성인 흡연자에서 심근경색증 조기 증상 인지와 관련된 요인:2021 지역사회 건강조사 자료 활용 (Factors associated with early recognition of myocardial infarction in adult smokers: Using data from 2021 Community Health Survey)

  • 본 연구는 성인 흡연자의 심근경색증 조기 증상 인지 정도를 파악하고 인지와 관련된 요인을 분석하기 위해 진행되었다. 2021년 지역사회 건강조사 원시 자료를 활용한 서술적 조사연구로 조사 완료자 229,942명 중 본 연구에 해당하는 설문에 응답이 불충분한 대상자 18,343명을 제외한 210,899명을 연구대상자로 선정하여 SAS 9.4 program을 이용하여 표본 분석, 빈도, 백분율 등의 기술통계, 카이 제곱 검정, 복합표본 로지스틱 회귀분석을 사용하였다. 나이, 혼인 여부, 교육수준, 금연계획 여부, 건강검진_암 검진 수검 여부, 당뇨병 진단 여부, 주관적 건강 수준, 사회 물리적 환경이 심근경색증 조기 증상 인지와 유의한 관련이 있었다. 연구 결과를 바탕으로 심근경색증 발생 위험이 높은 고위험 집단에 대한 적극적인 홍보 및 교육이 필요하다.

3T3-L1 지방 전구세포의 분화 및 지방 생성에 미치는 Fragaria Orientalis L. 물 추출물의 영향 (Effects of Fragaria Orientalis Water extract on Adipogenesis and Cell Differentiation in 3T3-L1 Cells)

  • Objective : Obesity, which has recently been rapidly increasing in the obese population, is caused by an imbalance in energy intake and consumption. The reason why we need to manage obesity well is that the prevalence of complications such as diabetes, atherosclerosis, insulin resistance, and cardiovascular disease increases. In this study, the effect of FO (Fragaria orientalis) water extract on fat metabolism in 3T3-L1 cells was observed to develop a new anti-obesity material based on Mongolian medical books. Methods : The effect of FO extract on adipogenesis in 3T3-L1 cells was observed using DPPH scavenging, pancreatic lipase inhibitory activity, MTT analysis and Oil-red-O staining method. And the expression of proteins related to lipid metabolism was analyzed by Western blot. Results : The FO group significantly increased the DPPH radical scavenging activity at 5 mg/ml compared to the positive control BHA at 0.1 mg/ml. In oil red O staining at a safe concentration without cytotoxicity, lipid accumulation was significantly inhibited by less than 80% compared to the control group at all concentrations. Moreover, treatment of FO significantly increased the expression of proteins related to lipid metabolism, such as p-AMPK and p-ACC, in 3T3-L1 cells, and the expression of CPT-1 tended to increase in a dose-dependent manner. However, the expression of PPAR-γ was significantly decreased in a dose-dependent manner. Conclusion : These results suggest that FO water extract has a potential anti-obesity effect and are expected to be utilized in the development of materials for obesity prevention and treatment.

Clinical Midterm Results of Surgical Aortic Valve Replacement with Sutureless Valves

  • Background: Sutureless aortic valves may enable shorter procedure times, which benefits patients with elevated surgical risk. We describe the outcomes of patients with aortic stenosis who underwent aortic valve replacement (AVR) using the sutureless Perceval aortic bioprosthesis. Methods: Data from a retrospective cohort were obtained from a clinical database. The study enrolled patients with symptomatic severe aortic stenosis who underwent surgical AVR with a sutureless bioprosthesis between August 2015 and December 2020. In total, 113 patients were included (mean age, 75.3±8.4 years; 57.5% women; median Society of Thoracic Surgeons score, 9.7%; mean follow-up period, 51.19±20.6 months). Of these patients, 41 were octogenarians (36.2%) and 3 were nonagenarians (2.6%). Transthoracic echocardiography was employed to assess changes in ejection fraction (EF), left ventricular mass index (LVMI), and mean pressure gradient (MPG). Results: The in-hospital mortality rate was 2.6%, and 13 patients developed new-onset atrial fibrillation. A permanent pacemaker was implanted in 3 patients (2.6%). The median intensive care unit stay was 1 day (interquartile range [IQR], 1-2 days), and the median hospital stay was 12 days (IQR, 9.5-15 days). The overall survival rate at 5 years was 95.9%. LVMI and MPG were reduced postoperatively, while EF increased over the follow-up period. No structural valve deterioration was observed, and no meaningful paravalvular leakage developed during follow-up. Conclusion: The use of a sutureless valve in the aortic position is safe and feasible, even for high-risk elderly patients requiring surgical AVR. LVMI and MPG decreased postoperatively, while EF increased over the follow-up period.

Arterial stiffness index, physical activity and food and nutrient intake: cross-sectional study in adults aged 40 years and older

  • Objectives: This study aimed to investigate arterial stiffness index, physical activity, and food and nutrient intake in middle-aged adults over 40 years when the incidence of cardiovascular disease begins to increase. Methods: This study included 106 subjects (48 males and 58 females) aged between 40 and 64 years. The arterial stiffness index (brachial-ankle pulse wave velocity [baPWV], and ankle-brachial index [ABI]) were measured using a blood pressure pulse wave testing device. Physical activity was assessed using the Korean version of the Global Physical Activity Questionnaire, and food and nutrient intake was calculated using the Food Frequency Questionnaire. Results: The mean age of the subjects was 54.4 years. Although the ABI of the subjects was within the normal range, they were divided into tertiles to compare physical activity and food and nutrient intake. In males, the time spent on moderate to vigorous physical activity (MVPA) was significantly higher in T3 (600.6 min/week) than in T1 (304.4 min/week). In females, the time spent in sedentary behavior was significantly lower in T3 (294.5 min/week) than in T1 (472.1 min/week). In addition, the frequency of fish consumption was significantly higher in T3 (1.27 frequency/day) than in T1 (0.64 frequency/day) in females. Polyunsaturated fatty acid (PUFA) and ω-3 fatty acid intake, adjusted for energy intake, were significantly positively correlated with ABI (r = 0.200 and r = 0.218, respectively). Conclusions: High MVPA (in males), low sedentary behavior (in females), and PUFA and ω-3 fatty acid intake through fish consumption may be associated with low peripheral artery stiffness. Therefore, arteriosclerosis can be prevented through physical activity and proper dietary therapy.

Differential Chemokine Signature between Human Preadipocytes and Adipocytes

  • Obesity is characterized as an accumulation of adipose tissue mass represented by chronic, low-grade inflammation. Obesity-derived inflammation involves chemokines as important regulators contributing to the pathophysiology of obesity-related diseases such as cardiovascular disease, diabetes and some cancers. The obesity-driven chemokine network is poorly understood. Here, we identified the profiles of chemokine signature between human preadipocytes and adipocytes, using PCR arrays and qRT-PCR. Both preadipocytes and adipocytes showed absent or low levels in chemokine receptors in spite of some changes. On the other hand, the chemokine levels of CCL2, CCL7-8, CCL11, CXCL1-3, CXCL6 and CXCL10-11 were dominantly expressed in preadipocytes compared to adipocytes. Interestingly, CXCL14 was the most dominant chemokine expressed in adipocytes compared to preadipocytes. Moreover, there is significantly higher protein level of CXCL14 in conditioned media from adipocytes. In addition, we analyzed the data of the chemokine signatures in adipocytes obtained from healthy lean and obese postmenopausal women based on Gene Expression Omnibus (GEO) dataset. Adipocytes from obese individuals had significantly higher levels in chemokine signature as follows: CCL2, CCL13, CCL18-19, CCL23, CCL26, CXCL1, CXCL3 and CXCL14, as compared to those from lean ones. Also, among the chemokine networks, CXCL14 appeared to be the highest levels in adipocytes from both lean and obese women. Taken together, these results identify CXCL14 as an important chemokine induced during adipogenesis, requiring further research elucidating its potential therapeutic benefits in obesity.

3차원 맥영상 검사로 살펴본 천왕보심단이 심혈관계에 미치는 영향 (Hemodynamic effects of Chunwangbosim-dan - A 3-dimensional radial pulse tonometry device study)

  • Objectives: This study was aimed to evaluate the hemodynamic feasibility using pulse parameters as a way to establish safe dose guidelines for Chunwangbosim-dan, and to provide a foundation for developing evidence-based guidelines for clinical use. Methods: Forty-one volunteers were recruited to participate in a study examining the changes in pulse wave characteristics following the ingestion of Chunwangbosim-dan, over a period of 2 weeks, and pulse wave measurements were taken before and after the administration. Pulse wave parameters were measured in this study using a 3-dimensional radial pulse tonometry device(DMP-Lifeplus). In addition, questionnaire, blood pressure, temperature, and body composition were also measured as secondary measures. Results: Fifteen minutes after administration of Chunwangbosim-dan, the non-adverse event group(non-AE) exhibited a statistically significant increase in several power and pressure-related parameters, including h1, h3, h4, h5, SA, PA and PW, while the adverse event group(AE) showed a trend of decreasing stroke volume and increasing Systemic Vascular Resistance Index(SVRI) and applied pressure. After 2 weeks of administration, non-adverse event group(non-AE) exhibited significant changes in standard deviation of pulse rate and HRV_LH ratio. Notably, there are significant differences between AE group and non-AE group in h4/h1, w/t, applied pressure, SV and pulse rate. Conclusion: These findings suggest that pulse parameters may be a useful way to establish safe dosing guidelines for Chunwangbosim-dan. Further research is needed to confirm these results and to develop evidence-based guidelines for clinical use.

Lipopolysaccharide로 활성화된 RAW264.7세포에서 염증반응사이토카인 발현에 대한 Epigallocatechin gallate의 억제효능연구 (Suppressive Effects of Epigallocatechin Gallate Pretreatment on the Expression of Inflammatory Cytokines in RAW264.7 Cells Activated by Lipopolysaccharide)

  • Epigallocatechin gallate (EGCG)는 녹차(Camellia sinensis, green tea)에 포함되어 있는 대표적인 카테킨(Catechin)으로, 당뇨(diabetes), 신경퇴행성 질환(neurodegenerative disorders), 암(cancer), 심혈관계 질환(cardiovascular disease), 비만(obesity) 등의 다양한 임상질환에 우수한 치료효과를 나타내는 것으로 알려왔다. 본 연구에서는 EGCG가 LPS (Lipopolysaccaride)로 자극된 RAW264.7 세포의 염증억제과정에서 Nitric oxide (NO)관련인자와 사이토카인 발현에 미치는 영향을 분석하기 위해, NO농도, inducible NO synthase (iNOS) 발현량, TNF-α, IL-1β, IL-6, IL-10 사이토카인의 발현량을 분석하였다. 먼저, EGCG (100-400 μM)를 처리한 Raw264.7 세포에서 특이적인 독성이 나타나지 않음을 확인하였으며, 이후 Raw264.7 세포에 4가지 다른 농도(0.1, 0.5, 1.0, 5.0 μg/ml)의 LPS를 처리한 후 MTT분석, NO농도분석, IL-6농도분석을 실시하여, 염증을 유발할 수 있는 최적 LPS 농도를 1 μg/ml로 설정하였다. NO농도를 분석한 결과, LPS를 처리한 그룹이 No그룹에 비하여 급속히 증가하여 63 μmol/l까지 증가하였으나 400 μM EGCG를 처리한 그룹에서만 68.2% 정도 감소하였으며, 동시에 iNOS 발현 양상은 200, 400 μM EGCG/LPS 처리 그룹에서 각각 12.3%, 17.4% 감소하였다. 또한, 감소비율에는 차이가 있었으나, anti-inflammatory 사이토카인(IL-1β, TNF-α)과 pro-inflammatory 사이토카인(IL-10)의 발현은 EGCG/LPS 처리에 따라 유의적으로 감소하였다. 그러나, IL-6단백질과 mRNA 농도는 Vehicle/LPS 처리 그룹과 EGCG/LPS 처리 그룹에서 유의적인 차이를 나타내지 않았다. 따라서, 본 연구의 결과는 EGCG가 LPS의 자극에 의해 활성화된 Raw264.7 세포를 효과적으로 억제하는 효능을 나타냄을 제시하고 있으며, 이러한 과정에 사이토카인들이 서로 다르게 특이적 반응을 중계함을 제시하고 있다.

비소세포성폐암의 수술후 MVP복합화학요법과 방사선병용치료 결과 (Postoperative Adjuvant MVP Chemotherapy and Radiotherapy for Non-Small Cell Lung Cancer)

  • Purpose : Since February 1991 a Prospective study for non-small cell lung cancer patients who underwent radical resection and had a risk factor of positive resection margin or regional lymph node metastasis has been conducted to evaluate the effect of MVP chemotherapy and radiotherapy on the pattern of failure, disease free and overall survival. and tolerance of combined treatment. Materials and Methods: Twenty nine patients were registered to this study until Sep. 1993; of these 26 received planned therapy Within 3 weeks after radical resection, two cycles of MVP(Mitomycin C $6mg/m^2,$ Vinblastin $6mg/m^2,$ Cisplatin $60mg/m^2$) chemotherapy was given with 4 weeks intervals. Radiotherapy (5040cGy tumor bed dose and 900cGy boost to high risk area) was started 3 to 4 weeks after chemotherapy. Results: One and two year overall survival rates were $76.5\%\;and\;58.6\%$ respectively. Locoregional failure developed in 6 patients$(23.1\%)$ and distant failure in 9 patients$(34.6\%)$ Number of involved lymph nodes, resection margin positivity showed some correlation with failure pattern but T-stage and N-stage showed no statistical significance. The group of patients who received chemotherapy within 2 weeks postoperatively and radiotherapy within 70 days showed lower incidence of distant metastasis. Postoperative combined therapy were well tolerated without definite increase of complication rate, and compliance rate in this study was $90\%$. Conclusion: 1) MVP chemotherapy showed no effect on locoregional recurrence, but appeared to decrease the distant metastasis rate and 2) combined treatments were well tolerated in all patients. 3) The group of patients who received chemotherapy within 2 weeks postoperatively and radiotherapy within 70days showed lower incidence of distant metastasis. 4) Addition of chemotherapy to radiotherapy failed to increase the overall or disease free survival.

No association between endothelin-1 gene polymorphisms and preeclampsia in Korean population

  • 목 적 : 자간전증은 임신부와 신생아의 사망 및 이환의 주된 원인으로 유전적 소질과 환경적 요인에 의해 발생하는 다요인성 질환이다. ET-1은 강력한 혈관수축 펩티드로 ET-1 시스템 내의 변경이 자간전증에서의 혈관수축을 자극하는 것으로 생각되고 있다. 본 연구에서는 정상 임신부와 자간전증 임신부에서 ET-1 유전자의 4가지 단일염기다형성들(c.1370T>G, c.137_139delinsA, c.3539+2T>C, and c.5665G >T)의 양상을 조사하여 비교함으로써 이러한 유전자 다형성들과 한국인 자간전증의 연관성에 대하여 알아보고자 하였다. 방 법 : 자간전증 임신부 206명과 임신기간 동안 자간전증이 발생하지 않은 정상 임신부 216명의 혈액으로부터 DNA를 추출하고 ET-1 유전자 다형성들의 양상을 SNapShot kit와 ABI Prism 3100 Genetic analyzer를 사용하여 분석하였다. 결 과 : 자간전증 환자군에서 ET-1 유전자의 4가지 단일 염기다형성 각각의 유전자형 및 대립유전자의 빈도는 대조군과 유의한 차이가 없었다. 또한 자간전증 환자군에서 3가지 일배체형(TDTG, GDCT, and TICT)의 빈도도 대조군과 유의한 차이가 없었다: 61%(TDTG), 13%(GDCT), 13%(TICT) vs. 62%, 14%, 12%. 나이, 미산부률, 분만주수, 신체질량지수 등의 자간전증 발생요인을 공변량으로 하여 유전자형과 자간전증 발생의 위험도 사이의 연관성을 확인하기 위해 다중회귀분석을 시행한 결과 열성모델과 우성모델에서 모두 ET-1 유전자의 단일염기다형성들에 대한 자간전증 발생의 위험도가 증가하지 않았다. 결 론 : 한국인 임신부에서 ET-1 유전자의 4가지 단일염기 다형성들은 자간전증 발생과 연관이 없는 것으로 사료된다.

Intercellular Adhesion Molecule-1 Gene Polymorphism (K469E) in Korean Preeclamptic Women

  • 목 적: 자간전증은 임신 특이적 질환으로, 면역 반응 관련 결합 요소의 증가에 의한 광범위한 혈관내피 기능손상을 나타낸다. 자간전증을 이끄는 생태병리학적 기전에 관여하는 세포내부 결합요소-1 (intracellular adhesion molecule-1; ICAM-1)은 면역반응의 시작을 유도하고, 그것의 혈장 정도와 기능은 유전적으로 영향을 받는다. 그러므로 우리는 이번 연구에서 한국인 자간전증 임산부에서 ICAM-1의 유전자 다형성 부위인 K469E의 분포를 평가하고 자간전증과 이 유전자 다형성 사이의 연관성을 조사하였다. 대상 및 방법: 이 유전자 다형성은 SNapShot kit와 ABI Prism 3100 Genetic analyzer를 사용하여 197명의 자간전증임산부와 193명의 정상 임산부의 말초 혈액에서 분석하였다. 결 과: ICAM-1 유전자 다형성인 K469E의 유전자형과 대립유전자 빈도는 자간전증 임산부와 정상 임산부 사이에 차이가 없었다. 유전자형인 KK, KE, EE는 자간전증 임산부에서 각각 40.6%, 43.7%, 15.7%였고, 정상 임산부에서 38.9%, 45.1%, 16.1%였다. 그리고 K와 E 대립유전자 빈도는 자간전증 임산부에서 각각 0.62, 0.38이었고 정상 임산부에서 0.61, 0.39였다. 다중회귀분석결과에서 자간전증 발생 위험도는 K469E의 이종접합 유전자형 (KE)이나 돌연변이 유전자형(EE)을 수반하는 그룹에서 증가되지 않았다. 결 론: 이 연구는 ICAM-1 유전자 다형성인 K469E가 한국인 임신부의 자간전증 발생과 연관이 없음을 나타낸다.
