Clinical Laboratory Aspect of Carbapenem-Resistant Enterobacteriaceae (카바페넴내성장내세균속균종의 임상검사 측면)

  • The correct distinction of carbapenem-resistant Enterobacteriaceae (CRE) and ccarbapenemase producing Enterobacteriaceae (CPE) and the rapid detection of CPE are important for instituting the correct treatment and management of clinical infections. Screening protocols are mainly based on cultures of rectal swab specimens on selective media followed by phenotypic tests to confirm a carbapenem-hydrolyzing activity, the rapid carbapenem inactivation method, lateral flow immunoassay, the matrix-assisted laser desorption ionization-time-of-flight test and molecular methods. The CPE is accurate for detection, and is essential for the clinical treatment and prevention of infections. A variety of phenotypic methods and gene-based methods are available for the rapid detection of carbapenemases, and these are expected to be routinely used in clinical microbiology laboratories. Therefore, to control the spread of carbapenemase, many laboratories around the world will need to use reliable, fast, high efficiency, simple and low cost methods. Optimal effects in patient applications would require rapid testing of CRE to provide reproducible support for antimicrobial management interventions or the treatment by various types of clinicians. For the optimal test method, it is necessary to combine complementary test methods to discriminate between various resistant bacterial species and to discover the genetic diversity of various types of carbapenemase for arriving at the best infection control strategy.

A Study of Depression Observed Among The Elderly Residing in Home For The Aged In Korea (양로원 재원노인의 우울에 관한 연구)

  • Using Zung's self-rating depression scale, the authors studied depression in 310 elderlies residing in the 9 home for the aged (HFA) in Kyungbuk area, and 234 elderlies living in Taegu city and 107 elderlies attending a Life-long education program in Taegu, which serve as a control group, Community resident elderlies(CRE). The research had been administered during the period from June to August, 1986. The results were as follows : 1. The mean scores of male. female. and the total of the elderlies in HFA were $38.95{\pm}11.55$, $44.18{\pm}14.15$:: and $42.8{\pm}13.7$ respectively while CRE marked $40.8{\pm}11.3$ for male, $45.2{\pm}12.0$ for female, and $43.4{\pm}11.9$ for the total. Therefore there were significant differences between male and female in both groups(p<0.01, p<0.001), though no significant difference between the two groups. However, the depression score of elderlies in HFA might be much higher than that of CRE if 81 elderlies(14.2%) in HFA who had been left out of the statistical evaluation due to their having severe depression, organic brain syndrom, or pseudoementia, etc had been included. 2. The score distribution by items for the elderlies in HFA were from highest scores hopelessness, worthlessness, emptiness, decreased appetite, confusion, while for CRE, hopelessness, decreased appetite, psychomotor retardation, indecisiveness, and worthlessness. Elderlies in HFA showed significantly high scores in depressed mood, weight loss, suicidal rumination(ideation) and psychomotor excitement, while CRE showed significantly high scores in decreased appetite, psychomotor retardation, indecisiveness, and dissatisfaction. 3. Elderlies who scored over 50 numbered 10 males(16%), 57 females(34%), and total of 67(29%) in HFA and 28 males(21%), 77 females(37%), and total of 105(31%) in CRE : female showed higher seores in both groups. 4. Psychosocial factors such as getting older(respectively p<0.01, p<0.01), being Buddhist(respectively p<0.01, p<0.01), and monthly pocket money less than \30,000(respectively p<0.001, p<0.001) were found to have a noticable impact on the depression level of the elderlies in both groups. Factors such as illiteracy (p<0.001, monthly pocket money less than \10,000(p<0.05), and having no family(p<0.01) recorded significantly higher scores among CRE than the elderlies in HFA.

Quality characteristics of Yanggaeng with extracts and powder of roasted coffee ground residue (커피박 추출물 및 분말 첨가 양갱의 품질 특성)

  • Quality characteristics of Yanggaeng prepared with water extract (CRE) and powder (CRP) of roasted coffee ground (CR) were investigated. Total polyphenol content and DPPH raidcal scavenging ability of water extract of CRE were 13.52 g/mL and 78.75%, respectively. The pH ranges of Yanggaeng prepared with CRE (CREY) and CRP (CRPY) were 7.10~7.29 and 6.95~7.15, respectively. The DPPH radical scavenging activities of CREY containing 0.1~1.0% CRE showed 8.77~43.10% and CRPY containing 0.1~1.0% CRP showed 5.28~14.92%. The total polyphenol contents and DPPH radical scavenging activity of CREY and CRPY increased significantly with increasing CRE and CRP concentrations (p<0.05). Sensory evaluation which includes taste, flavor, texture, and overall acceptability of CREY and CRPY were higher than that of control. The overall acceptability showed the highest levels in Yanggaeng containing 0.5% CRE and containing 0.3% CRP. These results indicate the potential use of roasted coffee ground residue as a valuable resource for development of side menu in coffee restaurants.

Anti-inflammatory effects of Coptidis Rhizoma in chronic cold stress-exposed mice (만성 저온 스트레스 동물모델에서의 황련(黃連)의 항염증 효능 연구)

  • Objectives : The aim of this study was to investigate whether the extract of Coptidis Rhizoma inhibits inflammation in chronic cold stress (CCS)-exposed mice or not. Methods : Coptidis Rhizoma extract (CRE) was made by reflux with distilled water. Male ICR mice (7 weeks old) were divided randomly into 5 groups: (1) control, (2) CCS, (3) CCS+CRE 100 mg/kg, (4) CCS+CRE 300 mg/kg, (5) CCS+CRE 1,000 mg/kg groups. Mice were orally administered once a day for 14 days starting from 1 day before CCS. Group (2)-(5) were exposed to CCS conditions that maintained at $4^{\circ}C$ for 2 h once a day for 14 days. The levels of serum cortisol and hypothalamic prostaglandin E1 (PGE1) and PGE2 were measured by enzyme-linked immunosorbent assay kit. The expression levels of several pro-inflammatory factors like heat shock protein 70 (HSP70), c-fos, and nuclear factor kappa-light-chain-enhancer of activated B cells (NF-kB) were measured by western blot analysis in mouse hypothalamus. Results : Oral administration of CRE 1,000 mg/kg significantly suppressed the increase of serum cortisol levels in mice exposed to CCS. CCS-exposed mice had significantly increased the expression of HSP70, c-fos, and NF-kB in hypothalamus, while CRE treatment significantly attenuated the elevation of these pro-inflammatory factors. The ratio of PGE2/PGE1 was also higher in CCS-exposed mice than control group. CRE treatment significantly reduced the increase of PGE2/PGE1 ratio induced by CCS. Conclusion : These findings suggest that Coptidis Rhizoma may work as a potential agent to modulate inflammatory responses under the condition of cold adaptation formed by CCS.

Enhancing Cellulase Production in Thermophilic Fungus Myceliophthora thermophila ATCC42464 by RNA Interference of cre1 Gene Expression

  • The role of CRE1 in a thermophilic fungus, Myceliophthora thermophila ATCC42464, was studied using RNA interference. In the cre1-silenced strain C88, the filter paper hydrolyzing activity and β-1,4-endoglucanase activity were 3.76-, and 1.31-fold higher, respectively, than those in the parental strain when the strains were cultured in inducing medium for 6 days. The activities of β-1,4-exoglucanase and cellobiase were 2.64-, and 5.59-fold higher, respectively, than those in the parental strain when the strains were cultured for 5 days. Quantitative reverse-transcription polymerase chain reaction showed that the gene expression of egl3, cbh1, and cbh2 was significantly increased in transformant C88 compared with the wild-type strain. Therefore, our findings suggest the feasibility of improving cellulase production by modifying the regulator expression, and an attractive approach to increasing the total cellulase productivity in thermophilic fungi.

Immunomodulatory Effects of Cimicifugae Rhizoma Extracts in Macrophages

  • Cimicifugae Rhizoma (CR) belongs to the Ranunculaceae family, which has been traditionally used to treat climacteric complaints, antipyretics and diaphoresis as an alternative medicine for estrogen hormone replacement therapy with estrogens. Recently, it has been reported that different extract fractions of CR have various effects such as anti-allergic, anti-inflammatory and anti-proliferative activities. The current study investigated the immunomodulatory effects of Cimicifugae Rhizoma water extracts (CRE) in the macrophage-like cell line, Raw 264.7. Our results showed that CRE ($1\sim50\;{\mu}g/mL$) stimulated tumoricidal activity and NO production, whereas phagocytic activity was inhibited at the same concentrations. Additionally, iNOS mRNA expression was significantly increased in Raw 264.7 exposed to CRE as demonstrated by RT-PCR. These results indicate that the tumoricidal activity induced by CRE may be mediated by the production of NO and these activities may be useful for the treatment of diseases such as cancer.

CReMeS: A CORBA COmpliant Reflective Memory based Real-time Communication Service

  • We present CReMeS a CORBA-compliant design and implementation of a new real-time communication service. It provides for efficient predictable and scalable communication between information producers and consumers. The CReMeS architecture is based on MidART's Real-Time Channel-based Reflective Memory (RT-CRM) abstraction. This architecture supports the separation of QoS specification between producer and consumer of data and employs a user-level scheduling scheme for communicating real-time tasks. These help us achieve end-to-end predictability and allows our service to scale. The CReMeS architecture provides a CORBA interface to applications and demands no changes to the ORB layer and the language mapping layer. Thus it can run on non real-time Off-The-Shelf ORBs enables applications on these ORBs to have scalable and end-to-end predictable asynchronous communication facility. In addition an application designer can select whether to use an out-of-band channel or the ORB GIOP/IIOP for data communication. This permits a trade-off between performance predictability and reliability. Experimental results demonstrate that our architecture can achieve better performance and predictability than a real-time implementation of the CORBA Even Service when the out-of-band channel is employed for data communication it delivers better predictability with comparable performance when the ORB GIOP/IIOP is used.

Cre-Lox: A Tool for Removal of Marker Genes to Make GM Foods Safe

  • The green revolution has significantly helped in increasing the food production. So far, various breeding methods have been exploited, besides them recombination DNA technology provides another approach for increasing the food production. By means of this technology the losses in food production incurred by various biotic and abiotic stresses can be effectively controlled. In most of the transgenic studies scientists have used antibiotic resistant genes as markers for easy selection of transformants but there are risks involved in use of GM foods. To make such foods safer and environment friendly we have discussed a novel strategy i.e. Cre-lox which involves site specific recombination. By means of Cre-lox the marker genes can be specifically removed once the selection of transformants is over. In addition, this strategy can be used to module the hybrid chromosomes, avoid gene silencing and incorporate single copy of a transgene for its higher expression.

Energy-Aware Self-Stabilizing Distributed Clustering Protocol for Ad Hoc Networks: the case of WSNs

  • In this paper, we present an Energy-Aware Self-Stabilizing Distributed Clustering protocol based on message-passing model for Ad Hoc networks. The latter does not require any initialization. Starting from an arbitrary configuration, the network converges to a stable state in a finite time. Our contribution is twofold. We firstly give the formal proof that the stabilization is reached after at most n+2 transitions and requires at most $n{\times}log(2n+{\kappa}+3)$ memory space, where n is the number of network nodes and ${\kappa}$ represents the maximum hops number in the clusters. Furthermore, using the OMNeT++ simulator, we perform an evaluation of our approach. Secondly, we propose an adaptation of our solution in the context of Wireless Sensor Networks (WSNs) with energy constraint. We notably show that our protocol can be easily used for constructing clusters according to multiple criteria in the election of cluster-heads, such as nodes' identity, residual energy or degree. We give a comparison under the different election metrics by evaluating their communication cost and energy consumption. Simulation results show that in terms of number of exchanged messages and energy consumption, it is better to use the Highest-ID metric for electing CHs.

Postretention stability after mandibular incisor extractions assessed by cast and radiograph - The American Board of Orthodontics cast and radiographic evaluation (모형과 방사선 사진을 이용한 하악 전치 발치 치료의 안정성 평가 - 미국교정학회에서 제시한 방법)

  • Purpose: The purpose of this study was to evaluate the improvement and posttreatment stability of patients treated with extraction of lower incisors. Materials and Methods: The total of 20 patients with extracted lower incisors were analyzed by means of diagnostic models and panoramic x-rays at the time of initial, final and after 2 years of retention period of the treatment. Irregularity index, overjet, overbite, tooth size-arch length discrepancy (TSALD), intercanine width, intermolar width and American Board of Orthodontics cast/radiographic evaluation (ABO-CRE) were analyzed. Statistical analysis was performed using Wilcoxon signed-rank test. Results: After treatment, irregularity index showed significant decrease (P = 0.000). TSALD showed significant increase (P = 0.028). During retention period, irregularity index showed significant increase (P = 0.001). For ABO-CRE, total score showed significant decrease after treatment (P = 0.000) and showed average decreased which was not significant result (P = 0.053). Conclusion: Through evaluation of stability of extraction of lower incisors by means of diagnostic models and panoramic x-rays, it can be concluded that lower incisor extraction treatment had been stable for 2 years after treatment.
