Laparoscopic Retroperitoneal Nephroureterectomy is a Safe and Adherent Modality for Obese Patients with Upper Urinary Tract Urothelial Carcinoma

  • Objective: We evaluated the association of body mass index (BMI) with perioperative outcomes in patients who underwent laparoscopic or open radical nephroureterectomy. Materials and Methods: This retrospective single-center study included 113 patients who had been diagnosed with upper urinary tract cancer from January 1998 to June 2013 and were treated with laparoscopic nephroureterectomy (Lap group, n=60) or open nephroureterectomy (Open group, n=53). Laparoscopic nephroureterectomy was performed via a retroperitoneal approach following an open partial cystectomy. The two surgical groups were stratified into a normal-BMI group (<25) and a high-BMI group ($BMI{\geq}25$). The high-BMI group included 27 patients: 13 in the Lap group and 14 in the Open group. Results: Estimated blood loss (EBL) in the Lap group was much lower than that in the Open group irrespective of BMI (p<0.01). Operative time was significantly prolonged in normal-BMI patients in the Lap group compared to those in the Open group (p=0.03), but there was no difference in operative time between the Open and Lap groups among the high-BMI patients. Multivariate logistic regression analysis of the data for all the cohorts revealed that the open procedure was a significant risk factor for high EBL (p<0.0001, hazard ratio 8.02). Normal BMI was an independent predictor for low EBL (p=0.01, hazard ratio 0.25). There was no significant risk factor for operative time in multivariate analysis. There were no differences in blood transfusion rates or adverse event rates between the two surgical groups. Conclusions: Laparoscopic radical nephroureterectomy via a retroperitoneal approach can be safely performed with significantly reduced EBL even in obese patients with upper urinary tract cancer.

Effects of Acupotomy and Selective Cryolysis including Herbal Medicine Treatment on Thigh Circumference: Case Series (한약치료를 포함한 도침치료와 냉각요법이 허벅지둘레에 미치는 영향: 증례군보고)

  • The aim of this study is to investigate the combined effect of acupotomy, selective cryolysis and herbal medicine therapy for treating regional fat accumulation of thigh. Twenty-four patients were selected for the study. A session of selective cryolysis and acupotomy treatments were performed on both thighs once a week, for four weeks, with administration of herbal medicine. Thigh circumference, weight, body mass index (BMI), skeletal muscle mass, and body fat rate were measured before and after the procedure. Statistical significance (P<0.05) was determined using the paired t-test. After treatments, thigh circumference was significantly reduced by $3.57{\pm}1.41cm$ in 4 weeks (P<0.001). Weight, BMI, skeletal muscle mass, and body fat rate too were significantly reduced (P<0.001). Of the 24 patients, 1 reported an adverse event of frostbite. We assume this combined therapy can be useful when dealing with localized fat accumulation. Long-term evaluation and further studies are required to identify the specific effect size.

Effects of Lifestyle and Depression on the Nutritional Status of Elderly People (일 복지관 이용 노인의 생활습관 및 우울이 영양상태에 미치는 영향)

  • Purpose: This study was conducted to evaluate nutritional status, and the effects of lifestyles and depression on nutritional status of elderly people (NSE). Method: The participants were 280 elders who visited the Y city Senior Welfare Center between August 2006 and October 2006. Data were collected using a structured questionnaire that included general characteristics, lifestyle checklist, depression scale, and nutritional risk index. Also, Body Mass Index (BMI) was calculated from height and weight. T-test, ANOVA, Duncan test, Pearson coefficients correlation and stepwise multiple regression were used to analyze the data using the SPSS Win 12.0 program. Results: The NSE mean score was mid level at 5.03 BMI was 23.42 which indicates overweight. Depression was the factor that influenced the nutritional risk index most strongly, accounting for 17.3% of the total variance in the NSE. A combination of significance of health, concerns about health, housing, condition of teeth, activity, and smoking pattern accounted for 29.8% of the variance in the NSE. Condition of teeth accounted for only 3.8% of the variance in BMI. Conclusion: It is necessary to be aware that variance in the NSE can be affected by several factors including depression and lifestyle and that depression is strongly related to poorer NSE.

A Study on the Prevalence of Obesity and the Related Factors Among High School Girls (인문계와 실업계 여고생의 비만실태와 관련요인에 관한연구)

  • The prevalence of obesity by measuring the amount of body fat and its related factors were investigated in 315 high school girls living in Seoul. Average body fat amount and body fat percentage measured by the body impedance analysis were 14.2$\pm$5.2kg and 25.9$\pm$7.0% respectively. Mean body mass index(BMI) was 2.1$\pm$2.7kg/m2 and 6.7% of the subjects showed over 25kg/m2 Correlative analysis reveal that the body fat percentage was significantly correlated to both relative body weight and BMI(r=0.66, P<0.01, r=0.68, P<0.01) Relative body weight of the subjects showed positive correlation to those of their mother and father. It was shown that many students skipped breakfast often and did not have a pleasant meal with enough time. There was no relationship between adiposity and dietary nutrient or energy intake. When subgroups of the subjects attending a non-vocational high school and those attending a vocational or technical school were compared obesity was more prevalent in the former. Therefore the burden caused by preparing the university entrance examination seems to play an important role in fat deposition.

  • Purpose : The purpose of this study was to find out how changes in body shape of obese patients could be affected by the combined management of diet and psychotherapy along with exercise therapy to control energy imbalance, the cause of obesity. Methods : In this study, the aerobic exercise program was conducted on 12 industrial workers of "D" company for a total of eight weeks for obese patients with a high body mass index (25 kg/㎡ or higher). The experimental group did not apply the program three times a week. The body type analysis was carried out using the body analyzer (Inbody 770, Inbody, Korea) and blood test and psychological test were performed after 8 weeks of exercise before and after exercise. Results : The results of the group showed a change in weight loss of 6.03 kg (p=.000) on average, which is nearly 10 % of the change in weight. It was also shown that the BMI (body mass index) decreased by 1.76 kg/㎡ (p=.000). Abdominal fat (AO) is also 0.14 % (p=.000), waist circumference (WC, 12.72 cm, p=.000), internal fat (VF, 25.12 %, p=.000), liver function levels AST (5.47 U/L, p=.04), ALT (13.64 U/L, p=.000), total cholesterol, pTC -14.22. Conclusion : Based on the results of this study, the obesity management program will be able to maximize the effects of obesity control if exercise programs and psychotherapy are combined.

Weight Control and Associated Factors among Health-related Major Female College Students in Seoul (서울지역 건강관련 전공 여대생의 체중조절 및 관련 요인)

  • This study was conducted to investigate weight control status and associated factors among health-related major female college students. The subjects consisted of 41 nutrition and 78 physical exercise major female college students. Nutrient intakes, biochemical index, nutrition knowledge (recognition and accuracy), interest of weight control, body satisfaction, self-recognition of health condition, self-evaluated body weight were studied. About 73% of nutrition and 79% of physical exercise major female students were in the normal range of BMI ($18.5{\sim}23$) and 2% of nutrition and 1% of physical exercise major female students were underweight, 10% of nutrition and 6% of physical exercise major female students were obese. There were no significant differences in height and weight by major but %body fat and WHR in physical exercise majors were significantly lower than nutrition major students (respectively p<0.01, p<0.05). Overall, nutrition intakes of subjects were not shown to be appropriate, especially Ca/P of subjects was shown $0.54{\sim}0.64$, fat% out of energy of subjects was shown $24.7{\sim}29.0$ and Na intake was shown above 2000mg. Recognition and accuracy of nutrition were higher than those of physical exercise majors (p<0.001). There were no significant differences in self-recognition of health condition, self-evaluated body weight, satisfaction of body shape by major and weight control attempt. But interest of weight control of attempter was higher than that of no-attempter in nutrition (p<0.05) and physical exercise major students (p<0.01). Significantly negative correlation was found in satisfaction of body and BMI, body fat mass, %bodyfat, WHR. And significantly positive correlation was found in interest of weight control and BMI, %bodyfat, WHR. It was noticeable to see that interest of weight control was positively correlated to accuracy and accuracy was negatively correlated to blood cholesterol level. Therefore, proper nutrition education for female college students is needed in order to improve their weight control-related health.

A Study on the Relationship between Time Spent on Lunch and Degree of Obesity, Eating Habits in Culinary College Male Students (조리전공 남자대학생의 점심식사 소요시간과 비만지표, 식습관의 관련성에 관한 연구)

  • This study was to investigate the relationship between time spent on lunch and degree of obesity, eating habits in culinary college male students. This survey was conducted using questionnaires for 106 male students in a Hongseung-located culinary college. Over 16 minutes of time spent on lunch group had significantly lower body weights, obesity degrees and body mass index (BMI) and tended to have lower body fat % than the other groups. It suggests that shorter time spent on lunch is related with lower body weight, obesity degree and BMI. Those who ate the meal prepared by mother had longer time spent on lunch. The shorter times spent on lunch group recognized their body shape was lean, the longer time spent on lunch group recognized the overweight or obesity significantly. The shorter time spent on lunch group tended to eat the cooked rice in the gug and recognized that they were not healthy. Those who had diseases in the past tended to have shorter time spent on lunch. The more they chewed cooked rice and then longer time spent on lunch they had, they recognized their eating speed was not significantly speedy. The shorter time spent on lunch they had, the fewer fruits and salty foods they ate significantly. This study suggests that more chewing time. slower eating speed and longer time spent on eatmg lunch is very related with lowering weight, BMI, body fat % and obesity degrees, so male college students should be educated to have correct eating habits.

A Comparison between Asia-Pacific Region Criteria and Entropy Model Criteria about Body Mass Index of Elderly Females Using Morbidity of Chronic Disease (만성질환 이환율을 이용한 여자노인의 체질량지수에 대한 아시아-태평양지역 기준과 Entropy모델 기준 비교)

  • Objectives: This study was conducted to propose the need of re-establishing the criteria of the body weight classification in the elderly. We compared the Asia-Pacific Region Criteria (APR-C) with Entropy Model Criteria (ENT-C) using Morbidity rate of chronic diseases which correlates significantly with Body Mass Index (BMI). Methods: Subjects were 886 elderly female participating in the 2007-2009 Korea National Health and Nutrition Examination Survey (KNHANES). We compared APR-C with those of ENT-C using Receiver Operating Characteristics (ROC) curve and logistic regression analysis. Results: In the case of the morbidity of hypertension, the results were as follows: Where it was in the T-off point of APR-C, sensitivity was 67.5%, specificity was 43.1%, and Youden's index was 10.6. While in the cut-off point of ENT-C, it was 56.7%, 56.6%, and 13.3 respectively. In the case of the morbidity of diabetes, the results were as follows: In the cut-off point of APR-C, Youden's index was 14.2. While in the cut-off point of ENT-C, it was 17.2 respectively. The Area Under the ROC Curve (AUC) of the subjects who had more than 2 diseases among hypertension, diabetes, and dyslipidemia was 0.615 (95% CI: 0.578-0.652). Compared to the normal group, the odds ratio of the hypertension group which will belong to the overweight or obesity was 1.79 (95% CI: 1.30-2.47) in the APR-C, and 2.04 (95% CI: 1.49-2.80) in the ENT-C (p < 0.001). Conclusions: We conclude that the optimal cut-off point of BMI to distinguish between normal weight and overweight was $24kg/m^2$ (ENT-C) rather than $23kg/m^2$ (APR-C).

The Effects of Wild Ginseng Complex Pharmacopuncture Combined with Hyperthermia on Abdominal Obesity in Post-Menopause Women: Case Report (온열요법을 결합한 산삼비만약침의 폐경기 복부비만 개선효과: 증례보고)

  • This study was conducted to investigate the effects of wild ginseng complex (WGC) pharmacopuncture combined with hyperthermia on abdominal obesity in post-menopause women. Two post-menopausal women were treated 5 times in 2 weeks with WGC pharmacopuncture on abdominal acupoints and 30 minutes of hyperthermia on abdominal area. Before treatment and after 5 treatments were finished, body weight (BW), body mass index (BMI), body fat ratio, waist circumference (WC), waist hip ratio, thickness of abdominal subcutaneous fat (ASF) were measured. In case 1, BW, BMI, WC, and ASF were reduced by 0.6 kg, $0.2kg/m^2$, 6 cm, and 18 mm respectively. In case 2, BW, BMI, WC, and ASF were reduced by 0.4 kg, $0.17kg/m^2$, 4.6 cm, and 12 mm respectively. This suggest that combined therapy of WGC pharmacopuncture and hyperthermia might be an effective treatment for abdominal obesity in post menopause women.

Weight Control and Cardiovascular Risk in Middle-Aged Women (중년여성의 체중관리 실태와 성인병 위험도)

  • The purpose of this study was to investigate weight control practices and cardiovascular risk in middle-aged women. 304 middle-aged women were selected as subjects from thirties to fifthies living at J city in Chung-Buk Do. Data were collected using a questionnaire, anthropometric measurements, BP & total cholesterol level in serum from April 1, to June 30, 2003. The results of this study were as follows: The middle-aged woman's age is average $43.95{\pm}7.09yr$ and mean BMI(body mass index) was $23.54{\pm}3.09\;kg/m^2$. Underweight, normal weight, overweight, obese women were 3.0%, 39.5%, 27.9% and 29.6%respectively. Subjects perceived own as 'Slim' 5.9%, 'ordinary' 45.7%, 'Fat' 48.4%. Their weight perception coincide their own actual body weight but as many as 41.8% of overweight and 9.0% of obese perceived themselves as being 'ordinary'. Middle age women with past weight control experience were 55.6%, and only 35.5% was doing weight control at present. Of these subjects, 71.4% reported wanting to lose weight and the primary reason of weight control was to improve their appearance(39.53%). The most frequently reported weight control behavior was 'exercise' followed 'dieting', but 39.6% reported using 'diet food', 12.4% 'behavior modification', 12.4% 'fasting', and 'diet-drug'(3.6%) or 'smoking'(3.6%). Effective weight control methods were thought regular exercise(97.1%) & dieting (79.3%). And behavior modification(71.4%) or diet camp(60%) were effective, too. An average waist circumference was $79.80{\pm}9.47cm$, waist/hip ratio was $86.63{\pm}6.78$, waist/height ratio was $50.43{\pm}6.10$. In the index of abdominal obesity, 79.7 % of middle-aged women was waist/height ratio over 0.46, 65.3% was waist/hip ratio over 0.85, 28.4% was waist circumference over 85cm. There were significant differences in the degree of abdominal obesity according to age and BMI. In the index of cardiovascualr risk, 10.9% of middle aged women was systolic hypertension over 140mmHg, 18.7% was diastolic hypertension over 90mmHg and 10.6% was hypercholesterolemia over 200mg/dl. There was significant difference in systolic hypertension ratio according to age. There was significant difference in diastolic hypertension ratio according to age and obesity. There were significant differences in hypercholesterolemia accorting to obesity. The abdominal obesity indices and the levels of T-cholesterol in the serum, systolic and diastolic BP increased significantly according to age. T-cholesterol in serum was predicted 2.6% by waist/height ratio. And systolic BP was predicted 15.2% by waist/height, add BMI to 16.8%. Subject's diastolic BP was predicted 12.1% by BMI. Therefore waist/height ratio and BMI were significant factors for the predictors of cardiovascular risk. There was significant correlation between index of obesity and cardiovascular risk. T-cholesterol in serum had correlation with waist/eight ratio(r=0.174) and waist circumference(r=0.48). Systolic BP had correlation with waist/height ratio(r=0.387), and BMI(=0.371). diastolic BP correlation had correlation with BMI(r=0.343) and waist/height ratio(r=0.327). In conclusion, The prevalence of obesity was 29.6% in 304 cases, and increased as age after menopause increased. Middle-aged women's weight perception and actual BMI coincide but some of them did not. Trial to reduce weight was attempted. But most of them did not actually. Undesirable weight control method such as using drugs, fasting, smoking was used by some women. It is important to educate about health weight control methods and raise their awareness of exact body figures. High frequency of abdominal obesity in middle-aged women had correlation with hypertension and hypercholesterolemia. Abdominal index such as waist/height ratio, waist circumference, waist/hip ratio was used. Women's hypertension, hypercholesterolemia significantly related to body mass index and abdominal obesity.
