핀 고정 천공형 티타늄막을 이용한 수직적 체조제증대술에 관한 연구 (Exophytic bone formation using porous titanium membrane combined with pins in rabbit calvarium.)

  • The purpose of this study was to evaluate exophytically vertical bone formation in rabbit calvaria by the concept of guided bone regeneration with a custom-made porous titanium membrane combined with bone graft materials. For this purpose, a total of 12 rabbits were used, and decorticated calvaria were created with round carbide bur to promote bleeding and blood clot formation in the wound area. Porous titanium membranes (0.5 mm in pore diameter, 10 mm in one side, 2 mm in inner height) were placed on the decorticated calvaria, fixed with metal pins and covered with full-thickness flap. Experimental group I was treated as titanium membrane only. Experimental group II, III, IV was treated as titanium membrane with BBM, titanium membrane with DFDB and titanium membrane with FDB. The animals were sacrificed at 8 and 12 weeks after surgery, and new bone formation was assessed by histomorphometric as well as statistical analysis. 1. Porous titanium membrane was biocompatable and capable of maintaining the regeneration space. 2. At 8 and 12 weeks, all groups demonstrated exophytic bone formation and there was a statistical significant difference among different groups only at 12 weeks. 3. The DFDB group revealed the most new bone formation compared to other groups (p<0.05). 4. At 12 weeks, DFDB and FDB groups showed the most significant resorption of graft materials (p<0.05). 5. The BBM was not resorbed at all until 12 weeks. 6. The fixation metal pin revealed excellent effect in peripheral sealing. On the basis of these findings, we conclude that a porous titanium membrane may be used as an augmentation membrane for guided bone regeneration, and DFDB as an effective bone forming graft material. The fixation of the membrane with pin will be helpful in GBR technique. However, further study is required to examine their efficacy in the intraoral experiments.

Demineralized dentin matrix combined with recombinant human bone morphogenetic protein-2 in rabbit calvarial defects

  • Objectives: The aim of this study was to compare the osteogenic effects of demineralized dentin matrix (DDM) combined with recombinant human bone morphogenetic protein-2 (rhBMP-2) in rabbit calvarial defects with DDM and anorganic bovine bone (ABB) combined with rhBMP-2. Materials and Methods: Four round defects with 8-mm diameters were created in each rabbit calvaria. Each defect was treated with one of the following: 1) DDM, 2) ABB/rhBMP-2, or 3) DDM/rhBMP-2. The rhBMP-2 was combined with DDM and ABB according to a stepwise dry and dip lyophilizing protocol. Histological and microcomputed tomography (${\mu}CT$) analyses were performed to measure the amount of bone formation and bone volume after 2- and 8-week healing intervals. Results: Upon histological observation at two weeks, the DDM and ABB/rhBMP-2 groups showed osteoconductive bone formation, while the DDM/rhBMP-2 group showed osteoconductive and osteoinductive bone formation. New bone formation was higher in DDM/rhBMP-2, DDM and ABB decreasing order. The amounts of bone formation were very similar at two weeks; however, at eight weeks, the DDM/rhBMP-2 group showed a twofold greater amount of bone formation compared to the DDM and ABB/rhBMP-2 groups. The ${\mu}CT$ analysis showed markedly increased bone volume in the DDM/rhBMP-2 group at eight weeks compared with that of the DDM group. Notably, there was a slight decrease in bone volume in the ABB/rhBMP-2 group at eight weeks. There were no significant differences among the DDM, ABB/rhBMP-2, and DDM/rhBMP-2 groups at two or eight weeks. Conclusion: Within the limitations of this study, DDM appears to be a suitable carrier for rhBMP-2 in orthotopic sites.

  • Purpose: The purpose of this study is to evaluate the effect of exposure of hydroxyapatite coating on the amount of bone formation and stability in the dogs. Methods: In this study, hydroxyapatite coated implants (HAPTITE) was placed over the femur bone surface of four dogs about 1 mm. The experimental group was divided into 4 weeks group of 8 implants and 8 weeks group of 8 implants, and then they were sacrificed. The stability of implants was evaluated twice with Osstell$^{TM}$ mentor (Osstell AB, Goteborg, Sweden) at right after placement and sacrifice. The amount of bone formation was evaluated through histomorphometric examination. Results: The stability of implants was in normal range, and tended to increase as time goes by. Mean percentages of new bone formation rates were $90.5{\pm}6.6$ at uppermost 1 mm bone level adjacent to soft tissues (level 1) and $92.9{\pm}4.1$ at next 1 mm bone level (level 2) in 4 weeks group, $90.1{\pm}11.5$ at level 1 and $95.9{\pm}2.3$ at level 2 in 8 weeks group. Mean percentages of bone-implant contact rates were $85.1{\pm}10.8$ at level 1 and $88.1{\pm}13.8$ at level 2 in 4 weeks group, $88.5{\pm}14.4$ at level 1 and $95.3{\pm}3.1$ at level 2 in 8 weeks group. There was no statistically significant difference of new bone formation rate and bone-implant contact rate between uppermost 1 mm bone level adjacent to soft tissues and next 1 mm bone level. However, there was no significant difference in bone formation between 4 and 8 weeks groups. Conclusion: These results suggested that exposed hydroxyapatite coated implants to soft tissue showed favorable bone formation and implant stability.

RBM surface 임플란트의 골형성에 대한 EPMA 분석 (EPMA analysis of bone formation around RBM surface implant)

  • Purpose: To evaluate bone formation on the implant surface between machined fixture and RBM surface fixture through analyzing of distribution and concentration of Ca and P by EPMA. Material and Methods: Fixtures had divided in 2 group: Machined(Group I), RBM (Group II). Total 4 fixtures were implanted on rabbit which sacrificed on 2 week and 4 week for the histological specimens. By these specimens EPMA value were measured, compared and analysed by each group to figure out the evidence of clinical use of RBM implant. Result: After 2 weeks and 4 weeks, it was analyzed that bone formation area, distribution and concentration of Ca and P by EPMA. In distribution and concentration of P Group II was higher than Group I, but there were no statistical significances. In new bone formation area, Group II was more higher than Group I with statistically significances. Both of group, after 4 weeks area is little bit higher than after 2 weeks area but there is no statistically significances. Conclusion: RBM implant was better than machined implant on the early bone formation.

혈소판유래 및 상피성장인자가 치주조직재생에 미치는 영향 (The Effects of Platelet-Derived Growth Factor and Epidermal Growth Factor on the Periodontal Tissue Regeneration)

  • 6 beagle dogs aged over one and half years and weighed 14 to 16 Kg were utilized in this study, Horizontal furcation defects were induced around 2nd, 3rd, and 4th premolars bilaterally, PDGF-BB in conjunction with EGF and PDGF-BB only were applied in the right and left premolars respectively. 2 animals were sacrificed at 4weeks, 8 weeks, and 12 weeks, after regenerative surgery respectively. Semi-thin sections using glass-knife were stained with toluidine blue for light microscopic study. The results were as follows: 1. At 4 weeks after regenerative surgery, bone formation in the PDGF-BB-applied site was thriving, but bone formation in the PDGF-BB-and-EGF-applied site was depressed. 2. Bony ankylosis was surely shown along the whole exposed root surface applied with PDGF-BB, but it was shown at the root surface near the base of the bone defect where was applied with PDGF-BB in conjunction with EGF. 3. Active bone formation was made from 8 weeks after regenerative surgery in the PDGF-BB- and-EGF-applied site. 4. Bone maturity as well as speed of bone formation in the PDGF-BB-applied site was superior to those in the PDGF-BB-and-EGF-applied site throughout the whole experimental period. Within the above results, PDGF-BB had the strong capability to form the new bone and EGF was not able to prevent the bony ankylosis thoroughly. However, EGF may have the possibility to prevent the bony ankylosis through the suppression of bone formation.

혈소판 농축혈장이 임플랜트 주위 골결손부 재생에 미치는 영향 (The Effects of Platelet-Rich Plasma on Regeneration around Dental Implant Defects)

  • The current interest in periodontal tissue regeneration has lead to research in bone graft, root surface treatments, guided-tissue regeneration, and the administration of growth factors as possible means of regenerating lost periodontal tissue. Several studies have shown that a strong correlation between platelet-rich plasma and the stimulation of remodeling and remineralization of grafted bone exists, resulting in a possible increase of 15-30% in the density of bone trabeculae. The purpose of this study was to study the histopathological correlation between the use of platelet-rich plasma and a bone xenograft used in conjunction with a non-resorbable guided-tissue membrane, e-PTFE, compared to a control group with regards to bone regeneration at the implant fixture site. Implant fixtures were inserted and graft materials placed into the left femur of in the experimental group, while the control group received only implant fixtures. In the first experimental group, platelet-rich plasma and BBP xenograft were placed at the implant fixture site, and the second experimental group had platelet-rich plasma, BBP xenograft, and the e-PTFE membrane placed at the fixture site. The degree of bone regeneration adjacent to the implant fixture was observed and compared histopathologically at 2 , 4, and 8 weeks after implant fixture insertion. The results of the experiment are as follows: 1. The rate of osseointegration to the fixture threads was found to be greater in the first experimental group compared to the control group. 2. The histopathological findings of the second experimental group showed rapid resorption of BBP with subsequent new bone formation replacing the resorbed BBP. 3. The second experimental group showed new bone formation in the area adjacent to the fixture threads beginning two weeks after fixture implantation, with continued bone remodeling in the areas mesial and distal to the fixture. 4. Significant new bone formation and bone remodeling was observed in both experimental groups near the implant fixture sites. 5. The rate of osseointegration at the fixture threads was greater in the second experimental group compared to the first group, and the formation of new bone and trabeculae around the fixture site occurred after the fourth week in the second experimental group. The results of the experiment suggest that a greater degree of new bone formation and osseointegration can occur at the implant fixture site by utilizing platelet-rich plasma and bone xenografts, and that these effects can be accelerated and enhanced by concurrent use of a non-resorbable guided tissue membrane.

  • Purpose: The purpose of this study is to assess the effectiveness of beta-tricalciumphosphate (${\beta}$-TCP) as a bone graft material on new bone formation and regeneration of mandible bone defect around dental implants. Methods: Both mandibular sites of ten rabbits were exposed. The experimental subjects were divided into two groups. Rabbits in the control group (right site of the mandible) had dental implants around cortical bone defects, without treatment, while, in the experimental group (left site of the mandible), ${\beta}$-TCP was grafted into the bone defect around the implant. Rabbits were sacrificed after one, two, three, four, and eight weeks, and histomorphometric evaluation and analysis of the bone implant contact rate were performed using an optical microscope. Results: Bone formation rates in the experimental group were greater than those in the control group from one to eight weeks, and percentages of implant surface contacted to bone were greater in the experimental group than in the control group from three weeks after implantation. Conclusion: These results suggest that the bone formation activity around dental implants was increased by osteoconduction activity of ${\beta}$-TCP.

  • Periosteum in general is described as a specialized fibrous membrane of mesenchymal origin consisting of two basis layers : outer fibrous layer consists of irregularly arranged dense connective-tissue with fibroblasts, and inner osteogenic or cambial layer is composed of more loosely arranged fibers, greater vascularity and flatted spindle-shaped pre-osteoblasts. This periosteum may serve in controlling bone growth, especially mandibular growth has been emphasized. But, the periosteum enwrapping the facial skeleton have been studied for many years leaving a controversy in opinion regarding the function of these structures. We evaluated the bone formation activity of te periosteum in allogeneic bone grafts which bones are made of freeze-dried preparation preoperatively. We made the calvarial bone defects, 5 ${\times}$ 7mm sized, amd grafted with allogeneic bone in rats, which a half of specimens has dissected the overlying periosteum and a rest intacted. After bone grafting, we evaluated the capacity ofbone formation of periosteum, 1, 2, 4, 6, 8 weeks postoperatively. There are subtle differences of bone formation during early healing period after demineralized allogeneic bone grafting between control groups with periosteum and experimental groups without periosteum.

성견 치근 이개부 병소에서 이종골 이식재의 치주조직 재생에 미치는 영향에 대한 비교 연구 (The Comparison of the Effects on the Regeneration with Xenografts on the Furcation Involvement in Beagle Dogs)

  • For the regeneration of osseous defect on the furcation area, autogeneous bone graft has been primarily used. But it has the limitation of donor site, additive surgical operation etc. Recently anorganic xenogenic bone graft materials of removing all organic components are commonly used for the regeneration of periodontal defects. This study was the comparison of the effect on the regeneration with two types xenografts($Bio-oss^{(R)}$ and Ca-P thin coated Bovine bone powder) on the furcation involvement in Beagle dogs. After surgically induced chronic periodontitis in bifurcation area of premolar, $Bio-oss^{(R)}$ and Ca-P BBP were grafted on the osseous defects. Tissue blocks including defects with soft tissues were harvested following a four-& eight-week healing interval and prepared for histologic analysis. The results of this study were as follows: 1. $Bio-oss^{(R)}$ group: there were significant differences among the $Bio-oss^{?}$ group at 4weeks and 8weeks, but the control group had various appearances : new bone formation, resorption of graft materials by multinuclear giant cells, connective tissue cells intervention in the bone graft sites etc. 2. Ca-P BBP group: lots of new bone formation were observed but the arrangement of periodontal ligament was not completed at 4weeks. New bone were replaced mature bone and the periodontal ligaments showed the functional arrangement at 8weeks. 3. By reason of undergrowing the epithelium within the osseous defects, new bone formation was not happened in the upper area of bifurcation in $Bio-oss^{(R)}$ group. 4. In Ca-P BBP group, epithelial undergrowth was not seen and generally showed much more new bone formation. 5. Ca-P BBP group showed the osteocyte-like cells at the inner portion of the graft materials 6. Both groups were similar to resorptive appearances of graft materials, but Ca-P BBP group had the better effects of osteoconduction.

Effects of fibrin glue on bone formation in combination with deproteinized bone xenografts in humans

  • Thirty-six sinus grafts were performed in 34 patients with an alveolar crest bone height in the posterior maxilla of 3 to 5 mm before grafting. The sinuses were grafted using Bio-Oss alone or mixed with fibrin glue. Group 1 was the control group and included 25 patients who received a xenograft mixed in saline. Group 2 comprised 9 patients who received a xenograft and fibrin glue. The study was further subdivided at the time of 9 months. This histologic study evaluated by hematoxylin-eosin (H&E) and histomorphometric analysis whether fibrin glue in combination with Bio-Oss enhances bone regeneration in sinus floor elevation in humans. The new bone formation was better in Group 2 than in Group 1, but the difference was not significant. The absorption of the graft material was faster in Group 2 than in Group 1, in the short term, but better in Group 1 over the long term, although the difference was not significant. Lamellar bone was formed earlier in Group 1 compared to Group 2, but the difference was not significant. Overall, the surgery site stabilized earlier with new bone formation in Group 2 than in Group 1, but the difference was not significant. Combining a fibrin sealant and Bio-Oss could lead to improved scaffolds for bone tissue engineering based on the synergistic effects of the biomaterials. Therefore, Bio-Oss or Bio-Oss plus Tisseel may be used depending on the situation.
