우리나라, 일본, 미국의 임상병리사 명칭은 의료체계, 교육제도, 업무영역에 따라 차이가 있다. 전통적으로 medical technologist (별칭 clinical laboratory technologist, medical laboratory technologist, clinical laboratory scientist, medical laboratory scientist)와 동등하거나 유사한 직종은 histotechnologist, cytotechnologist, genetic technologist이다. Extracorporeal technologist, medical electrophysiology technologist, medical sonographer는 medical technologist를 포함한 다른 보건의료인도 자격을 취득할 수 있는 직종이다. 임상병리사의 업무영역은 임상병리기술학, 병리기술학, 임상유전기술학, 핵의학기술학, 임상생리기술학으로 분류하였다. 새로운 전문임상병리사 종별은 CLMJ, CMAJ, ASCP, IAC, ABRET, ARMDS 등의 자격인정제도를 참고하였다. 현재 전문임상병리사 종별은 전문과목학회의 협조 하에 기존의 8종으로 혈액학, 수혈학, 화학, 면역학, 미생물학, 조직학, 세포학, 핵의학분야가 있으며 앞으로 미세질량분석, 감염관리, Pathologists' Assistant, 유세포분석, HLA, 세포유전학, 분자유전학, 심폐생리학, 신경생리학(뇌파/근전도/수면다원 등), 심장초음파학, 신경초음파학(뇌혈류/경동맥), 기기품질관리사 등 업무영역별로 20종으로 세분화할 수 있다.

본 연구의 목적은 임상병리사의 전공과 직업에 정체성을 부여할 수 있는 용어를 찾는 것이다. 임상병리학이라는 용어는 병리학의 모든 분야, 즉 해부병리학, 화학병리학, 혈액학, 미생물학 및 이들의 모든 하위 전문 분야를 포함하도록 의도되었다. 불행히도, 많은 나라에서 임상병리학이라는 용어는 해부병리학을 배제하는 것으로 생각되었는데, 이는 이름이 영어 이외의 언어로 번역되었을 때 문제가 복잡해졌다. 임상병리학(미국, 영국)은 의학의 한 전문 분야이다. 유사한 용어로는 검사의학(독일, 폴란드), 의료/임상생물학(프랑스, 네덜란드) 또는 임상분석학(스페인)이 있다. 의료기술은 개인, 회사 및 기관과 같이 질문하는 사람에 따라 약간 다르게 정의된다. 물론, 질문하는 언어에 따라 다르다. 의료기술은 임상검사기술학, 지원보건과학, 의료기기, 의생명공학, 보건기술로도 번역될 수 있다. 지원보건과학에서는 '임상병리기술학·병리기술학'이라는 용어를 사용하지 않는다. '보건계열 임상병리학 및 임상병리사'의 명칭은 '의생명검험과학·의생명검험사' 또는 '임상검사분석학·임상검사분석사'로 대체할 수 있다. 본 연구에서는 학문과 자격의 명칭을 '의생명'이 결합된 용어로 '의료생물학·의료생물사'로 변경할 것을 제안해 본다.

We investigated the latest trend of u-Health and relations with clinical technologist. Since 2000, Korea has become an aging society. Korea will become an aged society by 2018, and it is expected to enter the ultra-high aged society by 2026. Increase in over 65 years population means that the desire of medical service and care demand for the elderly is greatly required. In addition, many predicted Korean national health insurance would falter financially. Medical suppression policies and regulations are also amended continuously. U-Health based on the IT industry and development of related technologies and industries contains the variety concepts of telemedicine, telehealth, e-health, u-healthcare and POCT. Especially, the use of POCT supplied quick clinical examination is extending steadily in medical center and hospital, which will generate the field friction between nurses and medical technologists. In the transition situation from provider-centric service to consumer-driven health care system, this study recommended the principal role and correspondence of clinical laboratory workers and offered information about changes in healthcare market and the basic concept of expert system, measurement and the diagnosis principle to clinical technicians throughout the investigation of the recent research and government policy trends.

북한과 남한은 임상검사실 인력구조에서 차이가 있다. 북한에서는 남한의 '임상병리사'에 해당하는 유사인력을 '실험의사(검사의사)'라고 호칭한다. 본 연구자들은 탈북 보건의료인의 증언을 바탕으로 한 선행연구를 고찰해 보면, 검사의사(laboratory doctor)의 신분은 의사(physician)와 준의(feldsher, physician assistant)일 것이라고 사료된다. 의사와 준의는 5.5년제 의학대학 및 3년제 의학전문학교에서 양성되고 있다. 북한의 보건의료인력체계는 남한의 경우처럼 보건전문가들의 전문영역별 업무나 교육, 자격 및 법률이 세분화되어 있지 않다. 대한임상병리사협회는 한반도 통일을 대비하여 유관 단체와 함께 정책연구를 통해서 북한 임상검사인력에 대한 직제 전문화 구축과 발전 방향을 상호 모색해야 할 것으로 사료된다.

2000년대 병원경영전략의 변화로 조직구조가 계층제에서 팀제로 전환되었다. 계층제는 직무 중심, 직책연계, 수직적 관리가 특징이며 팀제는 직능 중심, 직책분리, 수평적 관리가 특징이다. 임상병리사 관리자 명칭은 1997년, 2007년, 2017년 세 차례에 걸쳐서 조사되었다. 수도권에 소재한 500병상 이상의 24개 병원에서 근무하는 직원들을 통해 확인되었다. 그 결과는 14개 병원에서 "팀장; 파트장" (59%), 7개 병원에서 "기사장; 수석기사" (29%), 3개 병원에서 "실장" (12%) 명칭을 사용 중에 있다. 본 저자들은 임상병리사가 현재 사용하고 있는 세 가지 직위나 직책명칭을 개선하기 위해 팀제를 기반으로 대안을 제안한다. 첫째, 기사장은 기사부장 또는 기사과장인지 지위가 불분명한 명칭이다. 기사장은 "팀장"으로 변경할 것을 제안한다. 둘째, 수석기사 또는 계장이 수간호사와 같은 직위 수준이라고 가정한다면, 수석기사 또는 계장을 파트장(유닛장)으로 변경할 것을 제안한다. 셋째, 직제규정은 계층제의 경우 검사의학과로만 표기되지만 팀제에서는 검사의학팀으로 표기된다. 임상병리사들은 팀제를 통해 소속감, 연대감, 친밀감을 더욱 갖게 된다.

The purpose of this study is to investigate various opinions on changes and revisions in the Medical Technician Act, to compare the curriculum of radiological technologist and physicians or dentists, and to compare the definitions and scope of work of radiological technologist in Korea and abroad. From the information, The goal is to review whether the phrase 'guidance of a doctor or dentist' specified in the definition of the 'Act on Medical Technicians, etc.' is realistically appropriate. radiological technologist receive specialized college education on radioligical science & medical imaging. The training hours for radiolgical science student are greater than medical students. In addition, radiological technologists are continuously developing their competencies for new knowledge and skills through continuing education in clinical fields. In particular, radiological technologist are making steady research efforts to reduce patient exposure and improve medical image quality. As a result of this investigation, it is considered that the term "guided by a doctor or dentist" as currently defined in the 'Act on Medical Technicians, etc.' may need to be revised in consideration of the professionalism of the radiological technologist.

The objectives of this study were to determine radiologic technologist image of some of the majored in department of radiology and collected data of 196 students were analyzed. The results obtained from this study are as follows 1. Department of radiology selective aspect appears the many have a positive opinion but now department satisfaction dropped 2. Department of radiology educational aspect appears the important things of clinical training programs and the national exam special lectures are high 3. Radiologic technologist's futural aspect appears high the many hope to get the university hospital for job plan after graduation and have a positive opinion 4. 임상병리사 명칭 변경과 관련한 의견을 수렴하기 위해 사단법인 대한임상병리사협회에서는 정회원으로 등록된 총 22,638명에게 온라인을 통해 제시된 명칭에서 2개를 선택하는 설문 답변을 요구하였으며 이 중 3,999명(17.66%)이 설문조사에 회신답변하였다. 명칭 선호도는 회원 1인당 2개를 선택하는 답변을 통해서 6,958건이 산출되었으며 그 중 5,555건(79.83%)가 '분석사' 레벨을 선호하였다. 명칭에 대한 조사결과는 진단검사분석사가 2,417건(34.73%)으로 가장 많았고, 임상검사분석사는 1,710건(24.57%), 의생명병리사는 758건(10.89%), 의생명분석사는 730건(10.49%), 의생명검사분석사는 730건(10.03%), 임상검사과학사는 646건(9.26%) 순으로 조사되었다. 따라서 조사된 회원들의 요구도, 자문, 문헌고찰, 표준직업분류, 주요대상국 현황을 기반으로 "진단검사분석사(Diagnostic Laboratory Analyst), 의생명검사분석사(Biomedical Laboratory Analyst), 의생명분석사(Biomedical Analyst)"를 명칭 정립으로 제언하고자 한다.
