뇌종양 환자의 양성자 치료 시 Range Compensator의 Smooth Thickness 적용에 따른 선량비교 (Dose comparison according to Smooth Thickness application of Range compensator during proton therapy for brain tumor patient)

  • 목 적 : 양성자 치료 시 사용되는 Range Compensator는 Target의 Distal Margin의 선량에 대해 정상조직에 전달되는 양성자 빔 선량을 보정하는 역할을 한다. 이에 뇌종양 치료에 사용되는 Range Compensator의 Smooth Thickness를 다르게 적용함에 따른 PTV와 OAR의 선량을 비교하여 대상 부위의 선량이 개선되는 것을 확인해 보고자 한다. 대상 및 방법 : 본원에서 양성자 치료를 받은 뇌종양 환자 10명을 대상으로 Eclipse Proton Planning System(Version 10.0, Varian, USA)의 Compensator Editor를 사용하여 Range Compensator에 적용되는 Smooth Thickness를 각각 1회에서 5회까지 순차적으로 적용하였다. 치료계획의 알고리즘은 Proton Convolution Superposition(version 8.1.20 or 10.0.28)을 사용하였고, Smooth Thickness를 단계적으로 적용함에 따른 PTV의 Dmax, Dmin, Homogeneity Index, Conformity Index 그리고 종양주위의 OAR 선량을 비교하였다. 결 과 : Smooth Thickness를 1회에서 5회까지 적용하였을 때 PTV의 최대선량(Dmax)은 최대 4.3%, 최소 0.8%, 평균 1.81% 감소하였으며, 최소선량(Dmin)은 최대 1.8%, 최소 0.2%, 평균 0.82% 증가하였고, 최대선량과 최소선량의 차이는 최대 5.9%, 최소 1.4%, 평균 2.63% 감소하였다. Homogeneity Index는 평균 0.018 감소하였고 Conformity Index는 거의 변화가 없었다. OAR 선량은 Brain Stem에서 최대 1.6%, 최소 0.1%, 평균 0.59% 감소하였으며, Optic Chiasm에서 최대 1.3%, 최소 0.3%, 평균 0.45% 감소하였으나, C와 E환자가 각각 0.3%, 0.6% 증가하였다. 그리고 Rt. Optic Nerve에서 최대 1.5%, 최소 0.3%, 평균 0.8% 감소하였으나, B환자가 0.1% 증가하였다. Lt. Optic Nerve에서는 최대 1.8%, 최소 0.3%, 평균 0.67% 감소하였으나, H환자가 0.4% 증가하였다. 결 론 : 뇌종양 환자의 양성자 치료에 사용되는 Range Compensator의 Smooth Thickness가 단계적으로 적용될수록 Compensator의 해상도가 증가하여 가장 최적화된 양성자 빔 선량을 전달할 수 있다. 이는 PTV에 좀 더 균일한 선량을 조사할 수 있고 또한 OAR에 작용하는 불필요한 선량을 감소시켜 부작용을 줄일 수 있을 것으로 사료된다.

다발성 후하소뇌동맥 해리성 동맥류 - 증 례 보 고 - (Multiple Dissecting Aneurysms of the PICA Trunk - Case Report -)

  • Apatient with multiple dissecting aneurysms of a posterior inferior cerebellar artery trunk who presented with SAH is reported. A 58-year-old woman presented with sudden occipital headache, dizziness and vomiting. The intial vertebral angiography revealed a suspicious pearl and string sign at the proximal posterior inferior cerebellar artery(PICA) segment. After 2 weeks, follow up angiography showed a progression of the proximal PICA dissection and newly developed dissecting aneurysm of the distal PICA segment. A far lateral suboccipital transcondylar appoach confirmed two dissecting aneurysms at distant sites of the PICA trunk. The dissection segments were wrapped with muslin wrap, which preserved the flow through the PICA and brain stem perforators. The angiographys at 3 weeks and 6 months after operation revealed serial disappearance of the dissecting aneurysms which is distal to proximal. The diagnosis, course and treatment of the dissecting aneurysms of the PICA are discussed with literature review.

후대뇌동맥 원위부에 발생한 거대동맥류 1례 - 증례보고 - (A Case of the Giant Aneurysm in the Distal Portion of the Posterior Cerebral Artery - A Case Report -)

  • The aneurysm arising from the posterior cerebral artery is relatively uncommon, and has been reported in 0.7- 2.2% of whole cerebral aneurysm. Moreover, only a few cases of the giant aneurysm over 2.5cm in diameter arising from the distal portion of the posterior cerebral artery have been reported. Neurologic complications may occur after surgical treatment of this aneurysm due to difficult surgical approach and neurovascular complexity around the brain stem. Authors experienced a case of the thrombosed giant aneurysm with approximately 2.7cm in diameter arising from the P2 segment of the left posterior cerebral artery. Complete removal of the thrombus in the aneurysmal sac and direct neck clipping was performed via left subtemporal approach.

Endoscopic Coagulation of Choroid Plexus in Hydranencephaly

  • Hydranencephaly is a rare congenital disease defined as an absence of cerebral hemispheres with an intact thalamus, brain stem, and cerebellum. Generally, patients with hydranencephaly require cerebro-spinal fluid diversion due to progressive hydrocephalus. An excellent view of the choroid plexus and advancement of the neurosurgical endoscope were possible, leading to easy coagulation of the choroid plexus to balance cerebro-spinal fluid production without the use of a shunt device. We present this rare case and good treatment outcome from endoscopic coagulation of the choroid plexus.

Staging in Vestibular Schwannoma Surgery : A Modified Technique

  • The authors herein propose the staged excision as a novel strategy to preserve facial nerve and minimize complication during microsurgery of large vestibular schwannoma (VS). At the first stage, for reducing mass effect on the brain stem and cerebellum, subtotal tumor resection was performed via a retrosigmoid craniotomy without intervention of meatal portion of tumor. With total resection of the remaining tumor, the facial nerve was decompressed and delineated during the second stage translabyrinthine approach at a later date. A 38-year-old female who underwent the staging operation for resection of her huge VS is illustrated.

Hemifacial Spasm Caused by Fusiform Aneurysm at Vertebral Artery-Posterior Inferior Cerebellar Artery Junction

  • Hemifacial spasm induced by intracranial aneurysm is a rare clinical condition. A 45-year-old male patient presented with a 3-year history of progressive involuntary twitching movement on right face. On radiological study, a dilated vascular lesion compressing the brain stem was found at the junction of vertebral artery and posterior inferior cerebellar artery. On operative field, we found the posterior inferior cerebellar artery and the fusiform aneurysm compressing root exit zone of facial nerve. Microvascular decompression was performed and the facial symptom was relieved without complications.

  • Hearing loss is one of the most common birth defects among infants. Most hearing-impaired children are not diagnosed until one to three years of age, which is too late to treat for normal speech and language development. If a hearing impairment is identified and treated in its early stage, child's speech and language skills could be comparable to his or her normal-hearing peers. In this study, we applied the 'Fsp' method to distinguish between normal and impaired hearing. We have developed a battery-operated portable A - ABR(automated auditory brain stem response) system that automatically detects hearing impairment for neonates or infants in a nursery room, as well as in a sound-proof room. We partially validated the accuracy of the system in five normal-hearing adults.

위스타 랫드의 충동성 조절에 대한 세로토닌의 역할 (Serotonin as a Possible Modulator of Impulsive Behavior in Wistar Rats)

  • We compared impulsive behaviors in Wistar rats and in Wistar-Kyoto rats. There was no significant difference in locomotor activity between them. However, Wistar rats showed high activity in 5-choice serial reaction time track. When Wistar rats were treated with atomoxetin (3 mg/kg), methylphenidate (2 mg/kg) or amphetamine (2 mg/kg), they showed less impulsive behavior. Serotonin contents in prefrontal cortex and brain stem also increased. In conclusion, we suggest that Wistar rats could be used as animal model for impulsive behavior analysis. In addition, serotonin might be related with this impulsivity.

Effects of Isopropyl Alcohol Infusions on the Ruminating Behavior of Goats

  • Metabolites, such as isopropyl alcohol (IPA) produced by rumen fermentation, were intravenously infused into a jugular vein of goats during feeding to explore the mechanism and roles of IPA in ruminating behavior (number of boli and ruminating time). Three female goats were confined in metabolism cages with a stanchion, The ruminating behavior measured by the number of ruminations, ruminating time, number of remastications, and remasticating time decreased (p<0,05) with intravenous IPA infusion. The IPA concentrations and VFA concentrations increased in the blood circulation. Our data suggest that sensitive receptors of rumination to IPA are more likely to be in an area such as the brain stem where they can respond to blood metabolite levels.

Effects of Intraruminal Isopropyl Alcohol Infusions on the Ruminating Behavior of Goats

  • Isopropyl alcohol (IPA), produced from acetone by rumen bacterial action, was infused into the rumen of three female goats kept in a climatically controlled experimental room during feeding to investigate the mechanism and roles of IPA in ruminating behavior (number of boli and ruminating time). The ruminating behavior measured by the number of boli, ruminating time, number of remastications, and remasticating time increased (p<0.05) with intraruminal IPA infusion. The concentrations of IPA and acetone in the rumen and the plasma significantly increased (p<0.05) during intraruminal IPA infusion. These data suggest that rumination receptors sensitive to IPA and acetone may be in an area such as the rumen epithelium and the brain stem where they can respond to metabolite levels.
