Tumor therapy using cytokines has been developed for last two decades. Several recombinant cytokines and tumor cell vaccines produced by cytokine gene transfer have been in clinical trials, but several side effects hamper routine clinical applications. Many cytokines are originally expressed as membrane-bound form and then processed to secretory form exerting paracrine effects. Though functional differences of these two types of cytokines are elusive yet, the membrane-bound form of cytokine may exert its effects on restricted target cells as a juxtacrine, which are in physical contacts. With the efforts to improve antitumor activities of cytokines in cancer patients, developing new strategies to alleviate life-threatening side effects became an inevitable goal of tumor immunologists. Among these, tumor cell vaccines expressing cytokines as membrane-bound form on tumor cell surface have been developed by genetic engineering techniques with the hope of selective stimulation of the target cells that are in cell-to-cell contacts. In this review, recent progress of tumor cell vaccines expressing membrane-bound form of cytokines will be discussed.
연구배경 : 종양괴사인자(tumor necrosis factor ; TNF)는 다양한 생물학적 기능을 가지고 있는 바, 그 중 생체 외에서 증명된 뚜렷한 항암 효과로 말미암아 최근 항암 유전자요법의 중요한 대상으로 관심을 모으고 있다. 현재 유전자 이입의 기술적 문제로 생체 외에서 암세포에 유전자 이입을 시행한 후 이를 다시 환자의 생체내로 이식하는 방법이 연구의 주종을 이루고 있다. 그러나 저자들의 과거의 연구를 포함한 여러 연구에서 TNF가 이입된 암세포는 TNF에 대해 내성을 보이는 이에는 새로이 방어 단백질을 합성하는 것이 관여할 것이라는 시사가 있었다. 이 획득내성의 기전을 밝히는 것이 종양생물학의 이해를 넓히고 보다 효과적인 항암 유전자요법을 개발하기위한 매우 중요한 과제로 생각된다. 저자들은 TNF 유전자 이입에 따른 암세포의 TNF에 대한 획득내성에, 일부 세포에서 TNF에 의해 발현이 유도된다는 것이 밝혀진, 항산화효소의 하나인 MnSOD의 발현의 변화가 관여하는 지를 규명하고자 본 실험을 수행하였다. 방 법 : TNF에 다양한 감수성을 보이는 인체 및 생쥐 기원의 4가지 암세포주(WEHI164, NCI-H2058, A549, ME180)에 TNF-$\alpha$ 유전자를 retroviral 이용하여 이입하고 TNF의 발현을 시도하여 PCR, ELISA, MTT assay로 확인하였고, TNF 유전자가 이입된 세포(WEHI164-TNF, NCI-H2058-TNF, A549-TNF, ME180-TNF)는 TNF에 내성을 보이는지 역시 MTT assay로 검증하였다. TNF 유전자 이입 전후의 MnSOD mRNA 발현의 차이는 Northern blot analysis를 통하여 비교하였다. 결 과 : 1) TNF-$\alpha$ 유천자 이입 및 발현 확인 PCR을 시행한 결과 TNF 유전자가 이입된 각 세포 790 base pair 표기의 진한 DNA band를 보인 반면 모세포주는 보이지 않아서 retroviral vector를 이용한 유전자 이입이 DNA 수준에서 이루어 있었다. 그리고 TNF 유전자가 이입된 세포의 배양상층액에서 TNF양을 ELISA로 측정한 결과 TNF를 세포에 따라 1.91ng/24hr/$10^6\;cells$에서 3.91ng/24hr/$10^6\;cells$ 생산함을 알 수 있었다. 2) TNF 유전자 이입 전후, 암세포의 TNF에 대한 감수성 비교 TNF 농도 100ng/ml에서 WEHI164-TNF와 ME180-TNF 세포는 통계적으로 유의하게 (p<0.01) TNF에 대한 내성을 획득함을 알 수 있었다. 3) TNF 유전자 이입 전후외 MnSOD mRNA 발현양상 TNF에 감수성을 보이는 WEHl164와 ME180세포는 TNF 유전자 이입 후에 MnSOD mRNA 발현이 증가되지 않았으며, TNF에 내성을 보이는 NCIH2058과 A549 세포는 TNF 유전자 이입 후에 MnSOD mRNA 발현의 증가가 관찰되었으나 이는 모세포에 외부에서 TNF를 주었을 때도 관찰되었다. 결 론 : 세포주에 TNF 유전자를 이입하여 TNF를 발현하게 하였을때 세포 자신은 TNF에 대해 내성을 하게 되는데, 이 획득내성은 MnSOD 발현 능력의 향상에 의한 것은 아닐 것으로 판단되나 각 세포주의 자연 상태의 TNF에 대한 감수성 여부는 MnSOD의 발현 차이가 관련될 것으로 생각된다.
Cathepsin은 리소좀에 존재하는 효소로, 엔도좀-리소좀 경로를 통하여 손상된 단백질의 분해를 유도한다. 이러한 cathepsin은 활성화되는 촉매 잔기 위치(catalytic residue site)에 따라 cysteine, aspartate, serine cathepsin으로 나뉜다. 다양한 암세포에서 cysteine cathepsin은 유전자 증폭이나 전사, 번역, 전사 후 단계를 통해 높은 발현을 유지한다. 특히 cathepsin S의 경우, 다른 cysteine cathepsin과 달리 중성 pH 환경에서도 안정적으로 활성도를 유지하며 특정 조직에서 발현이 제한적이라는 특징을 가지고 있기 때문에 질병의 미세환경에서 중요한 역할을 한다. 면역세포에서의 cathepsin S는 불변 사슬(invariant chain)을 분해하여 항원 제시에 중요한 MHC class II의 활성화를 통해 면역 반응을 조절하며, 암세포에서의 cathepsin S는 전이와 혈관신생을 조절함으로써 암세포의 성장을 증가시키는 역할을 한다. Cathepsin S의 발현 조절에 문제가 생기면 암, 관절염, 심혈관 질환을 포함한 많은 병리학적 현상에 영향을 미친다. 특히 암세포에서 cathepsin S의 발현 억제와 결함은 혈관신생을 억제시킬 뿐만아니라, 암세포의 성장과 전이를 감소시키고 세포사멸을 유도한다. 따라서, cathepsin S는 암 치료에 있어 좋은 표적이 될 수 있다.
목적: mdr1유전자를 표적으로 한 short hairpin RNA (shMDR)는 다약재내성을 나타내는 암세포에서 효과적으로 mdr1 유전자의 발현을 억제 할 수 있고 sodium iodide symporter (NIS)는 유전자 치료와 리포터로의 기능을 동시에 나타낼 수 있다. 이 연구에서는 사람 대장암세포(HCT15)에 shMDR과 NIS를 동시에 이입하고 Tc-99m sestamibi와 I-125 섭취를 측정하였고 doxorubicin과 I-131 치료효과도 관찰하였다. 대상 및 방법: 사람 태아 신장 세포주(Human Embryonic Kidney cells; HEK293)에 liposome 시약으로 shMDR을 이입하고 RT-PCR과 western blot으로 분석하였다. shMDR와 NIS 유전자가 발현하는 adenovirus를 만들고 HCT15 세포에 이입 후 48시간에 shMDR에 의한 Pgp의 기능 억제를 확인하기위해 Tc-99m sestamibi 섭취와 doxorubicin 세포독성을 측정하였다. 또한 NIS유전자의 기능을 확인 하기위해 I-125 섭취와 I-131 세포독성도 확인하였다. 결과: shMDR이 이입 된 HEK293 세포에서 mdr1의 mRNA와 Pgp의 발현이 각각 75%, 80% 감소하였다. NIS 유전자가 발현하는 adenovirus를 HCT15 세포에 이입하고 NIS 유전자 발현을 확인 한 결과 대조군에 비해 월등히 높게 발현하였다. Ad-shMDR 300 MOI, Ad-shMDR 300 MOI 와 Ad-NIS 10 MOI를 처리한 경우 Tc-99m sestamibi의 섭취가 대조군보다 1.5배 정도 증가하였다. HCT15 세포에 Ad-NIS 10 MOI를 감염시킨 경우 I-125 섭취가 대조군에 비해 25배 이상 증가였다. 또한 Ad-shMDR와 Ad-NIS를 동시 감염 시켰을 경우 doxorubicin의 세포 독성이 증가하여 나타났고 Ad-NIS 20 MOI를 감염시켰을 때 I-131에 의한 세포독성이 대조군보다 증가하였다. 결론: 세포에 shMDR의 이입으로 mdr1 유전자의 발현이 억제되고 Tc-99m sestamibi의 섭취와 doxorubicin의 세포독성이 증가하였으며 NIS 유전자의 이입으로 I-125의 섭취와 I-131의 세포독성이 증가하였다. 다약제내성세포에 shMDR와 NIS 유전자의 동시 이입은 doxorubicin과 방사성 옥소의 이중치료 효과를 높일 수 있을 것으로 본다.
The object of this study was to develop a gene therapy strategy for ovarian cancer. We have previously shown that AV with a L-plastin (LP) promoter infects breast and ovarian cancer cells and expressed ${\beta}$-galactosidase cDNA in preference to normal fibroblast cells and hematopoietic cells. We now report on the cytotoxicity of Ad.LP.CD, an AV carrying a CD cDNA which converts the pro-drug, 5-Fluorocytosine (5-FC) into the toxic drug 5-Fluorouracil (5-FU). Infection of Ad.LP.CD into either 293 cells or ovarian cancer cells generated the functional CD as measured by HPLC analysis. Using a ratio of AV to OVCAR3 cell of 100 and a 5-FC concentration of 100 ${\mu}$M, we achieve an over 95 % of cell growth inhibition. We are using flow cytometry analysis for ${\beta}$ -galactosidase and ovarian cancer associated folate receptor to screen primary ascites samples for infectivity after infection with an adenoviral vector, i.e., Ad.LP.LacZ. This vector system may be of value in the treatment of microscopic disease of ovarian cancer in the peritoneal cavity.
Resistance to chemotherapy treatment, which may lead to limited efficacy of systemic therapy in breast cancer patients, is multifactorial. Among the mechanisms of resistance to chemotherapy treatment, there are those closely related to estrogen receptor ${\alpha}$, P-glycoprotein, multidrug resistance-related protein, glutathione S-transferase pi and topoisomerase-II. $ER{\alpha}$ is ligand-activated transcription factor that regulates gene expression and plays a critical role in endocrine signaling. In previous preclinical and clinical studies, positive $ER{\alpha}$ expression in breast cancer cells was correlated with decreased sensitivity to chemotherapy. This article reviews current knowledge on the predictive value of $ER{\alpha}$ with regard to response to chemotherapy. Better understanding of its role may facilitate patient selection of therapeutic regimens and lead to optimal clinical outcomes.
Galectin-3 is a carbohydrate-binding protein and regulates diverse functions, including cell proliferation and differentiation, mRNA splicing, apoptosis induction, immune surveillance and inflammation, cell adhesion, angiogenesis, and cancer-cell metastasis. Galectin-3 is also recommended as a diagnostic or prognostic biomarker of various diseases, including heart disease, kidney disease, and cancer. Galectin-3 exists as a cytosol, is secreted in extracellular spaces on cells, and is also detected in nuclei. It has been found that galectin-3 has different functions in cellular localization: (i) Extracellular galectin-3 mediates cell attachment and detachment. (ii) cytosolic galectin-3 regulates cell survival by blocking the intrinsic apoptotic pathway, and (iii) nuclear galectin-3 supports the ability of the transcriptional factor for target gene expression. In this review, we focused on the role of galectin-3 on translocation from cytosol to nucleus, because it happens in a way independent of carbohydrate recognition and accelerates cancer progression. We also suggested here that intracellular galecin-3 could be a potent therapeutic target in cancer therapy.

Background: Despite consistent pharmacogenetic effects of CYP2D6 on tamoxifen exposure, there is considerable controversy regarding the validity of CYP2D6 as a predictor of tamoxifen outcome. Understanding the current state of evidence in this area and its limitations is important for the care of patients who require endocrine therapy for breast cancer. Materials and Methods: A total of 101 patients with breast cancer who received tamoxifen therapy for at least 3 years, were genotyped for common alleles of the CYP2D6 gene by nested-PCR and restriction fragment length polymorphism PCR. Patients were classified as extensive or poor metabolizers (PM) based on CYP2D6*4 alleles in 3 different groups according to the menopause, Her2-neu status, and stage 3. Results: The mean age of the patients with the disease recurrence was $50.8{\pm}6.4$ and in non recurrent patients was $48.2{\pm}6.8$. In this study 63.3% (n=64) patients were extensive metabolizers and 36.6% (n=37) were poor metabolizers. Sixty four of the 101 patients (63.3%) were Her2-neu positive. For tamoxifen-treated patients, no statistically significant difference in rate of recurrence observed between CYP2D6 metabolic variants in stage 3 and post-menopausal patients. However, there was a significant association between CYP2D6 genotype and recurrence in tamoxifen-treated Her2-neu positive patients. Compared with other women with breast cancer, those with Her2-neu positive breast cancer and extensive metabolizer alleles had a decreased likelihood of recurrence. Conclusions: This study for the first time demonstrated significant effects of CYP2D6 extensive metabolizer alleles on risk of recurrence in Her2-neu positive breast cancer patients receiving adjuvant tamoxifen therapy. Therefore, CYP2D6 metabolism, as measured by genetic variation, can be a predictor of breast cancer outcome in Her2-neu positive women receiving tamoxifen.
This study was conducted to investigate enhancement of anti-tumor effects of the lentogenic Newcastle disease virus Clone30 strain (NDV rClone30) expressing cytosine deaminase (CD) gene against tumor cells and in murine groin tumor-bearing models. Cytotoxic effects of the rClone30-CD/5-FC on the HepG2 cell line were examined by an MTT method. Anti-tumor activity of rClone30-CD/5-FC was examined in H22 tumor-bearing mice. Compared to the rClone30-CD virus treatment alone, NDV rClone30-CD/5-FC at 0.1 and 1 MOIs exerted significant cytotoxic effects (P<0.05) on HepG2 cells. For treatment of H22 tumor-bearing mice, recombinant NDV was injected together with 5-FC given by either intra-tumor injection or tail vein injection. When 5-FC was administered by intra-tumor injection, survival for the rClone30-CD/5-FC-treated mice was 4/6 for 80 days period vs 1/6, 0/6 and 0/6 for the mice treated with rClone30-CD, 5-FC and saline alone, respectively. When 5-FC was given by tail vein injection, survival for the rClone30-CD/5-FC-treated mice was 3/6 vs 2/6, 0/6 and 0/6 for the mice treated with rClone30-CD, 5-FC or saline alone, respectively. In this study, NDV was used for the first time to deliver the suicide gene for cancer therapy. Incorporation of the CD gene in the lentogenic NDV genome together with 5-FC significantly enhances cell death of HepG2 tumor cells in vitro, decreases tumor volume and increases survival of H22 tumor-bearing mice in vivo.
