간접심미수복을 부탁해 세라믹을 심미수복의 임상 접근 (Clinical approach with ceramic)

  • The requirements for the successful treatment of all-ceramic restorations are not so different from the ones of conventional restorations. "The provisional restoration followed by an adequate tooth reduction" and "the accurately fitting prostheses with corresponding to final impression" can be the examples of them. Nevertheless, the one which all-ceramic restorations are distinguished from conventional restorations is the additional procedure of so called "bonding". In addition to the application of resin cement between "inner surface of restoration and outer surface of abutment", bonding technology can be also applied to the treatment process of "Post and Core" in particular if the abutments are non-vital teeth. Core build-up for all-ceramic crown is conducted with fiber post and tooth colored composite by considering the properties of the restorations transmitting light. I would like to share my clinical experience about "silica based ceramic and non silica based ceramic restoration.

We know well that a vital abutment is easier than a non-vital one to get the targeted goals for clinical success in connection with esthetics and structure. The creation of "Post and Core" with bonding technique is a decisive factor for a long-term success if the abutment is non-vital tooth with dentinal collapse. I would like to share my clinical experience about "post & core build-up and all-ceramic restoration bonding" out of several success strategies of all-ceramic crown with this presentation.

  • A technique is presented through clinical cases for restoring anterior teeth to enhance dental esthetics using bonded partial ceramic restorations (BPCRs) without or with minimum reduction of tooth structures. These restorations offer the advantages of the superior properties of ceramic materials and the conservative nature of a direct composite restoration at the same time.

  • Even though a conventional metal ceramic restoration is widely in use, its laboratory procedure is still technique-sensitive, complex, and time-consuming. A ceramic-pressed-to-metal restoration (PTM) can be a reliable alternative. However, simplified laboratory procedure for a PTM is still necessary. The article is to propose a technique that reduces time and effort to fabricate a PTM with the aid of computer-aided design, computer-aided manufacturing and selective laser sintering technologies.

훼손 도자기 결실부 복원을 위한 3D 세라믹 프린팅 기술의 기초 적용성 연구 (A Study on the Applicability of 3D Ceramic Printing Technology for Restoration of the Missing Part of Damaged Ceramics)

  • 훼손 도자기의 결실부 복원은 ABS, PLA 등이 적용되고 있으나 도자기 재질과는 유사하지 않아 세라믹 수지에 대한 물성 및 적용성 연구를 진행해보고자 하였다. 기존 복원 방법인 거푸집과 ABS 수지 복원, 세라믹 수지의 실제 복원 및 물성 실험을 진행하였다. 연구 결과 세라믹 수지의 복원부 제작은 기존 재료에 비해 정밀도가 높아 미세한 형상의 출력이 가능하고, 표면 질감, 광택이 L30과 ABS 수지보다 우수하였다. 물성 측정 결과 수축, 변형이 없고 압축강도가 높아 기존 재료에 비해 내구성이 우수하나 비중과 경도 값은 복원부 제작 후 가공성이 저하될 수 있는 요소로 작용될 수도 있을 것으로 사료된다. 본 연구에서 적용된 세라믹 수지의 장기적인 모니터링과 신뢰성 및 안정성 평가, 너무 얇거나 긴 형태의 복원부 출력 시 원형복원성 등에 대한 추가적인 연구가 이루어져야 할 것으로 판단된다.

심미보철 치료의 경향과 이해 : part 1. IPS e.max (Understanding and trends of esthetic treatment in prosthodontics : IPS e.max)

  • With the increase of esthetic demands, most patients want to have restorations which are not only functional but also esthetic. For the esthetic restoration, many ceramic systems have been introduced and applied in dentistry. Among those ceramic restorations, IPS e.max system composed of lithium disilicate glass ceramic is one of the most commonly used systems because it has strength and esthetic characteristics. IPS e.max system is divided into IPS e.max Press and IPS e.max CAD according to the manufacturing methods. IPS e.max Press is fabricated through heat-pressed technique with ceramic ingot, which is very simple. The restorations which are made using IPS e.max system can apply to 3 units restoration for the anterior teeth and premolar, and single posterior tooth restoration. Cementation is one of the most important clinic procedure for the longevity of the restorations. All ceramics are bonded by resin cements, it is classified into three groups including adhesive, self-adhesive, and conventional. Variolink N, which is an adhesive resin cement and manufactured by same company with IPS e.max, is recommended for the bonding of IPS e.max restoration. Conventional and self-adhesive resin cement is also available. The aim of this review article is to provide the understanding of material properties, production procedure and clinical application of IPS e.max system.

전부도재수복물을 위한 수종의 코어 세라믹의 반투명도 및 색 안정성 (Translucency and Color Stability of Various Core Ceramics for All-Ceramic Restoration)

  • Purpose: This investigation was designed to determine the translucency and color stability of various core ceramics for all-ceramic restoration using the CIE $L^*a^*b^*$ system. Material and Methods: IPS e.max Press ceramic(Ivoclar-Vivadent, Liechtenstein), $LAVA^{TM}$ All Ceramic(3M-Espe, Germany), Cercon Smart Ceramic(Dentsply, Germany), and Z-match Ceramic(DentAim, Korea) were used for this study. For the specimens of zirconia oxide ceramics, the as-sintered cylindrical blanks($11.0{\times}25.0mm$) were machined into the shape of a disk(0.4, 0.8, 1.5 mm in thickness, 10 mm in diameters) with a diamond grind machine. The IPS e.max Press specimens ($0.8{\times}10mm$) were fabricated using the "lost wax" technique. CIE $L^*a^*b^*$ coordinates and light transmission were recorded for each specimen with a spectrophotometer(CM-2600d, Minolta, Japan). Color differences were calculated using the equation, ${\Delta}E^*ab=[({\Delta}L^*)2+({\Delta}a^*)2+({\Delta}b^*)2]1/2$. Results: The results were obtained as follows: 1. The most translucent group was IPS e.max Press ceramic that is a glass-ceramic, and $Lava^{TM}$ and Z-match ceramic were more translucent than Cercon Smart ceramic in zirconia ceramic group. 2. In the all groups, there was no significant translucent change after 6 times heat-treatments required to make a final restoration. 3. Colored zirconia was showed more yellowish and dark than uncolored zirconia. 4. After heat-pressing, IPS e.max Press ceramic was showed high ${\Delta}E^*ab$ value(4.1 of eM1, 6.8 of eM2) that means to be more whiter than before heat-pressing. However, there was no color change after additive heat treatments for final restoration. 5. In the colored zirconia groups, there was no significant color change after some heat-treatments required to make a final restoration.

열-가압을 이용한 금속 도재관과 전통적인 금속 도재관의 변연 적합도 비교 연구 (Comparative study in marginal fit of a pressed ceramic and feldspathic porcelain fused to metal restoration)

  • 연구 목적: 본 연구의 목적은 전통적인 금속 도재관과 열-가압을 이용한 금속-도재관의 변연 적합도를 비교하는 것이다. 연구 재료 및 방법: 상악 제1소구치 레진 치아를 교합면 2.0 mm, 협, 설면과 인접면을 1.0 mm 두께로 삭제하였으며, 변연 형태는 chamfer margin으로 그리고 축면 경사는 6도 taper로 삭제하였다. 삭제한 레진치를 복제하여 금속 다이 20개를 제작하였고, 이 금속 다이를 인상 채득하여 작업을 위한 석고 다이를 제작하였다. 금속 도재관과 $PoM^{(R)}$ 시스템 (Ivoclar vivadent., Liechtenstein)의 두 그룹으로 나누어 금속 코핑을 각각 10개씩 총 20개를 제작하였으며, 광학 현미경을 이용하여 변연 간격을 측정하였다. 도재 축성 후, 위와 동일한 방법으로 변연 간격을 측정하였다. 통계 처리는 Wilcoxon Signed Ranks test와 Mann Whitney U test를 이용하여 분석하였다. 결과: 본 연구의 결과는 다음과 같다. 1. 금속 도재관은 코핑 상태 ($64.93{\pm}12.48\;{\mu}m$)와 도재 축성 후 ($63.43{\pm}12.86\;{\mu}m$)의 변연 간격에 유의성 있는 차이를 보이지 않았다. 2. $PoM^{(R)}$ 시스템은 코핑 상태 ($50.00{\pm}12.28\;{\mu}m$)와도재축성후($56.72{\pm}13.80\;{\mu}m$)의 변연 간격에 유의성 있는 차이를 보이지 않았다. 3. 금속 도재관과 $PoM^{(R)}$ 시스템 변연 간격은 유의성 있는 차이를 보이지 않았다. 결론: $PoM^{(R)}$시스템은 왁스 소결법을 이용해 변화가 적은 기술적인 이점이 있으며, 세밀한 기술을 요구하는 축성 방법과는 대조적으로 왁스 성형으로 도재의 완전한 형태를 만들 수 있는 편리함을 제공하는 장점을 가진다. 본 연구 결과와 이런 장점을 토대로 앞으로 임상적 사용을 고려해 볼 수 있을 것으로 사료된다.

태안 마도 출토 도자기의 염에 의한 손상상태 연구 (A Study on the Soluble Salt and Deterioration of Ceramics from Taean Shipwreck)

  • Ceramics from Taean shipwreck were studied to investigate the deterioration by soluble salts and desalination pattern according to each ceramic characterization. NaCl crystals were found as a crystal growth on the ceramic microstructure and deteriorated to push off the glaze layer of some Puncheong ware sample. As a result of monitoring the desalination pattern of ceramics for 74days, the earthenware sample with big pores and no glaze removed soluble salts so fast, but Puncheong ware and Whiteware samples removed lots of soluble salts slowly and continuously. Generally, it takes long time to remove soluble salts from porcelains, due to small pores and glaze, but the long-term desalination probably can cause the damage on the surface layer such like glaze. As some soluble salts still remains in the ceramic microstructures after desalination, it is very important to control and keep the relative humidity stable through the proper storage. There are various ceramic deterioration patterns by soluble salts. Therefore, it needs to select the desalination method of ceramics from shipwreck, considering the material characteristics and the production techniques.
