Clinical Analysis of Intraoperative Rupture of Cerebral Aneurysms (수술 중 뇌동맥류 파열에 대한 임상 분석)

  • Objective : Intraoperative rupture of an intracranial aneurysm can interrupt a microsurgical procedure and jeopardize the patient's chance to favorable outcome. The purpose of this study was to analyse and evaluate intraoperative aneurysmal rupture and render ideal prevention and management to intraoperative rupture. Patients and Methods : The authors retrospectively analysed the results of 609 patients who underwent cerebral aneurysm surgery from January 1991 to December 2000. Results : 1) Intraoperative aneurysmal rupture occurred in 73 of 609 consecutive aneurysm surgery, so the incidence was about 12.0% and it was relatively lower than other reports. 2) Aneurysms arising from anterior communicating artery appeared more prone to intraoperative rupture. 3) The size of aneurysm and timing of operation didn't influence intraoperative aneurysmal rupture and temporary clipping didn't reduce the incidence of intraoperative aneurysmal rupture. 4) Intraoperative aneurysmal rupture occured during three specific periods : (1) dissection stage in 61%, (2) clip application stage in 29 %, (3) predissection stage in 10%. 5) In the patients with intraoperative aneurysmal rupture, surgical outcome was relatively good and there was no significant difference in outcome compared with unruptured group. Conclusion : Our suggestion for prevention methods of intraoperative aneurysmal rupture are as follows : 1) minimal brain retraction, 2) sharp and careful aneurysmal neck dissection, 3) gentle clipping with proper clip selection etc. Management methods after intraoperative aneurysmal rupture are as follows : 1) strong aspiration of bleeding point, 2) rapid application of temporary and/or tentative clip, 3) following rapid dissection of neck and proper clip application, 4) use of encircling clip etc.

  • During laparoscopic cholecystectomy, a surgical clip is used to control the cystic duct and cystic artery. In the past, metallic clips were usually used, but over recent years, interest in the use of Hem-o-lok clips has increased. Surgical clip migration into the common bile duct (CBD) after laparoscopic cholecystectomy has rarely been reported and the majority of reported cases involved metallic clips. In this report, we describe the case of a 53-year-old woman who presented with abdominal pain caused by migration of a Hem-o-lok clip into the CBD. The patient had undergone laparoscopic cholecystectomy 10 months previously. Abdominal CT revealed an indistinct, minute, radiation-impermeable object in the distal CBD. The object was successfully removed by sphincterotomy via ERCP using a stone basket and was identified as a Hem-o-lok clip.

  • Many rubber products are used in industrial products such as various hoses, rubber belts and oil seals etc. Especially, more then 200 rubber parts are used in the automobile, but design technology of these is largely dependent on Held experiences. These methods brought about too much time and cost in the developing procedures. However, with the help of recent rapid development of non-liner computer analysis, we can develop new sound products at low cost. Therefore in this study, optimizations of design variables such as location and number of gate in order to develop CLIP rubber product made of EPDM were performed by CAE in which Cross-WLF equations are adopted. The validity of proposed variables is evaluated by comparison with real forming results.

  • Owing to the focal wall defect covered with thin fibrous tissues, an aneurysm arising from the dorsal wall of the internal carotid artery (ICA) is difficult to manage either surgically or endovascularly and is often associated with high morbidity and mortality. Unfortunately, the definitive treatment modality of such highly risky aneurysm has not yet been demonstrated. Upon encountering the complex intracranial pathophysiology of such a highly precarious aneurysm, a neurosurgeon would be faced with a challenge to decide on an optimal approach. This is a case of multiple paraclinoid aneurysms including the ICA dorsal wall aneurysm, presented with spontaneous subarachnoid hemorrhage. With respect to treatment, direct clipping with a Sundt graft clip was performed after multiple endovascular interventions had failed. This surgical approach can be a treatment modality for a blood blister-like aneurysm after failed endovascular intervention(s).

  • Two concurrent signals of the pulse wave measured from both hands' radial artery in un-pressurization condition using the prototype model of two clip-type pulsimeters with a permanent magnet and Hall device are investigated. The phase differences of two pulse waves from 22 subjects have some distinct points according to the handedness. Thus, the propagation of the pulse wave calculated from phase difference is both fast and slow to each other. It is confirmed that this phenomenon comes from the difference of blood vessel hardness between right- and left-hand of each subject rather than a quantity of muscle.

  • We measured radial arterial pulse signals using a prototype of a clip-type pulsimeter equipped with a permanent magnet and a Hall device, which produced signals through a voltage-detecting circuit. The systolic peak time and the reflective peak time for a temporally pulsed signal were analyzed for an arbitrary pulse wave at one position of a small permanent magnet. The measured value of the peripheral pulse wave velocity was about 1.25-1.52 m/s, demonstrating the accuracy of this new method. To measure the peripheral blood flow velocity, we simultaneously connected the radial artery pulsimeter to a photoplethysmography meter. The average value of the peripheral blood flow velocity was about 0.27-0.50 m/s.

  • 본 연구에서는 비디오 하이라이트 검출 및 장면 추출을 위한 경량화된 모델인 Visual Context Learner (VCL)을 제안한다. 기존 연구에서는 매개변수가 고정된 CLIP을 비롯한 여러 피쳐 추출기에 학습 가능한 DETR과 같은 트랜스포머를 이어붙여서 학습을 한다. 하지만 본 연구는 경량화된 구조로 하이라이트 검출 성능을 개선시킬 수 있음을 보인다. 그리고 해당 형태로 장면 추출도 가능함을 보이며 장면 추출의 추가 연구 가능성을 시사한다. VCL은 매개변수가 고정된 CLIP에 학습가능한 프롬프트와 MLP로 하이라이트 검출과 장면 추출을 진행한다. 총 2,141개의 학습가능한 매개변수를 사용하여 하이라이트 검출의 HIT@1(>=Very Good) 성능을 기존 CLIP보다 2.71% 개선된 성능과 최소한의 장면 추출 성능을 보인다.

Intestinal Perforation Caused by Lumboperitoneal Shunt Insertion Repaired with an Over-the-Scope Clip

  • Lumboperitoneal or ventriculoperitoneal shunt insertion is a standard therapy for hydrocephalus that diverts cerebrospinal fluid from the subarachnoid space into the peritoneal cavity. Gastrointestinal perforations due to this procedure occur rarely; however, accepted treatment strategies have not yet been established. Hence, the most common treatment approaches are open surgery or spontaneous closure without endoscopy. We report the case of a small intestinal perforation in a 73-year-old-woman that occurred after the insertion of a lumboperitoneal shunt. A positive cerebrospinal fluid culture and high cerebrospinal fluid white blood cell count indicated a retrograde bacterial infection, and computed tomography revealed that the peritoneal tip of the shunt catheter was located in the lumen of the gastrointestinal tract. We repaired the perforation endoscopically using an over-the-scope clip, and the patient's recovery was uneventful. Use of an over-the-scope clip could be an effective and minimally invasive treatment for intestinal perforations caused by lumboperitoneal or ventriculoperitoneal shunt insertion.

Stability of the prosthetic screws of three types of craniofacial prostheses retention systems

  • Objectives: This study aimed to evaluate the stability of prosthetic screws from three types of craniofacial prostheses retention systems (bar-clip, ball/O-ring, and magnet) when submitted to mechanical cycling. Materials and Methods: Twelve models of acrylic resin were used with implants placed 20 mm from each other and separated into three groups: (1) bar-clip (Sistema INP, São Paulo, Brazil), (2) ball/O-ring (Sistema INP), and (3) magnet (Metalmag, São Paulo, Brazil), with four samples in each group. Each sample underwent a mechanical cycling removal and insertion test (f=0.5 Hz) to determine the torque and the detorque values of the retention screws. A servo-hydraulic MTS machine (810-Flextest 40; MTS Systems, Eden Prairie, MN, USA) was used to perform the cycling with 2.5 mm and a displacement of 10 mm/s. The screws of the retention systems received an initial torque of 30 Ncm and the torque values required for loosening the screw values were obtained in three cycles (1,080, 2,160, and 3,240). The screws were retorqued to 30 Ncm before each new cycle. Results: The sample was composed of 24 screws grouped as follows: bar-clip (n=8), ball/O-ring (n=8), and magnet (n=8). There were significant differences between the groups, with greater detorque values observed in the ball/O-ring group when compared to the bar-clip and magnet groups for the first cycle. However, the detorque value was greater in the bar-clip group for the second cycle. Conclusion: The results of this study indicate that all prosthetic screws will loosen slightly after an initial tightening torque, also the bar-clip retention system demonstrated greater loosening of the screws when compared with ball/O-ring and magnet retention systems.
