A Study on Home Care Service and the Level of Client Satisfaction (일 대학병원의 가정간호서비스 내용 및 만족도 조사)

  • This study was done to examine the home care service provided by home care nurses and the level of client satisfaction. Data were collected from 110 clients who received the home care service at the P-hospital in Pusan from 23th April, 2001 to 30th November, 2002. The tool for measurement of satisfaction was composed of 16 items and was 4 score scale. Data was analyzed by using SPSS/WIN 10.5 program, the results of this study are summarized as follows; 1) 77.3% among 110 clients were over sixty years old. By the disease pattern. subjects were distributed into cancer(52.7%), cerebro-vascular disease(22.7%) and the others. 36.4% of clients were received home visits from 10 to 20 times by home care nurse. 2) The total number of home care services was 20,828. And most of the clients received the health education and training. 3) The mean score of satisfaction on provided home care services was $3.36\pm0.45$, out of 4. among 16 items. 'The home care nurses were kind enough' was highest ($3.59\pm0.49$), the total level of satisfaction of home care services was very high. with total mean score $53.84\pm7.16$. As mentioned above. the satisfaction level of home care services was very high. so we can except that the prospect of home care services is very challengeable. Therefore. we should try to expand the service recipients with promotional education to the home care clients under the cooperation with medical staff and make more efforts to develop the management system about clients' information. to improve the quality of the service, to assure close ties with the public health centers for the continuous home care service linking.

The Levels of Health Literacy and Related Factors among Middle-aged Adults in Seoul, Korea (서울 지역 일부 중년 성인의 건강정보이해능력(health literacy) 실태와 관련 요인에 대한 연구)

  • Objectives: This study was performed to identify the level of health literacy and to investigate the relationship between the health literacy and preventive health care use in middle-aged adults in Korea. Methods: A total of 315 adults aged 40 to 64 years living in Seoul participated in the study. Data were collected from December 15-24, 2010 from outpatient hospitals, clinics, workplaces and other locations in the community. Health literacy was measured using the Functional Health Literacy and Self-rated Health Literacy Questionnaires. Preventive health service use was defined as receiving screening (general health checkups, gastric and colorectal cancer, mammogram, and pap smear) and influenza vaccination. Descriptive analysis, t-test, and ANOVA were used. Results: The mean of functional health literacy was 3.87 (score range 0-6) and the self-rated health literacy was 60.08 (score range 16-80). The most difficult items of the self-rated questionnaires were patient educational materials provided by health care providers and medical forms. The most difficult items of functional health literacy were information-based, including nutritional facts and clinical schedules. Association between health literacy and preventive health service use was not found. Conclusions: Further study is necessary with larger samples and with considerations for their education level, age, and preventive health care use.

Chromosomal Information of 1,144 Korean BAC Clones

  • We sequenced 1,841 BAC clones by terminal sequencing, and 1,830 of these clones were characterized with regard to their human chromosomal location and gene content using Korean BAC library constructed at the Korean Science (KCGS). Sequence analyses of the 1,830 BAC clones was performed for chromosomal assignment: 1,144 clones were assigned to a single chromosome, 190 clones apparently assigned to more than one chromosome, and 496 clones to no chromosome. Evaluating gene content of the 1,144 BAC clones, we found that 706 clones represented 1,069 genes of which 415 genes existed in the BAC clones covering the full sequence of the gene, 180 genes covering a $50%{\sim}99%$, and 474 genes covering less than 50% of the gene coverage. The estimated covering size of the KBAC clones was 73,379 kilobases (kb), in total corresponding to 2.3% of haploid human genome sequence. The identified BAC clones will be a public genomic resource for mapped clones for diagnostic and functional studies by Korean scientists and investigators worldwide.

  • The establishment of DNA microarray technology has enabled high-throughput analysis and molecular profiling of various types of cancers. By using the gene expression data from microarray analysis we are able to investigate diagnostic applications at the molecular level. The most important step in the application of microarray technology to cancer diagnostics is the selection of specific markers from gene expression profiles. In order to select markers of Immortalization and transformation we used c-myc and $H-ras^{V12}$ oncogene-transfected NIH3T3 cells as our model system. We have identified 8751 differentially expressed genes in the immortalization/transformation model by multivariate permutation F-test (95% confidence, FDR<0.01). Using the support vector machine algorithm, we selected 13 discriminative genes which could be used to predict immortalization and transformation with perfect accuracy. We assayed $H-ras^{V12}$-transfected 'transformed' cells to validate our immortalization/transformation dassification system. The selected molecular markers generated valuable additional information for tumor diagnosis, prognosis and therapy development.

  • Breast self-exam is easiest, safe and cost effective to be recommended as an important method for early detection of breast cancer. This experimental research with non-equal control design was to develop the efficacy expectation program for breast self-exam based on Bandura's self-efficacy information source. The study objects selected from two local churches in Busan. Twenty from S church were selected as a study group, twenty from D church as a control group. Efficacy expectation promoting program was based on Bandura's efficacy promoting source and was two hour lecture including slide, video tape, demonstration, pamphlet. After this, there were individual interviews with them and consultations through telephone f or verbal persuasion on the weekly basis between the first week and the fifth week. The effect from the experiment were measured on the first week after education and on the fifth week. Data from control group was collected during the period from Jan 11, 2 001 to Feb 15, 2001 and data from study group was collected during the period from Jan 12, 2001 to Feb 16, 2001. Measurement instrument for this study was developed by the author with the advice of specialist in order to measure self-efficacy and breast self-exam practice. Data analysis was done by using SPSS/10.0 PC program $with^2-$ test. t-test and ANCOVA. Proved results for hypothesis were as follows. 1) It was supported that "Study group educated by efficacy expectation promoting program has higher score in change rat e of self-efficacy than control group." (1 week after education F=18.395, p=.000 5 weeks after education F=28.972, p=.000) 2) It was supported that "Study group educated by efficacy expectation promoting program has higher score in change rate of exam practice than control group." (1 week after education F=37.984, p=.000 5 weeks after education F=28.972, p=.000) In conclusion, efficacy expectation promoting program for breast self-exam developed by this study may increase s elf-efficacy and breast self-exam practice.

Development and Application of the Constipation Assessment Tool for Patients with Terminal Cancers (말기 암환자의 변비 사정도구 개발 및 적용)

  • Purpose: The purposes of this study were to develop the constipation assessment tool for identifying their bowel movement patterns, and to provide basic information for the assessment and intervention protocol on constipation for patients with terminal cancers. Methods: The study followed the steps: the first step was to build a conceptual framework based on literature review; the second step was to develop a tentative instrument by analyzing the conceptual framework and existing instruments; the third step was to test content validity and reliability; and the final step was to apply the tool to patients with terminal cancers (N=112). Results: The constipation assessment tool was consisted of total nine items; under the categories of subjective and objective data for diagnosis had 4 items, and under the initial assessment category which includes stool type, physical examination, and abdomen X-ray had 5 items. Conclusion: The constipation assessment tool developed in this study is very easy to use and useful in nursing practice, especially in hospice and palliative care setting. Particularly this tool has items on patient assessment which would be considered as an evidence for choosing nursing interventions. Based on the constipation assessment tool, the development and application of intervention protocol on constipation for patients with terminal cancers is warranted in future research.

Diagnostic Value of Preoperative Ultrasonographic Evaluation on Thyroid Cancer (갑상선 암의 판정에 대한 술전 초음파의 의의)

  • Background and Objectives: It is critical to distinguish benign from malignant thyroid nodule and to select a patient for surgery. Even though the U/S study dose not make great contribution to diagnose a malignant thyroid nodule, it is widely used in the evaluation of anatomic feature of thyroid. The authors tried to estimate the efficacy of the U/S study in preoperative diagnosis of malignant thyroid nodule. Materials and Method: At the department of General Surgery of Ajou University, 75 patients who were operated after diagnosis with thyroid nodule by U/S study between July 1996 to June 1997 were retrospectively analyzed. By comparing the U/S impression that implies malignant thyroid nodule to FNAC and post-operative pathologic results ware as follows. Results: 1) Absence of cystic change, presence of internal hypoechogenicity, lobulation, calcification, thick and irregular halo, and nodule more than 4cm in diameter on U/S were considered significant statistically for the diagnosis of malignancy(Chi-square test, p<0.05) 2) Presence of internal hypoechogenicity or thick and irregular halo has the validity in Logistic regression analysis. 3) FNAC was done in 65 case. 19 case were malignant, 11 case were suspicious and 46 patients were benign (sensitivity 52.6%, specificity 87%). 4) The findings of U/S which are hypoechogenic and thick and irregular halo show 82% sensitivity and 97% specificity. In combination with the findings of FNAC that imply benign or suspicious lesions, the sensitivity was 100% and the specificity was 97%. Conclusion: This study suggest that the hypoechogenicity and thick and irregular halo on U/S are important information for the diagnosis of thyroid malignancy which were considered benign or suspicious after FNAC.

  • Radioactive iodine treatment that uses the 2 people isolation room is to cause unnecessary radiation exposure between patients. This research is to be tested safety of 2 people Isolation treatment room and dose-rate through conservative perspective except physiology characteristic and biology information on the assumption that patient have iodine without excretion in 2 people isolation treatment room. This research shows that 364 keV gamma rays emitted by the radioiodine was to determine that the air layer about 30 cm or lead shield 3 mm a half-layer. In addition, In addition, patients in the distance, and lead shielding, length of hospital stay (48 hours) for external radiation exposure that is received from the other patients, two of treatment as appears to be lower than the legal isolation standard dose less than 5 mSv isolation room effective analyzed that manageable.

Difference of gut microbiota composition based on the body condition scores in dogs

  • Microorganism residing in the gut has been known to have important roles in the animal body. Microbes and host microenvironment are highly related with host's health including energy metabolism and immune system. Moreover, it reported that gut microbiome is correlated with diseases like obesity in human and dogs. There have been many studies to identify and characterize microbes and their genes in human body. However, there was little information of microbiome in companion animals. Here, we investigated microbiota communities in feaces from twenty - four Beagles (aged 2 years old) and analyzed the taxonomy profile using metagenomics to study the difference among gut microbiome based on body condition score (BCS). gDNA was isolated from feaces, sequenced and clustered. Taxonomy profiling was performed based on the NCBI database. BCS was evaluated once a week according to the description provided by World Small Animal Veterinary Association. Firmicutes phylum was the most abundant followed by Bacteroidetes, Fusobacteria, Proteobacteria and Actinobacteria. That main microbiota in gut were differently distributed based on the BCS. Fusobacteria has been known to be associated with colon cancer in human. Interestingly, Fusobacteria was in the third level from the top in healthy dog's gut microbiome. In addition, Fusobacteria was especially higher in overweight dogs which had 6 scales of BCS. Species Fusobacterium perfoetens was also more abundant when dogs were in BCS 6. It implied that F. perfoetens would be positively related with overweight in dogs. These finding would contribute to further studies of gut microbiome and their functions to improve dog's diets and health condition.

  • Asbestos is a toxic material that can lead to lung cancer and other diseases. There is no information regarding areas in Korea that contain asbestos in nature; consequently we need to manage such areas with care. The purpose of this study was to construct a local graded map of asbestos exposure risk based on the natural occurrence of asbestos in rocks. We first developed a means of evaluating the asbestos exposure risk and produced thematic maps based on a field survey. In addition, we constructed a knowledge base for asbestos through analysis, representation and processes about asbestos data and prepare for the development of an evaluation model for asbestos exposure risk. The spatial analysis of asbestos exposure risk is based on a weighted-overlay analysis using expert opinion and the literature, and a fuzzy-overlay analysis using the uncertainty in the data. The map of asbestos exposure risk, compiled according to the weighted and fuzzy operations, is expected to be used to ensure safety and to reduce the risk of exposure to asbestos.
