백반증(白斑症) 사용약물(使用藥物)에 대한 문헌적(文獻的) 고찰(考察) -관련문헌(關聯文獻)에 나타난 백반증(白斑症)의 상용약물(常用藥物) 종류(種類), 허실(虛實), 시대(時代), 원인별(原因別) 분류(分類)를 중심(中心)으로- (A Documentary Study on Herb, Dmgs used for Vitiligo -With an emphasis on classifying kinds, excess and weakness syndrome, the changes of medical methods and factors by each epoch mentioned in the relative documentary records)

  • Arranging 63 kinds of separate volumes and papers published on Oriental medicine, I could get the result as follows. 1. The herbs for internal application used commonly in vitiligo are 155 kinds totally. The herbs for external application are 67 kinds. The herbs for external and internal both application examined into 23 sorts. 2. Herbs for weakness syndrome in vitiligo are 49 kinds. Another type, excess syndrome is 105 sorts, the'latter is roughly twice as many as the former. 3. It is as follows that the results of study in relations to kinds, factors, and medical treatments of herbs about vitiligo in and out of the country with the division of former times to 1900, 1901 to 1980, 1981 to 1990, and 1991 to the latest day. In comparative study of inner and outer of thc country about factors and medical treatments of vitiligo in chronicle classification, its factors in the internal documents are classified by outside factors. Although there arc the differences of factors by each epoch, however, the factors of vitiligo according to external documents are blood stasis(血熱), deficiency of um of the liver and kidneys(肝腎陰虛), deficiency of blood(血虛), excess of exhaustion(勞倦過多) etc. Moreover, the medical treatment is more diverse and the differences by each epoch as to the medical treatment is also put down more saliantiy than in internal documents. 4. In comparison with herbs in experimental and no experimental documents, herbs applied for weakness syndromes in experimental method are 40 kinds totally. The herbs in no experimental methods are 35 kinds. The herbs used by experimental method are 65 kinds. The common herbs for excess syndrome by no experimental method are 78 kinds. We can see comparable difference from kinds of herbs used by experimental method. In brief, there are the differences classified by each epoch in Oriental medicine for treatment. Especially one of the most important feature, the frequency in use of weakness syndrome herbs has increased more than that of excess syndrome herbs. In external documents (china) and experimental study, generally the differences of common herbs and factors have disappeared through many experimentsitudy. The classification of its factors have been fractionalized clinically. Besides, in Western medicine and Orienal medicine, vitiligo tends to be prescribed not to simple skin disease but a mental and physical disease, a whole body and an internal disease.

사상(四象) 체질별(體質別) 약재(藥材) 분류(分類)에 관한 연구(硏究)(소음인편(少陰人編)) (A Research on the Classification of Herbal Medicines based on the Sasang Constitution (Soeumin Part))

  • We analyzed 57 types of herbal medicines for Soeumin, clinically applied in Dongyisoosebowon. In order co discover the standard of herbal classification for the Sasang Constitutions, four concepts of Sasang Medicine were applied. These included 'Sadangron (theory of four groups)', 'Seungganggaehap(ascending-descending and gathering-dispersing)', 'Pyorihanyoul(exterior-interior and hot-cold)', and 'Hyungchiaekmi(fragrance, smell, bodily fluid and taste)'. According to these analyzing methods of herbal properties, we have reached the following conclusions: 1. Lee Je-ma first established the physiology of the internal organs of the Sasang Constitutions through the understanding of constitutional pathology. Dr. Lee then classified herbal medicines into four constitutional groups by observing the herbal effects according to such perspective of organ physiology and pathology. 2. Among the Sasang concepts applied in the analysis of herbal properties based on the physiology and pathology of Sasang constitutions, 'Seungganggaehap (ascending-descending and gathering-dispersing)' was found to be the most useful. Meanwhile, 'Sadangron, (the theory of four groups - Spirit, Qi, Blood and Essence),' along with 'Pyorihanyoul (exterior-interior and hot-cold)' and 'Hyungchiaekmi (fragrance, smell, bodily fluid and taste)' were partially useful. 3. Herbal medicines for Soeumin are characterized by ascending energetics that protect the Yang Qi. The 'Exterior heat disease' is treated with herbal medicines that raise the "Exterior Yang" in three different way: by tonifying Qi. by tonifying Blood and by releasing exterior. The 'Interior cold disease' is treated with herbal medicines that descend "Interior Yin" in two ways. One by warming the stomach, and the other by regulating Qi. 4. Cases where a whole category of herbs with the same effect have been applied co one constitution, are most frequent among Shaoyin herbs. There was an intimate connection between the treatment methods and the traditional effects of herbs for 'Soewnin'. These discoveries allowed us to see that the development of the pharmacology of the Sasang Constitutional pathology more progressed in 'Soeumin'. *analysing simple herbs of "DongEuiBoGam JangBu" we compare JangBu of traditional oriental medicine with JangBu of Sasang Constitutional Medicine. So we can comprehend the JangBu's difference of traditional-oriental with sasang-constitional medicine.

복부농양진단을 위한 Indium-111 표지백혈구스캔 (Scintigraphy with Indium-111-oxine Labeled Leukocytes for Localization of Abscesses)

  • Detection of deep-seated abscesses is sometimes difficult with ultrasonogrpahy or computed tome graphy alone. Indium-111-labeled leukocyte has widely used in the localization of abscesses after introduction by Segal and Thakur in 1976. But there are some difficulties in using indium-111-oxine in our country because of hardness to get the radiopharmaceutical timely and long time for labeling leukocytes. So we peformed the indium-111-labeled leukocyte scan for establishment of the labeling procedure and clinical application. We labeled the mixed leukocytes from 36 ml of patient's blood using 4 ml of ACD solution, 7 ml of 6% hydroxyethyl starch solution $(HESPAN^{(R)})$, 1 mCi of indium-111 oxine, 5 ml of normal saline and centrifuge. It took about 2 hours for the preparation of radiolabeled leukocytes and attention for contamination was needed. The average injected dose of labeled mixed leukocytes was 465 uCi. The average number of injected leukocytes was $2.5\times10^8$ and the labeling ratio was $57{\pm}13%$ (Table 2, Fig. 5). These number and ratio were sufficient for the localization of abscess. About twenty per cent of indium was labeled to red blood cells and platelets (Fig. 6) and the half-life of injected radiolabeled leukocytes was 8.3 hours. Scan was performed in 9 patients who were suspected to have abscesses clinically or radiologically. Three patients were positive, in one patient who had abscess close to lower lumbar vertebrae was surgically drained and another 2 positive cases did not show abscess clearly on computed tomography, so only antibiotics were administrated and treated successfully. The negative 6 patients were improved without specific treatment. In conclusion, the use of indium-111 oxine labeled leukocytes for localization of abscesses were very specific and helpful in the decision of treatment considering its relatively simple labeling method, and could be easily performed providing timely supply of the radiopharmaceutical.

  • 세포 내에서 매우 적은 양으로 존재하는 RNA 전사체와 기능적으로 동일하거나 비슷한 RNA를 RNA 중합효소를 써서 in vitro에서 생화학적으로 의미 있는 양 만큼을 합성할 수 있다. T7 RNA중합효소를 발현하는 재조합 유전자를 지닌 대장균주 BL21/pAR1219로부터 순수한 T7 RNA중합효소를 손쉽게 얻는 방법을 본 논문에서 소개한다. 황산암모늄 분획화와 sephadex SP 컬럼 크로마토그래피법으로써 여타의 방법과 비교하여 더 간단하고 빠르게, 그리고 경제적으로 T7 RNA 중합효소를 분리할 수 있었다. 정제된 T7 RNA중합효소를 이용하여 보통의 화학적 합성법으로 불가능한 긴 길이(1.54 kb)의 RNA전사체를 합성 하였다. 한편,정제된 T7 RNA중합효소에 의해 생성된 망치머리 리보자임은 표적 RNA를 in vitro에서 절단함으로써, 생성된 RNA가 생화학적 기능성을 유지한다는 것을 입증하였다 따라서 본 연구에서 소개되는 절차들은 다양한 길이의 RNA를 목적에 따라 간단하고 경제적으로 합성하는데 유용하게 이용될 수 있다.

뇌기능영상 측정법을 이용한 영재성 평가의 타당성 연구 (A Neurobiological Measure of General Intelligence in the Gifted)

  • 본 연구에서는 뇌영상기술(fMRI)을 이용하여 뇌신경활동성에 기반한 영재성 평가의 가능성을 타진하였다. 이를 위해 현행 영재교육 수혜자 및 일반 고교생 50명을 대상으로 국제적으로 공인된 다양한 지능검사(RAPM, WAIS)와 창의력 검사(TTCT-도형, TTCT-언어)를 실시하였으며 이들 중 40명의 학생을 대상으로 추론적 사고능력을 요구하는 지능과제 수행 시 두뇌활동성을 측정하였다. 일반지능(g) 수준에 따라 영재군과 일반군으로 구분하여 두뇌활동성을 비교 분석한 결과 두 그룹 모두 좌.우반구의 외측전전두엽피질(lateral PFC), 전대상피질(ACC), 후두정엽피질(PPC)에서 높은 활동성을 보였으며, 영재군이 일반군에 비해 높게 나타났다. 개인별 일반지능(g) 수준과 두뇌활동성 사이의 상관도를 분석한 결과 후두정엽피질에서 가장 높은 상관도$(r=0.73{\sim}0.74)$를 보였으며 다른 영역들 역시 비교적 높은 상관도$(r=0.53{\sim}0.66)$를 보였다. 한편 영재군은 일반군에 비해 지능지수에서는 월등히 높은 수치를 보였으나 창의력지수에서는 크게 차이를 보이지 않았다. 이러한 결과는 뇌기능영상기술이 영재성 평가에 적용될 수 있을 것이라는 가능성을 보여주며 영재선발 시 창의력에 대한 평가 비중을 강화시킬 필요성이 있음을 시사한다.

The Effect of Spray Dried Plasma, Lactose and Soybean Protein Sources on the Performance of Weaned Pigs

  • A total of 371 weaned pigs were used in three experiments to evaluate the effects of spray dried plasma (SDP), soybean protein sources, and lactose on growth performance. In Exp. 1, 128 pigs (5.99 kg, $18{\pm}2d$) were used to evaluate the effect of SDP (0 vs 7%), lactose (0 vs 30%), and two soybean protein sources [soybean meal (SBM) and extruded soybean protein concentrate (ESPC)] in phase I (d 0 to 14) diets on pig performance in a $2{\times}2{\times}2$ factorial arrangement. Spray-dried plasma increased phase I ADG (p<0.01) and ADFI (p<0.05) in the SBM diets, but not in the ESPC diets. Lactose improved ADG and gain/feed ratio (G/F) in phase I (p<0.01). In Exp. 2, 144 pigs (5.50 kg, $17{\pm}3d$) were used to evaluate the effect of SDP (0 vs. 3.5%) and three soybean protein sources [SBM, ESPC, and soybean protein concentrate (SPC)] in phase I diets, and the effects of two different phase II (d 14 to 28) diets (simple vs complex) in a $2{\times}3{\times}2$ factorial arrangement of treatments. In phase I, SDP increased ADG (p<0.01) and improved G/F (p<0.05). Pigs fed SBM had the highest ADG and ADFI, with a G/F similar to the pigs fed ESPC. In phase II, pigs fed the complex diet had improved ADG (p<0.01), ADFI (p<0.05), and G/F (p<0.05) compared to the simple diet. In Exp. 3, 99 weaned pigs (5.77 kg, $17{\pm}3$d) were used to evaluate the effect of SBM, ESPC, and ESPC with SDP in the phase I diets. Pigs fed SBM with no blood product in the diet had the lowest ADG (p<0.01), ADFI (p<0.01), and G/F (p<0.05) in the first week of phase I. There were no differences in soybean protein sources fed in phase I diets on overall pig performance. These experiments are indicated that SDP and lactose improve the phase I performance. Soybean meal can be used as the major protein source in phase I diets with SDP.

단순 외상팀 활성화 조건이 중증 외상 환자의 치료 결과에 미치는 영향 (Effectiveness of Simple Trauma Team Activation Criteria on Prognosis of Severe Trauma Patients)

  • Purpose: The goal of this study was to compare the outcome of the after trauma team (AfterTT) group to the before trauma team (BeforeTT) group. Methods: All trauma patients who visited to emergency room (ER) between July 1, 2006 and February 29,2008 based on trauma registry, with systolic blood pressure (SBP) < 90 mmHg or GCS < 9 were included in this study. We compared the amount of packed RBC transfusion, the ER stay time, the ER visit to CT evaluation time, the ER visit to operation time, the length of ICU stay, the length of hospital admission and the survival discharge rate between the AfterTT group and the BeforeTT group. Patients with brain injuries had little chance of survival. Burn patients, who visited the ER 24 hours after injury and patients who were dead on arrival (DOA) were excluded from this study. Results: Total of 93 patients were included in this study: 42 in the AfterTT group and 51 in the BeforeTT group. The AfterTT group and the Before TT group showed no differences in Revised Trauma Score (RTS) and mean age. The amount of packed RBC transfusion was lower in the AfterTT group, but no statistically significant difference was noted (AfterTT 11${\pm}$11units, BeforeTT 16${\pm}$15units, p=0.136). The ER visit to operation time was shorter in the AfterTT group, but there were no statistically significant difference between the groups (AfterTT 251${\pm}$223 minutes, BeforeTT 486${\pm}$460 minutes, p=0.082). The length of ICU stay was shorter in the AfterTT group, but the difference was not statistically significant (AfterTT 11${\pm}$12 days, Before TT 15${\pm}$30 days, p=0.438). The length of Hospital admission was shorter in the AfterTT group (AfterTT 43${\pm}$37 days, BeforeTT 68${\pm}$70 days, p=0.032), but this difference was not statistically significant. Conclusion: Simple Trauma team activation criteria decreased the amount of packed RBC transfusion and the hospital admission duration. Hemodynamic instability (SBP < 90 mmHg) and decreased mental state (GCS<9) are good indices for activating the trauma team.

데이터 마이닝을 이용한 서울시교직원의 피로요인 탐색연구 (An Exploratory Study of Fatigue Related Factors among School Personnelin Seoul by Data mining)

  • Purpose : To identify general characteristics of school personnel with recent fatigue which was the most frequent symptom among subjective symptoms and to explore fatigue-related factors by evaluating physical and perceived health status, life style, and symptoms through data mining techniques. Methods : We collected a data of the 1,147(male 545, female 602) who were elementary, middle, or high school personnel, answered a questionnaire, and received physical examination in Seoul School Health Center from September to November in 2000. And we investigated the differences between fatigue group and non-fatigue group for demographic characteristics, physical health status, perceived health status, symptoms, and laboratory values by frequency, chi-square test, t-test, or simple logistic regression analysis by SAS package 8.1, and then selected significant variables as input variables of a decision tree analysis of CART model by SAS E-miner. Results : In general characteristics, the fatigue consisted of 41.1%(male 35.2%, female 46.4%) among 1,147 school personnel. In classical statistics, factors related with fatigue were female, lower means of systolic and diastolic pressure, young age, personnel in middle school, irregular eating habit, no exercise a week or less than 30minutes exercise a day, perception of unhealthy status, and subjective symptoms including short of breath at exercise. In simple logistic regression to examine the relationship between selected independent variables and fatigue as a dependent variable, the odds ratio of gender (female vs male) was 1.58 times, and young age ( 20s vs 60s) 20.67 times, and middle vs high school personnel 1.86 times. However, we mined combined several characteristics by SAS-E miner. In CART model, if health perception was healthy, and age was >= 37.5 years, the proportion of the fatigue was only 19.3%. but if health perception was not healthy and symptom was severe 'short of breath' during exercise and age was < 53.5 years, and BMI was >= 22.69, the proportion of the fatigue was up to 84.8%. Conclusions : The fatigue consisted of 41.1%(male 35.2%, female 46.4%). 가스크로마토그래피/질량분석기를 이용한 약물의 확인 및 간이 정량분석 프로그램 개발 (Development of Rapid and Simple Drug Identification and Semi Quantitative Analytical Program by Gas Chromatography-Mass Spectrometry)

  • Systematic toxicological analysis (STA) means the process for general unknown screening of drugs and toxic compounds in biological fluids. In order to establish STA, in previous study we investigated pattern of drugs & poisons in autopsy cases during 2007~2009 in Korea, and finally selected 62 drugs as target drugs for STA. In this study, rapid and simple drug identification and quantitative analytical program by gas chromatography-mass spectrometry(GC-MS) was developed. The in-house program, "DrugMan", consisted of modified chemstation data analysis menu and newly developed macro modules. Total 55 drugs among 62 target drugs were applied to this program, they were 14 antidepressants, 8 anti-histamines, 5 sedatives/hypnotics, 5 narcotic analgesics, 3 antipsychotic drugs, and etc. For calibration curves, fifty five drugs were divided into four groups of range considering their therapeutic or toxic concentrations in blood specimen, i.e. 0.05~1 mg/l, 0.1~1 mg/l, 0.1~5 mg/l or 0.5~10 mg/l. Standards spiked bloods were extracted by solid-phase extraction (SPE) with trimipramine-D3 as internal standard. Parameters such as retention times, 3 mass fragment ions, and calibration curves for each drug were registered to DrugMan. A series of identification, semi quantitation of target drugs and reporting the results were performed automatically. Calibration curves for most drugs were linear with correlation coefficients exceeding 0.98. Sensitivity rate of DrugMan was 0.90 (90%) for 55 drugs at the level of 0.5 mg/l. For standard spiked bloods at the level of 0.5 mg/l for 29 drugs, semi quantitative concentrations were ranged 0.36~0.64 mg/l by DrugMan. If more drugs are registered to database in DrugMan in further study, it will be useful tools for STA in forensic toxicology.

텔레비전 드라마의 간호사 이미지에 대한 분석 (Contents Analysis on the Image of Nurses in the Television Drama)

  • The purpose of this study is to inquire the people's views on nursing for nurses, correct the image of nurse and take it as basis to be applied on nursing education examining the image of nursing on Television drama playing important role of mass media. 22 nurses of the characters in drama is applied to the analysis object of this study by selecting 6 dramas of Television ones the nurse play on the prime time from June 1 to August 31 in 1997. Contents analysis method was used in Data Analysis, 4 items was used after Coders previously modify and compensate it based on research documents of 1m Milim(1996) 2 Coders made the Coding the article on each person by them seeing the recorded film making the Coding Paper each items is written by the character. The average of reliability degree was 90% which measured the reliability degree by the mathod of Holsti. The statisic method of frequency, percentage was used SPSS Program in data processing The results were as follows. 1. Relative importance of 86.2% nurses in drama was depicted as extra characters 2. The affair attitude of nurses shown on drama was revealed as mechanical(84.7%), passive(45.5%), dependent(54.4%) unkind(68.2%). 3. The activity of nurses was classified with professional! simple affair. The professional affairs such as I.V., Blood Pressure Check, Rounding, Nursing Recording, Patient Education, Assist of Operation, Assistant meal of Patient, etc is mainly depicted and the screen of simple affair such as Receiving telephone, Carrying Tray or Dragging, Stretcher Car, Dressing Car and or Wheel Chair than professional affair. 4. The appearance feature of nurses was shown on thin physique(68.2%), common stature(68.2), dirty costume(45.4%), common appearance(81.9%), unnoble action(63.6%). The image of nurses is illuminated as the exterial scene of technical affair such as assisting the doctors and affair focused on accident and educational activity of nureses or extended role is nor depicted on Television drama. Therefore, the people regard the nurse as sexual object with good appearance than professional worker working professional nursing We want the following, epigraph based on above conclusion. 1. The continuous research is required on the image of nurse shown on various mass media. 2. The later research is required on appliction strategy of mass media for advancing the image of nurse. 3. The research to strengthen the objectivity by comparing analyzed data on drama & analyzing it is required 4. Through the deep study, the standard to show a concrete and professional work of nurses to scenario writers of TV drama is suggested by the association. 5. The monitoring about the mass media must be activated, not by some nurses, on a national scale and much study on the basis of this is needed.
