Background: We aimed to investigate the prevalence of unrecognized depression in patients with chronic pain, but with no history of psychiatric diseases. Methods: Patients with chronic pain who did not have a history of psychiatric disease were selected for this study. The Beck Depression Index (BDI) was used to evaluate depression. Participants' socio-demographic characteristics and pain-related characteristics were also recorded. Results: The study included 94 consecutive patients with chronic pain (28 men and 66 women). Based on the BDI scores, 33/94 (35.1%) patients with chronic pain had comorbid depression. The prevalence of depression was significantly higher in our cohort than it was in the general population (P < 0.001). The standardized incidence ratio, adjusted for age and sex, was 2.77 in men and 2.60 in women. Patients who were unmarried (odds ratio [OR] = 3.714, P = 0.044), and who had subjective sleep disturbance (OR = 8.885, P < 0.001), were more likely to have moderate to severe depression. Patients with high education levels (OR = 0.244, P = 0.016), and who were economically active (OR = 0.284, P = 0.023), were less likely to have moderate to severe depression. Conclusions: Our results indicate that unrecognized depression in patients with chronic pain is common. Therefore, pain physicians should actively seek to identify these problems rather than relying on the patient to volunteer such information.
Purpose: this study was to examine the differences of the level of pain, depression and self-efficacy according to the classifications of pain among chronic pain patients. Method: The data were collected by means of self-reported questionnaire from 164 patients with chronic pain visited in one university hospital and one local pain clinic in Busan, from October 7 to November 16, 2002. Analysis was done by ANOVA, and Scheffe test using SPSS program. Result: The subjects were divided into five classifications of chronic pain : 26.2% low back and extremity pain, 23.2% neck, shoulder and upper extremity pain, 19.5% postherpetic neuralgia, 15.9% complex regional pain syndrome and 15.2% peripheral neuralgia. There were significant differences in pain (p=.000), depression (p=.000) and self-efficacy (p=.003) according to the 5 kinds of chronic pain. With the results of the Scheffe test, the patients with peripheral neuralgia experienced pain and depression higher than those with the other kinds of chronic pain. The patients with neck, shoulder and upper extremity pain experienced self-efficacy higher than those with peripheral neuralgia. Conclusion: Chronic pain patients should be provided effective individualized intervention depending on the classifications of chronic pain. Therefore the development of interventions for pain management according to the classifications of pain in chronic pain patients is needed.
Purpose: The purpose of this study was to compare the effects of scrambler and transcutaneous electrical nerve stimulation therapy on pain, functional disability, and depression in patients with chronic low back pain. Methods: Twenty patients with chronic stroke were assigned randomly to an experimental (n=10) or control (n=10) group. The experimental group performed scrambler therapy. The control group performed electrical nerve stimulation therapy. Training was conducted once a day for 30 minutes, five days per week, for three weeks. The pain was measured using the numeric rating scale. Functional disability was measured using the Roland-Morris disability questionnaire. Depression was measured using the Beck depression inventory. Results: As a result of comparison between the groups, the experimental and control groups showed significant difference for pain, functional disability and depression after the experiment (p<0.05). In a comparison between the two groups, the experimental group, in which scrambler therapy was applied, showed a more significant reduction in pain, functional disability and depression than the control group (p<0.05). Conclusion: Based on these results, scrambler therapy shows positive effects on pain, functional disability, and depression in patients with chronic low back pain.

Purpose: This study was done to investigate the relationship among the variables, chronic pain, pain coping, fatigue, self-esteem, and depression in elders. Method: Data were collected by self-reported questionnaires from 270 older adults. Data analysis was done with SPSS 10.1 for descriptive statistics and a PC LISREL program for covariance structural analysis. Results: According to modified model, chronic pain was found to have a significant direct and total effect on pain coping. Chronic pain and pain coping were found to have a significant direct and total effect on fatigue. Chronic pain, pain coping and fatigue were found to have a significant direct and total effect on self-esteem. Chronic pain, pain coping, and self-esteem were found to have a significant direct effect on depression. Conclusion: This modified model is considered appropriate for explaining the relationship among chronic pain, pain coping, fatigue, self-esteem, and depression in elders. Also, the findings support the development of an intervention strategy to relieve chronic pain in elders.

Purpose: The study compared the muscle activity and ratio of upper trapezius (UT) to lower trapezius (LT) activity between acute and chronic whiplash-associated disorder (WAD) patients. Methods: Twelve healthy (male: 7), 14 acute WAD (male: 7), and 11 chronic WAD (male: 3) volunteers participated in this study. Electromyography using a surface EMG recorded the activity of the upper trapezius and lower trapezius of both shoulders (dominant and non-dominant) during $120^{\circ}$ elevation when standing and shoulder depression when sitting. The testing order was selected randomly. Subjects were asked to maintain each experimental position for 5 seconds at end range. EMG activity was normalized using the maximal voluntary isometric contraction (MVIC) elicited using a manual muscle-testing technique. One-way repeated measures analysis of variance (ANOVA) was used to compare the average root mean square (RMS) value of EMG activity for each condition. Results: The EMG activity of the dominant UT for chronic WAD subjects was significantly higher than for acute WAD subjects during $120^{\circ}$ elevation (p<.05). The EMG activity of the dominant LT for acute WAD subjects was significantly lower than for the control group (p<.05) during $120^{\circ}$ elevation. The EMG activity of the dominant LT for WAD patients was significantly lower than for the control group during shoulder depression (p<.05), and the chronic WAD patients scored significantly the lowest (p<.01). The dominant UT/LT for chronic WAD patients was significantly higher than the ratio for acute WAD subjects during shoulder depression (p<.05); this result was higher than for the control group (p<.01). The non-dominant UT/LT (ratio) for chronic WAD patients was significantly higher than the ratio for acute WAD subjects during shoulder depression (p<.05). Conclusion: The UT for chronic WAD subjects was hyperactive when compared to the acute WAD subjects, and the was hypoactive for both acute and chronic patients, therefore intra-trapezius imbalance was more prevalent during shoulder depression.
Purpose: The aim of this study was to investigate the effects of chronic musculoskeletal pain and depression on health-related quality of life (HRQoL) according to gender in community-dwelling older adults. Methods: The subjects of this study were 209 elderly individuals who were receiving visiting nursing services from a public health center located in Gangwon-do. Data were collected using a structured questionnaire from March to April, 2008. SPSS/WIN 13.0 was used for data analysis. Results: Depression and HRQoL showed a significant difference between male and female subjects. There was a negative correlation between chronic musculoskeletal pain, depression, and HRQoL. In the male elderly, depression was the most significant predictor of HRQoL, while in the female elderly, chronic musculoskeletal pain was the most significant predictor of HRQoL. Conclusion: The study showed that chronic musculoskeletal pain was the variable with the highest explanatory power for HRQoL in the female elderly. Therefore, chronic musculoskeletal pain needs to be assessed and managed first in nursing interventions to improve HRQoL of the female elderly.
Objectives : The purpose of this study was to investigate the depressive change and predictors of chronic illnesses and to examine whether depressive change predictors are different according to chronic disease types. Methods : Data from the 1st to the 10th year of the Korean Welfare Panel of 5,023 adults over 20 years of age who were treated for chronic diseases for more than three months were examined. We assessed demographic, sociological, and socioeconomic factors including depression predictors of chronically ill patients, and analyzed the data using latent growth modeling and multiple group analysis. Results : Depression symptoms of chronically ill patients gradually decreased, showing a linear relationship across time. In addition, multiple group analysis was performed for each type of chronic disease. Conclusions : We described the phenomenon of depression in chronically ill patients, and discussed the practical and policy interventions required to improve depression in chronically ill patients.

The etiology of most psychiatric disorders is still incompletely understood. However, growing evidence suggests that stress is a potent environmental risk factor for depression and anxiety. In rodents, various stress paradigms have been developed, but psychosocial stress paradigms have received more attention than non-social stress paradigms because psychosocial stress is more prevalent in humans. Interestingly, some recent studies suggest that chronic psychosocial stress and social isolation affects mainly anxiety-related behaviors in mice. However, it is unclear whether chronic non-social stress induces both depression- and anxiety-related phenotypes or induces one specific phenotype in mice. In the present study, we examined the behavioral consequences of three chronic non-social stress paradigms: chronic predictable (restraint) stress (CPS), chronic unpredictable stress (CUS), and repeated corticosterone-HBC complex injection (RCI). Each of the three paradigms induced mild to severe depression/despair-like behaviors in mice and resulted in increased immobility in a tail suspension test. However, anxiety-related phenotypes, thigmotaxis and explorative behaviors, were not changed by the three paradigms. These results suggest that depression- and anxiety-related phenotypes can be dissociated in mouse stress models and that social and non-social stressors might affect brain circuits and behaviors differently.

PURPOSE: This study was conducted to identify the relationships among stress response inventory, hospital anxiety and depression, muscle tone and stiffness, and hand strength in chronic stroke patients. METHODS: A total of 14 chronic stroke patients voluntarily agreed to this experiment and were included in this study. All measurements were performed in one day and in a room without noise. The tests conducted in this study were as follows: muscle tone and stiffness of the upper trapezius hand grip measurement. Subjects were also asked to complete surveys describing the following: stress response inventory and hospital anxiety and depression scale. RESULTS: There were significant correlations among stress response inventory and hospital anxiety and depression, stress response inventory and hand strength, and hospital anxiety and depression and hand strength (P<.05). There were high positive correlations between stress response inventory and hospital anxiety and depression (r=.979), while there were moderate negative correlations between stress response inventory and hand strength (r=-.415) and between hospital anxiety and depression and hand strength (r=-.420). CONCLUSION: The results of the present study indicate that there is a relationship among stress response inventory, hospital anxiety and depression, and hand strength in patients with chronic stroke.
While depression is certainly a prevalent disorder, it is often severe and debilitating and does not always have the good prognosis we have been led to expect. Social approaches to affective disorders have not been subjected to the same level of scrutiny as the interventions used in the management of schizophrenia. Psychosocial Rehabilitation is now at a critical stage. Psychoeducation, social skill training, cognitive remediation, family education, vocational rehabilitation and case management programs are essential for the rehabilitation of chronic depression.
