Substituent Effect on the Structure and Biological Property of 99mTc-Labeled Diphosphonates: Theoretical Studies

  • Theoretical calculations based on density functional theory (DFT) were performed to study the substituent effect on the geometric and electronic structures as well as the biological behavior of technetium-99m-labeled diphosphonate complexes. Optimized structures of these complexes are surrounded by six ligands in an octahedral environment with three unpaired 4d electrons ($d^3$ state) and the optimized geometry of $^{99m}Tc$-MDP agrees with experimental data. With the increase of electron-donating substituent or tether between phosphate groups, the energy gap between frontier orbitals increases and the probability of non-radiative deactivation via d-d electron transfer decreases. The charge distribution reflects a significant ligand-to-metal electron donation. Based on the calculated geometric and electronic structures and biologic properties of $^{99m}Tc$-diphosphonate complexes, several structure-activity relationships (SARs) were established. These results may be instructive for the design and synthesis of novel $^{99m}Tc$-diphosphonate bone imaging agent and other $^{99m}Tc$-based radiopharmaceuticals.

Bone Marrow Immunoscintigraphy for the Detection of Skeletal Metastasis in Malignant Tumors: A Comparison with $^{99m}Tc$-MDP Bone Scan (악성종양에서 골수면역신티그라피를 이용한 골수전이의 평가 : $^{99m}Tc$-MDP 뼈스캔과의 비교)

  • Although bone scan is a highly sensitive test for detecting bone metastasis, its findings are often limited in specificity and cannot be used for assessing the bone marrow. Bone marrow scintigraphy may provide useful information but previous experience with radiolabelled colloid has been disappointing. Recently, $^{99m}Tc$ labeled anti-granulocyte monoclonal antibody (anti-NCA-95 MAb) has been introduced as a new bone marrow imaging agent. To evaluate the usefulness of $^{99m}Tc$ anti-NCA MAb bone marrow scans for detecting skeletal metastasis, bone marrow scans of 44 malignant tumor patients were evaluated and compared with bone scan findings. Bone scan showed abnormal lesions in 26(59%) cases, and 18 of these patients also had an abnormal bone marrow scan. Seven of the 8 patients who had normal bone marrow scan despite bone scan lesions were confirmed to be free from metastasis. There was one case with a marrow defect despite normal bone scan but the presence of metastasis was not determined due to loss of follow up. Bone scan demonstrated a total of 64 lesions while bone marrow scan showed 38 lesions. Fifty percent (32/64) of the bone scan lesions had matching marrow defects while the remaining 50% did not. Most of these non matched lesions were suggested to be nonspecific lesions such as rib fractures or degenerative change. Meanwhile bone marrow scan was able to detect 6 new lesions not detected by bone scan, bit metastasis in each lesion was not confirmed. Bone marrow scan was also helpful in assessing equivocal bone scan lesions to be of metastatic nature in 10 patients by demonstrating a matched marrow defect. Thus $^{99m}Tc$ anti-NCA MAb bone marrow scan can help exclude metastasis in patients with nonspecific bone scan lesions and may be able to detect metastatic lesions not seen with bone scan. It appears useful as a complementary study to bone scan in evaluating malignant tumor patients.

  • Technetium-99m MIBI was developed as a myocardiac perfusion imagine agent and has been used effectively in the detection and post-therapeutic evaluation of various neoplasm such as thyroid, lung, bone and breast tumors. As an infrequent findings, Tc-99m MIBI agent has shown in non-neoplastic pulmonary conditions Including fibrosing alveolitis, pulmonary actinomycosis, active pulmonary sarcoidosis, pulmonary interstitial fibrosis in progressive systemic sclerosis and active osteomyelitis. In a recent report conducted by Cetin Oncel, Tc-99m MIBI imaging is an effective method in the detection and follow-up of pulmonary tuberculosis We have also experienced Tc-99m MIBI uptake in active pulmonary tuberculosis incidentally found in a patient with suspected proliferative villonodular synovitis of the left ankle.

Recalcitrant Low Back Pain Diagnosed as Hypophosphatemic Osteomalacia Induced by Antiviral Medication (항바이러스제에 의한 저인산성 골연화증으로 진단된 난치성 요통)

  • We report a rare case of anti-viral agent induced hypophosphatemic osteomalacia presented with localized and radicular pain. A 51-year-old man, who had been taking adefovir for chronic hepatitis, had experienced low back pain radiating to his right thigh for 2 years. With impression of lumbar disc herniation, he underwent magnetic resonance imaging and found multi-level disc herniation with facet joint synovial cysts. He received transforaminal epidural steroid injections, however, symptoms did not improve. To find other possible causes, additional tests were performed. Blood tests revealed hypophosphatemia and increased serum alkaline phosphatase, and osteoporosis was noted in dual-energy X-ray absorptiometry with multiple hot uptakes in bone scan. After replacement of adefovir to entecavir and supplement of phosphate and vitamin D, phosphate level and the clinical symptoms were improved. This is the first to report the presentation of osteomalacia due to anti-viral agent as radicular low back pain with facet synovial cysts.

A Study on Effective Application of $Methylenediphosphonate-^{99m}Tc$ ($Methylenediphosphonate-^{99m}Tc$의 효과적 이용에 관한 연구)

  • Biodistribution studies has been carried out to elucidate the cause of poor bone imagings often encountered in using $methylenediphosphonate(MDP)-^{99m}Tc$ and to establish effective conditions in using the popular bone imaging agent. After 150 minutes from the I.V. injection of $MDP-^{99m}Tc$ to mice, the radioactivities accumulated at bone(B), liver(L), and stomach(S) were counted. The radiochemical purity (RCP), the volume, the radioactivity concentration and the amount of radioactivity of $MDP-^{99m}Tc$ were controlled. Data were expressed either in %cpm/g organ or % cpm/organ. The organ distribution ratios(B/L and B/S) were correlated with the RCP, the volume of injection, the radioactivity concentration etc. Results indicated that the RCP plays a major role in biodistributions. High radioactivity concentration and injection of a small amount is recommended. Negligible effect was observed with the amount of radioactivity. It has been confirmed that the up-to-date methods of RCP determinations cannot sensitively detect the sharply affecting trace impurities. A particular biodistribution pattern of crossed B/L and B/S lines was observed in case of using $MDP-^{99m}Tc$ of low RCP. In such a case, rather a higher dosage would be effective for improving the contrast between bone and liver.

Diffuse Hepatic Uptake of $^{99m}Tc$-DPD on Whole Body Bone Scan: The Influence of MRI Contrast (전신 뼈 검사에서 $^{99m}Tc$-DPD의 미만성 간 섭취: MRI 조영제의 영향)

  • Purpose : The whole body bone scan on nuclear medicine is a widely accepted examination and procedure. However, unusual nonosseous uptake can be observed, which reflects a rare interaction between the radiopharmacceutical and the patient. This study aimed to evaluate the influence of MRI(Magnetic Resonance Imaging) contrast and $^{99m}Tc$-DPD(Dicarboxpropane diphosphonate) on whole body bone scan. Materials and Methods : We analyzed the 982 patients who were examined by $^{99m}Tc$-DPD on whole body bone scan in nuclear medicine department of pusan national university hospital from january to december 2010. All these 982 patients had MRI contrast administration prior to whole body bone scan. We analyzed laboratory test. Results : 46 patients(men 39, women 7) showed diffuse hepatic uptake on whole body bone scan. These uptakes were disappeared on the follow-up whole body bone scan. There were no significant difference of CBC test, liver function tests and renal function tests. Conclusion : The study might be an indirect evidence that diffuse hepatic and splenic uptake of 99mTc-DPD on whole body bone scan after intravenous administration of Gadolinium(Gd) MRI contrast. To perform a precise examination, Gd-contrast agent should be removed from the body before performing a whole body bone scan.

Conservation and Scientific Analysis of Human Bone Excavated in Sabi Period of Baekje from Eungpyeong-ri, Buyeo (부여 응평리 출토 백제 사비기 인골 보존처리 및 과학적 분석)

  • The stone chamber tomb in Eungpyeong-ri, Buyeo, is a joint tomb that contains the bodies of two individuals. This paper investigates the relationship between the buried persons and the characteristics of the stone chamber tomb. Based on the geographical location, relics, and the excavated human bones, it was determined that the tomb was built during the Sabi Period of the Baekje Dynasty and that the buried individuals were most probably residents of high stature or government officials. To study the excavated bones, the remains were carefully collected and conservation was carried out. Before collecting samples from the human bones for the analytical research, the results of near-infrared analysis were used to collect the samples for the isotope analysis and DNA analysis. The most important issue when handling the excavation site was the reinforcing agent and the concentration of the agent used. In situations like this, Paraloid B-72 is the most suitable agent. When the shape of human bones was difficult to distinguish from the soil, conservation was performed using X-ray and CT imaging data. The same chemical used for the reinforcement of the site was used to complete a minimum level of conservation to the surface areas where the conservation treatment of removing foreign substances, the reinforcement areas, and bonded areas were carried out. The collagen yield from the sample obtained at selected position was 3.8% to 6.1%. The results of analyzing the stable isotopes of carbon and nitrogen found in the extracted collagen showed that the stable isotope ratios came out to δ13C -18.3‰±0.1‰, -19.0‰±0.1‰ for EBW and δ15N 10.7‰±0.5‰, 10.6‰±0.1‰ for EBE. It is believed the two individuals consumed small amounts of minor cereals, mainly from C3 plants, and protein was obtained from eating terrestrial animals. What's more, the deviations in data obtained from the two individuals were so small that it could be inferred that the individuals ate similar foods. Considering the preservation state of the sample, amplifying DNA for the DNA analysis would have been very difficult since the amount of surviving DNA was so deficient. For DNA analysis, it is anticipated that the results could be derived by applying improved extraction methods that will be developed in the future. In this research, any association between scientific analysis(DNA and stable isotope ratio) and near-infrared spectroscopy was difficult to establish. Further research is needed on the utilization of near-infrared analysis for gathering samples from human bones.

Effect of Carrier on Labeling and Biodistribution of Re-188-Hydroxyethylidene diphosphonate (담체가 Re-188-Hydroxyethylidene Diphosphonate의 표지와 생체내분포에 미치는 영향)

  • Purpose: Re-188-Hydroxyethylidene diphosphonate (HEDP) is a new cost-effective agent for systemic radioisotope therapy of metastatic bone pain. We investigated the influence of carrier for labeling and biodistribution of Re-188-HEDP using HEDP kit with or without carrier ($KReO_4$). Materials and Methods: The kits (HEDP 15 mg, gentisic acid 4 mg and $SnCl_2.2H_2O_2$ 4.5 mg) with or without carrier ($KReO_4$ 0.1 mg) were labeled with Re-188 solution, made available from an in-house generator by boiling for 15 min. We compared the labeling efficiency and stability of carrier-added and carrier-free preparations of Re-188-HEDP Biodistribution and imaging studies of each preparation were performed in ICR mice ($1.85{\sim}3.7MBq/0.1ml$) and SD rats ($74.1{\sim}85.2MBq/0.5ml$). Results: The carrier-added preparation showed high labeling efficiency (95% at pH 5) and high stability in serum (88%, 3 hr). However, the carrier-free preparation showed low labeling efficiency (59% at pH 5) and low stability (43%, 3 hr). The carrier-added preparation showed high uptake in bone and low uptake in stomach and kidneys. However, the carrier-free preparation showed lower uptake in bone and higher uptake in both stomach and kidneys, which is supposed to be due to released perrhenate. The carrier-added preparation also showed better images with higher skeletal accumulation, lower uptake in other organs and lower soft tissue uptake than the carrier-free preparation. Conclusion: The results of these studies clearly demonstrate that addition of carrier perrhenate is required for high labeling efficiency, stability, bone uptake and good image quality of Re-188-HEDP.
