골수내 정상적인 혈구형성은 줄기세포와 주위의 미세환경 사이에 일어나는 상호작용에 의해서 좌우된다고 알려져 있다. 이때 미세환경은 성장요소, 기질세포 및 기질세포가 분비한 세포외기질의 복잡한 망상구조로 이루어져 있다. 골수의 세포외기질은 거대분자로 구성되어 있으며 이중 구성요소의 일부인 당단백이 혈구형성의 조절에 주요한 요소가 된다고 많은 학자들이 주장해 왔다. 골수의 기질을 이루고 있는 망상세포, 지방세포, 섬유모세포, 혈관내피세포 및 혈관주위세포 등이 분비하는 세포외기질에 여러종류의 laminin 동형체가 함유되어 있으며 여러 종류의 혈구형성에 주요한 역할을 한다는 연구보고가 있다. 저자는 골수내 종양을 치료하기 위해 빈번히 사용되는 cyclophosphamide가 골수에 미치는 영향을 규명하고자 실험동물의 혈구형성에 영향을 미치는 laminin-1의 발현의 변화를 골수의 기질세포에서 추적하고자 하였다. 건강한 SD계 흰쥐를 실험동물로 사용하여 cyclophosphamide를 체중 kg당 75 mg을 복강내로 1회 투여하고 3일, 1주, 3주 및 5주 후 희생하여 골수조직을 채취하여 면역조직염색법으로 염색하여 laminin-1의 골수내 분포의 변화를 관찰하였고, 일부 조직은 전자현미경표본을 제작하여 1차 항체(rabbit anti-laminin)로 반응시킨 후 직경 12 nm의 금과립이 섞인 제2차 항체(biotinylated goat anti-rabbit IgG)로 다시 반응시키고 uranyl acetate로 단일염색 후 전자현미경으로 관찰하여 다음과 같은 결과를 얻었다. 1. Cyclophosphamide 투여 후 3일 경과군의 흰쥐 골수조직은 정맥동이 확장 되었고, 일부 조직이 괴사되었으며, 혈구형성세포가 감소되었다. 망상조직과 지방세포사이의 조직에서 laminin-1의 면역반응이 강하게 나타났다. 2. Cyclophosphamide 투여 후 1주, 2주 및 5주 투여군의 골수에서 laminin-1의 약한 면역반응이 기질조직에 계속되었고 혈구형성세포의 수는 증가되었다. 3. Laminin-1의 면역반응을 나타내는 금과립은 cyclophosphamide 투여 3일군의 골수 기질세포에서 가장 많은 수가 관찰되었고, 투여 1주, 3주 및 5주군에서는 금과립의 수량이 감소되었다. 이상과 같은 실험결과는 cyclophosphamide가 흰쥐에 투여되었을 때 골수의 혈구형성의 미세환경을 유지하는 기질세포의 laminin-1의 발현을 증가시키는 것으로 생각되었다.
조혈 줄기 세포에의 효과적인 유전자 전달은 유전자 치료의 새로운 가능성을 제시할 수 있다. 레트로바이러스를 이용한 유전자 전달 기술은 많은 기초 연구와 임상 시도가 이루어진 대표적인 바이러스이다. 그러나 현재 사용되고 있는 in vitro에서의 조혈 줄기 세포에의 유전자 도입은 조혈 줄기 세포의 분화 유도, 자기 복제 능력과homing 능력의 저하 등 많은 문제점이 있다. 본 연구는 이러한 문제점을 극복하기 위한 방법으로서 마우스의 대퇴골에 직접 레트로바이러스를 이식하는 IBM (Intra-Bone Marrow) 방법을 이용하여 조혈 줄기 세포에의 효과적인 유전자 도입을 시도하였다. IBM 이식 2주 후 마우스의 각 조직을 분석한 결과, 골수뿐 아니라 림파절, 비장, 간장 세포 등에서 유전자가 안정적으로 발현하는 것을 관찰하였다. 또한, $6.4{\pm}2.7%$의 골수조직 존재 조혈줄기/전구세포에서 도입된 유전자가 안정적으로 발현하고 있는 사실을 확인하였다. 본 연구의 결과를 바탕으로 IBM 이식 방법을 이용한 생체 조직 내 레트로바이러스의 유전자 도입은 조혈 줄기 세포를 이용한 유전자 치료에 매우 효과적인 방법이라는 사실을 시사해주고 있다.
In this study, Kami-honghwa-tang (KH-19) was designed and animal study was conducted to evaluate its efficacy on the reduction of the side effect of radiotherapy. Bone marrow toxicity is one of the major side effect of radiotherapy which cause the reduction of blood cells, and KH-19 was designed to protect and enforce blood. C57BL/6 mice were irradiated with 4 Gy of gamma ray, and divided into control group which was treated with water and KH-19 group which was treated with 1.5g/Kg of KH-19 up to 4 weeks. KH-19 group showed significantly increased white blood cells, lymphocytes and platelet count compared with control group (p<0.05). When bone marrows were examined, KH-19 group showed higher cell densities than control group (p=0.06). KH-19 may increase blood cell count after radiation by its protective effects on bone marrow.
Lethally irradited C3H/HeN mice were transplanted with syngeneic bone marrow. The B cell regeneration levels of spontaneous serum Ig, fecal igA and specific ig to diphtheria toxoid were determined at various time points. The number of B220+ cells reached normal range at 4 weeks after bone marrow transplantation(BMT) in spleen and lymph node. The B cell number of spleen returned to normal relatively soon than in the lymph node. Within 5 to 7 weeks after BMT, the transplanted mice contained nearly normal levels of spontaneous serum IgA, IgG2b and fecal IgA, but 2 fold lower levels of serum IgG2a, IgM and IgG3. Especially IgG3 levels were within low-normal range throughout the study. One to two weeks after immunization the predominant anti-diphtheria toxoid subtype was IgM. The levels of specific serum Ig were very low and after booster immunization at week 6, the short-lasting increase of Ig production was notd.
Background: Non-Hodgkin's lymphoma (NHL) is a heterogeneous type of neoplasm of the lymphatic system. To have a more accurate and early diagnosis we need to know signs, symptoms and complications of lymphoma in early stages besides pathology and immunohistochemistry. Materials and Methods: This prospective study included 110 cases of NHL that were followed since February 2012 till November 2013. Biopsies were taken from all the patients besides bone marrow study. Signs and symptoms were categorized into "B" symptoms, general, lymphadenopathy and extranodal involvement and we compared the frequencies by stage and grade. Results: Of 110 cases, 88.9% had B-cell and 11.1% T-cell type with mean age $48.5{\pm}18.6$ years. "B" symptoms and lymphadenopathy were more common in men. Cervical lymphadenopathy was the most common sign (44.8%). and hematologic, bone marrow, bone and neurologic lesions were the most common complications. All complications were more common in males. "B" symptoms were seen mostly in stage III, general signs and symptoms in stage IV, and lymphadenopathy in stage II. Intermediate grade was also the most common in all signs and symptoms. In this study 12 (10.9%) patients had relapse, with neurologic and bone marrow as the most common sites of tumor recurrence. Conclusions: There is a meaningful relationship between male gender for NHL and anemia that can be due in part to higher incidence of bone marrow involvement and stage IV disease in male cases. We also found a strong relationship between low grade NHL and age. On the other hand extranodal involvement is more common in female groups.
Inherited bone marrow failure syndrome (IBMFS) encompasses a heterogeneous and complex group of genetic disorders characterized by physical malformations, insufficient blood cell production, and increased risk of malignancies. They often have substantial phenotype overlap, and therefore, genotyping is often a critical means of establishing a diagnosis. Current advances in the field of IBMFSs have identified multiple genes associated with IBMFSs and their pathways: genes involved in ribosome biogenesis, such as those associated with Diamond-Blackfan anemia and Shwachman-Diamond syndrome; genes involved in telomere maintenance, such as dyskeratosis congenita genes; genes encoding neutrophil elastase or neutrophil adhesion and mobility associated with severe congenital neutropenia; and genes involved in DNA recombination repair, such as those associated with Fanconi anemia. Early and adequate genetic diagnosis is required for proper management and follow-up in clinical practice. Recent advances using new molecular technologies, including next generation sequencing (NGS), have helped identify new candidate genes associated with the development of bone marrow failure. Targeted NGS using panels of large numbers of genes is rapidly gaining potential for use as a cost-effective diagnostic tool for the identification of mutations in newly diagnosed patients. In this review, we have described recent insights into IBMFS and how they are advancing our understanding of the disease's pathophysiology; we have also discussed the possible implications they will have in clinical practice for Korean patients.

Chronic myeloid leukemia (CML) is a stem cell disorder characterized by unrestricted proliferation of the myeloid series that occurs due to the BCR-ABL fusion oncogene as a result of reciprocal translocation t(9;22) (q34;q11). This discovery has made this particular domain a target for future efforts to cure CML. Imatinib revolutionized the treatment options for CML and gave encouraging results both in case of safety as well as tolerability profile as compared to agents such as hydroxyurea or busulfan given before Imatinib. However, about 2-4% of patients show resistance and mutations have been found to be one of the reasons for its development. European Leukemianet gives recommendations for BCR-ABL mutational analysis along with other tyrosine kinase inhibitors (TKIs) that should be administered according to the mutations harbored in a patient. The following overview gives recommendations for monitoring patients on the basis of their mutational status.

Bone marrow stroma is a complex tissue encompassing a number of cell types and supports hematopiesis, differentiation of erythreid, nyel and lymphoid lineages, and also maintains undifferentiated hematopoietic stem cells. Marrow-derived stem cells were composed of two populations, namely, hematopoietic stem cells that can differentiate into blood elements and mesenchymal stem cells that can give rise to connective tissues such as bone, cartilage, muscle, tendon, adipose and stroma. Differentiation requires environmental factors and unique intracellular signaling. For example, $TGF-{\beta}$ or BMP2 induces osteoblastic differentiation of mesenchymal stem are very exciting. However, the intrinsic controls involved in differentiation of stem cells are yet to be understood properly in order to exploit the same. This review presents an overview of the recent developments made in mesenchymal stem cell research with respect to osteogenesis.
