A case study of competing risk analysis in the presence of missing data

  • Observational data with missing or incomplete data are common in biomedical research. Multiple imputation is an effective approach to handle missing data with the ability to decrease bias while increasing statistical power and efficiency. In recent years propensity score (PS) matching has been increasingly used in observational studies to estimate treatment effect as it can reduce confounding due to measured baseline covariates. In this paper, we describe in detail approaches to competing risk analysis in the setting of incomplete observational data when using PS matching. First, we used multiple imputation to impute several missing variables simultaneously, then conducted propensity-score matching to match statin-exposed patients with those unexposed. Afterwards, we assessed the effect of statin exposure on the risk of heart failure-related hospitalizations or emergency visits by estimating both relative and absolute effects. Collectively, we provided a general methodological framework to assess treatment effect in incomplete observational data. In addition, we presented a practical approach to produce overall cumulative incidence function (CIF) based on estimates from multiple imputed and PS-matched samples.

Effects of Gargle Using Natural Substances on Oral Environment

  • In this study, natural mouthwash was developed based on natural substances based on cinnamon and ginger, which are familiar to the public, to check the effectiveness of mouthwash and changes in microbial distribution. This study was conducted on 24 people who fully explained the purpose and method of the study and agreed to participate among those who visited D University from October 2021 to December 2021. The subject's oral cavity was examined and placed in three groups to have similar conditions, and after using mouthwash for a week, a survey, saliva test, breath measurement test, and PCR test were performed. As a result of the experiment, the amount of saliva after using natural mouthwash increased compared to the control group (P<0.05). In the comparison of bacteria, it was confirmed that Pg and Fn bacteria decreased after using natural mouthwash (Pg t=4.852, P=.002, Fn t=2.888, P=.023). Following this study, it is expected that research on the development and efficacy of natural mouthwash will be conducted through various clinical applications, which will be useful in real life and dental care institutions.

Generation and Application of Directly Reprogrammed Endothelial Cells

  • Cell-based therapy has emerged as a promising option for treating advanced ischemic cardiovascular disease by inducing vascular regeneration. However, clinical trials with adult cells turned out disappointing in general. As a newer approach, direct reprogramming has emerged to efficiently generate endothelial cells (ECs), which can promote neovascularization and vascular regeneration. This review provides recent updates on the direct endothelial reprogramming. In general, directly reprogrammed ECs can be generated by two approaches: one by transitioning through a plastic intermediate state and the other in a one-step transition without any intermediate states toward pluripotency. Moreover, the methods to deliver reprogramming factors and chemicals for the fate conversion are highlighted. Next, the therapeutic effects of the directly reprogrammed ECs on animal models are reviewed in detail. Other applications using directly reprogrammed ECs, such as tissue engineering and disease modeling, are also discussed. Lastly, the remaining questions and foremost challenges are addressed.

  • Electrical impedance tomography(EIT) can produce functional images with conductivity distributions associated with physiological events such as cardiac and respiratory cycles. EIT has been proposed as a clinical imaging tool for the detection of stroke and breast cancer, pulmonary function monitoring, cardiac imaging and other clinical applications. However EIT still suffers from technical challenges such as the electrode interface, hardware limitations, lack of animal or human trials, and interpretation of conductivity variations in reconstructed images. We improved the KHU Mark2 EIT system by introducing an EIT electrode interface consisting of nano-web fabric electrodes and by adding a synchronized biosignal measurement system for gated conductivity imaging. ECG and respiration signals are collected to analyze the relationship between the changes in conductivity images and cardiac activity or respiration. The biosignal measurement system provides a trigger to the EIT system to commence imaging and the EIT system produces an output trigger. This EIT acquisition time trigger signal will also allow us to operate the EIT system synchronously with other clinical devices. This type of biosignal gated conductivity imaging enables capture of fast cardiac events and may also improve images and the signal-to-noise ratio (SNR) by using signal averaging methods at the same point in cardiac or respiration cycles. As an example we monitored the beat by beat cardiac-related change of conductivity in the EIT images obtained at a common state over multiple respiration cycles. We showed that the gated conductivity imaging method reveals cardiac perfusion changes in the heart region of the EIT images on a canine animal model. These changes appear to have the expected timing relationship to the ECG and ventilator settings that were used to control respiration. As EIT is radiation free and displays high timing resolution its ability to reveal perfusion changes may be of use in intensive care units for continuous monitoring of cardiopulmonary function.

유체의 성질에 따른 자계용적맥파의 검출 특성에 관한 연구 (A Study on the Detection Characteristics of the Magneto-Plethysmography According to Fluid Properties)

  • Photo-plethysmography (PPG), which measures changes in the peripheral blood flow of a human body using difference in absorption rate of light, is a measurement method that is studied and used in clinical and various applications due to its simple circuit configuration and measurement convenience. Magneto-plethysmography (MPG), which is newly developed by our team, is a method of measuring changes in the conductivity of biological tissues by using a eddy current induced by a time-varying magnetic field, and is not subject to optical interference. In this study, we investigated the detection characteristics of MPG according to the change of the conductivity of the object and fluid to be measured by simultaneously measuring PPG and MPG. In order to control the speed of fluid known in advance, a blood flow simulator was implemented and used. The fluid used in the experiment was general mineral water and physiological saline (0.9% NaCl) solution. Experimental results show that the amplitude change of the measured PPG was 0.3% in normal water and saline solution, and that of MPG was 77.3%. Therefore, it is considered that the magneto-plethysmography (MPG) has a strong correlation with the conductivity of the fluid.

635 nm Photodynamic Therapy (PDT)를 이용한 개의 난치성 외이염 치료 (Photodynamic Therapy Using a 632 nm Diode Laser on Otitis Externa of the Dog)

  • Photodynamic therapy (PDT) is an effective method for infectious disease, but as yet, no clinical trial on otitis externa has been conducted in animals. This report describes 4 cases of PDT treatment on severe otitis externa in canine. Four canines were treated with various concentrations of 5-aminolevulinic acid hydrochloride (5-ALA) and with various J (second ${\times}$ power) and irradiated by light-emitting diode light source at 635 nm, respectively. After the PDT applications, canines incubated in the dark for 5 or 6 hr. All patients showed clinical resolution of otitis externa after PDT treatment at the dose of 0.16~0.3 g ALA and J (54~162 $J/cm^2$). On the basis of these findings, PDT can be considered to be a new therapeutic approach to severe otitis externa in canine.

  • During laser irradiation, mechanically deformed cartilage undergoes a temperature dependent phase transformation resulting in accelerated stress relaxation. Clinically, laser-assisted cartilage reshaping may be used to recreate the underlying cartilaginous framework in structures such as ear, larynx, trachea, and nose. Therefore, research and identification of the biophysical transformations in cartilage accompanying laser heating are valuable to identify critical laser dosimetry and phase transformation of cartilage for many clinical applications. quasi-elastic light scattering was investigated using Ho : YAG laser $(\lambda=2.12{\mu}m\;;\;t_p\sim450{\mu}s)$ and Nd:YAG Laser $(\lambda=1.32{\mu}m\;;\;t_p\sim700{\mu}s)$ for heating sources and He : Ne $(\lambda=632.8nm)$ laser, high-power diode pumped laser $(\lambda=532nm)$, and Ti : $Al_2O_3$ femtosecond laser $(\lambda=850nm)$ for light scattering sources. A spectrometer and infrared radiometric sensor were used to monitor the backscattered light spectrum and transient temperature changes from cartilage following laser irradiation. Analysis of the optical, thermal, and quasi-elastic light scattering properties may indicate internal dynamics of proteoglycan movement within the cartilage framework during laser irradiation.

Protective Immunity of Pichia pastoris-Expressed Recombinant Envelope Protein of Japanese Encephalitis Virus

  • Japanese encephalitis virus (JEV) envelope (E) protein holds great promise for use in the development of a recombinant vaccine. Purified recombinant E (rE) protein may be useful for numerous clinical applications; however, there are limitations in using the Escherichia coli expression system for producing high-quality rE protein. Therefore, in this study, the yeast expression system was used to generate the rE protein. For protein production using the yeast system, the full-length JEV E gene was cloned into Pichia pastoris. SDS-PAGE and immunoblotting analysis demonstrated that the rE protein had a molecular mass of 58 kDa and was glycosylated. The predicted size of the mature unmodified E protein is 53 kDa, suggesting that post-translational modifications resulted in the higher molecular mass. The rE protein was purified to greater than 95% purity using combined ammonium sulfate precipitation and a SP-Sepharose Fast Flow column. This purified rE protein was evaluated for immunogenicity and protective efficacy in mice. The survival rates of mice immunized with the rE protein were significantly increased over that of Hyphantria cunea nuclear polyhedrosis virus E protein (HcE). Our results indicate that the rE protein expressed in the P. pastoris expression system holds great promise for use in the development of a subunit vaccine against JEV.

  • [ $1.06/1.32{\mu}m$ ] dual-wavelength medical laser was developed and preliminary clinical comparisons at these two wavelengths were performed for dental application. We could develop a compact laser system 1) by lasing two wavelengths from the same Nd:YAG rod, and 2) by introducing high-voltage switching power supply modules. Experiment on gingiva of pig jaw showed higher thermal damage at $1.32{\mu}m$. Depending on particular procedures, each wavelength has its own advantages and disadvantages. For cutting, however, using conical tips rather than bare fibers provided better results with low threshold of cutting energy and less surrounding thermal damage. Appling light-absorbing dye on target area appeared to induce more damage during laser irradiation. However, histological studies showed no significant difference whether dye was applied or not.

Identification of Differentially Expressed Genes in Human Mesenchymal Stem Cell-Derived Neurons

  • Mesenchymal stem cells (MSCs) have greater potential for immediate clinical and toxicological applications, due to their ability to self-renew, proliferate, and differentiate into a variety of cell types. To identify novel candidate genes that were specifically expressed during transdifferentiation of human MSCs to neuronal cells, we performed a differential expression analysis with random priming approach using annealing control primer-based differential display reverse transcription-polymerase chain reaction approach. We identified genes for acyl-CoA thioesterase, tissue inhibitor of metalloproteinases-1, brain glycogen phosphorylase, ubiquitin C-terminal hydrolase and aldehyde reductase were up-regualted, whereas genes for transgelin and heparan sulfate proteoglycan were down-regulated in MSC-derived neurons. These differentially expressed genes may have potential role in regulation of neurogenesis. This study could be applied to environmental toxicology in the field of testing the toxicity of a chemical or a physical agent.
