수공예활동과 글씨쓰기활동이 양손협응(Bilateral coordination)에 미치는 영향 (Study on Effect of Crafts and Hand-writing on Bilateral Coordination)

  • 목적 : 본 연구는 수공예활동과 글씨쓰기활동이 과제를 수행할 때에 양손협응에 미치는 영향에 대해 알아보고자 하였다. 연구방법 : 손 기능에 장애가 없는 20대 대학생 30명을 선정하여 무작위방법으로 10명씩 세 군으로 나누었다. 실험군 1(수공예활동), 실험군 2(글씨쓰기활동), 대조군으로 배분하였다. 중재 전, 후에 각각의 양손협응의 정도를 비교하기 위하여 Jebsen-taylor hand function test, Purdue pegboard test, Minnesota manual dexterity test를 시행하였다. 결론 : 수공예활동과 글씨쓰기 훈련 전, 후를 비교한 결과 중재 후에 실험군 1(수공예활동)과 실험군 2(글씨쓰기활동)에서 손 기능 및 양손협응에 긍정적인 영향을 미치는 것을 알 수 있었다. 특히, 실험군 1(수공예활동)은 Jebsen-taylor hand function test 항목에서 실험군 2(글씨쓰기활동)는 Purdue pegboard test 항목에서 중재 후에 통계적으로 유의한 차이를 보였다. 결론 : 수공예활동은 과제를 수행할 때에 양손협응을 향상시키고 글씨쓰기활동은 손의 기민성을 향상시킨다는 것을 알 수 있었다. 즉, 수행하는 과제에 따라 수공예활동과 글씨쓰기활동이 미치는 영향이 다르다는 것을 알 수 있었다. 후속 연구에서는 모든 연령과 다양한 수공예활동과 글쓰기활동에 대한 다각도의 연구가 수행된다면 협응능력 제한이 있는 사람의 작업 중재 방법에 도움이 되리라 생각한다.

편측 뇌성마비 아동(hemiplegic cerebral palsy)의 양손 협응 향상을 위한 집중적 양손활동훈련에 관한 체계적 고찰 (A Systematic Review of the Bimanual Intensive Training to Improve Bilateral Coordination for Children With Hemiplegic Cerebral Palsy)

  • 목적 : 본 연구의 목적은 집중적 양손활동훈련을 편측 뇌성마비 아동에게 적용한 후 상지 재활 효과와 양손 협응 능력의 변화를 살펴본 연구에 대해서 분석하는 것이다. 연구방법 : 2004년부터 2014년 현재까지 국내 외 학회지에 게재된 논문을 RISS, Pubmed를 통해 검색하였다. 1, 2차 분류를 통해 총 6개의 실험연구 논문이 대상논문으로 선정되었다. 결과 : 선정된 6편의 문헌은 모두 국외 논문이었으며, PEDro score는 5점 이상으로 높은 편 이었다. 사용된 중재는 강제유도운동치료의 변형형태가 가장 많았으며, 상지기능과 양손 협응 측정 위한 평가도구로는 AHA와 JTTHF가 가장 많은 빈도를 보였고, 그 외 QUEST, ABILHAND-Kids, COPM, GAS, PEDI, 3-D movement kinematics가 중재효과의 다양한 측면을 평가하는데 사용되었다. 고찰 : 본 연구를 통하여 집중적 양손활동훈련이 편측 뇌성마비아동의 상지 기능뿐만 아니라 양손 협응에 긍정적인 영향을 준다는 것을 확인하였고, 더 나아가 양손 협응이 요구되는 대부분의 일상생활의 참여에 도움을 줄 수 있음을 알아보았다. 이와 같은 결과는 작업치료사로서 국내 임상가들에게 편측 뇌성마비 아동을 위한 새로운 중재방법의 기초자료를 제시하고, 효과적인 치료 접근법을 알려주는 데 도움이 될 것이다.

불안정한 지지면에서 양측성 과제운동이 뇌졸중 환자의 다리 근활성도와 균형에 미치는 영향 (Effects of Bilateral Arm Motor Coordination Exercises Conducted on Unstable Support Surfaces on Leg Muscle Activity and Balance in Stroke Patients)

  • PURPOSE: This study assessed the influence of bilateral coordination exercises on unstable support surfaces on leg muscle activation and balance in stroke patients. METHODS: Two groups were recruited for comparison: an experimental group of 10 individuals who performed bilateral coordination exercises on unstable surfaces and a control group of 10 individuals who performed the same exercises on stable surfaces. All participants were assigned randomly. Pre-tests were conducted to measure the leg muscle activation and balance levels of the participants prior to the experiment. The intervention was comprised of three 30-minute weekly sessions for four weeks, followed by a post-test after the four-week period. RESULTS: Significant differences were identified within the experimental group in relation to all muscles (p < .01) and balance (p < .05). Within the control group, significant differences were identified in relation to the rectus femoris muscle, biceps femoris muscle, and balance (p < .05). Significant differences between the two groups were only observed in relation to the tibialis anterior and soleus muscles (p < .05). CONCLUSION: Only the tibialis anterior and soleus muscles showed significant differences between the two groups. This effectiveness may be attributed to using an ankle strategy to maintain body balance during exercise on unstable surfaces.

편측 협응훈련에 의한 운동신경망의 재조직 및 교차훈련의 효과 -사례연구- (Reorganization of Motor Network and the Effect of Cross Education Derived From Unilateral Coordination Training)

  • We report the reorganization of motor network resulted from intensive unilateral coordination training and the effect of cross education on the untrained side in patient with traumatic brain injury using functional magnetic resonance imaging (fMRI). A 22 year-old male patient who had suffered from diffuse axonal injury for 58 months showed coordination deficit in the left hand at initial examination. Intensive motor training including complex finger movements and coordination activities using a metronome was introduced to the patient 4 hours per day for a week. FMRI was performed on a 3T ISOL Forte scanner. All functional images were analyzed using SPM-99 software. Hand function was improved after training not only in the trained left hand, but also in the untrained right hand. There was no activation in the right primary motor area (M1) during left hand movement before training whereas robust activation of left M1 was demonstrated by the right hand movement. Profuse activation of bilateral prefrontal lobes was seen during both hand movements before training. After training of left hand, right M1 became prominently activated during the left hand motion. The activation of bilateral prefrontal lobes disappeared after training not only for the left hand movement but also for the right, which clearly demonstrated the effect of cross education. This case report demonstrated the learning-dependent reorganization of the M1 and the effect of cross education.

Sternocleidomastoid and Posterior Cervical Muscle Coordination in Response to Symmetrical and Asymmetrical Jaw Functions in Normal Adults

  • Purpose: The aim of this study was to elucidate the coordination patterns of the sternocleidomastoid and posterior cervical muscles in response to symmetrical and asymmetrical jaw functions in normal adults. Methods: Twenty-seven healthy volunteers (8 females, 19 males; mean age, $30.4{\pm}2.5$ years) participated in this study. Surface electromyography (EMG) was used to record activities in the masseter, suprahyoid, sternocleidomastoid, and posterior cervical muscles at rest and during maximum tooth clenching, biting of a cotton roll with the anterior teeth, unilateral biting of a cotton roll with the posterior teeth, bilateral biting of cotton rolls with the posterior teeth, and jaw opening while seated. Normalized amplitude, activity indices, and asymmetry indices were compared between the muscles and the jaw tasks. Results: During symmetrical jaw functions (e.g., tooth clenching, biting with the anterior teeth, bilateral biting with the posterior teeth, jaw opening), the sternocleidomastoid and posterior cervical muscles showed elevated EMG amplitudes compared with the resting condition. The co-activation pattern of the sternocleidomastoid muscle was more pronounced than those of the posterior cervical muscles during these tasks. During asymmetrical jaw functions (e.g., unilateral biting with the posterior teeth), the ipsilateral sternocleidomastoid and masseter muscles showed higher contraction activity than did the contralateral muscles, but the contralateral posterior cervical muscles were more active than the ipsilateral muscles. Conclusions: The sternocleidomastoid and posterior cervical muscles were shown to be co-activated and coordinated anteroposteriorly or bilaterally according to symmetrical or asymmetrical jaw function. These results suggest an integrated neural control mechanism for the jaw and neck muscles, and provide further evidence supporting the intimate functional coupling between the trigeminal and cervical neuromuscular systems.

COMPARATIVE ELECTROMYOGRAPHIC ANALYSIS OF MASTICATORY MUSCLES BETWEEN BILATERAL AND UNILATERAL MASTICATORS

  • There are several variations in normal mastication. In them, unilateral mastication is chewing, predominantly on a preferred side of the dentition and hardly on e non-preferred side. Continual unilateral mastication may alter the coordination of masticatory muscles. Although they studied about these EMG of masticatory muscles, there were no information about characteristics of masticatory muscle activity in unilateral mastication. Therefore, In this study, we investigated the activity of the masseter and anterior temporal muscles during rest, clenching in maximum intercuspation and gum chewing in habitually unilateral mastication group compared with normal group and tried to know effects of continual unilateral mastication on activity of masticatory muscles. The results of this study were as follows 1. In electromyographic activity during rest, in bilateral mastication group pattern of muscle activity of right and left side was symmetrical. But, in unilateral mastication group, records of anterior part of temporal muscle was higher than that of bilateral mastication group (p<.01) and patterns of muscle activity of right and left side in both muscle were asymmetrical.(p<.05) 2. In electromyographic activity during clenching in maximum intercuspation, records of superficial part of masseter muscle were higher than anterior part of temporal muscle in both group. Muscle activity of temporal muscle in unilateral mastication group was a little higher han bilateral mastication group and asymmetry of activity pattern in temporal and masseter muscle was shown but these differences were not statistically significant. (p<.05) 3. In electromyographic activity during gum chewing, temporal muscle was activated earlier than masseter muscle and maximum bite force is derived from masseter muscle in both group. In unilateral mastication group, electromyographic activity of masseter and temporal muscle of preferred chewing side, regardless of right or left side chewing, was higher than that of bilateral mastication group and especially, difference in masseter muscle was statistically significant. (p<.01) Based on the above results, our study suggested that recording of masticatory muscle activity will be helpful in the effective diagnosis and treatment of some types of the parafunctional habits.

항공규제 완화와 항공사간 전략적 제휴에 관한 연구 (A Study on the Aviation Deregulation Act and Global Airline Alliances)

  • This study explores how strategic alliance activities are evolving and the factors that impact on the formation and development of airline alliances. Findings show the initiation of regional and more liberalized bilateral, or open skies, agreements have removed some of the impediments to structural changes in international aviation. Airlines in more liberal markets enter into greater numbers and more integrative forms of alliances. Also, airlines, on average, achieve better results of operation if the market is more liberal. Essentially, there is a positive relationship between the developments of alliances and the liberalization of air transport markets. It has been questioned that the US bilateral open skies agreements provide its carriers more access to the global market and countries that do not enter into such agreements with the US risk a loss of traffic. Consequently, open skies agreements may enable carriers who have the freedom to exercise market power to be dominant in the markets. This suggests that regulatory coordination and liberalization of international aviation reinforce each other and should therefore be pursued simultaneously.

승마 속보 시 등자 길이에 따른 체간기울기와 양측 하지의 협응성 비교분석 : 비대칭 지수 및 전체이동지수 알고리즘 개발 (Analysis of the Coordination of the Trunk Tilting Angle and Bilateral Lower Limbs According to the Stirrups Length during Trot in Equestrian: Asymmetric Index Development of Overall Movement Index Algorithm)

  • Purpose : The purpose of this study was to analyze the coordination of the trunk tilting angle and bilateral lower limbs according to the stirrups length during trot in equestrian. Methods : Participants selected as subject were consisted of adult male(n=7, mean age: $45.00{\pm}3.78yrs$, mean height: $172.50{\pm}2.44cm$, mean body mass: $76.95{\pm}4.40kg$, mean, mean leg length: $97.30{\pm}2.60cm$). They were divided into 3-types of stirrups lengths(67 cm, 72 cm, 77 cm) during trot. The variables analyzed were consisted of the trunk front-rear angle, lower limb joint(Right Left hip, knee, ankle), overall movement index(OMI) of the lower limbs(thigh, shank, foot) and asymmetry index(AI%) during trot. Results : The average angle in hip and knee joint showed more extended posture according to the increase of stirrups lengths and ankle angle showed more plantarflexion posture according to increase of stirrups length during 1 stride in trot. Also, average angle showed more extended posture in right hip and ankle joint than that of left. The angle of knee joint didn't show significant difference statistically between right and left. Also asymmetric index in average angle of hip, knee and ankle joint didn't show significant difference statistically in between lower limbs, but hip joint showed higher asymmetric index in stirrup length of 77 cm and ankle joint showed higher asymmetric index in stirrup length of 67 cm than that of the others respectively. The FR angle in trunk of horse-rider showed relative backward leaning motions at stirrup length of 67 cm and 77 cm than that of stirrup length of 72 cm during stance and swing phase. OMI in thigh, shank, and foot limbs didn't show significant difference statistically according to the stirrups length of right and left lower limbs, but left lower limbs showed higher index than that of right lower limb. Stirrup length of 72 cm in shank and foot limbs showed higher index than that of stirrup length of 67 cm and 77 cm. But stirrup length of 72 cm showed higher asymmetric index than that of stirrups length of 67 cm and 77 cm. Conclusions : When considering the above, 72 cm(ratio of lower limb 74.04%) stirrup lengths could be useful in posture correction and stabilization than 67cm(ratio of lower limb 68.69%) and 77 cm(ratio of lower limb 79.18%) stirrup lengths during trot in horse back riding.

성공적 노화를 위한 양손 운동 듀오콕 프로그램의 적용 및 효과성 검정: 양측 운동 (Application and Effectiveness of the Two-Handed Exercise Duocock Program for Successful Aging: Bilateral Exercise)

  • Objective: This study was conducted with the aim of verifying the effectiveness of the duocock exercise, which is being utilized at the newly developed site to promote balance maintenance, basic physical strength and muscle strength among various age groups, in order to prevent fall and successfully age. Method: In this study, we conducted the duocock Exercise Program for 12 weeks (twice a week for an hour) for 10 senior citizens aged 65 or older (68.8±4.76 yrs, height: 15±6.00 cm) using Western-based health institutions in Daejeon, and compared the pre- and post- effects on basic physical strength and posture balance. All the data obtained from this study were used in statistical program SPSS 24 to perform paired t-test. The significance level for all statistical analyses was set at the level of p<0.05. Results: The 12 week duocock program showed statistically significant improvements in lower extremity muscle strength, equilibrium, agility, balance, and coordination in the basic fitness factors underlying successful aging (p<0.05). In addition, the trunk imbalance, which acts as a factor of falls and body balance, was statistically significant (p<0.05) and the posture balance of the sagittal plane also showed the effect of proper body adjustment. Conclusion: Based on the results of this study, duocock provides a new exercise program in the form of a sustainable sport as a two-handed exercise, and is very effective for the elderly to improve their basic physical strength as well as to control postural imbalances, strengthen and increase muscle strength.

Effect of Neurosteroid Modulation on Global Ischaemia-Reperfusion-Induced Cerebral Injury in Mice

  • The present study was designed to investigate the putative effect of neurosteroid modulation on global ischaemia-reperfusion-induced cerebral injury in mice. Bilateral carotid artery occlusion followed by reperfusion, produced a significant rise in cerebral infarct size along with impairment of grip strength and motor coordination in Swiss albino mice. Administration of carbamazepine (16 mg/kg, i.p.) before global cerebral ischaemia significantly attenuated cerebral infarct size and improved the motor performance. However, administration of indomethacin (100 mg/kg, i.p.) attenuated the neuroprotective effect of carbamazepine. Mexiletine (50 mg/kg, i.p.) did not produce significant neuroprotective effect. It may be concluded that the neuroprotective effect of carbamazepine may be due to increase in synthesis of neurosteroids perhaps by activating enzyme ($3{\alpha}$ HSD) as indomethacin attenuated the neuroprotective effect of carbamazepine. The sodium channel blocking effect of carbamazepine may not be involved in neuroprotection as mexiletine, a sodium channel blocker, did not produce significant neuroprotective effect.
