Effects of Cervico-Thoracic Mobilization Technique and Therapeutic Exercise on Muscle Activity, Functional Disability, Craniovertebral angle in Patient with Neck Pain (목 통증 환자에게 목-등뼈 가동술과 치료적 운동이 근활성도, 기능장애, 머리척추각에 미치는 영향)

  • Purpose: This study aimed to determine the effects of cervico-thoracic (C-T) mobilization technique and therapeutic exercise on muscle activity, craniovertebral angle (CVA), and neck disability index (NDI) in patients with neck pain. Methods: : A total of 30 patients with neck pain were included and divided into C-T mobilization combined with Pilates group (Experimental group; EG) and thoracic mobilization and self-exercise (Control group; CG) group; each group consisted of 15 patients. The exercises were performed by patients in both groups three times a week, for four weeks. NDI was used to measure functional disability and sEMG was used to measure muscle activity. Results: The EG participants showed significant improvement in the CVA and NDI after the intervention than the CG (p<.05). While both groups presented after intervention decreased muscle activity(upper trapezius), there were no statistically significant differences between the groups (p>.05). Conclusion: The findings of this study suggest that thoracic mobilization technique combined with Pilates exercise may have beneficial effects on CVA and NDI in patients with neck pain and forward head posture.

Yearly Report on CVA Patients (뇌졸중 환자에 대한 연례보고(2005년))

  • Objectives : This study was prepared for investigating the clinical features of stroke patients. Methods : We retrospectively reviewed the medical records of 75 patients who were admitted to Department of Internal Medicine, College of Oriental Medicine, Kyung-Won University with a diagnosis of stroke from Jan. 1, 2005 to Dec. 31, 2005. Results : Ischemic stroke(include TIA, 93.3%) was more common than hemorrhagic stroke(6.7%). The incidence in male was 36.0%, in female was 64.0% and the most prevalent age group is over-sixties. Cerebral infarction was most frequently noticed in MCA-infarction and hemorrhage in putamen & thalamus. Hypertention, the most preceding diseases, followed by diabetes mellitus and CVA. The rate of recurrence was 21.3%. Cerebral infarction and cerebral hemorrhage was much occurred after rising. The most patients visited the hospital within 24 Hours. The most common symptoms in admission time were motor weakness and speech disorder. The complication was mostly urinary tract inflamation. Conclusions : Our study on CVA patients was similar to previous studies from 1994 to 2004. In most cases, western and oriental treatment and medicine were given synthetically.

The rate of recurrence was 27.45%. Cerebral infarction and cerebral hemorrhage was much occurred after rising. The most patients visited the hospital within 24 Hours. The most common symptoms in admission time were motor weakness and speech disorder. The complication was mostly mental disorder. Conclusion : Our study on CVA patients was similar to previous studies from 1994 to 2003. In most cases, western and oriental treatment and medicine were given synthetically.

Effect of Exercise Intervention on Craniovertebral Angle and Neck Pain in Individuals With Forward Head Posture in South Korea: Literature Review

  • Forward head posture (FHP) is a musculoskeletal disorder that causes neck pain. Several exercise interventions have been used in South Korea to improve craniovertebral angle (CVA) and relieve neck pain. There has been no domestic literature review study over the past 5 years that has investigated trends and effects of exercise intervention methods for CVA with neck pain. This domestic literature review aimed to evaluate the trends and effects of exercise interventions on CVA and neck pain in persons with FHP. A review of domestic literature published in Korean or English language between 2018 and 2022 was performed. Literature search was conducted on Google Scholar and Korea Citation Index by using the following keywords: "exercise," "exercise therapy," "exercise program," "forward head posture," and "neck pain." Ten studies were included in this review. All of the studies showed positive improvements after intervention programs that included exercises. Notably, four of these studies demonstrated significant differences in results between the experimental and control groups. Among the 10 studies, nine measured visual analogue scale or numerical rating scale scores and reported significant reductions in pain following interventions, including exercise programs. Five of these studies showed significant differences in results between the experimental and control groups. Furthermore, six studies that used neck disability index exhibited a significant decrease in symptoms after implementing intervention programs that included exercise, and significant differences in results were found between the experimental and control groups. This domestic literature review provides consistent evidence to support the application of various exercise intervention programs to improve CVA and relieve neck pain from FHP. Further studies are warranted to review the effects of various exercise interventions on FHP reported not only in domestic but also in international literature.

  • The purpose of this study suggest that Park Golf for rehabilitation training of the patients with hemiplegia. The increase of the aged caused various problems. Among the problems, chronic illnesses the old people suffer from charge their family with mental, physical, social and economic burden as well as the person. Especially, the more old people are the more risks and attacks of cerebral vascular accident(CVA) exist. Therefore, the increase of the aged means the increase of cerebral vascular accident(CVA) patients. The cerebral vascular accident(CVA) patient get into difficulties to secure their livelihood due to troubles and disoders, so medical care facilities and rehabilitation training are really necessary to help them return to their normal life with mental, physical and social capability. The rehabilitation of patient with hemiplegia is ongoing process. It may possibly accomplish throughout the whole life because it takes a lot of time and effort. In addition, an ultimate aim of the rehabilitation is returning and adaptation to society. In this way, it is hardly to expect a satisfied results of the rehabilitation without motive. Therefore, patient with hemiplegia rehabilitation motive will is the most important. If interesting of park golf the patient with hemiplegia can be absorbed in easily are applied to the rehabilitation training, the patient are will to participate in the training without troubling.

The Effects of Task-oriented Training on Kinetic Factors and Muscle Activities of CVA Patients (과제지향성 훈련이 뇌졸중 환자의 운동역학적 변인 및 근활성도에 미치는 영향)

  • The purpose of this study was to examine the effects of Task-oriented training on the balance, lapse time and MVC(Maximum Voluntary Contraction) of CVA(cerebral vascular accident) patients. The active balance equipment was used to measure of the static balance, lapse time and task-oriented training. The EMG technique was used to record muscle activitie of affected side of gluteus medius and vastus medialis. The raw EMG data were filtered with band pass filter (60Hz) to remove artifacts and then low pass filtered (20Hz) to find the linear envelope which resemble muscle tension curve. The experiment had been conducted at the department of physical therapy in J hospital in M city during 8weeks. The thirty patients experienced the stroke were participated: training group (15), control group (15). They were ambulatory with or without an assistive device. They were assessed on central perturbation(mm) in the static balance, lapse time(s) and MVC test(mV). The data were analyzed using repeated measured ANOVA. The results were as follows: After Task-oriented training, central perturbation and lapse time was significantly differences in both groups(p<.001), and MVC in gluteus medius were no significant differences in both groups, but vastus medialis was significant differences in both groups (p<.001).

  • Nowadays Asthma is considered to be an inflamatory disease characterized by airway hyperresponsiveness and pulmonary eosinophilia. Hominis Placenta is the dried placenta of a healthy women. It has correspondence to the meridians of lung and kindey. Hominis Placenta acupuncture therapy has effect on invigoration of vital energy nourishing blood and tonifying the essence. It can be applied to the disease as Asthma, pulmonary tuberculosis, chronic hepatitis, liver cirrhosis, degenerative change and cerebrovascular disease. We treated two patients of Asthma with CVA by Homins Placenta Aqua-Acupuncture. The effect of Homins Placenta Aqua-Acupuncture was assessed by analyzing the pulmonary function test(PFT) and Quality of Life Questionnaire for adult Korean Asthmatics(QLQAKA) in patients before and after treatment. Total score was increased. The patients are satisfied our treatment. But further research concerning this is still necessary.

Effects of the BeHaS Exercise Program on Hand strength, Depression and Life Satisfaction in Institutionalized Elders with CVA (베하스(BeHaS) 운동프로그램이 시설에 거주하는 뇌졸중 노인의 악력과 우울 및 생활 만족도에 미치는 효과)

  • Purpose: This study was done to identify the effects of the BeHaS exercise program consisting of exercise, education, and cognitive supports on the hand strength, depression and life satisfaction in institutionalized elderly people with CVA. Method: A quasi-experimental design with an experimental group (n=18) and a control group (n=18) was used. The elders in the experimental group participated in the program once a week for eight weeks. Data were analyzed using descriptive statistics, Chi-square and t-test with SPSS/Win 18.0. Results: Hypothesis 1 was supported as the hand strength of the elders in experimental group significantly increased compared to those in the control group. Hypothesis 2 was also supported as depression scores for elders in the experimental group significantly decreased compared to those in the control group. Hypothesis 3 was supported as life satisfaction scores for elders in the experimental group significantly increased compared to those in the control group. Conclusion: The results of this study indicate that the BeHaS exercise program can play a useful role in improving depression and life satisfaction in institutionalized elderly people with CVA.

The Biomechanical Correlation Analysis of Upper Body according to Forward Head Posture (머리전방자세에 따른 상체의 생체역학적 상관분석)

  • Background: The purpose of this study is to analysis of correlation upper body according to forward head posture. Methods: The subjects of this study were 40 female university students who were equally and randomly allocated to a forward head posture group, normal group. Using general posture system, electromyograph, visual analogue scale, tape measurement, neck disability index were evaluated. Results: There was positive correlation between posture analysis and Sternocleidomastoid, neck flexion (p<.05). There was positive correlation between Craniovertebral angle (CVA) and trapezius upper, VAS (p<.05). There was negative correlation between posture analysis and CVA (p<.05). There was negative correlation between Cranial rotation angle and CVA (p<.05). Conclusion: Increased forward head posture lead to increase of pain, muscles activity, so it suggests to be necessary on the prevention of dysfunction and limited activities daily living.

  • Objectives : This study was prepared for investigating the clinical features of stroke patients. Methods : We retrospectively reviewed the medical records of 177 patients who were admitted to the Dept. of Internal Medicine, College of Oriental Medicine, Kyungwon University with a diagnosis of stroke from Jan. I, 1999 to Dec. 31, 1999. Results : Ischemic stroke (74.0%) was more common than hemorrhagic stroke (19.8%). The proportion of males was 53.7%, of females was 46.3%, and the most prevalent age group is those in their sixties. Cerebral infarction was most frequently noticed in MCA territory, hemorrhage in putamen. Hypertension was the most common preceding disease, followed by diabetes mellitus. The rate of recurrence was high in cerebral infarction. The onset mode of cerebral infarction was sleeping and rising and of cerebral hemorrhage was acting. Most patients visited the hospital within 24 hours. The most common symptoms at admission were motor weakness and speech disorder. The most common complication was urinary tract infection. Admission period was 29.5 days. Physical treatment from onset in cerebral infarction and in cerebral hemorrhage was 17.4 and 22.0 days, respectively. Conclusions : Our study of CVA patients was similar to previous studies from 1994 to 1998. In most cases, western and oriental treatment and medicine were given synthetically. Prescription of sasang constitutional medicine had wide application.
