A Study on the Cultural Adaptation Stress Experience of Female Marriage Immigrants: Focused on the Healthy family support centers and Multi-cultural family support centers in Incheon (여성 결혼이민자의 문화적응 스트레스 경험에 관한 연구: 인천시 건강가정·다문화가족지원센터 중심으로)

  • This study was a qualitative research to investigate the adaptation process and stress experiences of female marriage immigrants to Korean culture, and to explore life experiences and meanings to cope with them. Participants in the study collected data through individual in-depth interviews targeting 5 female marriage immigrants with more than 5 years of marriage using the Healthy Families·Multicultural Family Support Center located in ◯◯-gu, Incheon City. Using the six steps of Braun & Clarke's topic analysis method, 3 main topics and 13 subtopics were derived. Research Results First, cultural differences experienced by female marriage immigrants in Korean life (daily life culture, tradition, family relations) Second, the cultural adaptation process experienced by female marriage immigrants (difficulties and fears-understanding cultural differences-efforts to overcome) Third, it was found that the cultural adaptation of female marriage immigrants is that Korean lifestyle becomes natural and familiar by knowing the difference in lifestyle between their mother country and Korean culture, and finding and practicing the way they want to live with Korean people. Based on the research results, a practical intervention plan for successful cultural adaptation of female marriage immigrants was presented.

Development and Perception of a Course on Lifestyle and Health Promotion by Utilizing Blended Learning for University Students (블랜디드 러닝을 활용한 대학생을 위한 생활습관과 건강증진 교양과목 개발과 학생의 인식)

  • Backgroud & Objectives: The purpose of the study was to develop an innovative blended learning method on life style and health promotion and evaluate the educational effects for university students. Methods: The blended learning was developed to combine face-to-face lecture(off-line lecture) and on-line lecture that applied the subject of life style and health promotion. This course is a coordinated effort towards providing 5 topics of lifestyle such as smoking, alcohol, exercise, diet, and stress management. This has been verified by an expert in the field of nursing, education, e-learning technician and students. Participants were different part of university students (n=28) with major enrolled in a general culture course for 2 credits which composed of 8 sessions of each 2-hour in the first semester of 2010. The study was a one group posttest design. A self-report about health knowledge, attitude, and health behavior was organized by content analysis after the sessions. Results: Positive feedbacks from students were reflected in the outcome. Student regarded good lifestyle as being the most important. Student concerned those on-line lectures are not only available at most time and site, but also good for individualization, visual understanding and interest. Face-to-face lecture provided student a chance to integrate with knowledge and experience and had desire to improve good lifestyle and health promotion. Conclusions: The blended learning method on good lifestyle and health could make a best use of improvement for knowledge, attitude and behavior concerning. It is needed to identify the long term effects of a blended learning for further study.

The Role of Resectional Surgery for the Treatment of Localized Multi-drug Resistant Pulmonary Tuberculosis (국소 병변의 다제 내성 폐결핵 환자에서 폐절제술의 역할)

  • Background : Surgery may have a role when medical treatment alone is not successful in patients with multidrug resistant (MDR) pulmonary tuberculosis (PTB). To document the role of resection in MDR PTB, we analyzed 4 years of our experience. Methods : A retrospective review was performed on thirteen patients that underwent pulmonary resection for MDR PTB between May 1996 and February 2000. All patients had organisms resistant to many of the first-line drugs including isoniazid (INH) and rifampicin (RFP). Results : The thirteen patients were $37.5{\pm}12.4$ years old (mean${\pm}$S.D.)(M : F=5:8), and their sputum was culture positive even with adequate medication for prolonged periods ($109.7{\pm}132.0$ months), resistant to 2-8 drugs including isoniazid and rifampin. All patients had localized lesion(s) and most (92.3%) had cavities. At least 3 sensitive anti-TB medications were started before surgery in all patients according to the drug sensitivity test. The preoperative $FEV_1$ was $2.37{\pm}0.83$ L. Lobectomy was performed in 11 patients and pleuropneumonectomy in two. Postoperative mortality did not occur, but pneumonia occurred as a complication in one (7.7%). After $41.5{\pm}58.9$ days (range 1~150 days) follow up, negative conversion of sputum culture was achieved in all patients within 5 months. Only one patient (7.7%) recurred 32 months after lung resection. Conclusion : When medical treatment alone is not successful, surgical resection can be a good treatment option in patients with localized MDR PTB.

Active Surveillance of Pertussis in Infants Under 6 Months of Age: A Single Center Experience from 2011 to 2013 (생후 6개월 미만의 영아에서 백일해에 대한 적극적인 감시활동: 2011-2013년 단일기관 연구)

  • Purpose: The objective of this study was to assess and investigate the epidemiology of pertussis in infants under 6 months of age. Methods: A prospective study was conducted between October 1, 2011 and April 30, 2013 in CHA Bundang Medical Center, Seongnam, South Korea. Polymerase chain reaction (PCR) or culture was used to detect Bordetella pertussis in nasopharyngeal aspirates from case patients who were hospitalized for acute lower respiratory tract infection (LRTI). In addition, multiplex real-time PCR assays were also performed to detect 6 etiologic viruses, including adenovirus, human metapeumo-virus, influenza virus, parainfluenza virus, respiratory syncytial virus and rhinovirus. Results: Of the 79 enrolled case patients, whose median age was 2 months of age, the most common diagnoses uncovered in this study were acute bronchiolitis (60%) and pneumonia (28%). B. pertussis infection was found in 13 cases (16%), in which 7 (53%) was coinfected with respiratory syncytial virus and 1 (7%) with influenza A virus. Of the 13 patients with B. pertussis infection, 6 (46%) were not vaccinated with the diphtheria, tetanus toxoid, and acellular pertussis vaccine, while 6 (46%) received 1 dose, and 1 (8%) received 2 doses. Conclusion: B. pertussis infection was present in 16% of under 6 month-old infants, who were hospitalized for acute LRTI. Therefore, a nationwide epidemiological surveillance of pertussis, including institutions that cater to infants under 6 months of age is necessary and needed.

A Study of the Classification of Nursing Diagnoses (간호진단 분석 일 연구)

  • This study was conducted to analyze the nursing diagnoses applied for case studies of nursing students through their clinical practices, and to provide the educational basis of nursing diagnoses with its results. The data were collected for two years(1995 and 1996) from 70 case studies reported by the 2nd and 3rd year nursing junior college students. The students made 259 nursing diagnoses among which 230 diagnoses qualified NANDA classification and were taken for analysis. The results of the analysis were as follows : 1. The number of diagnoses indicating response patterns was 35(35.7%), whereas 98 diagnoses in NANDA table. Among the 35 diagnoses, the pattern of exchange was most frequent, then feeling, moving, knowing in rank. 2. The diagnoses were analyzed in the categories of response patterns. For Instance, 'Altered in Nutrition' was most frequent in exhange, then Risk for Infection', 'Ineffective Airway Clearance', in rank. 3. Among 230 diagnoses, 'Knowle Deficit' was most frequently mentioned, then 'Activity Intolerance' 'Anxiety', 'Pain', 'Altered in Nutrition', 'Risk for Infection', 'Ineffective airway clearance', in rank. 4. The types of word expression of each diagnoses were various. 'Activity Intolerance' was expressed in 6 types. 5. The relating factors applied to each diagnosis were analyzed. For Instance, the relating factor of 'Knowledge Deficit' were illness, and therapeutic process, lack of motivation, occurrance of complication, short experience, operation, and so on. From the above study, the researcher would like to recommend as follows : 1) The current diagnoses need to be verified its content validity, when they are applied to our culture. 2) The most effective educational method for applying nursing diagnoses should be explored. 3) Further study could be focused on not only 'relating factors' but also 'sign and symptoms'.

Clinical Patterns of Acute Poststreptococcal Glomerulonephritis: A Single Center's Experience (단일 기관 내의 급성 연쇄구균감염후 사구체신염 발생의 임상적 고찰)

  • Purpose: The aim of this study is to investigate the change in incidence and clinical characteristics of acute poststreptococcal glomerulonephritis (APSGN) through a single center's experience. Methods: We retrospectively analyzed the medical records of 53 children who were diagnosed with glomerular nephritis at National Health Insurance Corporation Ilsan Hospital between March 2000 and December 2009. Twenty-six of 53 patients falled to meet the dragnostic criteria of APSGN and were excluded. Chief complaints, physical examinations, urinalysis, urine culture, laboratory results, chest x-ray, and clinical manifestations were all retrieved from the remaining 27 patients. Results: The incidence of APSGN by age was highest in children between six to seven years of age, with similar distributions in both genders, and seasonal incidence was higher from November to February when compared to other months, which was similar to the previously reported studies. However, the annual number of APSGN patients did not show a significant decrease from 2000 to 2009. The incidences of edema and hypertension were 59.3% and 48.1% respectively, suggesting lower incidences than those from previous studies. Six patients (22.2%) experienced hematuria for more than 6 months. Hypertensive encephalopathy, one of APSGN critical complications occurred in one patient but resolved with conservative treatment. Conclusion: Our study shows that APSGN still occur as a common renal disease but the disease severity seems to decrease with milder clinical manifestations and less complications compared to past studies.

The Significance and Utilization of the Jungwon Historical and Cultural Area (중원역사문화권 설정의 의미와 활용 방안)

  • This paper proposed the validity and necessity of establishing the Jungwon Historical and Cultural Area by reviewing the historical characteristics of the name Jungwon and its temporal and spatial range. We also explored ways to utilize the Jungwon historical and cultural Area under the Special Act on the Maintenance of Historical and Cultural Areas. Jungwon, which means the center of the country, has strategic value as a key area connecting the Korean Peninsula to the north and south through the Geripryeong and Jukryeong. It also includes Chungju, which connects the Namhan and Bukhan Rivers . From the Mahan period, the areas of Cheongju and Jincheon, the Miho Stream basin to the west, and Chungju to the east were closely connected to each other. Afterwards, Baekje, Koguryo, and Silla engaged in a fierce battle to occupy the Jungwon area. Jungwon culture was born through the negotiation and fusion of the cultures of the three kingdoms. In particular, Silla continued to value the Jungwon area, demonstrated by the installation of Seowongyeong in Cheongju and Bukwongyeong in Wonju, centering on the Jungwongyeong. Since we have a historical view with a focus on ancient kingdoms, we have overlooked the full value of the history and culture of Jungwon. We need to recognize the importance of Jungwon area and think about how to utilize it. Therefore, we suggested in this paper that there be more public information services on the cultural heritage of Jungwon, the historical heritage experience, and the education programs for local residents. The development and operation of Jungwon's history, culture, and tourism contents, as well as the various academic research papers should be used to reveal the identity of the Jungwon historical and cultural area.

Sod Production and Current Status of Cultivation Management in Korea (우리나라 잔디 생산과 재배 관리실태)

  • To investigate the actual condition of production and management of sod, a questionnaire was ask to 57 farmers in major cultivated areas of turf sod in Korea in 2010 and 2011. The results of the turfgrass management situation analysis showed a mixed farming where the ratio of the principal work and the side work progress similarly, and for the landholding type depending on the management scale, the rental ratio was high for a scale over 0.5 ha. Sod production farmers has high-profile in new varieties of turfgrass, integrated management of the fertilization and soil or disease and insect pest however the future of the turfgrass industry does not seem optimistic due to the problems such as market stability or scarcity of worker. Obtaining of information on culture methods depended neighbor's experience (81.8%). Concerning the varieties preference analysis, the 32.1% of farmer answered that the reproductive rate was a key factor for the new variety. Sod production farmers want to receive financial support (28.1%), improvement of distribution structure of sod (26.6%), and spread of good variety turfgrass (23.4%).

Overview of the Korean Marine Industry and VPP Analysis of a 28ft Sailing Yacht (대한민국의 해양 레저 시장 및 28ft급 세일요트의 VPP 성능해석 연구)

  • The South Korean marine industry is emerging as a significant market, driven by the growing popularity of various water leisure activities, including sailing. This trend suggests a rising demand for sailing yachts. Consequently, since 2022, the design and development of a 28ft sailing yacht have been ongoing, supported by the government and the Ministry of Oceans and Fisheries, to promote yachting culture in South Korea. The Velocity Prediction Program (VPP) analysis was conducted using WinDesign during the preliminary design stage to evaluate performance and determine design parameters. The hydrodynamic model used for this vessel is based on regression methods developed from years of experience in naval architecture and yacht research at the Wolfson Unit, providing reliable estimates for most modern yachts. However, owing to the lack of specific hydrodynamic data from towing tank tests or CFD numerical analysis, verification of the hydrodynamic model has faced some challenges. Additionally, an incomplete weight estimate resulted in variable VCG values, potentially affecting stability and overall performance. The optimal boat speed for this vessel was determined at true wind speeds (TWS) of 4, 8, 12, 16, and 20 knots, using both the jib (up to 120° TWA) and the spinnaker (from 80° TWA). The optimized speed of the yacht was found to be comparable to that of international similar-class yachts.

Service Design for Healthcare Quality Improvement: An Implementation Approach for Enhancing Patient Experience (의료 질 향상을 위한 서비스디자인: 환자경험 증진을 위한 실행 접근법)

  • Purpose:This study aims to suggest the future direction for applying service design to improve the quality of healthcare as part of hospital service innovation and present implementation plans in Korea, based on a review of quality improvement activities and the current status of service design applications. Methods: Through a literature review, we examined the status of service design introduction and application in the healthcare field, focusing on cases in the US and Europe. The possibility and limitations of service design in the healthcare field were examined through a comparison of oversea and domestic cases. Results: Recently, service design has begun to be applied to the healthcare field worldwide. Service design shows the possibility of an alternative that alleviates and complements the limitations of existing quality improvement activities. It also offers the possibility of creating new organizational improvement and innovation approaches through integration and convergence with existing quality improvement activities and management innovation. Conclusion: To effectively apply service design to hospitals, it is necessary to integrate internal organizations related to service improvement, combine methods, and objectively measure and evaluate performance. To this end, we propose the operation of a nationwide education and training center for quality improvement and service design led by academic society. Service design will provide an opportunity to change the management innovation and organizational culture of hospitals beyond the scope of the current quality improvement, which deals only with micro-subjects of individual hospitals.
