수술실간호사와 병동간호사의 감염관련특성과 환자안전문화가 혈행성 감염예방 인식에 미치는 영향 (The Influence of Infection-related Characteristics and Patient Safety Culture on Awareness of Blood-borne Infection Prevention in Operating Room Nurses and General Ward Nurses)

  • Purpose: This study aimed to identify the factors influencing infection-related characteristics and patient safety culture on awareness of blood-borne infection prevention between operating room nurses and general ward nurses. Methods: Participants were 198 nurses(operating room nurses 98 and general ward nurses: 100) working at three general hospitals and three university hospitals in three cities. Data were collected using a structured questionnaire from September 11 to October 14, 2020. Data were analyzed using descriptive statistics, t-tests, ANOVA, Pearson's correlation coefficient, and multiple regression with IBM SPSS/WIN 26.0 program. Results: Typically, 39.8% of nurses in the operating room and 24.0% of ward nurses experienced injuries such as needles and sharp instruments used by the patient. The awareness of patient safety culture was identified to be higher for the ward nurses. Factors influencing the awareness of blood-borne infection prevention in operating room nurses were patient safety culture and wearing protective equipment for infection prevention while nursing infected patients. Moreover, the explanatory power of these variables was 19.4%. In general ward nurses, the patient safety culture was identified as a significant predictor, which accounted for 16.5% of awareness of blood-borne infection prevention. Conclusion: To prevent hospital infection, a strategy is needed to improve the level of awareness of blood-borne infection prevention and patient safety culture of operating room nurses. To this end, the difference in infection-related characteristics and influencing factors between the operating room nurses and the general ward nurses should be considered and planned.

소아두부 컴퓨터단층촬영검사에서 방사선피폭선량 저감화를 위한 근무자의 인식도와 의식도 조사 (Awareness and Consciousness Survey of Worker's for Radiation Exposure Dose Reduction from Pediatric Brain CT Examination)

  • 본 연구에서는 부산지역의 컴퓨터단층촬영검사실의 근무자를 대상으로 소아 두부 CT 검사를 시행함에 있어 방사선방어에 대한 지식정도와 행위에 대한 인식도 및 의식도를 설문조사하여 기관별로 분류하여 상급종합병원, 종합병원, 병원간의 인식도와 의식도 점수를 비교하여 영향을 미치는 요인이 무엇인지 알아보고자 하였다. 연구결과 기관별 인식도 평균점수는 상급종합병원이 42.29, 종합병원 38.43, 병원 34.06으로 상급종합병원이 가장 높게 나타났으며 종합병원, 병원 순으로 나타났다. 기관별 의식도 평균점수는 상급종합병원이 21.37, 종합병원 24.68, 병원 29.19로 병원이 가장 높은 것으로 나타났으며 종합병원, 상급종합병원 순으로 나타났다. 따라서 종합병원이나 병원의 CT 근무자들의 인식도를 높이기 위해서는 보수교육 및 학회 등을 통해 방사선에 대한 인식을 함양시키려는 노력이 필요할 것으로 사료되며 또한 상급종합병원의 CT 근무자들의 의식도를 높이기 위해서 방사선 방어의 최적화를 모색하고 환자에 대한 방사선 피폭선량 감소에 대해 노력을 기울여야 할 것으로 판단된다.

재활전문병원 간호사의 환자안전문화에 대한 인식과 안전간호활동 수행의 관계 (Relationship between Perception of Patient Safety Culture and Performance for Safety Care Activity in Rehabilitation Hospital Nurse)

  • Purpose: This study was to examine the relationships between nurse's perception of patient safety culture and performance for safety nursing activities at rehabilitation hospitals. Methods: This study applied a descriptive research design. Participants were 194 nurses who have provided nursing services for more than 6 months at 4 rehabilitation hospitals located in B metropolitan city. Data were analyzed using descriptive statistics, t-test, ANOVA, $Scheff{\acute{e}}$ test, Pearson's correlation coefficients and multiple regression. Results: There was a positive correlation between the awareness of patient safety culture and safety nursing activity. Multivariate analysis showed that hospital work environment, experience of education, hospital climate, frequency of reported events, and marital status were significantly associated with the safety nursing activity. Overall, approximately 23.1% of total variability in the safety nursing activity could be explained by the 5 variables ($R^2=0.231$, p<.005). Conclusion: Nurses at rehabilitation hospitals are relatively positive about patient safety culture. Therefore, we need to develop safety education programs at the level of organization in order to improve patient safety through performing effective safety nursing activities in addition to increase awareness of patient safety culture among nurses. Furthermore, we need hospital's strategies at the system level for open communication and outcome reports regarding patient safety.

요양병원 간호사의 연명치료중단에 대한 태도에 미치는 영향 요인 (Factors affecting the attitude toward withdrawal of life-sustaining treatment of nurses working at long-term care hospitals)

  • Purpose: This study was a descriptive study to identify factors affecting the attitude toward life-sustaining treatment of nurses working at long-term care hospitals. Methods: Data were collected through structured questionnaires from August 2nd to 27th, 2019. Study participants consisted of 163 nurses who were working for at least 6 months from 7 long-term care hospitals in B and K city. Data were analyzed using t-test, ANOVA, Scheffe' test, Pearson's correlation coefficients, and hierarchical regression with SPSS WIN v 21.0. Results: There were significantly positive correlations between awareness of good death (r=.46, p<.001) and perception of patients' rights (r=.32, p<.001). The factors affecting participants' attitude toward life-sustaining treatment were awareness of good death (β=.35, p<.001) and their own view of death (β=.24, p=.001), which explained about 27.0% of the attitude toward life-sustaining treatment. Conclusion: Based on these results, it is necessary to develop nursing educational materials that can establish values for deaths, and cultivate legal and ethical knowledge related to attitude toward life-sustaining treatment. In addition, since the severity of a patient's condition varies and the characteristics of the institution vary depending on the type of hospital, a study is needed on the relevance of variables considering the hospital environment.

경기도내 병원에 종사하는 임상병리사의 표준주의 수행도에 영향을 미치는 요인: 건강신념모델을 중심으로 (Awareness and Compliance with Standard Precautions among the Hospital Laboratory Room Technicians in Gyeonggi Province: Based on Health Belief Model)

  • Objectives: The current study intended to find the awareness and compliance with the standard precautions of the laboratory technicians working in the hospitals in Gyeonggi Province. Understanding on the relationships among factors would be helpful in developing work environment of the laboratory technicians to prevent infection and improving the quality of infection control in hospitals. Methods: Data were collected from the 313 laboratory technicians in hospitals in Gyeonggi Province. SPSS 21.0. and AMOS 18 were adopted to analyze the data. Results: The results of the analysis confirmed that the influence of perceived sensitivity and cues to action influenced perceived threats of disease. Perceived threats and perceived barriers to compliance with the standard precautions and perceived benefits of compliance with the standard precautions significantly influenced the probability of compliance with the standard precautions. Conclusion: It was recommended that hospitals need to prepare plans to prevent the employees from infection in the hospitals. The plans must include education, regular exams, injury prevention, and provision of personal protective equipments.

응급실 간호사의 노인학대 개입의도 영향요인 (Factors Influencing Intention of Intervening towards Elder Abuse among Nurses in the Emergency Department)

  • Purpose: The purpose of this study was to examine the factors influencing intention of intervening towards elder abuse among nurses in the emergency department. Methods: A cross-sectional descriptive design was used. A convenience sample of 209 emergency room nurses working in university hospitals and general hospitals completed the questionnaires. Multiple regression analysis was performed to examine predictors of intention of intervening towards elder abuse. Result: Awareness of elder abuse, legal and institutional knowledge, attitude, subjective norm, and self-efficacy were positively correlated to the intention of intervening towards elder abuse. Factors of the intention of intervening towards elder abuse in emergency room nurses were awareness of elder abuse, followed by subjective norm and attitude. Conclusion: The findings suggest that it is necessary to educate emergency room nurses in order to improve awareness and knowledge of elder abuse. From the nursing practice and hospital organization level, specific interventions and strategies to increase the assessment and intervention of elder abuse are needed.

연명치료중단에 관한 중환자실간호사, 의사 및 중환자가족의 태도 및 인식 (Attitudes and Awareness towards the Withdrawal of Life-Sustaining Treatment among Nurses, Physicians, and Families of Intensive Care Unit Patients)

  • Purpose: This study was aimed to investigate the awareness and attitudes towards withdrawal of the life-sustaining treatment among nurses, physicians, and the families of intensive care unit (ICU) patients in general hospitals. Methods: The data were collected using a questionnaire from 80 ICU nurses, 80 physicians, and 80 families of ICU patients in general hospitals. Data were collected from February 22nd to May 31st, 2010. Rusults: ICU nurses, physicians, and families of ICU patients felt that objective and ethical guidelines were needed in making a decision to withdraw the life-sustaining treatment. The main reason for withdrawal of the life-sustaining treatment was found that the patients could not recovered despite many efforts. The role of nurses in decision making process on withdrawal of the life-sustaining treatment was considered very positive from the view of physicians and family members. The most important role of nurses for those patients in ICU was found to try their best to care for the patients. Conclusion: ICU nurses should play a major coordinating role in communication among patients, their families, and medical teams. Also, an appropriate roles of nurses in the process of the withdrawal of the life-sustaining treatment should be established.

종합병원 의료종사자의 감염관리 표준주의 수행도 영향요인 (Influencing Factors on Performance for Standard Precaution of Healthcare Workers of General Hospital for Infection Control)

  • 본 연구 목적은 종합병원 의료종사자 감염관리 표준주의에 대한 지식, 태도, 안전환경 및 인지도가 표준주의 수행도에 미치는 영향요인 분석으로 연구방법은 서술적 조사연구이며 대상은 종합병원 의료종사자 262명으로 표준주의에 대한 지식, 태도, 안전환경, 인지도 및 수행도를 측정하는 설문지를 이용하였다. 자료분석은 SPSS 프로그램을 이용하여 기술통계, t-test, ANOVA, Pearson's correlation coefficient, Multiple regression으로 분석하였다. 연구결과 표준주의에 대한 수행도는 태도(r=.18, p=.003), 안전환경(r=.42, p<.001), 인지도(r=.63, p<.001)와 양의 상관관계로 나타났으며 다중회귀분석 결과, 표준주의 수행도에 영향을 미치는 요인은 인지도(${\beta}=0.57$)와 안전환경(${\beta}=0.31$)이었다. 따라서 표준주의 수행 향상을 위해 인지도와 안전환경을 향상시키고 직종별 특성을 고려한 차별화된 감염관리 교육프로그램과 모니터링이 요구된다.

응급구조(학)과 학생들의 아동학대에 대한 인식과 신고의무태도 (Awareness of child abuse and reporting obligations attitude of paramedic students)

  • Purpose: We aimed to identify the awareness and reporting obligation attitude toward child abuse among paramedic students who will become paramedics in the future and play an important role as those with an obligation to report child abuse. Methods: Data were collected from the paramedic students after consent to this study through online. The period of data collection was from July 10 to August 2, 2023 and 142 questionnaires were analyzed using SPSS 27.0 program. Results: 79.6% of the subjects knew who was required to report child abuse, but only 46.5% knew the reporting agency, and 30.3% knew the reporting procedure. Additionally, 83.8% were willing to report child abuse when discovered. Awareness of child abuse was 4.58±0.29 points out of 5. The reporting obligation attitude toward child abuse was 3.53±0.74 out of 5. Awareness of child abuse was determined by whether one was required to report child abuse (p<.05), whether one was aware of the reporting procedure (p<.05), and whether one was willing to report child abuse (p<.05), there appeared to be a significant difference. Attitude toward reporting obligation regarding child abuse was determined by gender (p<.05), whether one was aware of the obligation to report child abuse (p<.05), and willingness to report upon discovery of child abuse (p<.01), it was found that there was a significant difference. Conclusion: It is necessary to develop realistic educational content that can be linked to suspected abuse sites in hospitals and out of hospitals so that students who will become paramedics in the future can play an active role as mandatory reporters of child abuse.

요양보호사의 노인 구강건강관리 실천도 연구 : 노인 구강건강관리 교육과 행위 필요도의 매개효과 (Care workers' practices in oral healthcare for the elderly : mediating role of elderly oral healthcare education and behavioral needs)

  • Objectives: This study aimed to assess the impact of care workers' knowledge of elderly oral health, education on elderly oral healthcare and behavioral needs, awareness of elderly oral healthcare, and actual practices in elderly oral healthcare. Furthermore, it explores the mediating effects of elderly oral healthcare education and behavioral needs on the relationship between awareness and practice. Methods: Data were collected from October 11 to December 29, 2023, from 172 certified care workers employed in nursing hospitals and other workplaces. For analyzing the data, t-test, one-way ANOVA, Pearson's correlation, and hierarchical regression were conducted using SPSS Statistics 21.0. Results: Elderly oral health knowledge, education and behavioral needs, awareness, and practice were significantly higher among: those aged 51 years and older, female, primarily working in nursing hospitals, with a total career span of 10 years, higher job satisfaction, in organizations prioritizing elderly oral healthcare, and with superior education in elderly oral health. Elderly oral healthcare education and behavioral needs had partial mediating effects on the relationship between care workers' awareness and practice of elderly oral healthcare. Conclusions: The current practices and challenges in care workers' oral healthcare for the elderly will be analyzed, and recommendations and strategies for improving practices will be formulated.
