The choice of suitable hip implant is one of important factors in the total hip arthroplasty (THA). In clinical view point, an improper adaptation of hip implant might induce abnormal stress distribution to the bone, which can shorten the lifespan of replaced hip implant. Currently, interest in the custom-designed hip implants has increased as studies reveal the significance of geometric shape of patient's femur in modeling and designing the implants. In this study, we have developed custom-designed hip implant models with various sizes, and analyzed the stress distribution in the bone and bone cement using the Finite Element Method. It was found that minimizing the gap between implant stem and femoral cavity is crucial to minimize the stress concentration in the bone.
Purpose: Modeling has been used for elucidating the mechanism of complex biosystems. In spite of importance and uniqueness of adolescent calcium (Ca) metabolism characterized by a threshold Ca intake, its regulatory mechanism has not been covered and even not proposed. Hence, this study aims at model-based proposing potential mechanisms regulating adolescent Ca metabolism. Methods: Two different hypothetic mechanisms were proposed. The main mechanism is conceived based on Ca-protein binding which induces renal Ca filtration, while additional mechanism assumed that active renal Ca re-absorption regulated Ca metabolism in adolescents. Mathematical models were developed to represent the proposed mechanism and simulated them whether they could produce adolescent Ca profiles in serum and urine. Results: Simulation showed that both mechanisms resulted in the unique behavior of Ca metabolism in adolescents. Based on the simulation insulin-like growth factor-1 (IGF-1) is suggested as a potential regulator because it is related to both growth, a remarkable characteristic of adolescence, and Ca metabolism including absorption and bone accretion. Then, descriptive modeling is employed to conceptualize the hypothesized mechanisms governing adolescent Ca metabolism. Conclusions: This study demonstrated that modeling is a powerful tool for elucidating an unknown mechanism by simulating potential regulatory mechanisms in adolescent Ca metabolism. It is expected that various analytic applications would be plausible in the study of biosystems, particularly with combination of experimental and modeling approaches.
Objective: The aim of this study was to evaluate the biomechanical aspects of peri-implant bone upon root contact of orthodontic microimplant. Methods: Axisymmetric finite element modeling scheme was used to analyze the compressive strength of the orthodontic microimplant (Absoanchor SH1312-7, Dentos Inc., Daegu, Korea) placed into inter-radicular bone covered by 1 mm thick cortical bone, with its apical tip contacting adjacent root surface. A stepwise analysis technique was adopted to simulate the response of peri-implant bone. Areas of the bone that were subject to higher stresses than the maximum compressive strength (in case of cancellous bone) or threshold stress of 54.8MPa, which was assumed to impair the physiological remodeling of cortical bone, were removed from the FE mesh in a stepwise manner. For comparison, a control model was analyzed which simulated normal orthodontic force of 5 N at the head of the microimplant. Results: Stresses in cancellous bone were high enough to cause mechanical failure across its entire thickness. Stresses in cortical bone were more likely to cause resorptive bone remodeling than mechanical failure. The overloaded zone, initially located at the lower part of cortical plate, proliferated upward in a positive feedback mode, unaffected by stress redistribution, until the whole thickness was engaged. Conclusions: Stresses induced around a microimplant by root contact may lead to a irreversible loss of microimplant stability.

The aim of the study is to interpret the distribution of occlusal force by 3-dimensional finite element analysis of ISP(Implant Supported Prosthesis) supported by minimum number of implant to restore the edentulous patients. For this study, the Astra Tech implant system is used. Geometric modeling for 6 and 4 fixture ISP group is performed with respect to the bone, implant and one piece superstructure, respectively. Implants are arbitrarily placed according to the anatomical limit of lower jaw and for the favorable distribution of occlusal force, which is applied at the end of cantilever extension of ISP with 30mm. Element type is tetrahedral for finite element model and the typical mechanical properties, Young's modulus and Poisson's ratio of each material, cortical, cancellous bone and implant material are utilized for the finite element analysis. From this study, we can see the distribution of equivalent stress equal to real situation and speculate the difference in the stress distribution in the whole model and at each implant fixture, From the analysis, the area of maximum stress is distributed on distal contact area between bone and fixture in the crestal bone. The maximum stress is 53MPa at the 0.2mm area from the bone-implant interface in the maximum side for 300N load condition for 4 fixture case, which is slightly less than the stress calculated from allowable strain. This stress has not been deduced to directly cause the loss of crestal bone around implant fixture, but the stress can be much reduced as the old peoples may have lower chewing force. Thus, clinical trial may be performed with this treatment protocol to use 4 fixtured ISP for old patients.
Background: The European Organisation for Research and Treatment of Cancer Quality of Life Questionnaire Bone Metastases Module (EORTC QLQ-BM22) is a recently designed supplement to EORTC Quality of Life Questionnaire-C30 (EORTC QLQ-C30). Additional psychometric properties, especially using confirmatory factor analysis (CFA) and the Rasch model, are warranted. Materials and Methods: A total of 573 patients with bone metastases were enrolled from eight countries with a mean${\pm}$SD age of $55.8{\pm}13.7years$. Slightly more than two thirds of them were female (n=383; 66.8%). CFA was used to examine the BM22 framework; Rasch models were applied to understand misfit items and differential item functioning (DIF). Results: The fit indices were satisfactory in CFA (comparative fit index=0.972, Tucker-Lewis index=0.964, root mean square error of approximation=0.076, and standardized root mean square residual=0.045). All items fit well in the Rasch models (mean square values were between 0.5 and 1.5), and only one item (number 17) displayed DIF across gender. However, there were six DIF items across Canada and Taiwan, ten across Canada and Iran, and six across Taiwan and Iran. Conclusions: The BM22 has satisfactory psychometric properties, and could assess the QoL of patients with bone metastases specifically focusing on their symptoms. Clinicians may want to use it to capture the underlying QoL for patients with bone metastases. However, the score of item 17 should be interpreted with caution when comparing male and female patients. In addition, researchers should note that variation in DIF items may occur when conducting an international study.
In 3D printing technology, materials that can be printed are increasing along with the development of material engineering, and materials that can be used in the field of radiation are also increasing. Therefore, depending on the composition and density of the materials used, the applied field can be different and applied, so the composition and characteristics of the materials must also be considered. In this study, 10 filaments with different properties were selected using a 3D printer of the FDM (Fused Deposition Modeling) method, and the brightness change of each filament was checked using a diagnostic X-ray generator, and the CT number was measured through CT. I wanted to find a material similar to bone. As a result, a material called silicon carbide was found, which has a similar brightness and CT number to bone. It is thought that further research will be presented as basic data for various studies with a density similar to that of human bones.
DICOM images of patellofemoral bones were converted into a stereolithography file, and a Unigraphics CAD program was used to create a CAD modeling in which there exists point, line and facet information. The modeling extraction of joint facets was performed by linking two adjacent points into lines in the stereolithography file by using the Unigraphics rapid spacing function and then linking the lines into facets to complete the entire modeling. This modeling extraction was performed based on the anatomical knowledge of joint facet directions. As a result, a personalized space modeling and solid modeling were produced for the joint facets of patellofemoral bones. This was followed by a CAM control computing operation of solid modeling on graphite materials and 5-axis machining of patellofemoral bones. That is the description of a method for a personalized implant modeling by using DICOM images of patellofemoral bones.
The 3D printing selective laser sintering (SLS) and stereo lithography apparatus (SLA) method used for bone model production has good precision and resolution, but the printers are expensive and need professional knowledge for operation. The program that converts computed tomography digital imaging and communications in medicine (DICOM) file into STL (stereolithography) file is also expensive so requesting 3D printing companies takes a lot of time and cost, which is why they are not generally utilized in surgery. To produce bone models of fractured patients, the use of 3D imaging conversion program and 3D printing system should be convenient, and the cost of device and operation should be low. Besides, they should be able to produce big size bone models for application to surgery. Therefore, by using an fused deposition modeling (FDM) method 3D printer that uses thermoplastic materials such as DICOM Viewer OsiriX and plastic wires, this study developed 3D printing system for Fracture surgery Patients customized bone model production for many clinics to use for surgery of fracture patients by universalizing with no limit in printing sizes and low maintenance and production cost. It is expected to be widely applied to the overall areas of orthopedics' education, research and clinic. It is also expected to be conveniently used in not only university hospitals but also regular general hospitals.

A method to visualize human body is proposed for various human pose. The method affords three 3D-styles of the same body: firstly, one which wares clothes specified from pattern of dresses, second, body shape, lastly bone structure of body. For this objective, standard body data are prepared which is constructed from CT images. Individual body is measured by 3D body scanner. The present status of our research is limited to offer still images, though we are engaged to accommodate various poses.
