mucosa, edema of nasal mucosa, postnasal drip, and complication). Results : 1. Serum IgE decreased 2.62 point. which showed no significance. 2. Nasal obstruction, watery rhiorrhea, sneezing, nasal pruritus, hyposmia, quality of life, color of nasal mucosa, edema of nasal mucosa, and postnasal drip were improved. (p<0.005) 3. Complication showed no significance. Conclusions : After treatment, we had a lower symptom index scores. Kamibojungikgi-tang with intradermal acupuncture treatment had an effect on the allergic rhinitis.

Reviewing Research on the Application of Intranasal Phototherapy in Allergic Rhinitis (알레르기 비염의 비강 내 광치료에 관한 논문 경향 분석)

  • Objective : The purpose of this study is to analyze research trends on the effects of intranasal phototherapy on allergic rhinitis. Methods : We searched potentially relevant studies using electronic databases, such as PubMed, OASIS, KMbase, and RISS. Our review of clinical research in allergic rhinitis was restricted to an analysis of papers published within the last 5 years. Independent reviewers extracted data using a standardized form. Results : In total, twenty four studies were included. Eleven studies were analyzed, and five randomized trials were identified. They suggested to be effective and no serious side-effects. Conclusions : Intranasal phototherapy can be effectively used as an alternative to the treatment of allergic rhinitis. Further studies are needed on this topic in order to demonstrate the effectiveness clearly.

A Clinical Study on Sasang Constitutional Medical Treatment Effectiveness of Allergic Rhinitis Patients in Taeumin (태음인(太陰人) 알레르기성 비염환자의 사상의학적(四象醫學的) 치료효과(治療效果)에 관한 임상 연구)

  • 1. Objectives The present study purposed to examine Taeumin after Sasang Constitutional medical treatment showed faster improvement in sneezing or rhinorrhea or nasal obstruction, the main symptoms of allergic rhinitis. 2. Methods One group treated by Sasang Constitutional medicine in allergic rhinitis patients of T aeumin is 20 cases. The other group treated by other herb medication in alletgic rhinitis patients of Taeumin is 15 cases. We analyzed the improvement of the main symptoms in 2 group who had allergic rhinitis for less than five years in the 2nd, 4th, 6th and 8th week of treatment. 3. Results & Conclusions (1) Sneezing showed a significant decrease from the 6th week of treatment in the group treated by Sasang Constitutional medicine and from the 6th week in the group treated by other herb medication. (2) Rhinorrhea showed a significant decrease from the 2nd week of treatment in the group treated by Sasang Constitutional medicine and from the 4th week in the group treated by other herb medication. (3) Nasal obstruction showed a significant decrease from the 4th week of treatment in the group treated by Sasang Constitutional medicine and from the 4th week in the group treated by other herb medication. (4) Satisfaction with subjective symptoms was higher in the group treated by Sasang Constitutional medicine than in the group treated by other herb medication but the difference was not significant (p>0.01). (5) Change in total symptom score showed a significant decrease from the 2nd week in the group treated by Sasang Constitutional medicine and from the 4th week in the group treated by other herb medication(p>0.01).

The Relationship between Allergic Diseases and Functional Gastrointestinal Disorders in Children and Adolescents (소아·청소년의 알레르기 질환 (천식, 알레르기비염, 아토피피부염)과 기능성 위장관 질환과의 관계 (단면 조사 연구))

  • Objectives The purpose of this study is to establish relationship between allergic diseases and functional gastrointestinal disorders in children and adolescents using objective criteria and questionnaires. Methods This study surveyed 237 children and adolescents who visited the department of Pediatrics of ${\bigcirc}{\bigcirc}$ Korean Medicine Hospital from September 23rd, 2013 to December 26th, 2013. The Korean version of International Study of Asthma and Allergies in Childhood (ISAAC) questionnaire and the Korean-translated Questionnaire on Pediatric Gastrointestinal Symptoms-Rome III (QPGS-Rome III) were used. We analyzed the data by using PASW Statistics 18.0 with Chi-square test, Fisher's exact test and Linear by linear association. Results There was no significant difference between prevalence of allergic diseases and functional gastrointestinal disorders. Also, there was no strong relationship between the number of allergic diseases and the functional gastrointestinal disorders. However, some parts showed significant relationships - such as asthma symptoms ever and belch; asthma symptoms last 12 months and belch; allergic rhinitis diagnosis ever and belly aches and abdominal pain around and below the belly button; allergic rhinitis treatment last 12 months and belly aches and abdominal pain around and below the belly button; and atopic dermatitis diagnosis ever and irritable bowel syndrome (respectively; p=0.046, p=0.008, p=0.004, p=0.029, p=0.035). And as the number of allergic diseases increases, the prevalence of functional gastrointestinal disorders are 19.4%, 31.3%, 41.7%, 31.0% respectively. Conclusions Although there was no significant relationship between allergic diseases and functional gastrointestinal disorders, some gastrointestinal symptoms were related to allergic diseases.

  • As the coronavirus disease 2019 (COVID-19) pandemic continues, doctors face many difficulties in managing patients with respiratory allergies. As research continues, much new knowledge has emerged about the link between COVID-19 infection and respiratory allergies. Studies to date have shown that there is no significant increase in the risk of COVID-19 infection in patients with respiratory allergies. However, in patients undergoing treatment for moderate to severe asthma or allergic rhinitis, more severe clinical manifestations, increased hospitalization period, and increased mortality may occur, so patients need to be managed with more care. In addition to personal efforts, social consideration should be accompanied to prevent deterioration in patients' quality of life with respiratory allergies due to COVID-19. Lastly, as clinical doctors, we should treat patients with respiratory allergies to protect ourselves appropriately by actively utilizing remote medical treatment and minimizing tests that generate droplets. Also, by appropriately educating patients and continuing drug treatment, efforts should be made to ensure that patients receive the best treatment even in a pandemic situation.

Current status of allergic rhinitis patients in Korean Medicine hospitals - Exploratory study based on electronic medical records of 3 hospitals (국내 한방병원의 알레르기 비염 환자 현황에 대한 탐색적 연구 - 3개 대학한방병원의 전자의무기록 자료를 중심으로)

  • Objective : To study current status on characteristics of allergic rhinitis patients by using the electronic medical record (EMR) data and to explore the feasibility of clinical studies using the EMR data in three different Korean medical hospitals. Methods : We studied allergic rhinitis patients who visited the department of ophthalmology, otolaryngology and dermatology in the three different Korean hospitals from January 1, 2012 to December 31, 2012. We retrospectively collected medical history and characteristics of study subjects using data of EMR. Results : In hospital A, we were able to collect data of 18 years of age or older. In hospital C, we could only collect data after July 1, 2012. Therefore, each hospital's data had different settings in measuring them. Men and women were accounted for similar percentage, and teens were the highest in the age group. J30.4, unspecified allergic rhinitis, was the highest in diagnosis of the allergic rhinitis. Most of the patients have received acupuncture treatments. Moreover, 74.6 percent of the total patients were prescribed with Chinese medicine. Conclusions : Based on this exploratory study, further studies were needed on clinical studies using data of systematic EMR.
