카데바 실습교육이 간호대학생의 자기효능감, 죽음태도 및 간호전문직 자아개념에 미치는 효과 (Effect of Education in Cadaver Practice on the Self-efficacy, Attitudes to death and Professional self -concept of Nursing students)

  • 본 연구는 카데바 실습교육이 간호대학생의 자기효능감, 죽음태도와 간호전문직 자아개념에 미치는 효과를 알아보고자 시도하였다. 연구방법은 비동등성 대조군 전후설계의 유사실험 연구이며, 자료수집 기간은 2016년 11월 1일에서 11월 30일까지이다. 연구대상자는 D시 소재의 1개 대학교 2학년 중 연구 참여를 희망하고 동의한 실험군 29명, 대조군 30명으로 총 59명이다. 카데바 실습교육은 회기 당 이론교육 3시간, 실습교육 4시간으로 구성하였고, 총 3회기를 실시하였다. 자료분석 방법은 SPSS/WIN 21.0 program을 이용하여 기술통계, ${\chi}^2$ test와 t-test로 분석하였다. 카데바 실습교육 적용 전후의 두 집단 간의 자기효능감, 죽음태도 및 간호전문직 자아개념을 비교하여 그 효과를 분석하였다. 연구결과는 카데바 실습교육 실시 전후 두 집단 간 차이를 분석한 결과, 자기효능감(t=2.62, p=.028), 죽음태도(t=5.67, p=.000)와 간호전문직 자아개념t=2.57, p=.013)은 통계적으로 유의미한 차이가 있었다. 따라서 본 연구의 카데바 실습교육은 간호대학생의 자기효능감이 향상되었고, 죽음태도와 간호전문직 자아개념에 긍정적인 영향을 미치는 것을 알 수 있다. 이에 간호대학생의 해부학 실습교육의 기초자료로 활용하기를 기대한다.

해부 실습 시 포름알데히드 노출이 학생들의 신경행동기능에 미치는 영향 (The effect of formaldehyde on neurobehavioral performance of student during cadaver dissection)

  • Background: Formaldehyde is used to preserve cadaver in medical schools, and students are exposed to formaldehyde during cadaver dissection classes. When humans are exposed to formaldehyde, it induces mucosal inflammation, skin inflammation, and declining of neurobehavioral function including attention and memory executive functions. The purpose of this study is to identify the effects of formaldehyde exposure on student's neurobehavioral performance during cadaver dissection classes. Methods: The level of formaldehyde was measured in a cadaver dissection class. A total of 16 students were randomly divided into two groups. One group wore respiratory protection masks, while the other group did not. Among many subtests in Korean Computerized Neurobehavioral test, backward digit span was tested on all subjects before and after the class. Results: The length of memorized digit span between the two groups was not significant; however there was a greater decrease in neurobehavioral function after formaldehyde exposure in the non-mask group than the mask group. Conclusion: Formaldehyde exposure during cadaver dissection may likely decrease neurobehavioral performance of students. Therefore, proper ventilation system and respiratory protective equipment are necessary to protect medical school students from adverse effects of formaldehyde exposure.

한국인 Cadaver Test에 대한 상완골 골수정 개발에 관한 연구 (A Study on Development of Humeral Intramedullary Fixation Nail based on Korean Cadaver Tests)

  • The aim of this study is to develop a humeral Intramedullary fixation nail (HIFN) suitable for Korean people. In this study, CT images were obtained from 72 Korean cadaveric humeral bones and 3D Korean humeral bone models were reconstructed based on the CT images to investigate anatomical characteristics. Major design parameters of HIFN were selected using the morphological measurement information of the Korean humeral bone models. Through finite element analysis and mechanical tests, the developed HIFN prototype was compared with the Polarus HIFN ($ACUMED^{(R)}$, USA), and it was found that the HIFN prototype showed similar and/or superior mechanical performance compared to the Polarus HIFN. Also, clinical validation for the HIFN prototype was carried out to check predictable troubles in surgical operations. Finally, optimal design modification was proposed to prevent the possible axillary nerve injury due to the locking screw system of the HIFN prototype.

The accuracy of linear measurements of maxillary and mandibular edentulous sites in conebeam computed tomography images with different fields of view and voxel sizes under simulated clinical conditions

  • Purpose: The objective of this study was to investigate the effect of varying resolutions of cone-beam computed tomography images on the accuracy of linear measurements of edentulous areas in human cadaver heads. Intact cadaver heads were used to simulate a clinical situation. Materials and Methods: Fiduciary markers were placed in the edentulous areas of 4 intact embalmed cadaver heads. The heads were scanned with two different CBCT units using a large field of view ($13cm{\times}16cm$) and small field of view ($5cm{\times}8cm$) at varying voxel sizes (0.3 mm, 0.2 mm, and 0.16 mm). The ground truth was established with digital caliper measurements. The imaging measurements were then compared with caliper measurements to determine accuracy. Results: The Wilcoxon signed rank test revealed no statistically significant difference between the medians of the physical measurements obtained with calipers and the medians of the CBCT measurements. A comparison of accuracy among the different imaging protocols revealed no significant differences as determined by the Friedman test. The intraclass correlation coefficient was 0.961, indicating excellent reproducibility. Inter-observer variability was determined graphically with a Bland-Altman plot and by calculating the intraclass correlation coefficient. The Bland-Altman plot indicated very good reproducibility for smaller measurements but larger discrepancies with larger measurements. Conclusion: The CBCT-based linear measurements in the edentulous sites using different voxel sizes and FOVs are accurate compared with the direct caliper measurements of these sites. Higher resolution CBCT images with smaller voxel size did not result in greater accuracy of the linear measurements.

임상실습과정 의과대학생을 대상으로 카데바 모델을 이용한 단순 봉합술 교육의 효율성 (Evaluation of the Educational Efficacy of a Cadaver-based Model for Teaching a Simple Suture Technique to Medical Students)

  • Purpose: There is ongoing demand to deliver better procedural training to medical students in the emergency department. Thus, we studied the efficacy of a cadaver-based training model for teaching simple suture techniques to medical students. Methods: We investigated ten fourth-year medical students, who were rounding and being trained in the Emergency Department of Chungnam National University Hospital. They were educated with slides about a simple suture technique for 30 minutes to evaluate the efficacy of the cadaver-based training model. We prospectively measured their skill by administering a test on the cadaver-based simple suture technique in 3cm sized linar wound separately to each of them. Results: A total of ten fourth-year medical students completed the investigation. The tension, the direction of suture, the degree of cleanness, the number of sutures, the adequacy of the cutting thread length, and the suturing time in the first attempt were compared with those in the second one. The second results were compared with those in the third one, and the third results were compared with those in the fourth one. All the results had statistical significance. Conclusion: These findings support the value of the cadaver-based simple suture technique training model as a medical student teaching model. The cadaver-based simple suture technique teaching model is a good way of teaching several medical skills to medical students.

해부학수업에서 교수매체 적용에 따른 학습효과 (The Learning Effects of Instructional Media on Anatomy Classes in a Nursing College)

  • Purpose: It is to verify learning effect of the instructional media on anatomy classes at a nursing college and to develop an alternative instructional media instead of cadaver. Method: Four groups pretest-posttest experimental design were used. One hundred twenty students who attended an Anatomy lecture in September, 2009 were selected After attending the anatomy lecture, the subjects were divided into four group (30 for each group) conveniently. The heart anatomy knowledge level were measured by a self evaluation questionnaire and quiz before and after a different instructional media being applied for each group including making heart shape using colored clay, taking picture of a real heart, sketching the heart model with color pencil and drawing heart presented in the anatomy textbook. Data was analyzed by t-test, ANNOVA test using the SPSS/PC WIN 12 version. Result: A statistically significant differences in the level of heart anatomy knowledge acquirement was noted after four different instructional media being applied, and four different instructional media was effective to the anatomy practice education. However, no difference in statistical post test results was noted among the four groups. Conclusion: It is recommended that further comparative studies on the learning effect between human cadaver practice and different instructional media is necessary.

Analysis of Traumatic Brain Injury Using a Finite Element Model

  • In this study, head injury by impact force was evaluated by numerical analysis with 3-dimensional finite element (FE) model. Brain deformation by frontal head impact was analyzed to evaluate traumatic brain injury (TBI). The variations of head acceleration and intra-cranial pressure (ICP) during the impact were analyzed. Relative displacement between the skull and the brain due to head impact was investigated from this simulation. In addition, pathological severity was evaluated according to head injury criterion (HIC) from simulation with FE model. The analytic result of brain damage was accorded with that of the cadaver test performed by Nahum et al.(1977) and many medical reports. The main emphasis of this study is that our FE model was valid to simulate the traumatic brain injury by head impact and the variation of the HIC value was evaluated according to various impact conditions using the FE model.

피부흡수 대체시험법의 조건설정을 통한 수용성, 지용성 물질의 투과 특성 연구 (A Permeation Characteristics Study of Water- or Oil-soluble Substances through Condition Setting for the In Vitro Skin Absorption Method)

  • Objectives: The purpose of this study was to compare permeation characteristics in three skin types using oil-soluble benzoic acid and water-soluble caffeine after method condition optimization based on OECD guideline 428. Methods: A Franz diffusion cell, a reliable alternative method for skin permeation, was used. One-milliliter samples were taken and immediately replaced with fresh solution in the receptor chamber at regular time intervals (1, 2, 4, 7, 10 and 24 hr). The amount of test substances was measured by LC-MS/MS. Results: The permeation rate increased dose-dependently, and the permeation orders were $KeraSkin^{TM}$ > hairless mouse full skin > human cadaver epidermis for skin types, and benzoic acid solution > caffeine solution > benzoic acid cream > caffeine cream for type of test materials. Conclusion: According to the definitions of Marzulli, benzoic acid and caffeine would be classified as 'fast' and 'moderate' compared with the permeation of other chemical species. The setting conditions and permeation characteristics performed in this study are expected to contribute to future permeation studies.

  • 본 논문에서는 작은 여성 승객의 충돌해석을 위한 유한요소 모델링에 대하여 Part I 의 분절된 강체 모델에 이어 소개하고 있다. 추가로 모델링 된 상세한 팔, 다리 및 내부 요소들이 분절된 강체 모델에 합체되어 충돌 상해 즉 골절 또는 탈골 등을 예측하게 된다. 이러한 사지 및 내부 요소들의 거동 특성은 확보된 사체 실험 결과와 비교, 검증하였다. 따라서 본 논문에서 제안하고 있는 작은 여성 인체 모델은 충돌 상해 기구학의 조사 및 현존하는 충돌 더미의 생체 충실도를 보완하는데 활용될 수 있다.
