요양보호사의 임파워먼트와 직무만족도 (A Study on Empowerment and Job Satisfaction of Caregivers Aged)

  • Purpose: The descriptive study was done to examine empowerment and job satisfaction of caregivers caring for elders in long term care facilities. Methods: Data were gathered during September 2009, through a survey methodology. The participants were 189 caregivers who worked in long-term care institutions and home care clinics located in city and rural areas. The collected data were analyzed using t-test, ANOVA, Pearson correlation coefficient and stepwise multiple regression analysis. Results: For the relationship between empowerment and job satisfaction, there was a significant positive correlation between empowerment and job satisfaction (r=.503, p<.01). Empowerment explained 52.5% of the variance in job satisfaction. Conclusion: The results indicate that the level of empowerment influences job satisfaction. Therefore human resources management planning should focus on increasing empowerment in these caregivers in order to raise their job satisfaction.

호스피스 자원봉사활동의 실태 및 지원에 관한 연구 - 대전ㆍ충청권을 중심으로

  • This study was conducted to examine the present state, types of services, difficulties, education programs, motivation, satisfaction and attitude of hospice volunteers and also to suggest supporting programs for the volunteer. For these purposes, a questionnaire was distributed among the 200 hospice volunteers in ten hospice organizations in Daejeon and Chungcheong province. In this research, 85.5% of the respondents were women and most of them were housewives in their forties and fifties. The majority of hospice volunteers had an education background of more than high school. Christianity (94%) was the dominant religion of volunteers. Among the service area of the hospice, emotional caring, such as listening closely and establishing empathy were the most frequently done by the respondents and the next one was spiritual care. Hospice volunteers who had started with altruistic motivation show more dynamic activities in emotional, spiritual, physical caring and so on, than those with non-altruistic motivation. Most of the respondents expressed that they felt a hospice volunteer was valuable. However they had a hard time to overcome the sorrow coming from sharing empathy with the dying patients. Those who had started with altruistic motivation usually took part in two times per a week and had more positive attitude. Volunteers who had been working more than five years showed better attitude than those who had been working less than three years. The longer the voluntary services, the more positive attitude experienced by volunteers. Hospice volunteers encountered the most considerable difficulty with lack of background knowledge and their actual capability. Insufficiency of time was the next reason for the considerable difficulty of the hospice volunteer. On the other hand, those who served more than twice per week replied that lack of background knowledge and their capability were the least considerable difficulty. Insufficiency of time was the reason of difficulty for male volunteers, as well as for females under thirty and those in their forties and fifties respectively. Generally most of the hospice volunteers (70%) were satisfied with their services and they usually satisfied with physical caring, spiritual caring, hospice's family caring. The less difficulty in voluntary services, more satisfaction for the volunteers in the end. There is a positive correlation between a attitude of the hospice volunteer and frequency of voluntary services while the volunteers' difficulty affect negatively their attitude as well as the frequency of voluntary services. Most of the hospice volunteers want to have more education about basic skills, volunteer's attitude and role, spirituality, the way of attending on the hospice and so on. Most of them considered discussion and sharing different cases as the most effective method. They also wish to had more supporting programs for the hospice volunteers(in the order of their needs) such as regular events, newsletters, personal concern, social meetings among the volunteers, insurance and minimum expenses. Based on the study results, more programs should be run in order to activate the voluntary services regardless of their gender and education background. A continuous practical supporting policy and education programs are required in order to provide special education and training courses considering every field of voluntary services. Expanding the role of the volunteers allows them more opportunities to take part in voluntary services and th activate participation. It is necessary to establish a new hospice system as a part public medical system, which can not only facilitate the voluntary services for a hospice but also enhance professional hospice volunteers. Finally, experts are needed to operate the voluntary services effectively.

중환자실 환자의 간호지속성 유지를 위한 중환자실-병동간 연계형 전환간호 프로그램 개발 및 효과 (Development and Effects of the Collaborative Transitional Care Program for Continuity of Care in Patients Transferred to General Wards from ICUs)

  • Purpose: This study was conducted to develop and evaluate the collaborate transitional care program for improving continuity of care in patients transferred to general wards from ICUs. Methods: 18 years and older who were hospitalized in adult intensive care units at A university affiliated medical center was recruited for the study. The experimental group for patients transferred from an ICU consisted of 33 patients and family caregivers; 34 patients and family caregivers for the control group. This study was utilized a quasi-experimental research design. The collaborative transitional care program was administered in transfer process. Data were collected two times by interviews, medical records, and telephone using questionnaires. Results: There were statistically significant differences between the two groups on relocation stress (p<.001), perceived health status (p<.001), satisfaction of caring (p=.011), physical domain (p=.022) and mental domain (p<.001) of the QOL. There were significant differences between the families of the two groups on burden (p<.001) and satisfaction of caring (p<.001). Conclusion: The collaborative transitional care programs administered in transfer process to general wards from an ICU have positive effects on patients and families' intrinsic and extrinsic factors. This program will be able to be utilized in clinical fields to improve continuity of care for patients and families between ICUs and general wards.

The association of physicians' caring attitude with patient satisfaction: an analysis of the national e-survey data

  • 환자의 만족도는 더 나은 의료서비스 결과물을 위한 중요 요소이기에, 환자만족도와 관련 요인에 대한 연구는 그 필요성이 대두하고 있다. 따라서, 본 연구는 의사의 환자를 향한 공감적(empathetic) 태도와 같은 잠재적 요인이 환자만족도와 어떤 연관성을 가졌는지를 분석했다. 이를 위해, 편의표본추출법을 통한 환자설문조사 자료 중 총 273,994 개의 사례들을 분석한 단면연구가 수행되었다. 연구의 독립변수 및 종속변수들은 각각 의사의 태도 및 의사와 진료소(office setting or clinic) 관련 환자만족도로 정의되었다. 연구결과에 의하면, 의사와 진료소 관련 환자만족도는 각각 100점 만점에 평균 78.08과 78.62로 나타났고 상응하는 표준편차는 각각 0.14와 0.12였다. 또한, 의사의 태도가 의사와 진료소 관련 환자만족도와 유의한 관계에 있음을 확인하였다(p < 0.001). 의료에 대한 환자만족도를 높이기 위해서는, 의사의 공감적 의사소통 능력개발을 위한 지속적인 노력이 기울여져서 환자에 의한 의사의 공감적 태도가 인지되어 질 수 있기를 본 연구는 제언한다.

중년기 남성의 가족 및 직업관련 변인과 생활만족도의 관계 (The relationships between Middle-Aged Men's Family and Work related variables and satisfaction)

  • The purpose of this study was to investigate the relationships of middle-aged men's family and work related variables and satisfaction. Based on the results of the study, the following was concluded ; First, there were significant differences in men's satisfaction across education level, economic variables, and occupation. Second, family related variables, work related variables, and work-family conflict were significantly correlated with satisfaction. In reference to family related variable, satisfaction was higher when marital satisfaction is good, parental stress, adult caring burden, and work-family conflict are lower. In reference to work related variables, satisfaction is higher when job stress is lower, job security is lower, and colleague relationship is good. Third, family related variables, work related variables, and work-family conflict were significantly correlated with mutually one another. Specifically, parental stress is correlated with work related variables. A higher correction was reported between work-family conflict and job stress. Finally, results of multiple regression analysis showed that these variable influenced middle-aged men's satisfaction. Moreover, the degree of explanation for satisfaction increased more, when family and work related variables, and work-family conflict add in individual character variables.

외조모의 손자녀 양육지원에 관한 탐색적 연구: 외조모와 어머니의 보고 중심으로 (An Exploratory Study on Child Rearing Grandmothers: Based on the Interview of Grandmothers and Mothers)

  • Objective: The purpose this study is to explore the relationship between grandmothers and mothers when grandmothers take care of their grandchildren, and to understand the particular meaning of it. Methods: The Participants were 5 grandmothers who have supported or have experience in caring for their grandchildren, and their daughters(5 working mothers) who have entrusted their children to them. In-depth interviews were conducted and analysed by the phenomenological qualitative research method. Results: Seventeen sub-constructs and 5 constructs resulted in: (1) Motivation of grandmothers and mothers for taking care of grandchildren. (2) Pressure of grandmothers and satisfaction of mothers for the support. (3) Grandmothers stepping back and saying 'Not my child' and mothers saying 'It's my child' and thinking their influence is big. (4) Consistent sacrifice of grandmothers and mothers who cannot do the same. (5) Grandmothers feeling empty after caring for their grandchildren and mothers realizing their efforts. Conclusion/Implications: This study attempted to understand the relationship between grandmothers and mothers when grandmothers take care of their grandchildren from multi-perspectives. Through the findings, the need for developing healthy relationships and a caring environment for grandmothers and their grandchildren is stressed.

Effectiveness of Simulation-Based Education for Caring Patients with COVID-19

  • Purpose: The role of medical staff gained immense significance in the context of the prolonged coronavirus disease (COVID-19) pandemic. However, few studies had explored the impact of simulation-based education on the ability of nursing students to care for the patients of COVID-19. This study provided nursing students with simulation-based education in caring for the patients of COVID-19 and confirmed its effectiveness. Methods: This study used a non-equivalent control group pretest-posttest design. The participants were recruited from the nursing departments of two universities in Korea through convenience sampling. A total of 79 participants were included: 37 in the intervention group and 42 in the control group. The intervention group received four sessions of simulation training based on the National League for Nursing Jeffries simulation theory. Results: The intervention group showed an improvement compared to the control group in terms of knowledge related to coronavirus, confidence in performing infection control skills, and perception of preparedness for caring for the patients of COVID-19, with a high-level of satisfaction and self-confidence in learning. There was no significant difference between the two groups in terms of anxiety. Conclusion: This simulation is expected to be a significant strategy for alleviating the global burden in terms of staff safety and patient outcomes by improving the competencies of prospective medical staff in responding to pandemics.

청소년의 배려와 나눔 실천을 위한 측정도구 개발 연구 (Measurement Tools for the Practice of Caring and Sharing by Adolescents)

  • This study develops a scale to measure the practice of consideration and sharing by adolescents. This data as an object of high school students in regards to items generated from the content validity test and preliminary study stage based on the consideration scale, literature review and expert for elementary school students on a 5-point Likert scale. The factor analysis of the collected data using SPSS 20.0 2 indicated the final fators. The three fanal factors designated by the measuring items are: 'practice of consideration and sharing', 'recognition of consideration and sharing', and 'self-recognition'. As a result of analyzing students' individual variables, this study found that family relationship satisfaction and school life satisfaction had positive relationship in self-recognition, recognition of consideration and sharing, practice of consideration and sharing, and total personality score. The measurement tools developed in this study, are expected to act as valid tools to develop a program that can increase the practice of consideration and sharing activities.

노인장기요양보험에서 1등급 노인과 3등급 치매노인을 케어하는 요양보호사의 직무스트레스와 직무만족에 관한 비교연구 (A Comparative Study on Job Stress and Job Satisfaction of Long-Term Care Staffs for Physically Disabled Seniors with Rating 1 and for Demented Seniors with Rating 3)

  • 본 연구는 노인장기요양시설에 입소한 노인 중 1등급 노인과 3등급 치매노인을 케어하는 요양보호사들의 직무스트레스(전체, 서비스대상 관계스트레스, 서비스제공 스트레스)를 비교하고 직무만족의 하위변인(대우만족, 상사관계 만족, 동료관계만족, 성장만족)에 영향을 미치는 변수를 알아보기 위한 것으로, 경기북부 권 파주, 고양, 의정부시의 노인장기요양시설에 근무하는 요양보호사 212명을 대상으로 이루어졌다. 독립표본 t-검증과 다중회귀분석을 실시한 결과는 다음과 같다. 첫째, 두 집단(1등급 노인을 케어하는 요양보호사 집단, 3등급 치매노인을 케어하는 요양보호사 집단)의 직무스트레스는 '직무스트레스 전체', '서비스대상 관계스트레스', '서비스제공 과정스트레스'에서 통계적으로 유의미한 차이를 보였다. 둘째, 집단별 직무만족에 직무스트레스 하위변인이 어떻게 영향을 미치는지 회귀 분석을 실시한 결과 오직 3등급 치매노인을 케어하는 요양보호사 집단에만 직무스트레스가 '대우만족', '동료관계만족'에 영향을 미치는 예측변수로 나타났다. 구체적으로 '대우만족'에는 '과정스트레스'가, 그리고 '동료관계만족'에는 '관계스트레스', '과정스트레스', 그리고 '학력'이 유의한 변수로 나타났다. 본 연구의 의의는 기존 선행연구에서 다루지 못한 시설 내 요양보호사의 직무스트레스의 차이와 직무만족의 하위변인에 영향을 미치는 예측변수를 비교·분석한 연구의 시작이라는 것이며, 비록 같은 시설 내 근무하는 요양보호사라 할지라도 그들이 케어하는 노인의 등급과 상태에 따라 직무스트레스에 차이가 존재한다는 것을 주목할 필요가 있다. 그리고 직무만족의 하위변인에 영향을 주는 중요한 예측변수로 발견된 서비스대상 관계와 서비스제공 과정에서 발생하는 스트레스를 고려한 근무환경 개선과 스트레스를 경감시킬 수 있는 방안의 모색이 필요하다.

간호 대학생의 기본간호 임상실습에 관한 자율성, 교수효율성 및 임상실습 만족도 (Autonomy, Teaching Effectiveness, and Clinical Practice Satisfaction for Fundamentals Nursing Clinical Practice in Student Nurses)

  • Purpose: The purpose of this study was to investigate autonomy, teaching effectiveness, and clinical practice satisfaction for fundamentals nursing clinical practice in student nurses. Method: The participants were 244 sophomores, who had done the practice for 8 weeks from June 17 ${\sim}.33$ August 23, 2005. Autonomy the Caring Perspective(ACP) devised by Boughn(1995) was used to measure autonomy, Instrument to Measure Effectiveness of Clinical Instructor(IMECL) by Reeve(1994) for measuring teaching effectiveness, and an instrument by Moon(2002) for measuring satisfaction. Results: After practice, the role model was seen as rich in knowledge and experience(48.77%). Desirable categories for instructor were full-time professors with a practice background and head nurses(31.97%). Average score for autonomy was 3.71(${\pm}.33$), for teaching effectiveness, 3.67(${\pm}.48$) and for satisfaction 3.51(${\pm}.38$). Autonomy scores were high for students satisfied with their major(F=5.23, p=.006), and interested In practice(F=4.38, p=.014). Teaching effectiveness scores were high for students satisfied with practice (F=2.57, p=.038). Clinical practice satisfaction scores were high for students interested in practice(F=5.01, p=.007). Relationships between autonomy and teaching effectiveness (r=.174, p=.006), and between leaching effectiveness and satisfaction showed a positive correlation(r=.632, p=.000). Conclusion: Interest in clinical practice courses in first year affect autonomy, teaching effectiveness and satisfaction.
