Bisphenol A Exposure and Childhood Obesity (Bisphenol A 노출과 소아비만)

  • Over the a few decades, the incidences of obesity and associated metabolic syndrome diseases have been increased dramatically and resulted in a global health crisis. Recent findings suggest endocrine disrupting chemicals (EDCs) as an obesogen, because they disrupt normal development and homeostatic controls over adipogenesis and energy balance in obesity. Furthermore, risk of cardiovascular disease and mortality is elevated among those who were obese during childhood. Thus, we focused on etiology of obesity in children and performed biological monitoring of bisphenol A (BPA), which is a broadly exposed EDC in environment. Study subjects were age and sex-matched obese and normal children in Seoul (N=52; age, $8.67{\pm}1.46$ years). Exposure levels of BPA were analyzed with HPLC/FLD as a conjugated form in urine. As results, ranges of urinary BPA were 0~54.38 ${\mu}g/g$ creatinine (median, 4.57 ${\mu}g/g$ creatinine). Levels of urinary BPA were 1.7 fold higher in the obese children than those in the controls (medians of obese and control children, 7.31 and 4.25 ${\mu}g/g$ creatinine, respectively, p=0.22). In the near future, enlarge scaled studies should be performed to confirm the risk of BPA for obesity.

The Study of Relationship among Serum Leptin, Nutritional Status, Serum Glucose and Lipids of Middle-school Girls (일부 사춘기 여중생의 혈청 Leptin 함량과 영양소섭취상태 및 혈당, 혈청지질과의 상관관계 연구)

  • The purpose of this study was to investigate the relationship among urn leptin, nutritional status, and urn lipids, glucose of 99 middle school girls residing in Kyluggi-do, Korea. Subjects were assigned to one of three groups such as underweight(BMI < 20, n=38), normal(20$\leq$BMI<25, n=36) and over weight(BMI$\geq$25, n=25) groups. The nutritional status of the subjects was evaluated by anthropometric measurements, questionnaires, 24-hr dietary recalls and blood anal),sis. The results were as follows : The mean age, body weight, height and BMI were 13.93 years, 55.19kg, 158.40cm and 21.96kg/㎡ respectively. The mean menarche age of the subjects was 12.64 years and the mean daily energy intake was 94.22% off RDA for Koreans. The energy ratio from carbohydrate fat and protein was 63: 24: 13. Underweight group showed the lowest intakes of oils and fats among three groups(p<0.05). The mean serum levels of leptin, glucose, total cholesterol, HDL-cholesterol. LDL-cholesterol and triglyceride were 10.57ng/ml 78.80mg/dl, 12.29mg/dl, 48.07mg/dl, 93.50mg/dl and 78.61mg/d1, respectively. The mean atherogenic index was 2.37. There were significant increases in serum leptin, glucose, LDL-cholesterol, triglyceride and atherogenic index of subjects with increases in their BMIs. Serum leptin showed a significantly positive correlation with BMI(p<0.001), body fat percent(p<0.001). When BMI was adjused, serum leptin showed a significantly positive partial correlations with intakes of animal protein(p<0.05), milks and animal foods(p<0.05), Serum leptin showed a significantly positive correlation with serum glucose(p<0.05) total cholesterol(p<0.01), LDL-cholesterol(p<0.001), triglyceride(p<0.001) and atherogenic index(p<0.001), however, it had significantly negative correlations with HDL-cholesterol(p<0.05). Therefore, these results suggest that the increase of serum levels of leptin may elevate the risk of cardiovascular disease and lipids and some nutrient intakes may be related with serum leptin levels. The Effect of Nutrition Education and Excercise Program on Body Composition and Dietary Intakes, Blood Lipid and Physical Fitness in Obese Women (영양교육과 운동중재 프로그램이 성인비만여성의 신체성분과 식이섭취, 혈중지질 및 기초체력에 미치는 효과)

  • This study investigated the effects of the nutrition education and exercise program on body composition, dietary intakes and physical fitness in obese women. The subjects were 44 obese women (BMI${\geq}$ 25 kg/$m^2$) who had been participating in nutrition education (1 time/week) and exercise program (3 time/week) for 12 weeks. Nutrition education was focused on low energy, high protein and low carbohydrate diet to reduce the body weight and improve the diet quality. To evaluate the effectiveness of the program, daily nutrient intakes were assessed by 24-hour recall method. Body composition, blood lipid profiles and physical fitness test were assessed before and after the intervention. After the intervention, body weight, BMI, percent body fat, soft lean mass and waist/hip ratio were significantly decreased (p < 0.001). Fasting blood sucrose, total cholesterol, LDL-cholesterol and atherogenic index (AI) were significantly decreased (p < 0.001), while HDLcholesterol was significantly increased (p < 0.001). Energy adjusted protein, fiber, vitamin A, vitamin $B_2$, vitamin $B_6$, vitamin C, vitamin E, niacin and folate were significantly increased. After the intervention, the muscle endurance, muscle strength, agility, balance were significantly increased. The changes in obesity indices (body weight, BMI, percent body fat, waist-hip ratio) were correlated with the changes of the nutrient intakes, serum lipid profiles, physical fitness. These results show that nutrition education and exercise program was effective not only for weight reduction but also for the improvement of physical strength and cardiovascular disease risk factors in obese women.

Bradycardia after Dobutamine Administration in a Dog (Dobutamine 투여 후 발생한 개의 서맥 1례)

  • A 13-year-old, castrated male, Shih Tzu dog with a history of acute ataxia was referred to veterinary medical teaching hospital and anesthetized for diagnostic magnetic resonance imaging of cervical intervertebral disk disease. After preanesthetic evaluation including physical examination, blood chemistry, radiography and ultrasound, the patient was premedicated with intravenous butorphanol (0.2 mg/kg). Anesthesia was induced by intravenous propofol (6 mg/kg) and maintained with isoflurane at 1.2 minimal alveolar concentrations. Because the mean arterial pressure (MAP) decreased from 70 to 58 mmHg at 70 minutes after induction, dobutamine was administered by constant rate infusion ($5{\mu}g/kg/min$) to treat hypotension. However MAP did not increase, and heart rate rapidly decreased from 100 to 55 beats per minute (bpm). To treat bradycardia, intravenous glycopyrrolate ($5{\mu}g/kg$) was administered, and heart rate increased to 165 bpm. After extubation of endotracheal tube, the patient showed normal recovery without any problems related to cardiovascular system. Unexpected dobutamine-induced bradycardia was considered as Bezold-Jarisch reflex. It is recommended that clinicians know and prepare the possibility of bradycardia during dobutamine therapy under general anesthesia.

Aortic Root Replacement with Valve Preservation in a Patient with Annuloaortic Ectasia (대동맥판을 보존한 대동맥근부치환술 - 증례보고 -)

  • Patients with aortic root disease, frequently seen in Marfan syndrome have progressive dilatation of the aortic sinuses and dilatation and distortion of the aortic annulus, leading to aortic incompetence. They are currently treated with composite graft replacement of the ascending aorta and aortic valve and reimplantation of the coronary arteries. Recently, we experienced an aortic root replacement with aortic valve preservation in a patient with annuloaortic ectasia. The ascending aorta and sinus was excised except the aortic annulus and aortic valve. The aortic valve was reimplanted inside of a collagen-impregnated tubular Dacron graft. The coronary arteries were also reimplanted. The patient was followed up for six months and reevaluated with the echocardiography. Postoperative Doppler echocardiography revealed normal aortic valve function. With this technique, it is possible to preserve the native aortic valve if the aortic leaflets are anatomically normal.

Factors Associated with Early Adverse Events after Coronary Artery Bypass Grafting Subsequent to Percutaneous Coronary Intervention

  • Background: A previous percutaneous coronary intervention (PCI) may affect the outcomes of patients who undergo coronary artery bypass grafting (CABG). The objective of this study was to compare the early in-hospital postoperative outcomes between patients who underwent CABG with or without previous PCI. Methods: The present study included 160 patients who underwent isolated elective on-pump CABG at the department of cardiothoracic surgery, Minia University Hospital from January 2010 to December 2014. Patients who previously underwent PCI (n=38) were compared to patients who did not (n=122). Preoperative, operative, and early in-hospital postoperative data were analyzed. The end points of the study were in-hospital mortality and postoperative major adverse events. Results: Non-significant differences were found between the study groups regarding preoperative demographic data, risk factors, left ventricular ejection fraction, New York Heart Association class, EuroSCORE, the presence of left main disease, reoperation for bleeding, postoperative acute myocardial infarction, a neurological deficit, need for renal dialysis, hospital stay, and in-hospital mortality. The average time from PCI to CABG was $13.9{\pm}5.4$ years. The previous PCI group exhibited a significantly larger proportion of patients who experienced in-hospital major adverse events (15.8% vs. 2.5%, p=0.002). On multivariate analysis, only previous PCI was found to be a significant predictor of major adverse events (odds ratio, 0.16; 95% confidence interval, 0.03 to 0.71; p=0.01). Conclusion: Previous PCI was found to have a significant effect on the incidence of early major adverse events after CABG. Further large-scale and long-term studies are recommended.

  • Naturally occurring substances which strongly scavenge peroxynitrite(ONOO-) are evaluated to be beneficial for cardiovascular, diabetic, hypercholesterolemic and obese disease. To search for bioactive substances with peroxynitritescavenging activity from the young leaves of Heracleum moellendorffii(Umbelliferae) used for a mountainous vegetable, the experiment on peroxynitrite-scavenging assay, phytochemical isolation and HPLC analysis were undertaken. Activity-guided fractionation led to the separation of astragalin and hyperoside which were identified by physicochemical and spectroscopic data. The $IC_{50}$ values of astragalin and hyperoside were shown as $5.80{\pm}0.62$ and $0.560{\pm}0.26\;{\mu}M$, respectively, in the assay. The HPLC analysis led to the quantitative determination of astragalin and hyperoside by $30.0{\pm}0.01$ and $17.0{\pm}0.02\;mg/g$ dried weight, respectively.

Isolation and Biological Activity of $Resveratrol-3-O-{\beta}-D-Glucoside$ in Transgenic Rehmannia glutinosa L. Transformed by Peanut Resveratrol Synthase Gene (RS3)

  • Resveratrol, which is both a phytoalexin with antifungal activity and a phytochemical associated with reduced cancer risk and reduced cardiovascular disease, is synthesized in a limited number of plant species including peanut. Resveratrol synthesis is catalyzed by the enzyme stilbene synthase including resveratrol synthase (RS). Resveratrol synthase gene (RS3) obtained from peanut, Arachis hypogaea, Fabaceae has been transferred into chinese foxglove, Rehmannia glutinosa by using Agrobacterium mediated transformation. RS t-DNA introduced to chinese foxglove (R. glutinosa L) by transformation and its reaction product, $resveratrol-3-O-{\beta}-D-glucoside$ was isolated and characterized using HPLC. Also its biological effects was tested in inhibition of the lipid peroxidation of mouse LDL by glycosylated stilbenes derivatives obtained from transgenic plants. $Resveratrol-3-O-{\beta}-D-glucoside$ isolated from transgenic R. glutinosa L. showed antimicrobial activity of the growth inhibition zone against Escherichia coli and Salmonella typhimurium. Therefore, this compound can be contributed to be useful as a phytoalexin for plant health as well as a phytochemical for human health.

Effect of Autophagy-Related Beclin1 on Sensitivity of Cisplatin-Resistant Ovarian Cancer Cells to Chemotherapeutic Agents

  • The purpose of the study was to determine the effects of autophagy related gene Beclin1 at different levels of expression on the sensitivity of cisplatin-resistant ovarian cancer cells (SKOV3/DDP) to different chemotherapeutics. In pSUPER-Beclin1 transfected cells, real-time fluorescence quantitative RT-PCR and Western blot analysis showed that expression was significantly inhibited. Flow cytometry revealed that the mean fluorescence intensity (MDC), reflecting autophagy, and cells in the G0/G1 phase were markedly reduced. When compared with the blank control group, inhibition of Beclin1 expression in SKOV3/DDP cells not only increased the rate of apoptosis following treatment with chemotherapeutics, but also increased the sensitivity. These findings suggest that Beclin1 expression plays an important role in chemotherapeutic agent-induced death of SKOV3/DDP cells. Inhibition of autophagy related gene Beclin1 expression in SKOV3/DDP cells may increase the rate of apoptosis and elevate the sensitivity to chemotherapeutics.

Primary Idiopathic Myelofibrosis: Clinico-Epidemiological Profile and Risk Stratification in Pakistani Patients

  • Background: Primary idiopathic myelofibrosis (PMF) is a clonal Philadelphia chromosome-negative myeloproliferative neoplasm characterized by extramedullary hematopoiesis and marrow fibrosis. It is an uncommon hematopoietic malignancy which primarily affects elderly individuals. The rational of this study was to determine its clinico-epidemiological profile along with risk stratification in Pakistani patients. Materials and Methods: In this retrospective cross sectional study, 20 patients with idiopathic myelofibrosis were enrolled from January 2011 to December 2014. Data were analyzed with SPSS version 22. Results: The mean age was $57.9{\pm}16.5years$ with 70% of patients aged above 50. The male to female ratio was 3:1. Overall only 10% of patients were asymptomatic and the remainder presented with constitutional symptoms. In symptomatic patients, major complaints were weakness (80%), weight loss (75%), abdominal discomfort (60%), night sweats (13%), pruritus (5%) and cardiovascular accidents (5%). Physical examination revealed splenomegaly as a predominant finding detected in 17 patients (85%) with the mean splenic span of $22.2{\pm}2.04cm$. The mean hemoglobin was $9.16{\pm}2.52g/dl$ with the mean MCV of $88.2{\pm}19.7fl$. The total leukocyte count of $17.6{\pm}19.2{\times}10^9/l$ and platelets count were $346.5{\pm}321.9{\times}10^9/l$. Serum lactate dehydrogenase, serum creatinine and uric acid were $731.0{\pm}154.1$, $0.82{\pm}0.22$ and $4.76{\pm}1.33$ respectively. According to risk stratification, 35% were in high risk, 40% in intermediate risk and 25% in low risk groups. Conclusions: The majority of PMF patients were male and presented with constitutional symptoms in our setting. Risk stratification revealed predominance of advanced disease in our series.
