Factors Affecting Selection of Delivery Facilities Pregnant Women (산모의 분만기관 선택관련 요인)

  • This study was designed to investigate the mar factors affecting selection of delivery facilities by pregnant women. Five hundred women hospitalized at 23 Seoul-area delivery facilities, such as university hospitals, general hospitals, hospitals, and clinics were selected and given questionnaires from April 24 to May 7, 1990. A total of 350 questionnaires were collected and analysed for the study. The results are as follows ; 1. In general, variables which significantly affected the choice of delivery facilities included the age of women, their educational level, the educational level of their husbands, monthly average incomes and residential areas. 2. In analyzing the obstetrical characteristics of the women, those variables significantly affecting the choice of delivery facilities were the gestational period, the facilities for prenatal care, the frequency of prenatal care, the type of delivery, the frequency of miscarriage, previous delivery experiences and the awareness on prenatal care. 3. In comparing the motivation factors for selecting the delivery facilities, all the factors except convenience and need for hospitalization differed significantly among delivery facilities. 4. The factor analysis was assessed for twenty possible factors motivating the choice of delivery facilities. Six factors including personal service, scale of the facility, reputation, urgency, convenience, and experience were noted explaining by 57.7%. 5. In the discriminant analysis used to clarify the major factors affecting the selection of delivery facilities, the 16 significant variables were regarded as independent variables, and the type of delivery facilities was considered a dependent variable. The stepwise method was applied to the analysis. Detected discriminant variables were the facilities for prenatal care, scale factor, personal service factor, urgency factor, convenience factor, reputation factor, experience factor, gestational period, types of delivery, frequency of miscarriage, age and income. These 12 discriminant variables were tested, with reference to discriminant prediction, on their importance in the choice of the delivery facility, by the discriminant functional formula. The test showed a hit-rate of 67.7%. The results suggest that general characteristics, obstetrical characteristics, and motivations for selecting the delivery facilities differ significantly according to the types of the delivery facilities. This study implies that all types of delivery facilities should attempt to acommodate characteristics and motivations of pregnant women. The facilities should be prepared to increase their patients satisfaction with required medical conditions by improving service and responding to the pregnant women's preferences.

Development of a Nursing Professional Values Scale (간호전문직관 측정도구 개발)

  • Purpose: The purpose of this study was to develop an instrument to measure nursing professional values. Method: Forty preliminary items were selected by classifying 223 basic items extracted via a literature study and in-depth interviews of subjects and testing the relevance of their contents. In order to verify the reliability and relevance of the preliminary instrument, data was collected from 504 nurses in 3 general hospitals. Result: As a result of the item analysis, 29 items were selected from a total of 40 items. Five factors were extracted by factor analysis, and the total variance was $51.5\%$. For the explanation of variances by factors, the 1 st factor, 'self-concept of the profession' accounted for $14.8\%$, the 2nd factor, 'social awareness' $12.1\%$, the 3rd factor, 'professionalism of nursing' $9.8\%$, the 4th factor, 'the roles of nursing service' $9.1\%$, and the 5th factor, 'originality of nursing', $5.6\%$. Cronbach's Alpha of those 29 items was .9168, which was high. Conclusion: This paper is meaningful in a way that it developed a tool capable of measuring nursing professional values, which reflects the characteristics of our country. In order to re-verify the relevance and stability of this tool, it is necessary that comparative studies should be conducted.

The Influence of Focusing Manner and Emotional Labor on Nursing Performance of Clinical Nurses (임상간호사의 포커싱적 태도와 감정노동이 간호업무성과에 미치는 영향)

  • Purpose: Focusing manner is very powerful mind-body self-help and self-awareness competence. So focusing manner may affect nurses' emotional labor and nursing performance, but few are known about it to date. The purpose of this study was to identify the degree of focusing manner, emotional labor, nursing performance of clinical nurses, and the factors that affect nursing performance. Methods: In order to collect data, structured questionnaires were administered to 212 nurses who worked at 2 university hospitals located in D city. Data were analyzed by t-test, ANOVA, correlation and multiple regression using SPSS/WIN 20.0. Results: Nursing performance had positive relationship with focusing manner (r=.45, p<.001), and negative relationship with emotional labor (r=-.25, p<.001). And the most prediction factor influencing nursing performance was focusing manner (${\beta}=.41$, p<.001) and the other factors were age (${\beta}=.30$, p=.002), and emotional labor (${\beta}=.14$, p=.012). The total variance was 37.1% by those predictors (F=25.87, p<.001). Conclusion: Based on the results of this study, it is necessary to develop and educational program on focusing manner-oriented psychotherapy in order to improve the nursing performance. Also, the management system for controlling emotional labor needs to be set up for nurses in clinical settings.

A literature review of patient safety in Korea: focused on domestic studies (환자안전에 관한 문헌 연구: 국내연구를 중심으로)

  • Objectives: The purpose of this study is to provide basic data on the continuous management and institutional measures in the future by understanding the research trends of patient safety in healthcare field. Methods:The data were extracted from 2011-2016 KoreaMed, KMBase, KISS, NDSL and KISTI. Data were analysis by frequency analysis using the SPSS 14.0 program. Results: 87.0% of the studies were quantitative studies. As for the method of sampling, 'No use' was the highest at 56.5%. Most of the participants in the study were 'nurses' (50.7%). 19 hospitals (35.8%) were the most common. The subjects of the study consisted of 35 (51.5%) patients' safety culture (awareness) and 20 (29.4%) 'safety nursing activities'. Conclusions: Patient safety and patient safety should be maintained. Further, a mature patient safety culture should be settled through cooperation management among medical staff.

A Study on subjective awareness and management status of Dental Implant Patients (임플란트 환자의 보철물에 대한 인식 및 관리 실태에 관한 연구)

  • Purpose: The purpose of this study is to provide basic data for the improvement of dental care service for patient care. Methods: The survey period was from December 10, 2018 to June 10, 2019. The subjects were 180 patients who were treated with dental implant in 20 dental hospitals and clinics in Daejeon and Gyeonggi province. A total of 145 questionnaires were analyzed except for 35 copies which were unreliable or difficult to analyze. Results: The most important care method for prolonging the life time of dental implant prosthesis was the highest rate of correct brushing (39.4%), and a majority in the subjects used oral hygiene products (91.0%). With regard to discomfort after dental implant prosthesis, food particles between the teeth showed the highest response (49.7%), and discomfort with gum infections and bleeding and reduced mastication increased significantly with age (p<0.05), whereas feeling of irritation decreased significantly with age (p<0.05). Conclusion: This study is meaningful in that it approached the perception and discomfort of prosthesis from the patients' point of view. It needs to establish the basic data to improve dental treatment service for patient care.

Health Education for Health Professionals (보건의료인에 대한 보건교육 정책)

  • The hospital setting provides many opportunities for health promotion. There are many health professionals including physicians, nurses, medical technicians who have close contact with patients and their family. Health professionals are very influential to arouse the awareness about health and illness, and to motivate to change lifestyle among patients. Thus health professionals are most effective and important human resources for health education for patients to improve recovery rates and to promote health. In spite of the importance of health professionals' role in health promotion, the Korean government has provided little support for their health education for patients. Most of the health professionals have not learned about health education theories and skills, and have little attention to educate patients to change their lifestyle. Also the health professionals themselves have relatively poor lifestyle compared with advanced western countries. To improve health education for patients and their family, following strategies and policies should be considered: reinforcing curriculum for health education in college and training course, providing practical incentives for patient education, capacity building for health education and developing guideline for patient education, training health educators, networking and collaborating between community health center and hospitals, promoting the importance of health education among patients, researching and developing health education theory and practice including cost-benefit of health education.

  • The existing algorithms or programs of egress time estimations rule out the walking velocity of each single person. But this algorithm can not be applied to estimation of evacuation in a hospital, because most patients are handicapped or walking on various kinds of tools. This study measured the moving velocities of patients according to different types of physical handicap. Also evacuation drills in several hospitals were carried out to establish an algorithm for prediction of total egress time of wards. Besides these measurements awareness of staffs about safety was investigated with the questionnaire. The results of this study is divided into two serial papers.

A study on dental hygienists' job expectation (치과위생사의 직업기대에 관한 연구)

  • Objectives : The purpose of this study was to grasp association with dental hygienists' job expectation through correlation between curricular subjects for the department of dental hygiene and job. Methods : It carried out convenience sampling of dental hospitals where are located in Capital Area, and then conducted a self-administered questionnaire survey targeting 274 dental hygienists who are working here. The statistical program was analyzed by using PSAW 18.0. Results : Research subjects' 60.2% experienced turnover. Its reason accounted for 31.7% with the working condition. As a result of examining difference in experience of turnover according to dental hygienists' individual characteristic and working form, the significant difference was shown in age, marriage, annual salary, working institution, and career. In consequence of confirming association between curricular subjects and job, the majority showed high correlation. However, the correlation with oral physiology, clinical dental hygiene, and dental clinics showed low correlation as the insignificant variable. Conclusions : As a result of this study, the high correlation between curricular subjects and job can be considered to reflect dental hygienists' job expectation. On the other hand, the working condition, professional sense, and turnover experience, which are factors related to dental hygienists' job expectation, were indicated to fail to meet job expectation.

Education and Counseling of Pregnant Patients with Chronic Hepatitis B: Perspectives from Obstetricians and Perinatal Nurses in Santa Clara County, California

  • Background: This study aimed to better understand the barriers to perinatal hepatitis B prevention and to identify the reasons for poor hepatitis B knowledge and delivery of education to hepatitis B surface-antigen-positive pregnant women among healthcare providers in Santa Clara County, California. Materials and Methods: Qualitative interviews were conducted with 16 obstetricians and 17 perinatal nurses in Santa Clara County, California, which has one of the largest populations in the United States at high risk for perinatal hepatitis B transmission. Results: Most providers displayed a lack of self-efficacy attributed to insufficient hepatitis B training and education. They felt discouraged from counseling and educating their patients because of a lack of resources and discouraging patient attitudes such as stigma and apathy. Providers called for institutional changes from the government, hospitals, and nonprofit organizations to improve care for patients with chronic hepatitis B. Conclusions: Early and continuing provider training, increased public awareness, and development of comprehensive resources and new programs may contribute to reducing the barriers for health care professionals to provide counseling and education to pregnant patients with chronic hepatitis B infection.

  • The purpose of this study is to investigate the intra CSR for the sustainable hospital management. The company was proactive stance on CSR carried out in the environmental field as a preemptive countermeasures movement of citizens and government regulations. CSR, while performing for employees, is recognized as a problem within the enterprise, it was reluctant to respond to the performance and results published. This study aims to determine whether the effect is substantially reflect If you want to include in the process of demonstrating the impact of CSR within the organization to make decisions in labor union composed of employees of internal stakeholders as a parameter. In particular, research in the field of performing a public function such situations, hospitals are lacking. Internal CSR performance of the general hospital (professional) in this study was found to have mad that the generally positive effect on improving labor productivity within the organization. In particular, its performance was more positive than in the virtuous cycle that meets the expectations and internal performance improvements improve equity executives expect employees to be borne if your blood and your liver collaborative decision-making behavior. Therefore, the internal CSR awareness by investing in sustainable management of the partnership is expected to achieve improved financial performance and labor productivity by performing more active at the same time.
