  • Purpose: Diffuse axonal injury (DAI) is clinically defined as a coma of over six hours in a head trauma victim without a focal mass lesion. The emergency physician usually resuscitates and stabilizes a comatose head trauma victim in the emergency Department. After assessment and treatment, the prognosis is very important to both the victim and the physician. The prognosis for DAI is based on Glasgow Coma Scale (GCS) and other imaging data. We investigated the prognostic value of computed tomography (CT) and gradient-echo magnetic resonance imaging (GRI) for head trauma victims with DAI. Methods: Fifty-three(53) head trauma victims of DAI were enrolled in this study from 2007 to 2012. During the study period of six years, data on trauma victims were collected retrospectively. We analyzed the differences in the Glasgow Outcome Scale (GOS) result between the CT and the GRI modalities. Results: We classified the study group by using GOS. Between the good outcome subgroup (GOS scores of 4 and 5) and the poor outcome subgroup (GOS score of 1-3), there were no statistical difference in sex, age, initial vital signs and initial GCS score. The good outcome subgroup had non-hemorrhage on CT(52%), which was correlated with good outcome and a shorter awakening time, while a larger number and a deeper location of hemorrhagic lesions on in GRI were correlated with poor outcome in DAI. Conclusion: We conclude that the existence of hemorrhagic lesions on CT, and the number and location of those lesions on GRI had good prognostic value for head trauma victims with DAI.

  • In the present case, the effect and toxicity of carboplatin and cyclophosphamide chemotherapy combined with surgery of mammary tumors in a dog were examined. An 8-year old spayed female Beagle presented with a mammary tumor. Physical examination, radiography, ultrasonography, computed tomography (CT), and laboratory examination were performed. Metastasis of the mammary tumor was confirmed by CT scan. Chemotherapy using a combination of carboplatin and cyclophosphamide was initiated following surgery. The first cycle of chemotherapy was planned to last for 6 weeks; it was planned that carboplatin would be intravenously administered for the first week (1 day) and cyclophosphamide would be intravenously administered for the next 3 weeks (22 days). Between the end of cycle 1 and the beginning of cycle 2, based on CT, it was confirmed that the number and size of tumors were unchanged and the tumors had not spread to other organs. However, at the end of cycle 2 and the beginning of cycle 3, CT revealed an increase in the number and size of mass in the lung. Chemotherapy was associated with adverse effects such as lethargy, anorexia, leukopenia, and hair loss. In conclusion, this case showed that a combination of carboplatin and cyclophosphamide suppressed the development of new neoplasms as well as metastasis for a certain period of time but did not improve the survival time. Although more cases are required, this chemotherapeutic procedure remains challenging.

  • In this work, Rhizophora spp. particleboard phantoms were made using SPI-based adhesives, modified with sodium hydroxide and itaconic acid polyamidoamine-epichlorohydrin (0, 5, 10, and 15 wt%). An X-ray computed tomography (CT) imaging system was used to ascertain the CT numbers and density distribution profiles of the particleboards. The SPI-based/NaOH/IA-PAE/Rhizophora spp. particleboard phantoms with 15 wt% IA-PAE addition level had the highest solid content, flexural strength, flexural modulus, and internal bonding strength of 36.06 ± 1.08%, 18.61 ± 0.38 Nmm-2, 7605.76 ± 0.89 Nmm-2, and 0.463 ± 0.053 Nmm-2, respectively. The moisture content, mass density, water absorption, and dimensional stability were 6.93 ± 0.27%, 0.962 ± 0.037 gcm-3, 22.36 ± 2.47%, and 10.90 ± 0.86%, respectively. The results revealed that the mass attenuation coefficients and effective atomic number values within the 16.59-25.26 keV photon energy region, were close to the calculated XCOM values in water, with a p-value of 0.077. Moreover, the CT images showed that the dissimilarities in the discrepancy of the profile density decreased as the IA-PAE concentrations increased. Therefore, these results support the appropriateness of the SPI-based/NaOH/IA-PAE/Rhizophora spp. particleboard with 15 wt% IA-PAE adhesive as a suitable tissue-equivalent phantom material for medical health applications.

  • Objective : placement of ventricular catheter is important to achieve long-term patency of the shunt system. We describe a method of calculating the insertion site, trajectory, and the length of the ventricular catheter using CT parameters in shunt surgery. Patients and Methods : These can be rapidly obtained using a caliper from a scout cut and two axial scans at lateral ventricular and upper 3rd ventricular level. To compare this technique with traditional one, we analyzed 40 consecutive patients who underwent shunt surgery. Results : Of 20 patients undergoing ventriculoperitoneal shunt insertion using this technique, none had poor location of the proximal catheter. In the counterpart, 9 patients had poor location(p=0.001). The number of patients who required revision surgery were also lower in the group using this technique, but it was not statistically significant (4 versus 2, p=0.422). Mean follow-up period was shorter in this technique group. Conclusion : This technique provides an accurate placement of the proximal catheter without special instrument or additional expenses.

  • A new and modified neural network model Is proposed for CT image reconstruction from four projection directions only. The model uses the Resilient Back-Propagation (Rprop) algorithm, which is derived from the original Back-Propagation, for adaptation of its weights. In addition to the error in projection directions of the image being reconstructed, the proposed network makes use of errors in pixels between an image which passed the median filter and the reconstructed one. Improved reconstruction was obtained, and the proposed method was found to be very effective in CT image reconstruction when the given number of projection directions is very limited.

  • The cardiovascular disease has been known as a common cause of death for a long time in the west. The eating habits of Asia, including Korea, have changed recently, so that this disease is also a problem in Asia now. Annual Report on the Cause of Death Statistics from 1996 to 2006 reported that the cardiovascular disease would become the number one cause of death in the next $5{\sim}10$ years. Therefore we realize that more accurate examination is required. The aim of this study was to investigate the accuracy of Calcium-scoring CT and the relationship between risk factor and quantitative scores of Calcium-scoring CT. Through this study we expect that the national public health will be improved. Seventy patients with chest pain were chosen at random. The patients were undergone both coronary CT antigraphy and Calcium - scoring CT at G hospital in Incheon from February 1 to June 30, 2008. The result of the Calcium-scoring CT showed its usefulness for Ischemic cardiovascular disease, with an accuracy similar to that of exercise/pharmacologic stress or ECG when it is difficult for a patient to exercise due to joint problems, aging or for other reasons.

  • Background: Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is a standard procedure to evaluate suspicious lymph node involvement of lung cancer because computed tomography (CT) and $^{18}F$-fluorodeoxyglucose positron emission tomography-CT (PET-CT) have limitations in their sensitivity and specificity. There are a number of benign causes of false positive lymph node such as anthracosis or anthracofibrosis, pneumoconiosis, old or active tuberculosis, interstitial lung disease, and other infectious conditions including pneumonia. The purpose of this study was to evaluate possible causes of false positive lymph node detected in chest CT or PET-CT. Methods: Two hundred forty-seven patients who were initially diagnosed with lung cancer between May 2009 and December 2012, and underwent EBUS-TBNA to confirm suspicious lymph node involvement by chest CT or PET-CT were analyzed for the study. Results: Of 247 cases, EBUS-TBNA confirmed malignancy in at least one lymph node in 189. The remaining 58 patients whose EBUS-TBNA results were negative were analyzed. Age ${\geq}65$, squamous cell carcinoma as the histologic type, and pneumoconiosis were related with false-positive lymph node involvement on imaging studies such as chest CT and PET-CT. Conclusion: These findings suggest that lung cancer staging should be done more carefully when a patient has clinically benign lymph node characteristics including older age, squamous cell carcinoma, and benign lung conditions.

  • Objectives : The purpose on this study is to show the anti-obesity effect of Eugenia caryophyllata $T_{HUNB.}$ pharmacopuncture in mice fed a high fat diet. Methods : Male ICR mice were divided into three groups, which were fed a normal AIN diet(N), a high fat diet(CT), a high fat diet and injected with 50 mg/kg of Eugenia caryophyllata $T_{HUM.}$ pharmacopuncture(EC) for 8 weeks. Results : 1. The levels of body weight gains were significantly increased in CT compared with N and decreased in EC compared with CT. 2. The levels of absolute liver weight and adipose-tissue weight were significantly decreased in EC compared with CT. The levels of relative adipose-tissue weight were significantly decreased in EC compared with CT. 3. The levels of total cholesterol(TG), triglycerid(TC), high density lipoprotein(HDL) cholesterol and low density lipoprotein(LDL) cholesterol were significantly increased in CT compared with N and decreased in EC compared with CT. The levels of HDL/LDL ratio were shown there were no significant differences in all groups. 4. The levels of adiponectin in serum were remarkably decreased in CT compared with N and significantly increased in EC compared with CT. 5. The levels of measuring adipocyte number in adipose tissue was significantly decreased in CT compared with N and increased in EC compared with CT. Conclusions : These results suggest that Eugenia Caryophyllata $T_{HUNB.}$ has an anti-obesity effect in mice and that the effect is mediated by inhibition of fat gain.

  • The aim of this study was to achieve optimal portal phase while reducing contrast medium by applying weight-based dose protocol compared to standard fixed dose protocol to performing of pediatric abdominal CT examination. Discovery 750HD (General Electric Medical Systems, Milwaukee, USA) was used, and a total of 167 children consisting of 85 men and 82 women under the age of 18 were studied. The group in which the 300 mgI/ml(Xenetix, Guerbet, France) contrast medium was fixedly injected at twice body weight and the group injected with physiological saline while gradually decreasing the injection amount by 10% while applying the weight-based protocol were distinguished. Also, the CT number and SNR of abdominal organs were compared and evaluated while changing the scan delay time. Subjective image quality of enhancement and beam-hardening artifacts of around the heart was assessed with five-point criterion. The group adapted weight-based protocol with 20% reduction in contrast medium was most similar in contrast enhancement in the group with fixed injection at twice body weight. Furthermore, the group with a delay time of 20% had the highest contrast enhancement effect, and the difference in CT attenuation coefficient from the group scanned immediately after injection of the contrast media. Therefore, the appropriate delay time after injection of the contrast agent increased the contrast enhancement of the parenchymal organ. In addition, the weight-based injection protocol with normal saline reduced artifacts around the heart, and the effect of contrast enhancement could be maintained. In conclusion, it is possible to reduce dosage of contrast media through the application of weight-based injection protocols and appropriate latency, and to characterize optimal portal phase imaging on pediatric abdominal CT.

  • This study measured the exposure dose during abdominal-pelvic CT exam which occupies 70% of CT exam and tried to propose a protocol for optimized exposure dose in abdomen and pelvis without affecting the imagery interpretation. The study scanned abdomen-pelvis using the current clinical scan method, the 120 kVp, auto exposure control(AEC), as 1 phase. As for the newly proposed 2 phase scan method, the study divided into 1 phase abdomen exam and 2 phase pelvis exam and each conducted tube voltage 120 kVp, AEC for abdomen exam, and fixed tube current method in 120 kVp, 100, 150, 200, 250, 300, 350, 400 mA for pelvis exam. The exposure dose value was compared using $CTDI_{VOL}$, DLP value measured during scan, and average value of CT attenuation coefficient, noise, SNR from each scan image were obtained to evaluate the image. As for the result, scanning of 2 phase showed significant difference compared to 1 phase. In $CTDI_{VOL}$ value, the 2 phase showed 26% decrease in abdomen, 1.8~59.5% decrease in pelvis for 100~250 mA, 12.7%~30% increase in pelvis for 300~400 mA. Also, DLP value showed 53% decrease in abdomen and 41~81% decrease in pelvis when scanned by 2 phase compared to 1 phase, but it was not statistically significant. As for the SNR, when scanning 2 phase close to heart, scanning 1 phase close to pelvis, scanning and scanning 1 phase at upper and lower abdomen, it was higher when scanning 2 phase for 200~250 mA. Also, the CT number and noise was overall similar, but the noise was high close to pelvis. However, when scanning 2 phase for 250 mA close to pelvis, the noise value came out similar to 1 phase, and did not show statistically significant difference. It seems when separating pelvis to scan in 250 mA rather than 400 mA in 1 phase as before, it is expected to have reduced effect of exposure dose without difference in the quality of image. Thus, for patients who often get abdominal-pelvic CT exam, fertile women or children, this study proposes 2 phase exam for smaller exposure dose with same image quality.
