Longitudinal Splitting Free Vascularized Fibular Transplantation (종축 절골편을 이용한 생비골 부분이식술)

  • Free vascularized fibular transplantation is one of the most useful living bone reconstructive procedure in microsurgical field. Concerning about donor site morbidity, the donor has minor problems of ankle stability and muscle power weakness and transient peroneal nerve symptoms. That problems can be minimized with longitudinal splitted osteotomy on the donor fibula if the bone defect in recipient site is not so large. Half splitted fibula with peroneal arterial pedicle which contains nutrient artery and periosteal vessels, grafted bone can survive with those vascular supplies. Authors underwent five cases of half splitted free vascularized fibular transplantation from 1985. There were no evidence of devascularization in all cases, we can minimized donor morbidity with leaving half fibula intact on donor site. The problem of that technique is technically demanding in longitudinal splitting of bone without damage to peroneal nutrient vessels and periosteal soft tissues which attached to the bone. Authors can propose longitudinal half fibular transplantation is one of modification in free vascularized bone transplantation that minimize donor defect.

Enhancement of In Vivo Bone Regeneration Efficacy of Human Mesenchymal Stem Cells

  • We investigated whether transplantation of osteogenically differentiated bone marrow-derived mesenchymal stem cells (BMMSCs) and the use of an hydroxyapatite (HAp) scaffold can enhance the in vivo bone formation efficacy of human BMMSCs. Three months after implantation to the subcutaneous dorsum of athymic mice, transplantation of osteogenically differentiated human BMMSCs increased the bone formation area and calcium deposition to 7.1- and 6.2-folds, respectively, of those of transplantation of undifferentiated BMMSCs. The use of the HAp scaffold increased the bone formation area and calcium deposition to 3.7- and 3.5-folds, respectively, of those of a polymer scaffold. Moreover, a combination of transplantation of osteogenically differentiated BMMSCs and HAp scaffold further increased the bone formation area and calcium deposition to 10.6- and 9.3-folds, respectively, of those of transplantation of undifferentiated BMMSCs seeded onto polymer scaffolds. The factorial experimental analysis showed that osteogenic differentiation of BMMSCs prior to transplantation has a stronger positive effect than the HAp scaffold on in vivo bone formation.

  • Technetium-labeled phosphate bone scan was shown to detect bone fractures and bone metastasis in early stage than general radiographs. Therefore, bone scan has become one of the most frequently performed nuclear medicine imaging examination. However, non-osseous radiopharmaceutical uptake on the bone scan are unusual findings. We report a case of diffuse splenic absorption of Tc-99m dicarboxypropane diphosphonate in patients who undergo liver transplantation.

Reconstruction with Vascularized Fibular Epiphyseal Transplantation of Humeral Head Deformity by Septic Arthritis (생비골 성장판 이식술을 통하여 화농성 관절염에 의한 상완골두 변형의 재건)

  • Purpose: To report the clinical and radiological result of the vascularized fibular epiphyseal transplantation in the treatment of humeral head deformity by septic arthritis Material & Methods: A 3 years old male who has humeral head deformity and bone defect by septic arthritis on neonatal period. We replaced bone defect as vascularized fibular epiphyseal transplantation and lengthened humerus shaft for humerus discrepancy. We followed it up for 14 years. Result: We saw the callus formation 2 months after surgery and obtained bone union, one year after surgery. The transplanted fibular bone got hypertrophy. We could check full range of motion on lt. shoulder and The bone deformity was not worsened and The graft did not displaced on last follow up. Conclusion: Humeral head reconstruction by vascularized fibular epiphyseal transplantation showed good clinical outcome.

The Relationship between the Period of Engraftment and the Nutritional Status in Patients Undergoing Allogenic Bone Marrow Transplantation for Acute Myelogenous Leukemia (동종 골수이식을 받은 급성 골수성 백혈병 환자의 영양상태와 생착일수에 관한 연구)

  • This study was designed to investigate the association between the period of engraftment and the nutritional status of patient s undergoing bone marrow transplantation for acute myelogenous leukemia (AML). Nutritional status was evaluated by body mass index (BMI), percentage of ideal body weight (PIBW), percentage of weight loss, and serum albumin, total protein (T protein), hemoglobin (HGB) and hematocrit (HCT) levels on the day prior to transplantation and on the day of bone marrow transplantation. The periods of engraftment were determined by absolute neutrophil counts (ANC) above $500/mm^3$ from the day of bone marrow transplantation. The study subjects were 80 patients (55 males, 25 females) with acute myelogenous leukemia admitted to the University Hospital in Seoul. The result of this study is as follows : Tie nutritional status values of the majority of patients on the day prior to transplantation were in the normal range except for HGB and HCT ; however, during hospitalization, all of the levels of the nutritional status values were significantly decreased. The periods of engraftment of the abnormal group according to their BMI, PIBW, HGB and HCT levels on the day of bone marrow transplantation showed no difference when compared to the normal group. However, the periods of engraftment of the abnormal group, according to the percentage of weight loss, albumin, and T protein levels were significantly decreased when compared to the normal group. Therefore, the nutritional status of patients at the time of transplant had a noticeable influence on the periods of engraftment. Our results suggested that nutritional status is a critical factor of engraftment in BMT during pre-transplant and post-transplant. Futhermore, we recommend that the process of nutritional preparation for the transplant should initiate immediately after the transplant decision has been made.

Effect of CJ-50001 (rG-CSF) on the Recovery of the Neutrophil Numbers in the Mice with Bone Marrow Transplantation BMT) (CJ-50001 (rG-CSF)의 골수이식모델 마우스에 대한 호중구수 회복 촉진효과)

  • The peripheral neutrophil recovery test was conducted to determine the efficacy of CJ-50001, a drug developed in Cheil Jedang R&D center as a recombinant granulocyte-colony stimulating factor (rG-CSF). Grasin was used as control drug. CJ-50001 and Gratin were subcutaneously administered to ${\gamma}$-ray irradiated mice for 21 days at a dose of 10$\mu$g/kg after bone marrow transplantation and the recovery of neutrophil number was examined on the days of 9, 13, 17, and 21 after the drug administration. It was observed that the peripheral neutrophil number of the vehicle control group was recovered to the normal level on the day of 13 after the transplantation whereas the group administered with CJ-50001 and Grasin respectively, showed the normal level of peripheral neutrophil number on 9th day after the bone marrow transplantation. The number of peripheral neutrophils reached the highest level on the 21 st day of drug administration, and was recovered to the normal level on the 4th day after ceasing of the drug administration (on the 25th day of the transplantation). Thus, it was presumed that CJ-50001 showed efficacy similar to Grasin on the peripheral neutrophil recovery after bone marrow transplantation.

Reduced Osteogenic Differentiation Potential In Vivo in Acute Myeloid Leukaemia Patients Correlates with Decreased BMP4 Expression in Mesenchymal Stromal Cells

  • The osteogenic differentiation potential of mesenchymal stromal cells (hMSCs) is an essential process for the haematopoiesis and the maintenance of haematopoietic stem cells (HSCs). Therefore, the aim of this work was to evaluate this potential in hMSCs from AML patients (hMSCs-AML) and whether it is associated with BMP4 expression. The results showed that bone formation potential in vivo was reduced in hMSCs-AML compared to hMSCs from healthy donors (hMSCs-HD). Moreover, the fact that hMSCs-AML were not able to develop supportive haematopoietic cells or to differentiate into osteocytes suggests possible changes in the bone marrow microenvironment. Furthermore, the expression of BMP4 was decreased, indicating a lack of gene expression committed to the osteogenic lineage. Overall, these alterations could be associated with changes in the maintenance of HSCs, the leukaemic transformation process and the development of AML.

An Methodology Research of the Quality of Life of the Allogenic Bone Marrow Transplantation Survivors (동종골수이식 환자의 삶의 질 측정도구 개발)

  • This Research was designed to develop an instrument that can be used for measuring and analysing the degree of the quality of life of the Allogenic bone marrow transplantation survivors and to provide basic data to help them attain a better quality of life. The subjects of this study were a total of 101 people who were undergone Allogenic bone marrow transplantation and were over 100 days after transplantation. Data were collected during the period from September 3 to December 2. 2000. In Developing this instrument, the researcher established a conceptual framework based on the result of previous research, interview of patients and medical staff and researcher's experience. The scale for this instrument consisted of 51 items. Responses were obtained from the respondents through a self reporting method and each item had a possible score of 5. The collected data were analyzed using the SPSS program. The reliability of the scale was tested by Cronbach‘s alpha. For the examination of the component factors of the measuring instruments, factor analysis method was used. Examination of the difference in the quality of life as related to demographic variables was done using t-test, analysis of variance(ANOVA). For the examination of the major variables influencing the quality of life, Pearson's Correlations was used. The results were as follows: (1) The reliability of the instrument was 0.90 (Cronbach's alpha) (2) The validity of the instrument was tested by factor analysis. The result of the factor analysis of the 51 items is making up the instruments were classified into the following factor; emotional state, Interpersonal problem, physical problem, spiritual state, social problem, decrease of physical energy, physical problem-eye, physical problem-skin and mucosa, health awareness, elimination problem, worry about health, financial problem, sexual problem. (3) Demographic variables related to quality of life is the time after transplantation. GVHD experience, lose of job. Age at the transplantation, diagnosis, type of the transplantation, marital status, monthly income, educational background, sex, religion are not related to the quality of life.

  • AML (Acute myeloid leukemia) is a form of blood cancer where growth of myeloid cells occurs in the bone marrow. The prognosis is poor in general for many reasons. One is the presence of leukaemia-specific recognition markers such as FLT3 (fms-like tyrosine kinase 3). Another name of FLT3 is stem cell tyrosine kinase-1 (STK1), which is known to take part in proliferation, differentiation and apoptosis of hematopoietic cells, usually being present on haemopoietic progenitor cells in the bone marrow. FLT3 act as an independent prognostic factor for AML. Although a vast literature is available about the association of FLT3 with AML there still is a need of a brief up to date overview which draw a clear picture about this association and their effect on overall survival.

Challenges and innovations in hematopoietic stem cell transplantation: exploring bone marrow niches and new model systems

  • Hematopoietic stem cell transplantation (HSCT) remains an indispensable therapeutic strategy for various hematological diseases. This review discusses the pivotal role of bone marrow (BM) niches in influencing the efficacy of HSCT and evaluates the current animal models, emphasizing their limitations and the need for alternative models. Traditional animal models, mainly murine xenograft, have provided significant insights, but due to species-specific differences, are often constrained from accurately mimicking human physiological responses. These limitations highlight the importance of developing alternative models that can more realistically replicate human hematopoiesis. Emerging models that include BM organoids and BM-on-a-chip microfluidic systems promise enhanced understanding of HSCT dynamics. These models aim to provide more accurate simulations of the human BM microenvironment, potentially leading to improved preclinical assessments and therapeutic outcomes. This review highlights the complexities of the BM niche, discusses the limitations of current models, and suggests directions for future research using advanced model systems.
