Objective: The current study was to test if the developed sphygmomanometer was working well and blood pressure information could be collected and monitored systematically through the internet. We tested if the sphygmomanometer and services for blood pressure controlled high blood pressure significantly and the ubiquitous monitoring could be used further. Methods: Kyungwon University, KT Co., Gil Medical Center, LIG Nex1 Co., and Sujeong Health Center conducted an ubiquitous high blood control project in Sujeong-gu, Sungnam, Korea from Mar. 5 to May 16. We developed and applied sphygmomanometer. We distributed the devices to 27 high blood pressure patients. The blood pressures of the residents were monitored through the internet when they measured blood pressures in their homes. A nurse monitored and consulted their blood pressures in the monitoring center in Kyungwon University during the demonstration period. The consultant called them and consulted on their blood pressures in few seconds they used the sphygmomanometers. For the significance of change in blood pressure, we tested statistically with Generalized Additive Model(GAM) and Multi-level Analysis. Results: Both GAM and Multi-level Analysis showed that the blood pressures of persons with ubiquitous blood pressure management decreased significantly as time passed. Conclusions: The internet monitoring and services are considered to be promising because most of the participants were satisfied especially because somebody was caring their health. The decrease of blood pressures was significant by GAM and Multi-level Analysis. Thus, we can apply ubiquitous blood pressure management to health promotion projects.
This study was designed to investigate the effect of Ca supplementation of 1,000mg per day for 53 weeks on lipid, Na, and K metabolism and on blood pressure in postmenopausal women. The subjects were 12 healthy women aged from 60 to 70 years. They were divided into two groups : the placebo(control group) and the Ca supplemented(1,000 mg/day) group(Ca group). Metabolic studies were conducted twice in the 1st and the 53rd weeks. The results were as follows : Serum triglyceride, total cholesterol and LDL-choesterol levels tended to be decreased after the experiment. Serum VLDL-cholesterol lowering effect was observed with Ca supplementation(p<0.05), and also the significantly elevated HDL/(LDL+VLDL) ratio in Ca supplemented subjects whose average Na intake was as high as 4.9g per day. This phenomena was accompanied with increased Na retention and increased Na excretion in feces, but with decreased urinary Na in Ca supplemented group. However, considering much higher Na reteniton in the control group at the end of experiment(control va Ca ; 1272.3mg vs 732.9mg), Ca supplementation may have some beneficial effects on Na blance. Serum aldosterone level increased significantly in the Ca group after the exsperiment(p<0.05). With these normotensive subjects, there were no level increased significantly in the Ca group after the experiment(p<0.05). With these normotensive subjects, there were no pronounced effect of Ca supplementation on blood pressure, however, decrease in diastolic blood pressure were observed at the 14th week and end of the experiment(p<0.05). In summary, the Ca supplementation on postmenopausal Koran women appears to exert a desirables effect on blood lipid patterns related to the coronary heart diseases and to be beneficial in controlling diastolic blood pressure. Further studies with hypertensive or/and hyperlipidemic subjects are required to clarify the effect of Ca supplementation in Koreans.
Objective: To appreciate the effect of San-Yin-Jiao(SP-6) pressure on duratof delivery time and quantity of blood loss in order to verify the possibility of application to clinic of San-Yin-Jiao pressure. Methods: The design of this study is nonequivalent control group non- synchronized design. The subjects are 39 persons who are made up of SP-6 pressure applied(experimental) group 20 persons and to control group 19 persons. Collected data were analyzed as frequency, percentage, t-test, $x^2-test$ using SPSS 10.0 WIN Program. Conclusions: San-Yin-Jiao(SP-6) pressure not only make short duration of delivery time, but also decline quantity of blood loss in effect. 1. It could be necessary that the study of the effects of San-Yin-Jio(SP-6) pressure on duration of delivery time and quantity of blood loss for primipara be done repeatedly. 2. It could be necessary to increase this sort of study through free random experimental design in order to generalize this experimental result.

In this paper, we discuss a method for design of an ambulance stretcher which call decrease blood pressure fluctuation caused by ambulance acceleration. Recently, a lot of stretchers which can isolate the vertical vibration to reduce body resonances (4-10 Hz) have been used during ambulance transport. However, we have found that blood pressure of a patient laying in the stretcher fluctuates when the ambulance accelerates or decelerates. Since the enforced change of the blood pressure may deteriorate the patent's condition, a stretcher to cancel head-to-foot acceleration and to decrease the blood pressure variation (BPV) is expected for safe transport. We propose a method to design a stretcher which is tilted according to an adequate angle to cancel head-to-foot acceleration by gravity when the ambulance accelerates or decelerates. A control method of the stretcher is constructed by means of simulation analysis using acceleration data measured during ambulance transport. It is confirmed that the active controlled stretcher proposed has good performance for the BPV reduction.
The cause of essential hypertension is yet unknown, but in general is caused by interaction of hereditary factors, diet, obesity, lack of exercise and stress. The aging process influences various physiological mechanism related to regulation of blood pressure. So elderly hypertensives have a tendancy to consider the disease as a result of the physiological aging process. This attitude causes many complications, worsening of the disease and even early death because of inappropriate care. In order to improve self-management of elderly hypertensives in this study the researcher examined the effect of blood pressure regulation by an education program that improves self-care behavior, through increasing self-efficacy. The education program consisted of group education on hypertension and self-care strategies, and encouraging and reinforcing self-efficacy resources such as verbal persuation, performance accomplishment and vicarious experiences. A quasi-experimental pre-and post-test design was used. Thirty-two elderly hypertensives participated in the study. Eighteen in the education group and fourteen in the control group. The education program consisted of eight sessions twice a week for four weeks. There was no intervention for the control group. Data were analysed using SPSS for Windows(Version 8.0). The results were as follows. 1. There was a significant decrease in systolic and diastolic blood pressure between the experimental group and control group over three different times, and interaction by groups and over time. 2. There was no significant difference in the level of self-care behavior between the experimental group and control group over three different times, and interaction by groups and over time. 3. There was significant difference in selfefficacy of experimental and control group, depending on the measuring period, but there was no difference between these groups on interaction by groups and over time. 4. There was positive correlation between self efficacy and self care behavior depending on the measuring period. There was negative correlation between diastolic blood pressure and self-care behavior on the posttest of the program, and between systolic blood pressure and self-care behavior on the follow-up test of the program. Findings indicate that this study will contribute to develop nursing strategies for the regulation of blood pressure for the elderly, which is easy for the elderly to learn as a nonpharmacologic approach.
Purpose: To confirm the effect of tailored life style improvement program, provided by occupational health nurse for hypertensive workers on their blood pressure, self-efficacy, and life style changes in workplace. Method: The experimental and control group of thirty subjects each were randomly assigned for those who agreed to participate in the study among the pre-hypertensive workers. The tailored life style improvement program was provided to the experimental group and conducted once a week for 8 weeks. This program was composed of exercise and low salt diet to control high blood pressure and weight, and the measurement of carbon monoxide for non-smoking inducement. Result: In the experimental group, the systolic and diastolic blood pressure significantly decreased while self-efficacy increased after the program compared to the control group. Smoking and drinking habits of the experimental group were significantly decreased whereas exercise and nutritional habits were significantly improved compared to the control group. Conclusion: The tailored life style improvement program provided by occupational health nurses for the hypertensive workers was positively affected on their blood pressure, self-efficacy and life style changes in workplace.
It is generally accepted that the delivery of health care is undergoing many changes specially those related to acute, contagious disease care and to the increase of chronic illnesses which can not be cured but are controlable. The health care practitioner can not be soley responsible for the control of their clients' care. Because the clients will play a vital role in controlling their illnesses, long term participation by both the health care provider and the client is necessary. Since most individuals with hypertension do not experience signs or symptoms, the disease is difficult to detect and even when diagnosed, clients do not comply well with their hypertension regimens. The noncompliant client is at increased risk for compliants involving the heart, brain, kidney and other organs. In an effort to explore methods of increasing patient participation in and adherence to treatment programs for hypertension, the researcher used health contracting to promote self care. The research questions are; 1) Will the health contracting increase compliance in health behavior and reduce the blood pressure\ulcorner 2) If clients comply with their regimens will this reduce their blood pressure\ulcorner The research design utilized in this study was a quasi-experimental design. A purposive sample, was abtained from two churches in the 1. area, consisting of 64 clients with hypertension. The data was collected from the middle of January to the 1st of September 1985. Randomization was only of the two church groups into experimental and control groups. Compliance with health behavior related to the hypertensive regimen, blood pressure and body weight were measured, compared and analyzed. In the experimental group measurements were made 6 times; one month before the education program after education program when health contracting was done and 4 more times once a month for 4 months. In the control group measurements were made 3 times; one month before the education program after the education program, and once 4 months later. There was no health contracting. The data were analyzed by t-test, Pearson correlation and ANOVA according to purpose of the study. The result of this study may be summarized as follows: The result related to the hypothesis on the effect of health contracting are as follows: H$_1$; “The hypothesis that the experimental group, with a health contractual agreement will demonstrate increased compliance levels for health behavior than the control group” was supported(t=-5.29, df=62, p=.000). H$_2$; “The hypothesis that the experimental group, with a health contractual agreement, will demonstrate a greater reduction in blood pressure than the control group” was supported (for systolic blood pressure t=2.72, df=62, p=.009, for diastolic blood pressure t=1.95, df=62, p=.050). H$_3$; The hypothesis that the greater the compliance of the client with health behavior the lower the client's blood pressure will be was partially supported (for systolic pressure r=-.2981, p=.008, for diastolic pressure r=-.1720, p=.087). From the examination of the results of this study it can be concluded that the interaction between the nurse and the client, contracting to define goals and reinforcing compliant behavior, leads to improved compliance with health care behaviors and thus to an increase in the effectiveness of nursing care. Further consideration need to be given to the inclusion of the concept of health contracting in primary nursing and to further research in this area.
More and more non-injured operations are being implemented these days, thanks to the development of medical technology. Still, however, most operations leave direct scars on patient' bodies, as well as accompanying pain. The massage as an independent nursing intervention can stimulate the circulation of the blood of tissue and muscle and increase the relationship between a patient and a nurse. The purpose of this study is to investigate the effect of foot massage on pain in post abdominal operative patients. The nonequivalent control group, pre-post test design is used for this study. From July 7, 2000 to February 20, 2001, the 40 patients who were operated under general anesthesia in a university hospital in Seoul were studied. They were divided into two groups ; 20 patients were part of the experimental group, and the others, in the control group. In order to evaluate the effect of foot massage, severity of pain was checked with the VAS (Visual Analog Scale) and also each patients' vital signs were measured with pulse rate, systolic blood pressure and diastolic blood pressure. The collected datas were processed by SAS version 6.12 program and analyzed by the Chi-square, Fisher's exact test, t-test and repeated measures ANOVA. The results of this study were as follows. 1. The severity of pain decreased significantly in the experimental group as compared to the control group following foot massage (t=-3.317, p= .002). 2. Measured vital signs in the experimental group had more reduction of that than in the control group following foot massage. - The pulse rate in the experimental group was lower than that in the control group following foot massage (F=7.73, p=.008). - The systolic blood pressure in the experimental group was lower than that in the control group following foot massage (F=25.75, p=.000). - The diastolic blood pressure in the experimental group was lower than that in the control group following foot massage (F=15.27, p=.000). In conclusion, foot massage is an effective dependent nursing intervention for pain control of post abdominal operative patients.
In this paper we present an implantable pressure sensor to measure real-time blood pressure by monitoring mechanical movement of artery. Sensor is composed of inductors (L) and capacitors (C) which are formed by microfabrication and direct bonding on two biocompatible substrates (quartz). When electrical potential is applied to the sensor, the inductors and capacitors generates a LC resonance circuit and produce characteristic resonant frequencies. Real-time variation of the resonant frequency is monitored by an external measurement system using inductive coupling. Structural and electrical simulation was performed by Computer Aided Engineering (CAE) programs, ANSYS and HFSS, to optimize geometry of sensor. Ultrafast laser (femto-second) cutting and MEMS process were executed as sensor fabrication methods with consideration of brittleness of the substrate and small radial artery size. After whole fabrication processes, we got sensors of $3mm{\times}15mm{\times}0.5mm$. Resonant frequency of the sensor was around 90 MHz at atmosphere (760 mmHg), and the sensor has good linearity without any hysteresis. Longterm (5 years) stability of the sensor was verified by thermal acceleration testing with Arrhenius model. Moreover, in-vitro cytotoxicity test was done to show biocompatiblity of the sensor and validation of real-time blood pressure measurement was verified with animal test by implant of the sensor. By integration with development of external interrogation system, the proposed sensor system will be a promising method to measure real-time blood pressure.
We have developed an implantable wireless sensor for real time pressure monitoring of blood circulation system. MEMS (micro-electro-mechanical system) technology was adopted as a sensor development method. The sensor is composed of photolithographically patterned inductors and a distributed capacitor in gap between the inductors. A resulting LC resonant system produces its resonant frequency in range of 269 to 284 MHz at 740 mmHg. To read the resonant frequency changed by blood pressure variation, we developed a custom readout system based on a network analyzer functionality. The bench-top testing of the pressure sensors showed good mechanical and electrical functionality. A sensor was implanted into tibial artery of farm pig, and interrogated wirelessly with accurate readings of blood pressure. After 45 days, the sensor's electrical response and histopathology were studied with good frequency reading and biocompatibility.
