Survey of Food Allergy Perception of Teachers and Operators of Child Care Facilities - Focusing on Jung-gu and Dong-gu in Daejeon - (보육시설 교사 및 운영자의 식품알레르기 인지도 조사 - 대전광역시 동구, 중구 지역을 중심으로 -)

  • This study was conducted to investigate the status of food allergy knowledge and management of the teachers and operators of child care facilities in the Daejeon Area. Surveys of 262 teachers and 50 operators of child care facilities in Dong-gu and Jung-gu in Daejeon were conducted. The prevalence of food allergies was found to be 3.6% (102 children). The average score of teachers in the anaphylaxis knowledge survey was $0.18{\pm}0.22$. Additionally, the needs for food allergy education among teachers and operators was 88.2% and 98%, respectively. The survey revealed that all child care facilities in this study provide food service menus to parents, but only 14 facilities (28%) provided alternative food to children with food allergies, and some of these 14 facilities did not provide adequate alternatives. To ensure the safety and healthy growth of children with food allergies, it is necessary to provide food allergy education to teachers and operators, as well as to establish safe food service and allergy management systems in all child care facilities.

Child-Care Facility and Kindergarten's Demands on Foodservice Support by Center for Child-Care Foodservice Management (CCFSM) in Seoul and Gyeonggi-do (서울.경기지역 보육시설 및 유치원 대상 어린이급식관리지원센터 지원 요구도 분석)

  • The purpose of this study was to analyze the differences between child-care facilities and kindergartens towards the need for foodservice support by Center for Child-Care Foodservice Management (CCFSM). For this study, questionnaires were sent out from August of 2008 to April of 2009 to directors of 1,478 child care facilities and 299 kindergartens in Seoul and Gyeonggi-do via postal service. A total of 267 questionnaires were usable with 203 (13.7%) of child-care facilities and 64 (21.4%) of kindergartens. Statistical data were analyzed by SPSS 15.0 for descriptive analysis and t-test. For political and administrative support, government funding for foodservice was the highest need and hiring nutritionists was significantly different by type of facility (p < 0.01). Both child-care facilities (4.29) and kindergartens (4.41) demanded the balanced menu from CCFSM. There were significant differences of "information about food material sanitation management" (p < 0.05), "hygiene safety management method according to working process" (p < 0.05), "hygiene safety management method of foodservice facilities and equipment" (p < 0.05). In education and training contents from center, "types and methods to manage foodservice facilities and equipment" for directors, "dietary education by age" for teachers, and "the rules of personal sanitation and working process" and "gas, electricity and fire prevention" for culinary workers had the highest mean score of requirements.

Long-Term Durability of Infliximab for Pediatric Ulcerative Colitis: A Retrospective Data Review in a Tertiary Children's Hospital in Japan

  • Purpose: The long-term efficacy and safety of infliximab (IFX) in children with ulcerative colitis (UC) have not been well-evaluated. Here, we reviewed the long-term durability and safety of IFX in our single center pediatric cohort with UC. Methods: This retrospective study included 20 children with UC who were administered IFX. Results: For induction, 5 mg/kg IFX was administered at weeks 0, 2, and 6, followed by every 8 weeks for maintenance. The dose and interval of IFX were adjusted depending on clinical decisions. Corticosteroid (CS)-free remission without dose escalation (DE) occurred in 30% and 25% of patients at weeks 30 and 54, respectively. Patients who achieved CS-free remission without DE at week 30 sustained long-term IFX treatment without colectomy. However, one-third of the patients discontinued IFX treatment because of a primary nonresponse, and one-third experienced secondary loss of response (sLOR). IFX durability was higher in patients administered IFX plus azathioprine for >6 months. Four of five patients with very early onset UC had a primary nonresponse. Infusion reactions (IRs) occurred in 10 patients, resulting in discontinuation of IFX in four of these patients. No severe opportunistic infections occurred, except in one patient who developed acute focal bacterial nephritis. Three patients developed psoriasis-like lesions. Conclusion: IFX is relatively safe and effective for children with UC. Clinical remission at week 30 was associated with long-term durability of colectomy-free IFX treatment. However, approximately two-thirds of the patients were unable to continue IFX therapy because of primary nonresponse, sLOR, IRs, and other side effects.

A Survey on the Use of Visiting Nurses in Child Day Care Centers (지역사회 보육시설에서의 방문간호사 활용을 위한 기초조사연구)

  • Purpose: The purpose of this study was to contribute children's health care and health promotion by surveying the use of visiting nurses in child day care centers. Methods: This study is a descriptive study, and the study period covers July to September, 2010. The research subjects were 27 public health center managers, 166 visiting nurses and 137child care teachers. Results: Teachers' need of visiting health care services and visiting nurses' work performance of were statistically significantly different. The teachers' need of visiting health care was higher in all areas (health examination, health life practice, infectious disease control, safety accidents and disaster management, emergency measures and linkage, nutrition, parent education) but the visiting nurses had a low level of work performance. Conclusion: Child care teachers are not health professionals for child health care, and therefore they are in need of professional help. Thus, for the current public health centers in need of customized visit health care, new visiting nurses in charge of professional child health care need to be developed. Also, new models need to be developed for visiting nurses and child care teachers through the connection of community child health care.

  • This study aims to enhance child care teachers' ability to offer health care by developing an educational program for children's health care and safety. This study was carried out using a one-group pretest-posttest design based on a survey of 400 child care teachers in 3 cities. The educational program for child care teachers consisted of three parts: child health observations, disease management for child and emergency care for children. The educational program was conducted 3 times for 3 hours, totaling 9 hours. The educational program showed better grades of child care teachers in child health observation, emergency care for children, and disease management for children. The finding indicates that more child care teachers will adopt a health care education program to improve the quality of child care service.

The Study on Supporting Situation of the Community Child Care Service for the Work-Family Balance - Focused on Dual Income Families Living in Seoul and Gyeonggi Province (일-가정 균형을 위한 지역사회의 자녀 돌봄 지원 실태에 대한 연구 -서울, 경기지역 맞벌이 가정을 중심으로)

  • The purpose of this study is to research support systems of the community child care services for optimum work-family balance. For this purpose, the questionnaire to be completed combined four areas: the workplace, school, community and home. Then questions concerning demographic factors and community-related aspects were analysed for this article. After analyzing 197 questionnaires, supplementary questions were asked, by interview to 10 samples. Full-time working wives (eg. working from 9am to 6pm) who have 1 or more children under 10 years old, living in Seoul and Gyeonggi province were collected as samples. The results were as follows: First, child care from family and relatives, and community services was properly used during the week, but dual working parents took complete charge of child care on Saturdays. Second, emergency child care was usually provided by the family, while relatives and community child care services were used less. Third, parents spent 17% of their monthly income on total child care expenditure. Fourth, community child care services were not used often, but the level of satisfaction with them was very high. Finally, they used relatives' services for safety reasons and community services for location, and only based on informed decisions. Therefore, Saturday child care programmes need to be initiated to help physically and mentally tired parents, with more general provision of positive and diverse public support systems.

  • In this study, the problem of child care gaps has arisen due to the expansion of women's entry into the workforce and the increase in working couples, and the care cliff phenomenon after children enter elementary school has been identified as one of the causes of women's career disconnection and low birth rates, and child care services have been initiated to solve care problems and balance work and family. The importance of childcare services to the safety and well-being of children has been highlighted by the restrictions on school attendance and the absence of caregivers during the COVID-19 pandemic. The government has been making policy efforts to reduce the gap in child care, but problems with the effectiveness and efficiency of the child care system have arisen due to unstable target selection and delivery systems by ministries and projects in the implementation of child care services. Therefore, this study examines the child care services implemented by each ministry to reduce the blind spots of after-school care services in the community and prepare efficient operation plans for various delivery systems, and seeks directions for the development of child care services.

Safety Education Needs and Knowledge and Attitude of Injury Prevention of Elementary School Children (학령기 아동의 안전교육 요구도 및 사고예방에 대한 지식 및 태도)

  • Purpose: This study was conducted to suggest direction and strategy of safety education proper to elementary school children. Method: The subject of this survey consisted of 313 3rd- 5th grade from 4 elementary schools in Kyungi-Do and Kwangwon-Do. Data were collected from March to May, 2002 using a questionnaire about 「safety education needs」, 「knowledge about injury prevention」, 「attitude about injury prevention. Result: 1. The degree of safety education needs showed averaged 77.50 on the basis of 100 points. 2. The degree of knowledge and attitude about injury prevention showed averaged 72.81 and 81.74 seperately on the basis of 100 points. 3. With the respect to the demographic characteristics, there were stastically significant differences in safety education need according to children's grade(F=8.692, p=.003), sex(t=-2.059, p=.040), family type(t=-2.229, p=.027) and in knowledge & attitude about injury prevention, there statiscally significant difference according to experience of injury prevention education(t=3.058, p=.003; t=5.308, p=.000) each. 4. The level of safety education needs is correlated at signficant level with knowledge and attitude about injury prevention of childrens(r=.166, p=.048; r=.265, p=.001) and between knowledge and attitude about injury prevention, there was significant correlation (r=.427, p=.000). Conclusion: From this results, nurses can plan safety education program appropriate to children's needs, level of knowledge and attitude about injury prevention.

A Study on the Environmental Design Factors of Children's Park Access Area - A Survey Focusing on the Perceptions of Professional Groups - (어린이공원 주변공간의 환경계획요인에 관한 연구 - 전문가 의식조사를 중심으로 -)

  • This study seeks to identify elements of spatial planning for areas surrounding children's parks in order to improve child safety. It will identify different aspects of child safety and extract elements of safety planning for the areas surrounding children's parks from a literature review of both domestic and international research. The study classifies the space between one's residence and the children's park as either a "means of access", such as pedestrian walkways and streets, or a "boundary", which consists of the entrance, exit, and fences; then, the derived safety planning elements were categorized in accordance with this classification. In order to ensure the validity of the planning elements, an expert survey was conducted of environment planners who specialize in the palnning and designing of residential area as well as government employees who are in direct charge of managing children's parks. The survey findings were as follow. First, the pedestrian walkway(means of access) near the park is the most crucial factor in regards to the safety of children, followed by the entrance and exit(boundary), fency(boundary), and streets (means of access), in descending order of importance. Thus, improving the safety of the pedestrian walkway should be considered first and foremost, and it should precede improving the surrounding streets. Second, an investigation of the need for safety devices near the children's park showed that securing visibility, through the installation of an illegal parking prevention device, is imperative. Illegal parking near children's parks poses a grave risk to pedestrian safety and demands immediate action. Furthermore, a section of streets within 300m of the park entrance should be designated as a children protection zone, in addition to the designation of school zones near elementary schools.

A Study on the Danger of Play Apparatus in Elementary School- Case study of CHONJU City- (초등학교 놀이시설의 위험성에 관한 연구 -전주시를 중심으로-)

  • The purpose of this study was an analysis of the elementary school child's attitude and dangerous factors of play apparatuses n elementary schools. The results were summarized as follows; 1. 53.0% of the reply child thought that play apparatuses were dangerous, and 41.0% of the total reply had experienced accidents by apparatuses, the child under 10 had experienced the accidents manytimes. 2. The types of injuries most frequently occurred are falls from play apparatuses, and part of the body most frequently injured parts of the body are arms and legs. 3. The danger of fall in play apparatuses that is over 3M amounted 19.6%, it asked safety of bottom materials. 4. Destructed play apparatuses reached up to 20.0%, and rusted or decomposed apparatuses reached to 28.4%. 5. The distance of each apparatus were very close ; it was within 0.3~1.5M, which was 24.9%, it was very dangerous factor. 6. The most frequent hazards of educational play apparatuses are ; - inappropriate size of apparatuses(height, gap, thick etc.)- inappropriate shape and finished state(bad welding and cutting, projection part etc.)- unsuitable use of bottom material in playground and safety color.- insufficient use zone.-inappropriate maintenance and inspection(endurance passage, erosin of iron-ware and wooden ware, projection of foundation concrete)
