지지간호중재가 암환아의 신체상 및 자존감에 미치는 영향 (Study on the Effects of the Supportive Nursing Intervention Program for Body Image and Self Esteem of the Child with Cancer)

  • The increased potential for the lifespan of a child with cancer is largely due to advances in drug treatment, radiation treatment, and surgical techniques. In this generation cancer has become associated with chronic illness. Therefore supportive nursing intervention for children with cancer is needed to promote normal growth and development. This study was designed to develop and test the supportive nursing intervention program for promoting body image and self-esteem of children with cancer The supportive nursing intervention program involved emotional, physical, informational, and social support. The subjects were 41 children with cancer(20 in intervention group, 21 in control group) in K city. Measurements were taken concerning body image and self-esteem from both groups during pre and post test The data were analyzed using Cronbach's alpha, x$^2$-test, paired t-test and t-test. The results were as follows : The intervention group had a more positive body image as measured by the Body Cathexis Scale (t=2.436, p=.020) and a more self esteem as measured by the Cpopersmith self-inventory (t=2.768, p=.009) than the control group at post test. According to this study, the supportive nursing intervention program was effective for promoting the body image and self-esteem of children with cancer. Repeated research is needed to develop a refined supportive nursing intervention program for children with chronic illness.

국가암조기검진 수검자 만족도에 영향을 미치는 요인 (Factors affecting satisfaction in National Cancer Screening Program)

  • The objectives of this study were to evaluate the satisfaction of participants in the National Cancer Screening Program(NCSP) and to identify factors affecting the satisfaction. The telephone survey was conducted in July 2007 for the participants who were screened by the NCSP from January through May 2007. Student's t-tests and analysis of variance were performed first to determine if the mean satisfaction score differed by the characteristics of study objects, followed by multiple linear regression analyses to examine the factors affecting satisfaction. Dependent variable was general satisfaction for the screening service, and three dimensions of independent variables - 'sociodemographic characteristics', 'screening characteristics', and 'perceived service quality' - were used for the empirical analyses. Female, old-aged, less educated, Medicaid recipients, rural residents and the participants with normal results were more likely to be satisfied with the NCSP. The results of multiple regression analyses show that gender, age, location of residence, type of screening units, and perceived quality of screening services were significantly related to satisfaction. This study is meaningful as the first attempt to measure participant satisfaction with the NCSP, and to identify factors affecting the satisfaction. Among the identified factors, the NCSP needs to pay attention to perceived quality of service, in particular, to improve the satisfaction. This study is expected to contribute to raising the compliance rate and to improve the quality of the NCSP.

대장암 환자를 위한 불확실성 이론 기반 모바일 내비게이션 프로그램의 효과 (Effects of Mobile Navigation Program in Colorectal Cancer Patients based on Uncertainty Theory)

  • Purpose: This study aimed to examine the effects of a mobile navigation program on uncertainty, resilience, and growth through uncertainty in colorectal cancer patients. Methods: To verify the effectiveness of the mobile navigation program, 61 participants diagnosed with colorectal cancer undergoing surgery were selected. A nonequivalent control group nonsynchronized design was used to evaluate the program. Uncertainty was measured using the Korean version of the Uncertainty in Illness Scale, resilience was measured using the Korean version of the Connor-Davidson Resilience Scale, and growth through uncertainty was measured using the Growth through Uncertainty Scale. Results: Compared with the control group, patients in the mobile navigation program group showed significant differences in scores for uncertainty (F=7.22, p=.009) and resilience (F=4.31, p=.042), but not for growth through uncertainty (F=2.76, p=.102). Conclusion: These results suggest that the mobile navigation program has positive effects on decreasing uncertainty and increasing resilience among colorectal cancer patients. The mobile navigation program could play a significant role in assisting colorectal cancer patients in regard to the continuity and usability of the program.

마음챙김명상 프로그램이 유방암 환자의 스트레스 지각, 대처방식 및 스트레스 반응에 미치는 효과 (Effects of Mindfulness Meditation Program on Perceived Stress, Ways of Coping, and Stress Response in Breast Cancer Patients)

  • Purpose: Purpose of this study was to examine the effects of the Mindfulness Meditation program on perceived stress, ways of coping, salivary cortisol level, and psychological stress response in patients with breast cancer. Methods: This was a quasiexperimental study with a non-equivalent control group pre-post test design. Participants in this study were 50 patients who had completed breast cancer treatment (experimental group, 25, control group, 25). The experimental group received the Mindfulness Meditation program for 3 hours/session/ week for 8 weeks. Data were analyzed using $X^2$-test and t-test for subject homogeneity verification, and ANCOVA to examine the hypotheses. Results: The experimental group had significantly lower scores for perceived stress, emotional focused coping, salivary cortisol level, and psychological stress response compared to the control group. However, no significant differences were found between two groups for the scores on problem focused stress coping. Conclusion: According to the results, the Mindfulness Meditation program was useful for decreasing perceived stress, emotional focused coping, salivary cortisol level, and psychological stress response. Therefore, this program is an effective nursing intervention to decrease stress in patients with breast cancer.

대장암 생존자에게 적용한 전문간호사 주도 심리교육 프로그램의 효과 (Effects of the Advanced Practice Nurse-Led Psychoeducational Program for Colorectal Cancer Survivors)

  • Purpose: This study aimed to investigate the effects of an advanced practice nurse-led psychoeducational program on distress, anxiety, depression, coping with cancer (CWC), health promotion behavior (HPB), and quality of life (QOL) among colorectal cancer survivors. Methods: This study was designed as a quasi-experimental study with a non-equivalent control group pretest-posttest. The participants were survivors of colorectal cancer who underwent follow-up care. There were 39 survivors: 19 in the experimental group and 20 in the control group. The experimental group performed a psychoeducational program for 120 minutes per session, once a week for a total of six weeks, while the control group received routine education and counseling. Distress, anxiety, depression, CWC, HPB, and QOL were investigated before, immediately after, and 4 weeks after the intervention. The data were analyzed with SPSS/WIN ver. 24.0, using repeated measures ANOVA. Results: There were significant interactions between time and group for distress and anxiety. In addition, CWC interacted with the total of CWC and interpersonal coping, and QOL interacted with the total of QOL and functional status. However, there were no significant differences in the depression or HPB scores. Conclusion: Based on the results of this study, we expect that this program can be used as an effective intervention for colorectal cancer survivors.

유방암 환자의 자기초월감 증진을 위한 자조집단과정 개발 및 운영 효과 (Effects of the Self-help Group Program for Promoting Self-Transcendence in Women with Breast Cancer)

  • Cancer survivor's self-help group may promote psychosocial adjustment in cancer patients. Self-transcendence has been shown to be related with crisis and adjustment in women with breast cancer. The purpose of this study is to determine the effectiveness of self-transcendence promoting program to facilitate self-transcendence, physical, and emotional well-being in women with breast cancer. Thirty patients who received mastectomy were recruited, but 9 subjects of experimental group and 14 subjects of control group participated in this study. Subjects of experimental group attended the self-help group for self-transcendence once a week for 6 weeks. Each session of program was proceeded for 2hours and the program contained activities for promoting self-transcendence, stress management, communication skill, exercise and relaxation, symptom management, and family support. Data were collected by self-reporting questionnaires within seven days after mastectomy and after the program was finished in experimental group, within 7 days after mastectomy and after 3 months in control group. Self-transcendence was assessed using the self-transcendence scale (STS) developed by Reed. Physical well being was assessed using Symptom distress scale (SDS) developed by McCorkle and Young and emotional well being was assessed using Profile of mood states (POMS). As a result, no significant differences were found between both groups in changes of self-transcendence, emotional well-being, and physical well-being. Self-transcendence score was significantly decreased in control group (p =0.16). But that of experimental group showed no decrease. In conclusions, when the self-transcendence promoting program is provided to the patients with breast cancer, it can be helpful for promoting self- transcendence. For the further study, repeated research with appropriate sample size and more intensive nursing intervention to promote self-transcendence is suggested. And reassessment after 6 months is recommended for testing long term effects of the program.

동영상 운동프로그램이 항암 화학요법을 받는 암환자의 피로와 신체기능 및 정서상태에 미치는 효과 (The Effect of a Video Exercise Program on Cancer-related Fatigue, Physical Function and Emotional Status in Patients with Cancer during Chemotherapy)

  • Purpose: The purpose of this study was to evaluate the effects of a home-based video exercise program on cancer-related fatigue, physiological and psychological status in patients with colon and rectal cancers undergoing chemotherapy. Methods: The study design was a non-equivalent control group non-synchronized design. Data were collected from patients with colo-rectal cancers in Yonsei cancer center from July 5th to October 31st in 2011. There were 40 participants; 20 in the experimental group and 20 in the control group. The structured questionnaire was used to measure fatigue, physical function and emotional status. Data were analyzed using SPSS 18.0 and a chi-squre test, Fisher's exact test, Mann-Whitney U test and Wilcoxon signed- rank test were conducted to examine the homogeneity and the research hypotheses. Results: There was a statistically significant difference in White Blood Cell count in the experimental group compared with that of the control group. The exercise group showed a slight decrease of White Blood Cell count compared with that of the control group after 4 week program (z=-2.935, p=.003). However, there were no significant differences in fatigue, physiological and psychological status between the two groups. Conclusion: In this study, the developed video exercise program was effective in markedly slightly decreasing White Blood Cell count in patients with colo-rectal cancers undergoing chemotherapy. Therefore, utilizing the video exercise program can be an useful method to promote health among patients with cancer in clinical practice.

유방암 환자의 분노관리 자기효능감 증진 프로그램 개발 (Development of a Program for Anger Management Based on Self-efficacy in Patients with Breast Cancer)

  • Purpose: The purpose of this study was to develop a program for anger management based on self-efficacy. Methods: In this study, four types of approaches to enhance anger control were developed; 1) a role play for vicarious experiences; 2) an assertive training for anger expression; 3) a 30-minute long education program & a 20-minute long telephone call coaching for verbal persuasion; and 4) a booklet for anger management and self care behaviors. One group pretest-posttest design was used for evaluating the program. Study subjects were 6 cancer patients undergoing chemotherapy. The group were received a 4-week intervention, 60 ~ 90 minutes a day weekly, and measured the variables at baseline, 4 weeks later. Anger-in, anger-out, and anger-control were measured by STAXI-K. Data was analysed by Wilcoxon using SPSS/WIN 12.0 program. Anger situation was analysed according to primary anger-thoughts and secondary anger-thoughts based on cognitive theories of anger. Results: The program for anger management consisted of a role play, assertive training, education, telephone call coaching and a booklet. The program revealed significantly less Anger-in (Z = -1.997, p = .046), anger-out (Z = -2.207, p = .027). No difference, however, was found in anger control (Z= -1.826, p=.068). Conclusion: This evaluation suggested that more assertive training and longer intervention may be needed to maximize anger control.

Anthropometric and Reproductive Factors among Newly-Diagnosed Breast Cancer Patients and Healthy Women: A Case-Control Study

  • The objective of this case-control study was to determine anthropometric and reproductive factors associated with the development of breast cancer among women. Fifty-six newly diagnosed breast cancer patients were recruited from the Oncology Clinic, Universiti Sains Malaysia (USM), and 56 healthy female hospital employees were recruited as controls. Socio-demographic and reproductive data were obtained using a standard questionnaire. Anthropometric factors (body weight, height, body fat percentage, visceral fat and waist and hip circumference) were assessed. A high waist circumference (adjusted OR= 1.04, [95% CI: 1.00, 1.09]) and being more than 30 years of age at first full-term pregnancy (adjusted OR=3.77, [95% CI: 1.10, 12.90]) were predictors of breast cancer development. The results of this study indicate that weight and reproductive health management should be emphasized for breast cancer prevention in Malaysia.

암병동 신규간호사를 위한 행복 셀프코칭 프로그램 개발 및 효과 (Development and Evaluation of the Effect of a Happiness Self-Coaching Program for New Graduate Nurses Working in Cancer Care Unit)

  • Purpose: This study was conducted to examine the effect of a Happiness Self-Coaching program for new graduate nurses working in cancer care units. Methods: A quasi-experimental pretest-posttest, nonequivalent control-group design was applied to conduct the study. Study participants were new graduate nurses employed within 12 months and working as shift employees at G university hospital, located in J city. A total of 21 new graduate nurses participated in the study, 10 in the experimental group, and 11 in the control group. The happiness self-coaching program was conducted weekly for 70 minutes from February 1 to June 6, 2016 for 6 weeks. The experimental group received the Happiness Self-Coaching program through lectures, presentation, group activities, and strength card play. Data were analyzed by using repeated measure ANOVA and paired t-test. Results: The experimental group demonstrated significantly higher happiness (F=6.82, p=.003) and self-efficacy (F=3.38 p=.045) levels compared to the control group. Conclusion: The Happiness Self-Coaching program was effective in enhancing new graduate nurses' happiness and self efficacy. It is recommended that nursing organizations to apply the Happiness Self -Coaching program to facilitate adaption of new graduate nurses in cancer care units.
