교정용 메탈 브라켓에서 자가중합효소연쇄반응을 통한 치아우식증 원인균의 탈출 (Direct detection of cariogenic streptococci in metal brackets in vivo using polymerase chain reaction)

  • Streptococcus mutans와 Streptococcus sobrinus는 교정장치 주위의 법랑질 탈회를 유발하는 중요한 원인균들이다. 본 연구는 중합효소연쇄반응법을 이용하여 생체 브라켓에 존재하는 이들 균주의 이환율을 평가하기 위해 고안되었다. 상악과 하악치열의 절치 브라켓 각각 4개를 교정치료 종료 시점에서 채취하였다. 브라켓에 부착된 세균들의 DNA를 추출하고 각각의 세균에 해당하는 프라이머를 이용하여 덱스트란 분해효소를 증폭시켰다. 본 연구의 결과는 S. mutans와 S. sobrinus가 하악절치 브라켓에 비해 상악절치 브라켓에 더 많이 존재함을 보여주었으며, 이들 브라켓에 존재하는 세균의 빈도는 이전의 연구에서 구강내에 존재하는 빈도에 비해 더 적음을 보여주었다. S. mutans의 상악절치 브라켓 이환율은 $50.0\%$, 하악절치 브라켓 이환율은 $33.8\%$였고, S. sobrinus는 각각 $17.5,\;15.0\%$였다. 상악절치 브라켓에 두 종류의 세균이 모두 이환된 환자는 7명 $(8.8\%)$였으며, 하악절치 브라켓에 두 종류의 세균이 모두 이환된 환자는 5명 $(6.3\%)$였다. 본 연구의 결과는 법랑질 탈회를 유발하는 치아우식증 원인균이 절치 브라켓에 부착하는 상주균일 가능성을 제시한다.

Silver diamine fluoride가 타액 생물막에 미치는 영향 (The Effect of Silver Diamine Fluoride on Salivary Biofilm)

  • Silver diamine fluoride(SDF)는 우식의 정지를 위해 사용될 수 있는 효과적이고 경제적인 약제로, 협조가 힘든 환자나 의과적인 문제로 치료를 받기 힘든 경우 등에서 사용될 수 있다. 이 연구는 타액 생물막을 이용하여 SDF 의 항미생물효과를 평가하고자 시행되었다. 타액을 이용하여 획득피막 유사 구조를 형성한 후, 타액과 Streptococcus mutans를 첨가하여 우식원성의 타액 생물막을 형성하였다. 타액 생물막에 SDF 를 처리한 결과, 전체 미생물과 S. mutans의 수가 유의하게 감소하였다(p < 0.000). CLSM에서도 생물막 내 죽은 세포가 증가하였다. 유치 법랑질과 상아질 시편에 SDF를 처리한 후 우식원성의 생물막을 배양한 결과, SDF 를 처리할 경우 생물막 형성이 저해되는 것을 확인할 수 있었다(p = 0.029). 연구 결과 SDF의 뛰어난 항미생물효과를 확인할 수 있었으며, 이로 인한 임상에서의 항우식효과를 기대할 수 있다.

  • 이 연구는 우식원성 세균막 형성과 상아질 표면 거칠기에 대한 Silver diamine fluoride와 요오드화 칼륨(KI)의 영향을 실험실 환경에서 평가하는 것이다. 인공 우식을 유발한 48개의 Bovine dentin 시편을 제작하여 대조군, SDF 도포, SDF와 KI 도포(SDFKI)의 3개 군으로 나누었다. 각 군당 10개의 시편의 상면에 Streptococcus mutans, Lactobacillus casei 및 Candida albicans를 포함하는 다종 우식원성 세균을 24시간 동안 배양 후 세균 수를 측정하였다. 나머지 시편은 원자 힘 현미경과 주사전자현미경으로 관찰하였다. SDF와 SDFKI 도포 시 대조군에 비해 세균 수가 유의하게 감소했으며, KI는 SDF의 항균 효과를 유의하게 저해하지 않았다. 현미경 관찰 시 SDF와 SDFKI 도포 후 생성되는 입자가 상아질 표면에 침착 되는 것을 관찰했으나 표면 거칠기는 세 군 간 유의한 차이가 없었다.

Antimicrobial Effect of Polyphenon 60 against Streptococcus mutans and Streptococcus sobrinus

  • Polyphenon 60 refers to the mixture of catechins present in green tea. The aim of this study was to investigate the antimicrobial activities of polyphenon 60 against 4 strains of Streptococcus mutans and 2 strains of Streptococcus sorbrinus, which are the major causative bacteria of dental caries. The minimum bactericidal concentration (MBC) values of polyphenon 60 for S. mutans and S. sobrinus were determined and the effect of biofilm formation inhibition of that was evaluated. The MBC value of polyphenon 60 against the bacterial strains was 2.5 mg/ml except for one particular strain, S. mutans KCOM 1128 for which the value was 1.25 mg/ml. The results of biofilm formation inhibition assay revealed that polyphenon 60 inhibited biofilm formation more than 90% at a concentration of 2.5 mg/ml. It was apparent that polyphenon exhibited biofilm formation inhibition activity along with bactericidal effect against S. mutans and S. sobrinus. Therefore, it is proposed that polyphenon 60 as one of the components of bactericidal agents could be useful in developing oral hygiene products, toothpaste or gargling solution.

Antimicrobial Effect of Coptidis rhizome Extract against Mutans Streptococci and Periodontopathogens

  • The purpose of the study was to investigate the antimicrobial activity of the methanol extract of Coptidis rhizome against the type strains of cariogenic bacteria, Streptococcus mutans and Streptococcus sobrinus, and the periodontopathogens, Porphyromonas gingivalis, Prevotella intermedia, Treponema denticola and Aggregatibacter actinomycetemcomitans. The antimicrobial activities of the crude extract and the methanol extract fractions of Coptidis rhizome separated by silica gel chromatography were evaluated by determining the minimal bactericidal concentration (MBC) values, using the microdilution method. The cell viability test of the extracts of Coptidis rhizome on the KB cells was also studied by methyl thiazolyl tetrazolium (MTT) assay. Our results showed that the 11th fraction (F11) of the methanol extract had the greatest antimicrobial activity against the tested bacteria, with no associated cytotoxicity on the KB cells, upto a concentration of $50{\mu}g/ml$. These results suggest that the silica gel chromatography fraction F11 of the methanol extract of Coptidis rhizome, could be useful in the development of oral hygiene products as an antimicrobial agent for the prevention of dental caries and periodontal diseases.

Identification of Antimicrobial Peptide Hexamers against Oral Pathogens through Rapid Screening of a Synthetic Combinatorial Peptide Library

  • A positional scanning synthetic peptide combinatorial library (PS-SCL) was screened in order to identify antimicrobial peptides against the cariogenic oral bacteria, Streptococcus mutans. Activity against Streptococcus gordonii and Aggregatibacter actinomycetemcomitans was also examined. The library was comprised of six sub-libraries with the format $O_{(1-6)}XXXXX-NH_2$, where O represents one of 19 amino acids (excluding cysteine) and X represents equimolar mixture of these. Each sub-library was tested for antimicrobial activity against S. mutans and evaluated for antimicrobial activity against S. gordonii and A. actinomycetemcomitans. The effect of peptides was observed using transmission electron microscopy (TEM). Two semi-mixture peptides, RXXXXN-$NH_2$ (pep-1) and WXXXXN-$NH_2$ (pep-2), and one positioned peptide, RRRWRN-$NH_2$ (pep-3), were identified. Pep-1 and pep-2 showed significant antimicrobial activity against Gram positive bacteria (S. mutans and S. gordonii), but not against Gram negative bacteria (A. actinomycetemcomitans). However, pep-3 showed very low antimicrobial activity against all three bacteria. Pep-3 did not form an amphiphilic ${\alpha}$-helix, which is a required structure for most antimicrobial peptides. Pep-1 and pep-2 were able to disrupt the membrane of S. mutans. Small libraries of biochemically-constrained peptides can be used to generate antimicrobial peptides against S. mutans and other oral microbes. Peptides derived from such libraries may be candidate antimicrobial agents for the treatment of oral microorganisms.

Effects of Bamboo Salt with Sodium Fluoride on the Prevention of Dental Caries

  • Background: Dental caries is one of several prevalent oral diseases caused by dental plaque biofilms. This study evaluated the anti-cariogenic effects of a bamboo salt (BS) and sodium fluoride (NaF) mixture on oral bacteria. Methods: The effects of several mixtures of NaF and BS on acid production, growth, and adhesion to glass beads of Streptococcus mutans, and their anti-cariogenic properties were investigated. The growth of S. mutans was measured according to optical density at 3, 6, 9, 12, 15, 18, and 24 hours after treatment using spectrophotometry at a wavelength of 600 nm, while pH was measured using a pH meter. Adhesion of S. mutans was measured according to the weight of glass beads from each group before and after incubation. Gene expression was measured using real-time polymerase chain reaction. Acid production and growth patterns of S. mutans were compared using repeated measures analysis of variance, followed by Scheffe's post-hoc test. The Kruskal-Wallis test was used to compare adhesion, followed by the Mann-Whitney test. Gene expression in the experimental and control samples was compared using the Student's t-test. Results: Growth, acid production, and adhesion of S. mutans were inhibited in all experimental groups. Expression of gft and fructosyltransferase in S. mutans was inhibited in all groups. A mixture of NaF and BS significantly reduced growth, acid production, adhesion, and gene expression of S. mutans compared with the other groups. Conclusion: Results of the present study demonstrated that a mixture of NaF and BS was useful as a mouth rinse in preventing dental caries.

  • 시판되는 보리차 3종(티백형 2종 알곡형 1종)이 우식유발 세균인 Streptococcus mutans와Streptococcus sobrinus가 타액피복 수산화인회석에 부착되는 정도와 이들 세균의 체표소수성에 미치는 영향에 대해 평가하였다. 모든 실험군에서 보리차로 처리하였을 때 수산화인회석에의 부착능과 세균의 체표소수성은 감소하였고 이 값은 보리차의 종류와 처리방법에 따라 상이하였다. 이러한 결과는 보리차에 함유된 활성성분인 catechins와 melanoidins의 영향 때문인 것으로 추측되며 보리차로 전 처리한 수산화인회석에 세균이 부착하는 정도가 감소한 것은 활성물질이 흡착되어 치아표면의 수용체가 세균의 부착소와 작용하는 것을 방해함으로써 치아우식증 병인론의 초기 단계인 부착에 영향을 미칠 수 있다는 것을 의미한다. 본 연구결과 보리차가 우식유발 세균의 주요 독력인자 중 하나인 부착능을 억제하므로 보리차가 이러한 세균이 일으키는 치아우식증을 억제할 수도 있다는 가능성을 보여준다.

Comparative Evaluation of the Fluoride Releasing Ability and Microbial Attachment of Glass-Hybrid Restorative Material

  • This study aimed to compare the fluoride-releasing ability and degree of microbial attachment of a newly developed glass-hybrid restorative material (GH) with those of a high-viscosity glass ionomer (HvGIC), resin-modified glass ionomer (RMGI), and composite resin (CR). In addition, the correlation between fluoride-releasing ability and microbial attachment between materials was evaluated. Specimens were prepared in a disc shape and divided into 4 groups according to the materials (GH, HvGIC, RMGI, and CR). The fluoride release experiments were performed in each group (n = 15). The amount of fluoride released was measured on days 1, 3, 7, 14, 28, and 42 after storage. For the microbial attachment experiment, 12 specimens were produced per group using Mutans Streptococci (S.mutans ), a cariogenic microorganism. S. mutans was cultured on the specimens for 24 hours, and the number of bacteria was measured. GH had the highest cumulative fluoride release and showed a significant difference when compared with RMGI (p = 0.001) and CR (p < 0.0001). Microbial attachment was the lowest in GH; however, no significant difference was observed between the materials (p = 0.169). There was no significant correlation between fluoride release from materials and microbial attachment (p > 0.05). From this perspective, remineralization of low-mineralized areas could be expected due to the high fluoride release of GH, and the effect of delaying the progression of dental caries could be predicted from the low cariogenic microbial attachment. Therefore, GH might be a useful restorative material for treating immature permanent teeth with hypomineralized enamel. However, further studies are needed about the degree of remineralization of hypomineralized areas after restoration and the capacity to recharge fluoride.

불소유리 가철성 교정장치의 항균효과에 관한 연구 (A STUDY ON THE ANTIMICROBIAL EFFECT OF FLUORIDE-RELEASING REMOVABLE ORTHODONTIC APPLIANCES)

  • To study the antimicrobial effect of fluoride-releasing removable orthodontic appliances, 0 wt%(Group 1), 5 wt%(Group 2) and 10 wt%(Group 3) of $CaF_2$ were added to orthodontic resin. The specimens were then put in media in which S. mutans and Lactobacillus, known cariogenic bacteria, were cultured. Inhibiting zones were measured 24, 48 and 72 hours later. The following conclusions were reached: 1 In the Lactobacillus media, no zone of inhibition was observed regardless of the time passed. 2. In the S. mutans media, zones of inhibition were observed after 48 hours. 3. S. mutans showed greater inhibition in Group 3 compared to Group 2, after 72 hours rather than 48 hours(p<0.01).
