A Case of Cervical Spinal Stenosis Improved by Combined with Chuna Manual Therapy and Exercise Treatment (추나치료 및 상지 운동치료를 통해 호전된 경추 협착증 환자 치험례)

  • This study reports the effects of Korean medicine treatments including Chuna manual therapy (CMT) and exercise treatment for a patient suffering with cervical spinal stenosis. We treated patient diagnosed with cervical spinal stenosis. The patient was treated with CMT once a day for 26 days. Manual muscle testing (MMT), ratio and degree of numbness, numerical rating scale (NRS) were used as tools for evaluating the patient's progress. MMT was increased from 3+ to 5, ratio and degree of numbness were decreased from grade (Gr.) 9 to Gr. 4, NRS was decreased from 5 to 3. This study suggests that CMT and self-exercise can be considered as effective treatment for cervical spinal stenosis.

A Case Report on a Patient with Cervical Vertigo Who Improved with a Combination of Korean Medicine Treatments (한방 복합치료로 호전된 경추성 현훈 환자의 증례보고 1례)

  • Objective: To report the case of a patient suffering from cervical vertigo who was treated with a combination of Korean medicine treatments (including acupuncture, pharmaco-acupuncture, and Chuna manual therapy). Method: An outpatient with cervical vertigo was given a combination of Korean medicine treatments five times, from December 5 to 16, 2016. All of the treatments, acupuncture with electric stimulation and pharmaco-acupuncture, were applied on the trigger points (TP) of the cervical muscles and the upper trapezoid muscles. Chuna manual therapy was then applied on the cervical area. The Numeric Rating Scale (NRS) - according to outpatient's statement on vertigo, neck pain and headache - and the Korean Dizziness Handicap Inventory (DHI), which is a self-evaluating scale of dizziness in daily life, were used to evaluate the effects of this treatment. Results: At the end of the treatment, the vertigo and the headache disappeared. The NRS decreased from 9 to 0 for vertigo and from 5 to 0 for headache. Although the neck pain also decreased, it did not disappear; this NRS decreased from 7 to 1. The DHI score also decreased significantly, from 64 to 4. Conclusion: Since the symptoms of the patient with cervical vertigo improved after the very first treatment and the improvement of the symptoms was observed as the treatment progressed, this case report is worth supporting the efficacy of combined Korean medical treatments. However, because there was only one subject, it is hard to say that this efficacy can be generalized. Moreover, this report cannot prove the individual efficacy since various treatments were combined. While there are many researches related to cervical vertigo in Western medicine, the studies in Korean medicine are comparably insufficient; therefore, follow-up research on more subjects is needed.

  • Objective : There is no case report on the ossification of the posterior longitudinal ligament(OPLL) treated by CHUNA manual therapy in oriental medicine. We experienced a patient(32-year-old female) with OPLL improved by CHUNA manual therapy in X-ray image and visual analogue scale(VAS), and we report it clinically. Methods : During the patient with OPLL was treated CHUNA manual therapy, acupuncture, and physical therapy, we examined cervical X-ray, CT and Moire topography and evaluated the patient's symptoms by VAS. And we observed whether the changes exist or not in X-ray and VAS. Results : VAS in symptoms was improved significantly(p<0.05), and the radiologic X-ray image in OPLL patient was improved. Conclusion : OPLL was generally treated conservative treatment and operation in severe case. But CHUNA manual therapy may be more effective in conservative treatment of OPLL, if more clinical studies and reaches are accomplished.

A Systemic Review of Traction Therapy for the Patients of Cervical, Lumbar Vertebral Disease (경추 및 요추의 통증에 사용되는 견인요법에 대한 문헌 고찰)

  • Objectives : This study aims to evaluate domestic/foreign clinical studies related to the traction therapy and analyse points to consider about cervical/lumbar traction therapy. Methods : Seven databases were searched for related articles about cervical/lumbar traction therapy from 1950 to 2014. Fourteen clinical studies and several systemic reviews were included among 144 studies searched. Out of fourteen clinical studies, four were case series and ten were controlled trials. Results : Most of included studies reported favorable effects of traction group compared to baseline of controlled group. The various mechanical factors most relevant to traction are 1) angle of pull and direction, 2) traction force, 3) duration of traction and frequency of treatment. Conclusions : We found various mistake in the applications of statistical methodologies of traction therapy targeting patients of cervical, lumbar vertebral disease. It is necessary for more randomized controlled trials to evaluate effect of cervical/lumbar traction therapy targeting patients of cervical, lumbar vertebral disease.

The Clinical Study of Cervical Sprain by Car Accidents in Children (경항통 및 경추 염좌를 동반한 소아 교통사고 입원 환자 24명에 대한 임상적 고찰)

  • Objectives The purpose of this study is to report 24 patients who have neck pain or have been diagnosed with cervical sprain by car accidents and have been treated in the oriental medical hospital. Methods 24 children who have visited oriental medical hospital from January 2016 to September 2016 were analyzed by reviewing the patients' charts. Results In the study group, the most common chief complaint was having musculoskeletal partial pain. Among them, 18 patients had neck pain; 20 patients had C-spine sprain was diagnosed at and among 18 who had neck pain, 14 was diagnosed C-spine sprain and 4 was normal. All patients had been treated by acupuncture, herb medicine and chuna treatment (muscular manipulation) and the oriental medical treatment resulted in improvement of children patients' symptoms by car accidents. Conclusions The study result showed that the car accident could have caused cervical sprain and neck pain to children and the oriental medical treatment was effective in the children patients.

A Case Report of Prescribing Yanghyeolgeopung-tang(yangxuequfeng-tang) to Two Patients with Cervical Disc Herniation and Headache (양혈거풍탕(養血風湯)을 투여한 두통을 동반한 경추 추간판 탈출증 환자 치험 2례)

  • Objectives : The purpose of this study is to investigate the clinical application of Yanghyeolgeopung-tang(yangxuequfeng-tang) to two patients with cervical disc herniation and headache. Methods : The patients were hopitalized at the Dept. of Oriental Rehabilitation Medicine, Jaseng Oriental Medicine Hospital, and diagnosed as cervical disc herniation and treated mainly with herbal medicine; Yanghyeolgeopung-tang(yangxuequfeng-tang). Additional acupuncture and pharmaco-puncture was provided. NRS(Numeric Rating Scale) and NDI(Neck Disability Index) scores were used as outcome measures. Results : After taking Yanghyeolgeopung-tang(yangxuequfeng-tang), the patients' pain was controlled and the patients slept well after treatment. NRS and NDI scores both decreased. Conclusions : As seen in these two cases of cervical disc herniation with headache, Yanghyeolgeopung-tang(yangxuequfeng-tang) has a positive effect in controlling neck pain and headahce due to cervical disc herniation.

Systematic Review of Chuna Manual Therapy for Cervicogenic Headache (경추성 두통에 대한 추나 요법의 체계적 문헌고찰)

  • Objective : The purpose of this study is to review the randomized clinical trials of Chuna manual therapy for cervicogenic headache and provide a evidence for the efficacy of Chuna manual therapy. Methods : We searched randomized clinical trials that performed Chuna manual therapy for cervical headache up to Feb. 2017 in 6 databases. Randomized clinical trials were selected according to the inclusion criteria and the data were extracted and analyzed. The risk of bias was assessed using the Cochrane Risk of Bias Criteria. Results : 16 RCTs met the inclusion criteria. The meta-analysis of 13 RCTs showed favorable results for the use of chuna manual therapy compared to drug, physical treatment. Conclusions : In 16 RCTs, we found that Chuna manual therapy was effective in cervicogenic headache. However, all RCTs are exposed to a number of bias risks. Therefore, well designed clinical trial would be needed to raise the evidence level of Chuna manual therapy.

  • Objectives : The purpose of this study is to investigate the clinical application of acupuncture treatment to a cervical herniated intervertebral disc patient with hyperventilation syndrome developed after magnetic resonance imaging(MRI). Methods : A patient with cervical herniated intervertebral disc got hyperventilation syndrome after taking magnetic resonance imaging. To relieve symptoms, we took paperbag breathing first aid, but it had no effect. So we used acupuncture therapy with strong stimulation. To measure the outcome of the patient's improvement, we observed change of vital sign and verbal numerical rating scale(VNRS). Results : After 8 minutes of acupuncture treatment, the patient had significant improvement in change of vital sign and verbal numerical rating scale(VNRS). Conclusions : In emergency situation, acupuncture treatment as a first aid has a positive effect to control hyperventilation syndrome.

  • Objectives: This case is to report the effect of TMJ balancing therapy (TBT) and combined Korean Medicine (CKM) on a 15-year-old male patient with suspected cervical dizziness of unknown causes. Methods: He was treated with TBT and CKM included Yin-yang Balancing Therapy of TMJ, pelvic balancing therapy, cervical chuna therapy once a week for 3 months. He continued to wear accurate balancing appliance (ABA) in the mouth, did 12-way cervical gravity straightening exercise and took banhabakchulchunma-tang-gamibang for 3 months. The effects of treatment was evaluated using the numerical rating scale (NRS). Results: After the treatments, dizziness was relieved from NRS 7 to NRS 1. Conclusions: This case report suggests that TBT and CKM treatments can relieve suspected cervical dizziness of unknown causes.

Analysis of the Treatment and Adverse Events of Whiplash Associated Disorder Patients During Pregnancy in a Korean Medicine Hospital (임신 중 교통사고로 일개한방병원에 내원한 환자의 치료 분석 및 이상반응 보고)

  • Objectives: The purpose of this study is to analyze both treatment and adverse events of Korean medicine treatment for whiplash associated disorder patients during pregnancy. Methods: We investigated the medical records of 44 pregnant patients who had got into car accidents from April 1, 2018 to May 31, 2019. Results: 100 percent of the inpatients and 88 precent of the outpatients were treated by acupuncture. 79 percent of the inpatients and 72 precent of the outpatients were treated by chuna, and 86 percent of the inpatients and 64 precent of the outpatients were treated by pharmacopuncture. Out of 32 patients with low back pain, 94 percent were treated by acupuncture, 47 percent by phramacopuncture and 38 precent by chuna. 90 percent of 31 patients with cervical pain were treated by acupuncture, 71 percent were treated by pharmacopuncture and 77 percent were treated by chuna. There were adverse events like cough, rhinorrhea, dilatation of cervix and worsen low back pain during or after the treatment. Conclusions: Korean medicine treatment including chuna and pharmacopuncture can be applied to pregnant women injured by traffic accident.
