보행운동프로그램이 청소년 경직형뇌성마비아동의 자아효능감 및 부모의 양육태도에 미치는 영향 - 사례연구 (Effect of Gait Exercise Program on the Self-Efficacy and Parenting Attitude of Adolescent Spastic Cerebral Palsy - Case Study)

  • Purpose : The purpose of this study was to investigate the effect of increased gait function enhancement exercise in three adolescent convulsive cerebral palsy sinuses on children's large movement function, balance and gait function, self-efficacy, and guardian attitude. Methods : The purpose of this study was to conduct a program to strengthen trunk muscles and strengthen walking ability 5 times a week for 8 weeks in 3 children with convulsive cerebral palsy in adolescence. The main reinforcement of the program was 20 minutes of muscle strength and 20 minutes of walking on a treadmill five times a week. Exercises were focused on the reinforcement of the flexor muscles of the proprioceptive neuromuscular promotion (PNF) and the extension of the legs in the bridge posture exercise and squat movements. Results : The results of GMFM-88 to determine the effects of this functional enhancement program on the body of children with convulsive cerebral palsy, and PBS & TUG to determine the effects of balance and gait ability were improved. In addition, parenting attitudes of guardians, self-efficacy of children, and self-efficacy of parents increased positively due to mental consequences. Conclusion : The results of GMFM-88 to determine the effects of this functional enhancement program on the body of children with convulsive cerebral palsy, and PBS & TUG to determine the effects of balance and gait ability were improved. In addition, parenting attitudes of guardians, self-efficacy of children, and self-efficacy of parents increased positively due to mental consequences.

기능적인 저항훈련이 운동이상형 뇌성마비 아동의 대동작과 균형능력에 미치는 영향 : 단일사례설계 (Effects of Functional Resistance Training on Gross Motor and Balance Abilities in Children with Dyskinetic Cerebral Palsy : Single Case Design)

  • Purpose : The purpose of this research is to find clinical effects of functional resistance training using weighted vest on gross motor and balance abilities of children with dyskinetic cerebral palsy. Methods : This study selects 3 subjects for 8~12 years old who were diagnosed with children with dyskinetic cerebral palsy. The Design is ABA design of single-subject research design. Baseline(A) and TypeII Baseline(A : 12weeks) phases were received with NDT treatment, Intervention(B : 12weeks) phase provided with 40 minute functional resistance training using weighted vest in a session twice a week. In order to analyze the measure results of gross motor function and performance, balance abilities in children dyskinetic cerebral palsy during baseline, intervention and typeII baseline phase. Result : A statistically significant differences in the total GMFM including walking/running/jumping during baseline, intervention, typeII baseline, but no significant differences in the lying/rolling, sitting, crawling/kneeling and standing. A statistically significant differences in the total GMPM including dissociated movement, coordination, weight shift, stability during baseline, intervention, typeII baseline, but no significant differences in the body alignment domains. A statistically significant differences in the length and surface area ellipse of center of pressure during baseline, intervention, typeII baseline. Conclusion : The intervention method to facilitate multi-joint and closed kinematic chain movement equipped weighted vest applied functional resistance training on children with dyskinetic cerebral palsy effectively improve on gross motor function and performance, balance abilities.

경직형 뇌성마비 아동의 조음능력 관련 변인 연구 (A Study of Articulation Abilities and Feeding Functions on the Children With Spastic Cerebral Palsy)

  • 목적 : 본 연구의 목적은 경직형 뇌성마비 아동들의 조음과 관련이 있는 변인들 중에서 연령, 운동기능 정도 및 섭식기능과 조음능력 간 변인에 대해 연구하는데 있다. 연구 방법 : 본 연구는 서울, 경기, 대구, 대전 및 부산 등에 거주하면서 재활의학과 전문의에 의해서 경직형 뇌성마비로 진단을 받은 2세 2개월~7세 8개월(평균 4세 7개월) 범위의 남아 34명과 여아 27명을 대상으로 하였다. 이때 조음능력인 구어 명료도, 교호운동 속도 및 교호운동 명료도를 종속변인으로 하고, 연령, 운동기능 정도 및 섭식 재질과 방법에 따른 구어 및 비구어 기제 기능을 독립변인으로 하여 중다회귀분석 실시하였다. 연구 결과 : 본 연구의 결과 경직형 뇌성마비 아동들의 조음능력 중 구어 명료도는 섭식 방법 측면에서 컵으로 먹기와 숟가락으로 먹기 동안의 비구어 기제 기능에서 유의하였다. 교호운동 속도는 운동기능 정도와 섭식 재질 측면에서 죽으로 먹기 동안의 구어 기제 기능과 섭식 방법 측면에서 컵으로 먹기 동안의 비구어 기제 기능이 유의한 결과를 보였다. 또한, 교호운동 명료도는 섭식 재질 측면에서 죽으로 먹기 동안의 구어 기제 기능과 섭식 방법 측면에서 컵으로 먹기 동안의 비구어 기제 기능에서 유의한 결과를 나타내었다. 결론 : 본 연구 결과, 경직형 뇌성마비 아동들의 조음능력 관련 변인 중 섭식기능에 따른 구어 및 비구어 기제 기능과 운동기능 정도를 고려한 섭식기능 평가와 치료가 유용할 것으로 여겨진다. 또한, 후속 연구로 뇌성마비 아동에게 적합한 섭식기능 증진 프로그램을 개발하고 적용한 후의 조음능력 개선에 대한 연구가 이루어진다면 매우 유용할 것이라고 사료된다.

전방 보행기와 후방 보행기가 뇌성마비아동의 보행 특성과 신체 정렬상태에 미치는 영향 (The Effects of Anterior Walker and Posterior Walker on Gait Parameters and Body Alignment of Children With Cerebral Palsy)

  • The purpose of this study was to compare the effects of anterior walker and posterior walker on gait parameters and body alignment of children with cerebral palsy. The intraclass correlation coefficient was .99 for intertester reliability. Intratester reliability was between .96 and .99. The use of posterior walker increased gait velocity and facilitated more upright posture. The measurement of joint angle program was found to be reliable to measure range of motion. This study has a limitation of generalizing the results to all children with cerebral palsy. Research is required to investigate the effect of posterior walker on energy efficiency.

섭식장애가 있는 뇌성마비아동의 사회성 증진을 위한 구강 감각 놀이 치료프로그램의 효과연구 (A Study on the Effect of Oral Sense Play Treatment Program on the Social Enhancement in Dysphagic Children with Cerebral Palsy)

  • Purpose : In this study, we would like to apply oral sensory play therapy program to children with cerebral palsy who have sensitive oral senses to find out the effect on oral sensory ability, oral function, performance ability and sociality. Methods : The subjects of this study were 20 children diagnosed with cerebral palsy, and 10 experimental groups and 10 control groups. Oral sensory play therapy programs, including oral sensory programs, were conducted in the experimental group, while only oral sensory programs were conducted in the control group. Oral Sensory Treatment Level was used to identify the sensory profile and the overall oral function of the oral function was used, and the performance and satisfaction of Canadian occupational performance measure (COPM) were examined to find out the performance of the work, and social skills were examined for social skills (SMS). Results : The level of oral sensory treatment, overall oral function, work performance and sociality all increased significantly in the experimental group, but the oral sense and oral function in the control group were significantly increased, but not significantly in the case of work performance and sociality. Conclusion : Through this study, we confirmed that oral sensory play therapy program is an effective arbitration method for oral sensation, oral function, work performance and social enhancement of children with cerebral palsy. Based on the results of this study, the study of arbitration protocols for children with cerebral palsy with oral sensory problems will have to be more active.

장애아동 어머니가 인지하는 스트레스와 생활만족도의 영향요인분석 -뇌성마비 중심으로- (The Perceived Stress and Life-Satisfaction among Mothers of Children with Disability - with focus on the Cerebral Palsy -)

  • Purpose : The purpose of this study was to examine the levels and influencing factors of the perceived stress and life-satisfaction among mothers of children with disability. Methods : 55 mothers were selected for this study and the self-reporting method to the structured questionnaire were used to collect the data. The data were analyzed with the frequency, t-test, one-way ANOVA, Duncan test using SPSS 14.0 for window programs. Results : Firstly, the level of total perceived stress of the mothers of children with cerebral palsy was 44.28/75, and the average of the items of the perceived stress was 2.95/5. The highest level of subcategory of the perceived stress, 10.82/15, was in parent role stress, and the lowest level of subcategory of the perceived stress, 3.88/10, was in children developmental stress. There were statistically significant differences in the mother's level of stress according to their ages, employment, occupation, and daily activity time and sleeping time. Secondly, the level of life-satisfaction was 3.17/7, and statistically significant difference was found by the treatment cost. Conclusions : We suggest that these variables need to be considered when the stress intervention programs for the mothers of children with cerebral palsy are developed.

Parenting experiences among fathers of prematurely-born children with cerebral palsy in South Korea

  • Purpose: The symptoms and impairments caused by cerebral palsy usually require long-term treatment, resulting in a substantial burden on the family of affected children. This study explored the experiences of fathers with prematurely-born children with cerebral palsy, with a focus on how such experiences influenced their families. Methods: A qualitative case study method was used. Nine subjects were recruited from April 2018 to June 2019 at one hospital, and each was interviewed three times by a neonatal nurse. Results: Five core experiences of fathers were identified: "regret for an insufficient initial response", "confronting my child born as a premature baby", "the position of being a dad who can't do anything", "the process of treatment like a tunnel with no exit", and "a father's getting meaning in life through children". These stories covered an individual's timeline and family interactions. Conclusion: Our findings suggest that fathers of prematurely-born children tend to suppress their emotions; therefore, a novel intervention program to encourage fathers' emotional expression and to support healthier interactions with their families is needed. Moreover, our findings could contribute basic information for the construction of a community-based support system to aid families, including prematurely-born children and other persons with impairments.

The Effects of Insoles for Postural Correction on Spatial-temporal Changes of Gait in Spastic Cerebral Palsy Children

  • Improvement in functional gait is one of treatment goals in treatment of cerebral palsy children. This study intended to examine the effects of insoles for postural correction on gait in spastic cerebral palsy patients by investigating changes in gait temporal spatial parameters. As the subjects, 15 spastic bilateral cerebral palsy patients participated in this study. Temporal spatial parameters of gait were measured using GAITRite system under three gait conditions. Bare foot gait, gait in shoes, and gait in insoles for postural correction were conducted. In order to look at differences in temporal spatial parameters according to three gait conditions, repeated one way analysis of variance was conducted. As post hoc test, Bonferroni was conducted. A significant level was set at ${\alpha}=.05$. According to the result of this study, gait velocity, cadence, step length, stride length of the left lower extremity significantly changed. When the subjects put on customized insoles for postural correction, the effect was greatest. There were no significant changes in stance time, single support time, double support time, swing % of gait, and stance % of cycle. Therefore, gait with insoles for postural correction positively influenced functional gait improvement and will be able to be usefully employed for spastic cerebral palsy children as one of gait assistance devices.

  • Consideration for therapy method and oral motor function character of children with cerebral palsy. Therapists who treat for feeding disorder children owing the regression of oral motor function are necessary to gain knowledge about dysfunction of sensing, perception and cognition with baffling to eat and inhibition of primitive reflex, oral anatomy and function, and motor control (trunk, head, positioning of the upper limbs and the lower limbs and muscle tone). Oral motor function program is a comprehensive rehabilitation program which requires systematized enforcement and collaborated attempts to physiotherapists, occupational therapists, speech therapists, psychotherapists. Especially, physical therapists are not accustomed to oral motor program, hoping to provide diffusely and apply new therapy approach method for many areas (bell's palsy, respiratory failure, speech articulation). It will comprise to study owing to holistic approach with clinic.

The Comparison of 'Knowledge of Result' and 'Knowledge of Performance' in the Children with Cerebral Palsy

  • Purpose: The purpose of this study was to compare the effect of 'knowledge of result' and 'knowledge of performance', two types of extrinsic feedbacks, during the sit-to-stand movement in children with hemiplegic cerebral palsy. Methods: A total of ten children with hemiplegic cerebral palsy (ages 8 to 12 years) were recruited for the study. Subjects with hemiplegic cerebral palsy performed sit-to-stand movement in front of a mirror. Their performance was supervised and revised for normal movement by a pediatric physiotherapist. In the knowledge of the result, subjects performed sit-to-stand using a chair with an armrest in their mind with normal movement. In the knowledge of performance, subjects performed sit-to-stand under verbal instructions. Randomized cross over trials were used in this study. Main outcome measurements were as follows: mediolateral speed, anteroposterior speed, velocity moment, extent in mediolateral direction, extent in anteroposterior direction, and vertical distance of the center of pressure. Results: The mediolateral speed and extent of center of pressure was higher for 'knowledge of performance' in comparison with the other type of extrinsic feedbacks (p<0.05). The other parameters, including anteroposterior speed and extent, and vertical speed of the center of pressure, did not differ between the two types of extrinsic feedbacks (p>0.05). Conclusion: These findings suggested that training in sit-to-stand movement with 'knowledge of result' may result in better use of extrinsic feedback.
