임상치위생관리과정 수행 전·후에 따른 구강위생 상태 비교 (Comparison of oral hygiene status by clinical dental hygiene care performance)

  • Objectives : The purpose of this study was to investigate the oral hygiene status before and after the dental hygiene care performance to patients by the dental hygienists. Methods : Subjects were 55 adults visiting to dental clinics in Chungnam. The patients agreed to undergo a dental hygiene care performance. A skilled dental hygienist provided dental hygiene care performance including periodontal status, halitosis and oral hygiene status. Results : The periodontal pocket depth was compared before and after the performance. The depth dropped from 4.02 mm to 2.81 mm, and the value of halitosis dropped from 45.78 bbv to 35.76 bbv. The O'Leary index of the patients dropped form 49.37 to 32.84, and all the differences were statistically significant. Conclusions : Proper use of oral hygiene supplies and regular dental checkup can prevent periodontal diseases. This study will provide the useful information of the effective application of dental hygiene care performance.

치과위생사 업무에 따른 임상실습 만족도 연구 (A Study on the Satisfaction of Clinical Training According to Dental Hygienist Duties)

  • 본 연구는 치위생과 학생들의 임상실습에 대한 만족도와 주요 업무에 대한 만족도를 구체적으로 조사분석함으로써 보다 효율적인 임상실습 교육을 위한 기초자료로 활용하기 위하여 이루어졌다. 본 연구는 임상실습과정을 경험한 치위생과 23학년 재학생 427명을 대상으로 설문조사한 자료를 SPSS(Statistical Package for the Social Science) WIN 11.5로 분석하였고 t-검정, One-way ANOVA 그리고 ${\chi}^2$-검정을 실시하여 다음과 같은 결론을 얻었다. 임상실습 만족도는 3.34로 보통정도인 것으로 나타났으며, 실습내용에 대한 만족도와 임상실습 경험에 대한 업무별 만족도 역시 보통정도인 것으로 나타났다. 그리고 실습 내용의 전문성에 대한 만족도 중 실습 내용이 단순한 보조 업무라고 인식하는 학생이 44.9%로 높게 나타났다. 임상실습 기간에 대한 만족도는 비교적 적당하다고 인식하는 학생이 60%로 높게 나타났으며, 치과위생사 역할에 대한 만족도는 3.48로 높게 나타났다. 임상실습 만족도를 높이기 위하여 의료기관은 실습생들에게 단순 보조역할만 요구할 것이 아니라 학습한 이론을 실습현장에서 효과적으로 적용할 수 있도록 실습 환경을 조성해야 한다. 뿐만 아니라 교육기관과의 협조 하에 치과위생사의 업무분야를 구분한 체계적인 지도가 이루어지도록 하여 학생들이 임상실습을 통하여 실질적으로 임상에 대한 지식과 업무에 대한 자신감을 획득할 수 있도록 하여야 한다.

한국 치위생(학)과의 교육과정 개편 방향에 대한 요구도 (The demand for the change in Korean dental hygiene curriculum)

  • Objectives: The purpose of the study was to examined the demand for the change in Korean dental hygiene curriculum. This study compared the dental hygiene curriculum of domestic and overseas university and tired to provide the basic data for the development of standard curriculum that meets the education policy changes. Methods: Data were obtained from a cross-sectional survey of 1,192 participants including dental hygiene professors, dental hygienists and students. Data were collected by mail, telephone call from the professors and research assistants, email, and direct visit from June 10 to August 10, 2013. The questionnaire consisted of 4 questions of general characteristics of the subjects and 13 questions of dental hygiene curriculum. Results: The professors answered the development of integrated dental hygiene curriculum as the first priority(48.9%) and dental hygienists(51.6%) and students(42.6%) chose the development of new dental hygiene curriculum(p<0.001). In the analysis of validity of dental hygiene curriculum, the professors gave 3.29 to the validity of evaluation for basic job performance, and the clinical dental hygienists and the students gave 3.05 and 3.26 points, respectively(p<0.001). In relation to the necessity of the change of the dental hygiene curriculums, the professors gave 4.17 points which implied that curriculum change is the most important and necessary thing to do. The clinical dental hygienists and the students gave 3.90 and 3.47 points, respectively(p<0.001). Conclusions: The above-mentioned findings suggest that the revision of the current Korean dental hygiene curriculums is required. Therefore organizations related to dental hygiene including professor association and Korean Dental Hygienists Association should carefully rebuild the curriculum to improve the competency of dental hygienists. They should take the initiative in the development of a standardized curriculum.

치과병·의원에 근무하는 치과위생사의 업무 범위에 대한 질적 연구 (A qualitative research on work scope in dental healthcare hygienists)

  • Objectives: This qualitative research was conducted to clearly find and clinically understand dental hygienists' work experience in order to lay the legal foundation for their work range and safe execution. Methods: From March 1 to April 30, 2019, seven study participants were selected and participated in-depth interviews. Phenomenological research methodology was applied for intensive analysis. Results: They performed jobs that required no professionalism, such as dental assistance, except for scaling. Dental hygienists, nurses' aids, and dental technicians performed the same jobs regardless of difficulty level and legal work range. Dental hygienists failed to accurately recognize their legal work range. Since there was the wide gap between their legal work range and actual work range, it was necessary to expand the work range in line with actual conditions. Conclusions: Given the results, it is necessary to conduct in-depth discussions within the dental circle to make dental hygienists' work execution in line with reality by improving and discussing a legal system for job assignment and expansion of workers. Therefore, it is required to form a plan for the many jobs dental hygienists executed in a clinical setting.

노인치위생학 교육과정 개선에 대한 요구도 분석 (Analysis of educational needs for improving the geriatric dental hygiene curriculum)

  • Objectives: This study aimed to analyze the educational needs for geriatric dental hygiene and provide basic data for developing standard curricula for geriatric hygiene. Methods: To investigate the educational needs for geriatric dental hygiene, 212 students from dental hygiene departments and 205 dental hygienists engaged in clinical practice were enrolled. Results: Among the educational needs for geriatric dental hygiene, Geriatric oral health issues category was the highest with 4.06 points for students and 4.05 points for dental hygienists. Students who completed a geriatric dental hygiene course had higher scores for the following sub-domains compared to those who did not: need for geriatric dental hygiene-related theoretical education (p=0.002), needs for practice education with elderly people (p=0.001), and confidence in performing oral healthcare for elderly people after graduation (p<0.001). Meanwhile, clinical dental hygienists who not completed geriatric-related courses had higher scores for, need for geriatric dental hygiene-related theoretical education, need for practice education with elderly people, and confidence in performing oral healthcare for elderly people. but, there was no significant difference (p>0.05). Conclusions: The results of this study showed that students and dental hygienists had high educational needs for geriatric dental hygiene. In particular, there was a high demand for education related to Geriatric oral health issues as well as Geriatric dental hygiene care and skills.

일부 치위생(학)과 학생들의 치과위생사 국가고시 실기시험에서의 치주낭 측정 평가 필요성에 관한 연구 (Necessity of measuring the periodontal pocket depth according to dental hygiene studentsin the mock national board examination)

  • Background: This study was aimed at investigating the perception of the requirement of measuring the periodontal pocket depth of the dental hygiene students in the mock national board examination. Methods: SPSS 25.0 (IBM SPSS Statistics) was used to perform the independent t-test for the recognition level of dental hygiene students in the interdisciplinary practical national board examination. A p-value < 0.05 was set to indicate statistical significance. Results: Participants showed a high recognition level for the requirement of measuring the periodontal pocket depth without any difference among disciplines. When periodontal pocket depth measurement was included in the national board examination, the educational and clinical work of dental hygienists was expected to be unified, and the professionalism of dental hygienists was expected to be strengthened. Conclusion: The results of this study suggest that the feasibility of introducing periodontal pocket depth measurement in the practical national board examination for dental hygienists should be reviewed by a group of experts.

일부대학 치위생과 졸업생의 근무경력, 근무기관별 취업 시 고려사항 및 임상치위생계의 개선사항 (Considerations in work years and dental institution types among some dental hygiene graduates, and ways to improve the clinical dental hygiene society)

  • Objectives: The purpose of this study was to provide basic data on the current status and ways to improve the dental hygiene society by investigating working conditions, working experience, dental institution types among some dental hygienists, and ways to improve the dental hygiene society. Methods: This study conducted an online survey to 450 graduates of the department of dental hygiene in colleges in Gyeonggi, Jeolla and Gangwon Provinces. This study got some help in securing the list of graduates from the heads of the departments. SPSS WIN 23.0 was used to analyze the data. Results: To questions on what is the most important thing in considering to apply for jobs, the highest proportion (39.1%) of respondents designated employee welfare, followed by improvement of clinical dental hygiene (34.0%), of welfare related contents (26.0%), and salary (19.5%). In the relationship between employee welfare and dental institution type and between employee welfare and work experience, employee welfare tend to rise with the increase of work years. In the relationship between the director and staff members, it was found that as work years accumulate, the relationship tends to decrease. The dental clinic with no more than five staff members showed the best relationship between the leader and co-workers, while employee welfare was the highest in the other types of clinics, and the differences were statistically significant. Conclusions: Dental hygienists need to make various efforts to improve their statuses as professionals. In order to secure a part of professionalism, it is necessary for them to make more efforts to realize the health-care professionals being promoted by the Dental Hygienists Association of Korea.

치과위생사의 기대업무와 수행업무 격차가 이직의도와 직무만족도에 미치는 영향 (Effect of Gap between Expected Job and Performed Job on Turnover Intention and Job Satisfaction in Clinical Dental Hygienists)

  • 본 연구는 치과위생사의 기대업무 수준과 수행업무 수준의 차이가 직무만족도와 이직의도에 미치는 효과를 평가하기 위하여 의료기관에서 근무하는 치과위생사 2,655명을 편의추출하여 우편설문조사를 실시한 결과 612명이 회신하였다. 이 가운데 이직경험이 없는 치과위생사 295명을 대상으로 최종 분석하였다. 진료보조업무, 예방업무, 상담업무, 접수업무 등 네 가지 업무영역별로 기대업무 수준과 수행업무 수준의 격차를 측정하였다. 조사대상자의 일반적 특성, 사회인구학적 특성 및 직무관련 특성에 따른 기대업무와 수행업무 격차를 알아보기 위하여 독립표본 t 검정과 일원분산분석(ANOVA)를 수행하였다. 직무만족도에 영향을 미치는 요인과 이직의도에 영향을 미치는 요인을 알아보기 위하여 다중회귀분석을 실시하였다. 그 결과 다음과 같은 결론을 얻었다. 1. 연령과 근무경력에 따라 진료보조업무의 기대-수행업무 격차(p<0.01)와 접수업무의 기대-수행업무 격차(p<0.05)가 나타났다. 또한 연령, 근무경력, 학력에 따라 상담업무의 기대-수행업무 격차가 나타났다(p<0.01). 예방업무의 기대-수행업무 격차는 나타나지 않았다. 2. 직무만족도에 영향을 미치는 요인은 진료보조업무(${\beta}$=0.163, t=3.102, p=0.002), 급여만족도(${\beta}$=0.538, t=10.788, p=0.000), 월차유무(${\beta}$=0.115, t=2.244, p=0.026)로 나타났다. 모형의 결정계수($R^2$)는 0.608로써 통계적으로 유의하였다(F=12.162, p=0.000). 3. 이직의도에 영향을 미치는 요인은 자녀유무(${\beta}$=-0.254, t=-2.711, p=0.007), 진료보조업무 격차(${\beta}$=-0.186, t=-3.162, p=0.002), 급여만족도(${\beta}$=-0.313, t=-5.623, p=0.000)로 나타났다. 모형의 결정계수($R^2$)는 0.466으로써 유의한 것으로 나타났다(F=5.749, p=0.000). 결론적으로, 급여만족도와 진료보조업무격차가 이직의도와 직무만족도에 영향을 미치고 있으며, 월차유무는 직무만족도에 영향을 미치나 이직의도에는 영향을 미치지 않았다.

Clinical Dental Hygienists' Experience of the Prevention Based Incremental Oral Health Care: Applying Focus Group Interviews

  • Background: In this study, we tried to comprehensively explore clinical dental hygienist's experience of a prevention-based incremental oral health care program, which was pilot-operated by dental clinics, define prevention-based incremental oral health care as experienced in the field, and identify factors to be considered. Methods: This study conducted a focus group interview with five dental hygienists who participated in an ongoing oral management pilot project in 2016. The interview was conducted by a researcher, and the co-research team attended as progress assistants and recorded characteristics of the participants, main dictations, and non-verbal characteristics. All interviews were recorded and underwent thematic analysis to examine the questions of the study as the main axis. Results: As a result of the study, 65 meaningful statements were extracted by code, integrated into 24 sub-categories, and structured into 11 categories. Finally, four keywords were drawn: characteristics, facilitating factors, conflicting factors, and improvement measures for prevention-based incremental oral health care. Regarding prevention-based incremental oral health care in dental clinics, dental hygienists were highly aware of the physical and mental burdens of personalized treatment and education for each individual. They were responsible for the patient and for facilitating changes in the behavior of the client, leading to professional satisfaction. The dental team's cooperation and supportive attitude were found essential to continue oral health care in the dental clinic. Conclusion: Through dental team-based treatment philosophy sharing and collaboration, it is possible to provide prevention-based incremental oral health care in dental clinics. In future, it is necessary to develop a system for establishing a sustainable preventative management system for public health promotion.

치위생과정에 근거를 둔 임상치위생학의 교육 실태 (Current Status of Clinical Dental Hygiene Education Based on Dental Hygiene Process of Care)

  • 이 연구는 치위생과정에 근거한 임상치위생학 교육과정을 도입하기 위하여 현재 운영 중인 임상치위생학의 교육의 실태를 조사한 연구이다. 50개 대학의 (학)과장을 대상으로 구조화된 설문지를 이용하여 면접조사 및 전자우편 조사를 실시하였으며, 그 결과는 다음과 같다. 1. 설문에 응답한 대학의 53.1%는 1999년 이전 (1977-1999년)에 개설되었고, 46.9%는 2000년 이후(2000-2006년)에 개설되었다 학제는 3년제 치위생과가 28개교(87.5%)이었고 4년제 치위생학과가 4개교(12.5%)이었다. 입학정원은 40명 이하(37.4%)가 가장 많았다. 2. 임상치위생학 실습에서 교수 한 명이 지도하는 학생의 수는 평균 22명이었고, 응답 대학의 62.5%가 포괄치위생과정에 관한 교육을 하고 있었다. 실습 시 평균적으로 한 명의 환자가 실습실을 내원하는 횟수는 특별히 정해놓지 않는다(32.3%)가 가장 많았고, 그 다음으로 1회 (29.0%), 3회(16.1%), 2회 (12.9%), 4회(9.7%) 순이었다. 3. 치위생과정의 다섯 단계에 관한 강의여부에서 사정과 수행단계는 대부분의 대학에서 실시하고 있었으며, 치위생판단(68.8%), 계획수립 (65.6%), 평가(68.8%)는 비교적 낮았다. 4. 실제로 강의 시간에 치위생과정의 개념에 관한 교육이 있는지 조사한 결과, 56.7%의 대학에서 강의가 이루어지고 있었다. 강의에서 사용하는 주 교재는 'Clinical practice of the dental hygienist (Wilkins) 또는 임상치위생학 (김숙향 역)'이 40.0%로 가장 많았다. 이를 종합해 볼 때, 치위생과정을 임상치위생학 교육과정에 도입하기 위해서는 치위생판단올 위한 진단 모형, 계획수립의 절차, 평가 방법에 관한 교육을 보강해야 할 것이다.
