Targeting Orthotopic Glioma in Mice with Genetically Engineered Salmonella typhimurium

  • Objective : With the growing interests of bacteria as a targeting vector for cancer treatment, diverse genetically engineered Salmonella has been reported to be capable of targeting primary or metastatic tumor regions after intravenous injection into mouse tumor models. The purpose of this study was to investigate the capability of the genetically engineered Salmonella typhimurium (S. typhimurium) to access the glioma xenograft, which was monitored in mouse brain tumor models using optical bioluminescence imaging technique. Methods : U87 malignant glioma cells (U87-MG) stably transfected with firefly luciferase (Fluc) were implanted into BALB/cAnN nude mice by stereotactic injection into the striatum. After tumor formation, attenuated S. typhimurium expressing bacterial luciferase (Lux) was injected into the tail vein. Bioluminescence signals from transfected cells or bacteria were monitored using a cooled charge-coupled device camera to identify the tumor location or to trace the bacterial migration. Immunofluorescence staining was also performed in frozen sections of mouse glioma xenograft. Results : The injected S. typhimurium exclusively localized in the glioma xenograft region of U87-MG-bearing mouse. Immunofluorescence staining also demonstrated the accumulation of S. typhimurium in the brain tumors. Conclusion : The present study demonstrated that S. typhimurium can target glioma xenograft, and may provide a potentially therapeutic probe for glioma.

Role of Contrast-Enhanced Ultrasound as a Second-Line Diagnostic Modality in Noninvasive Diagnostic Algorithms for Hepatocellular Carcinoma

  • Objective: To investigate the diagnostic performance of contrast-enhanced ultrasound (CEUS) and its role as a second-line imaging modality after gadoxetate-enhanced MRI (Gd-EOB-MRI) in the diagnosis of hepatocellular carcinoma (HCC) among at risk observations. Materials and Methods: We prospectively enrolled participants at risk of HCC with treatment-naïve solid hepatic observations (≥ 1 cm) of Liver Imaging Reporting and Data System (LR)-3/4/5/M during surveillance and performed Gd-EOB-MRI. A total of one hundred and three participants with 103 hepatic observations (mean size, 28.2 ± 24.5 mm; HCCs [n = 79], non-HCC malignancies [n = 15], benign [n = 9]; diagnosed by pathology [n = 57], or noninvasive method [n = 46]) were included in this study. The participants underwent CEUS with sulfur hexafluoride. Arterial phase hyperenhancement (APHE) and washout on Gd-EOB-MRI and CEUS were evaluated. The distinctive washout in CEUS was defined as mild washout 60 seconds after contrast injection. The diagnostic ability of Gd-EOB-MRI and of CEUS as a second-line modality for HCC were determined according to the European Association for the Study of the Liver (EASL) and the Korean Liver Cancer Association and National Cancer Center (KLCA-NCC) guidelines. The diagnostic abilities of both imaging modalities were compared using the McNemar's test. Results: The sensitivity of CEUS (60.8%) was lower than that of Gd-EOB-MRI (72.2%, p = 0.06 by EASL; 86.1%, p < 0.01 by KLCA-NCC); however, the specificity was 100%. By performing CEUS on the inconclusive observations in Gd-EOB-MRI, HCCs without APHE (n = 10) or washout (n = 12) on Gd-EOB-MRI further presented APHE (80.0%, 8/10) or distinctive washout (66.7%, 8/12) on CEUS, and more HCCs were diagnosed than with Gd-EOB-MRI alone (sensitivity: 72.2% vs. 83.5% by EASL, p < 0.01; 86.1% vs. 91.1% by KCLA-NCC, p = 0.04). There were no false-positive cases for HCC on CEUS. Conclusion: The addition of CEUS to Gd-EOB-MRI as a second-line diagnostic modality increases the frequency of HCC diagnosis without changing the specificities.

수술현미경에서의 다중형광영상을 이용한 뇌종양과 혈관영상 검출 시스템 연구 (Study of a Brain Tumor and Blood Vessel Detection System Using Multiple Fluorescence Imaging by a Surgical Microscope)

  • 본 연구에서는 뇌 종양 수술에서 다수의 광원과 빔 스플리터 모듈을 사용해 종양과 혈관의 형광영상을 동시에 검출하고 획득한 형광영상을 동일한 디스플레이 장치에 표시함으로써 시술자에게 종양과 혈관의 정확한 정보를 실시간으로 제공할 수 있는 현미경 시스템을 제안한다. 5-ALA(5-Aminolevulinic acid) 와 ICG(Indocyanine green) 의 형광영상의 동시 검출을 위해 빔 스플리터(beam-splitter : BS)모듈을 사용하였고 5-ALA는 600nm, ICG는 800nm이상의 파장 대역에서 가장 효율이 뛰어나도록 구성하였다. 빔 스플리터 모듈은 파장 대역에 따라 광학기기의 구조를 변경할 수 있고 필터를 탈, 착 가능한 구조로 설계하여 필요에 따라 빔 스플리터와 필터의 종류를 변경할 수 있으며 5-ALA 및 ICG 이외의 형광염료를 사용한 시술에서 사용할 수 있다. 빔 스플리터 모듈을 통한 형광영상은 5-ALA는 가시광역, ICG는 근적외선 영역을 검출 할 수 있는 CCD 카메라를 장착해 동일한 디스플레이에서 확인할 수 있고 획득한 형광영상은 닮음 변환(similarity transform)을 이용해 원영상과 정합하여 실시간으로 시술자에게 제공하는 시스템을 구현하였다.

간암환자에서 Electronic Portal Imaging Device(EPID)를 이용한 자세 오차 및 종양 이동 거리의 객관적 측정 (Measurements of Setup Error and Physiological Movement of Liver by Using Electronic Portal Imaging Device in Patients with Hepatocellular Carcinoma)

  • 목적 : 간암의 3차원 입체조형치료계획시 electronic portal imaging device (EPID)를 이용한 검증영상으로 경계여유 결정에 필요한 객관적 기초자료를 얻으므로써 방사선치료의 정확성을 높이고 본 기기를 이용하여 방사선치료의 정도관리 측면에서의 유용성을 알아보고자 하였다. 대상 및 방법: 10명의 간암 환자에서 EPID를 이용하여 1회 방사선 치료 중 약 4극회의 검증영상을 획득하였고 총 10회 분할 치료기간 동안 반복하여 시행하였다. 방사선치료 자세 고정시 발생할 수 있는 각 개인의 자세 오차를 구하여 치료간 발생하는 환자의 위치 이동을 분석하였고, 호흡으로 인한 간의 상하 움직임의 정도를 측정하여 계획용표적체적(Planning Target Volume, PTV)의 결정시 필요한 적정 경계 여유를 구하였다. 결과 : 검증영상 분석 결과 간암의 치료시 자세이동에 의한 자세 오차는 x축으로 3.0 mm, 표준편차는 1.70 mm 였고 y축으로 3.7 mm, 표준편차 1.88 mm 였다. 따라서 각 축을 중심으로 5 mm 이내였다. 또한 호흡에 의한 간의 상하 운동 범위는 평균 8.63 mm이었고 표준편차는 1.48 mm 였다. 따라서 계획용표적체적 결정시 종양에 적어도 15 mm의 경계여유가 더 필요하다 결론 : EPID는 3차원 입체조형치료계획시 종양의 경계여유 결정에 매우 유용하며 나아가서 방사선치료의 정도관리에도 크게 기여할 것이다

  • X-ray is currently most effective method in detecting small malignant breast tumors but has the several problems due to suppressing breast, ionizing radiation and not detecting small cancer. In this paper, a new method is proposed by using dielectric characteristics of pathological tissues and time delay of backscattered response. We have developed a detection algorithm and verified it by numerical simulation and measurement for a prototype system. For a prototype system, we have fabricated experimental model(artificial breast with a cancer) and UWB(ultra-wideband) antenna. The results of the measurement simulation show an excellent detection capability of a cancer tissue. It is found that a good UWB antenna is a key element of such detection system. Further study is ongoing to develop a commercial system.

대장-직장 및 항문암에서 F-18 FDG PET (PET/CT)의 임상 이용 (Clinical Application of F-18 FDG PET (PET/CT) in Colo-rectal and Anal Cancer)

  • In the management of colo-retal and anal cancer, accurate staging, treatment evaluation, early detection of recurrence are main clinical problems. F-18 FDG PET (PET/CT) has been reported as useful in the management of colo-rectal and anal cancer because that PET has high diagnostic performance comparing to conventional studies. In case of liver metastases, for confirmation of no extrahepatic metastases, in case of high risk of metastasis, for avoiding unnecessary operation, PET (PET/CT) is expected more useful. In anal cancer, PET is expected useful in lymph node staging. For the early prediction of chemotherapy or radiation therapy effect PET has been reported as useful, also. In early detection of recurrence by PET, cost-benefit advantages has been suggested, also. PET/CT is expected to have higher diagnostic performance than PET alone.

2D Microwave Image Reconstruction of Breast Cancer Detector Using a Simplex Method and Method of Moments

  • This paper presents a tumor detection system for breast cancer that utilizes two-dimensional (2D) image reconstruction with microwave tomographic imaging. The breast cancer detection system under development consists of 16 transmit/receive antennas, and the microwave tomography system operates at 900 MHz. To solve a 2D inverse scattering problem, the method of moments (MoM) is employed for forward problem solving, and the simplex method employed as an optimization algorithm. The results of the reconstructed image show that the method accurately shows the position of a breast tumor.

  • The postoperative respiratory complications such as pneumonia and ARDS, are poor prognostic factors after esophagectomy in patients with esophageal cancer. To avoid these complications, there have been attempts to use minimally invasive approach. Recently introduced daVinciTM S surgical system is used in esophagectomy because of its advantages of minimal invasiveness, clear 3-dimensional imaging and precise handling of robotic arms. We report a 83-year-old esophageal cancer patient who underwent daVinciTMS robot-assisted esophagectomy, laparoscopic stomach mobilization followed by cervical esophagogastrostomy.

Bombesin-based Radiopharmaceuticals for Imaging and Therapy of Cancers Expressing Gastrin-releasing Peptide Receptor

  • Bombesin has a high binding affinity to gastrin-releasing peptide receptor (GRPR) and can be used as a targeting ligand in GRPR-related cancers. Because GRPR is overexpressed in prostate cancer, bombesin analogues have been investigated extensively for diagnosis and treatment of prostate cancer. In nuclear medicine, bombesin derivatives labeled with radiometals such as 55/57Co, 64Cu, 68Ga, 99mTc, and 177Lu or radiohalogen such as 131I and 211At were developed as markers for early detection of tumors and theragnostic tool for cancer treatment. This review focuses on the introduction of bombesin-based radiopharmaceuticals that are studied in pre-clinical or clinical research.

예측모형의 구축과 검증: 소화기암연구 사례를 중심으로 (Development and Validation of a Prediction Model: Application to Digestive Cancer Research)

  • Prediction is a significant topic in clinical research. The development and validation of a prediction model has been increasingly published in clinical research. In this review, we investigated analytical methods and validation schemes for a clinical prediction model used in digestive cancer research. Deep learning and logistic regression, with split-sample validation as an internal or external validation, were the most commonly used methods. Furthermore, we briefly introduced and summarized the advantages and disadvantages of each method. Finally, we discussed several points to consider when conducting prediction model studies.
