Rituximab (RTX) is a chimeric monoclonal antibody that inhibits CD20-mediated B-cell proliferation and differentiation. Several studies have examined its use in intractable nephrotic syndrome (NS) with some positive results. However, those studies examined such effects for a short-term period of 1 year, and some patients continued to relapse after a lapse in RTX treatment. Our use of RTX as a maintenance therapy (RTX injection when the CD19 cell count exceeded $100-200/{\mu}L$ before relapse) showed some noticeable efficacy. We used RTX in 19 patients with steroid-dependent NS (SDNS). In 12 patients treated with RTX maintenance therapy, only one relapse occurred. The mean treatment period was $23.4{\pm}12.7months$, and the mean number of RTX administrations was $3.9{\pm}1.6$. The relapse rates were decreased (from 2.68/year to 0.04/year), and the drug-free period also increased (from 22.5 days/year to 357.1 days/year) during maintenance therapy. The other seven patients were treated with one cycle of RTX or additional cycles in case of relapse (non-maintenance therapy). Relapse rates were significantly decreased after RTX treatment (from 1.76/year to 0.96/year, P=0.017). The relapse-free period was $15.55{\pm}7.38$ (range, 5.3-30.7) months. No severe side effects of RTX were found except for a hypersensitivity reaction such as fever and chills during its infusion. In conclusion, RTX is considered an effective and safe option to reduce the relapse rate by a single- or maintenance-interval therapy in SDNS.

Harlequin ichthyosis (HI, OMIM #242500) is one of the most severe skin diseases among the autosomal recessive congenital ichthyoses, with high morbidity and mortality, particularly in newborns. Clinically, it is characterized by a typical appearance of generalized, thick, yellowish, hyperkeratotic plates with deep erythematous fissures on the skin. Herein, we present the case of a newborn girl with HI that was genetically confirmed by targeted gene panel analysis. The premature baby was encased in an opaque white membrane with erosion covering the skin of the entire body except the lips, with her hands and feet restricted by the membrane. Humidification, emollient, and retinoic acid treatment were started; the thick ichthyosis gradually peeled off and the underlying skin was only covered with thin scales. Targeted gene panel analysis using next-generation sequencing and validation with Sanger sequencing and quantitative polymerase chain reaction analyses confirmed compound heterozygous mutations of the ABCA12 gene (p.N1380S and a partial gene deletion encompassing exon 9). The parents were carriers for each of the identified mutations. Early recognition of the genetic etiology of congenital ichthyosis can, thus, facilitate genetic counseling for patients and their families.
Purpose. A natural disaster negatively affects children's emotional and behavioral adjustment. The purpose of this paper was to examine the prevalence, symptoms, and correlates of PTSD after the occurrence of Typhoon Rusa. Method. 261 elementary school children living in Kimcheon, which was a devastated rural area in South Korea by Typhoon Rusa, were selected. Data were collected 4 months after the disaster using the PTSD Reaction Index categories recommended by Frederick, severity of PTSD. Results. 12.3% of the children had either moderate or severe PTSD symptoms; 22.7% reported mild symptoms; and the remaining 65% had sub-clinical symptoms of PTSD. The most frequent symptom was recurrenct fear(67.0%). 13% to 17.2% of children exhibited difficulty in concentration, sleep disturbance, and guilt feeling. The regression model of severity of PTSD was composed of the level of exposure to traumatic experiences, grade in school, gender, negative coping style, and social support, and explained 34.3% for PTSD symptoms. Exposure to traumatic experiences was the strongest factor of all predictors. Conclusion. Emotional support from friends and coping style were correlated with PTSD severity. School-based interventions that emphasizes coping with disaster related problems and problem-solving may prove to be useful, and may aid in building close and supportive ties with teachers, classmates, and friends.
Although listening to music is the most basic musical experience that is ahead of all the other musical activities, it tends to be neglected due to lack of awareness and difficult teaching methods. This study is to compose integrative music appreciation by reflecting thoughts of children, draw children's attention to listening to music, and let them to discover and create musical concept and structures by themselves, accompanied by related activities through which children can express their thoughts and feelings with children's poem. Considering the peculiarities of three-four year-olds, it may be difficult for them to create poems individually, so we let them to write poems through small group discussion in which they can share their opinions and observe their peers' reaction. We provided a teaching method to teachers who find activities of listening to music and writing poems difficult, then we analyzed the effect. When children finished writing poems in small groups through integrative music appreciation reflecting their thoughts, they placed greater weight on preparing to listen to music at first, but going through the program, they discovered musical concepts and became active in music appreciation. In the related activity, writing poems inspired them to think creatively, listening to their peer's stories. Even children who were not interested in children's poem showed higher participation. Teachers found children's creative words to be interesting, discovering the joy of creation.
Objective: This study analyzed mother-child interactions in a stressful situation each second by mother's emotional regulation level. Methods: The study was conducted with 16 mothers and their 5-year-old children playing a teaching task for 15 min. During the interactions, the participants were videotaped and examined. Furthermore, qualitative analysis was used for analyzing mother-child interactions in detail by creating a situation that maximizes the stress and frustration of the mother and child. Results: The results showed that maternal humor and affection were significantly related to child positive emotion and that maternal coaching closely correlated with the child pride, pleasure, and whining. Additionally, maternal intrusive behavior showed a positive correlation with child anger. Lastly, mothers with higher levels of emotional regulation more often expressed affection to their children. They were more actively involved in the tasks and used fewer positive or negative directive expressions. Therefore, children of this group expressed more positive emotions. Conclusion: These findings suggests that programs improving parental emotional reaction and emotion regulation should be developed.
With the exhibition materials and exhibition spaces are provided, it is the reaction of the visitors that completes the function of science museum. To satisfy the visitors, it is necessary to understand their needs, interests, and motives, apart from the physical environment of the museum. The purpose of this research is to verify the effect of children's personal characteristics on the satisfaction in science museum. In doing so, an interview survey was conducted: 210 primary and middle school students who had finished the exhibition tour were randomly selected and interviewed. The data was analysed by a T-test and an one-way ANOVA to find out the correlation. The results of analysis are as follows: First, a meaningful statistical differences are found by the age groups in terms of motivation, expectation, satisfaction, enjoyment, and total staying time at the museum. The lower the grade, students are satisfied with their everyday living; and this is reflected on their intrinsic motivations and higher expectations on visit. Also, their actual satisfaction and enjoyment are higher and they stay at the museum longer than the other group. Second, the group with intrinsic motivation and higher expectation is more satisfied with the contents of the exhibition than the group with extrinsic motivation. Third, the level of higher education of mothers, rather than that of fathers had mere effect on the learning of the children at the museum. Also, the children who are brought up by full-time house wives have bigger achievement of learning from the exhibition than those with working mothers.
The purpose of this study has placed its meaning in the use as the basic material for the research of the person's facial expressions, by researching and analyzing the visual reactions of recognition of children according to the facial expressions of emotion and by surveying the verbal reactions of boys and girls according to the individual expressions of emotion. The subjects of this study were 108 children at the age of 6 - 8 (55 males, 53 females) who were able to understand the presented research tool, and the response survey conducted twice were used in the method of data collection by individual interviews and self administered questionnaires. The research tool using in the questionnaires were classified into 6 types of joy, sadness, anger, surprise, disgust, and fear which could derive the specific and accurate responses. Regarding children's visual reactions of recognition, both of boys and girls showed the high frequency in the facial expressions of joy, sadness, anger, surprise, and the low frequency in fear, disgust. Regarding verbal reactions, it showed the high frequency in the heuristic responses either to explore or the responds to the impressive parts reminiscent to the facial appearances in all the joy, sadness, anger, surprise, disgust, fear. And it came out that the imaginary responses created new stories reminiscent to the facial expression in surprise, disgust, and fear.

This study aims to reveal whether attachment relationship with parents can differ from children being accepted and rejected in peer relations, and whether children's rejection sensitivity can differ from children having secure and insecure attachment relationship with parents. Subjects were 218 students in fifth and sixth grade of primary school. Inventory of Parent and Peer Attachment(IPPA), Children's Rejection Sensitivity Questionnaire(CRSQ), and peer relation sociometrics were used as measurements. The results showed that children in secure attachment with parents had lower scores on the subscales of CRSQ, such as anxiety, anger, belief, and psychological reaction, than children in insure attachment relationship with parents. And IPPA scores were higher in children being accepted in peer relation than children being rejected. These results can be considered that early attachment relationship with parents may influence children's rejection sensitivity and peer relation.

Purpose: This study aimed to investigate the clinical and socioenvironmental characteristics of sudden cardiorespiratory arrest after venipuncture in children. Methods: We conducted a retrospective email-based survey of all members of the Korean Pediatric Society. The questionnaire included items on patient demographics, socioenvironmental circumstances of the venipuncture, type of cardiorespiratory arrest, symptoms and signs, treatment, prognosis, and presumed cause of the arrest. Results: Fourteen patients were identified. Of these, 13 were young children (<2 years old), and 1 was 14 years old. All patients had been previously healthy and had no specific risk factors for sudden cardiorespiratory arrest. Most cases (n=11, 79%) were defined as cardiac or cardiorespiratory arrest, while the remaining cases (n=3, 21%) were defined as respiratory arrest. Aspiration (n=3), acute myocarditis (n=2), and laryngeal chemoreflex (n=1) were presumed as the causes; however, the exact causes were unclear. The overall prognosis was poor (death, n=7; morbidity, n=5; full recovery, n=2). The medical institutions faced severe backlash because of these incidents (out-of-court settlement, n=5; medical lawsuit, n=5; continuous harassment, n=3). Conclusion: Cardiorespiratory arrest after venipuncture is unpredictable and the probable cause of most cases is a vasovagal reaction. Medical personnel must be aware of the risk of unexpected cardiorespiratory arrest during routine intravenous procedures.
Using grounded theory method, this study analyzed the in-depth interviews with 11 parents of institutionalized children in order to find out how the experience of the parents on the process of the children's family reunification. According to the results, it was 'leaving one's child' that composed causal conditions, and also it was 'sentenced life' and 'adapting to a life without child' that composed existing conditions. Thus, the contextual condition was found out as 'a belief of family' and 'a belief of child caring' and the mediation conditions were 'power to recover' and 'not being as one's intentions'. Also, reaction/interaction strategies turned out as 'preparing a basis of child-care', 'playing a parent', 'standing against being adapted' and 'adjusting as their children back to family'. Finally, the analysis showed 'burdens on caring', 'wanting to leave one' child again', 'having stronger family membership' and 'being hopeful in a future' as results. The specific levels of the process were found out as following 4 steps, as time goes by. 1)Fostering children at institutions: become harsh parents(or a harsh parent), 2)After fostering: rebuild collapsed family in order to take the children back, 3)Family reunification: become aboveboard to oneself, family and the world, 4)After family reunification: try to keep the family taking precautions against resending. While taking their children back, the parents turned out to experience long, unremitting tension. Reflecting results of the analysis above, and in order to promote sending institutionalized children back to their families this article suggests practical alternatives for parents who left their children in institutions.
