Comparative study of prosthetic complications associated with the bar-clip, milled bar, and Locator attachments for implant overdentures: a retrospective study

  • Purpose. The purpose of this study was to compare the type and frequency of prosthetic complications associated with attachment types for implant overdenture. Material and methods. In this retrospective study, 38 patients (mean age, 63.5 years) have been treated with implant overdentures from 2007 to 2014. Ten patients received a bar-clip attachment. Eleven patients had received a milled bar with Locator attachment. Seventeen patients had received a Locator attachment. The mean follow-up period was 36.9 months (range, 15-83 months). The type and frequency of prosthetic complications was recorded. The frequency was analyzed to determine the statistical difference among the 3 different attachments by using one-way ANOVA (${\alpha}=.05$) and Bonferroni post hoc method at a 5% level of significance. Results. The total number of prosthetic complications was higher in the bar-clip attachment (55 events) than in the milled bar with Locator attachment (39 events) and the Locator attachment (34 events). There were no statistically significant differences, and the most common prosthetic complication was the loss of retention. In the bar-clip attachment group, the average frequency of prosthetic complications was 3.0 events per prosthesis during the first year. In the milled bar with Locator attachment and Locator attachment groups, the average frequencies were 1.45 events and 2.35 events, respectively. Statistically significant differences were observed in the frequency of the complication. (p = .043) Conclusions. Compared to the bar-clip attachment, implant overdentures that use milled bars with the Locator attachment have a significantly lower incidence of prosthetic complications in the first year of follow-up after placement.

Effects of type of magnet attachment and implant angulation in two implant overdenture models

  • PURPOSE. The purpose of this study is to evaluate the effects of type of magnet attachment and implant angulation in two implant overdenture models. MATERIALS AND METHODS. Magnet attachments used in this study were flat and dome types (MGT5515, MGT5520D, Dentium Co., Seoul, Korea). Two implants with keepers were inserted in the resin blocks at a distance of 24 mm. For the first model, the implants were parallel to the vertical and perpendicular to the horizontal; for the second model, both were angulated 5 degrees to the mesial; for the third model, both were angulated 10 degrees toward the mesial. The retentive force was measured in both vertical and lateral directions. Statistical analyses were performed using SPSS software version 22.0 (α=.05). RESULTS. The flat type magnet attachment showed the highest lateral retentive force in the 20° divergent group (P<.05) and the dome type magnet attachment showed the highest lateral retentive force in the parallel group (P<.05). The vertical and lateral retentive force of the dome type magnet attachment was greater than that of the flat type magnet attachment in every direction (P<.05). CONCLUSION. Within the limitations of this study, the dome shape magnet attachment can resist vertical and lateral retentive force more superiorly than the flat type magnet attachment, regardless of angle, in the mandibular two implant model.

Attachment distribution of pectoral muscle origins identified in dual-plane breast implant insertion

  • Background Implant-based dual-plane augmentation mammoplasty requires accurate separation of the pectoralis major muscle (PMM) at its origins. The authors identified the PMM origins during breast reconstruction surgery with the goal of providing additional information on subpectoral implant insertion for reconstructive or aesthetic purposes. Methods This study was conducted on 67 patients who underwent breast reconstruction surgery at the breast center of our hospital between November 2016 and June 2018. In total, 34 left and 39 right hemithoraces were examined. The left and right hemithoraces were each divided into 15 zones to determine the percentage of PMM attachments in each zone. The distribution of PMM origins in each zone was examined to identify any statistically significant differences. Results There were no statistically significant differences in the origins of the PMM between the right and left hemithoraces. The percentage of attachments increased moving from the fourth to the sixth rib and from the lateral to the medial aspect. Conclusions The anatomical findings of this study could be used as a reference for accurate dissection of the origins of the PMM for the preparation of the subpectoral pocket for subpectoral implant placement.

The Effect of Insecure Attachments, Depression, and Life Satisfaction on SNS Addiction Tendency in University Students (대학생들의 불안정애착, 우울, 삶의 만족도, 성별이 SNS 중독 경향성에 미치는 영향)

  • This study examined the effect of insecure attachments, depression, life satisfaction on SNS addiction tendency. Participants were recruited from 3 universities in Seoul, Daejeon and Kyungbook Province. Participants were given the survey including instruments that measured adult attachment, depression, life satisfaction, and SNS addiction tendency. Total 310 participants' data were used for analysis. Multiple regression revealed that SNS addiction tendency was predicted by anxious attachment(${\beta}=.32$, p<.001), depression(${\beta}=.17$, p=.003), and gender(${\beta}=-.25$, p<.001), except life satisfaction, and 26% of the variance was explained by these variables. These findings can help to expand the understanding and the development of the effective prevention/intervention programs of SNS addiction tendency for university students.

Effects of a tunnel ventilation system within the tie-stall barn environment upon the productivity of dairy cattle during the winter season

  • Objective: The objective of this study was to examine the effect of using a tunnel ventilation system within the dairy barn environment upon the productivity of dairy cows during the winter season. Methods: The study was performed at the University Farm, Faculty of Agriculture, Utsunomiya University. Twenty-one Holstein dairy cows (5 heifers and 16 multiparous) were enclosed in a stall barn. Unventilated (UV) and tunnel-ventilated (TV) was operated by turns every other week, and a number of key parameters were measured in the barn, including tunnel ventilation output, temperature, relative humidity, gas concentrations (oxygen [$O_2$], carbon dioxide [$CO_2$], and ammonia [$NH_3$]). Also, skin and rectal temperature, respiratory rate, blood gas concentrations, and bacterial count were measured from nipple attachments on ten cows. The amount of fodder left uneaten, and general components and somatic cell count of the milk were measured. Results: As for our dairy barn environment, air temperature dropped significantly with the passage of time with TV. Humidity was significantly higher with TV at 0600 h compared to UV, while $CO_2$ and $NH_3$ concentrations with UV were significantly higher than with TV at 0000 h and 0600 h. Skin temperature was significantly lower with TV compared to UV at 0000 h and 0600 h. Respiratory rate was also significantly lower at 0600 h with TV than with UV. Bacterial count for the nipple attachments was significantly lower with TV than with UV at 0600 h. The amount of leftover fodder was significantly less with TV in comparison with UV. Conclusion: Our results suggest that a TV system in the winter barn results in environmental improvements, such as reductions in unfavorable gas concentrations and bacterial growth. Consequently, it is expected that barns utilizing a TV system will be beneficial for both animal health and production.

Predicting patient experience of Invisalign treatment: An analysis using artificial neural network

  • Objective: Poor experience with Invisalign treatment affects patient compliance and, thus, treatment outcome. Knowing the potential discomfort level in advance can help orthodontists better prepare the patient to overcome the difficult stage. This study aimed to construct artificial neural networks (ANNs) to predict patient experience in the early stages of Invisalign treatment. Methods: In total, 196 patients were enrolled. Data collection included questionnaires on pain, anxiety, and quality of life (QoL). A four-layer fully connected multilayer perception with three backpropagations was constructed to predict patient experience of the treatment. The input data comprised 17 clinical features. The partial derivative method was used to calculate the relative contributions of each input in the ANNs. Results: The predictive success rates for pain, anxiety, and QoL were 87.7%, 93.4%, and 92.4%, respectively. ANNs for predicting pain, anxiety, and QoL yielded areas under the curve of 0.963, 0.992, and 0.982, respectively. The number of teeth with lingual attachments was the most important factor affecting the outcome of negative experience, followed by the number of lingual buttons and upper incisors with attachments. Conclusions: The constructed ANNs in this preliminary study show good accuracy in predicting patient experience (i.e., pain, anxiety, and QoL) of Invisalign treatment. Artificial intelligence system developed for predicting patient comfort has potential for clinical application to enhance patient compliance.

A new method of predicting hotspot stresses for longitudinal attachments with reduced element sensitivities

  • For the complicated structural details in ships and offshore structures, the traditional hotspot stress approaches are known to be sensitive to the element variables of element topologies, sizes, and integration schemes. This motivated to develop a new approach for predicting reasonable hotspot stresses, which is less sensitive to the element variables and easy to be implemented the real marine structures. The three-point bending tests were conducted for the longitudinal attachments with the round and rectangular weld toes. The tests were reproduced in the numerical simulations using the solid and shell element models, and the simulation technique was validated by comparing the experimental stresses with the simulated ones. This paper considered three hotspot stress approaches: the ESM method based on surface stress extrapolation, the Dong's method based on nodal forces along a weld toe, and the proposed method based on nodal forces perpendicular to an imaginary vertical plane at a weld toe. In order to study the element sensitivities of each method, 16 solid element models and 8 shell element models were generated under the bending and tension loads, respectively. The element sensitivity was analyzed in terms of Stress Concentration Factors (SCFs) in viewpoints of two statistical quantities of mean and bias with respect to the reference SCFs. The average SCFs predicted by the proposed method were remarkably in good agreement with the reference SCFs based on the experiments and the ship rules. Negligibly small Coefficients of Variation (CVs) of the SCFs, which is measure of statistical bias, were drawn by the proposed method.

  • Anterior talofibular ligament (ATFL) injuries are the most common cause of ankle sprains. To ensure anatomically accurate surgery and ultrasound imaging of the ATFL, anatomical knowledge of the bony landmarks around the ATFL attachment to the distal fibula is required. The purpose of the present study was to anatomically investigate the ATFL attachment to the fibula with respect to bone morphology and attachment structures. First, we analyzed 36 feet using micro-computed tomography. After excluding 9 feet for deformities, the remaining 27 feet were used for chemically debrided bone analysis and macroscopic and histological observations. Ten feet of living specimens were observed using ultrasonography. We found that a bony ridge was present at the boundary between the attachments of the ATFL and calcaneofibular ligament (CFL) to the fibula. These two attachments could be distinguished based on a difference in fiber orientation. Histologically, the ATFL was attached to the anterodistal part of the fibula via fibrocartilage anterior to the bony ridge indicating the border with the CFL attachment. Using ultrasonography in living specimens, the bony ridge and hyperechoic fibrillar pattern of the ATFL could be visualized. We established that the bony ridge corresponded to the posterior margin of the ATFL attachment itself. The ridge was obvious, and the superior fibers of the ATFL have directly attached anteriorly to it. This bony ridge could become a valuable and easy-to-use landmark for ultrasound imaging of the ATFL attachment if combined with the identification of the fibrillar pattern of the ATFL.

  • Computerized 3D virtual dental models are currently available, and their use has started to improve treatment outcomes. The accuracy of digital models has been demonstrated by many studies and various intra-oral scanners are innovated for short scanning time and high precision. Recently, a digital model was combined with a high technology computer-driven system, which was developed for the application of a digital set-up and indirect bonding of lingual attachments. In this section, virtual treatment planning using a virtual set-up program is be introduced, and the clinical applications and accuracy of computer-generated indirect bonding are discussed.

Taegyo in Korea (한국에서의 태교)

  • Purpose: This study was done to review Taegyo types, the practice of Taegyo, factors of Taegyo, and the effect of Taegyo programs in Korea. Method: This study reviewed the preceding research related to Taegyo through websites, articles, and books. Result: As a result of a review of Taegyo types, the practice of Taegyo, factors of Taegyo, and the effect of Taegyo programs through websites, articles, and books, it was summarized that pregnant women were independent people able to practice Taegyo from pre-pregnancy to childbirth for baby's health. If she practiced encouraging behaviors of Taegyo with her husband's physical, emotional, and economical support, she and her husband would easily transition to parenthood and form strong attachments with their baby. In addition, the rate of mother's postpartum depression was decreased. However, some pregnant women felt a psychological, social, and economical pressure to practice Taegyo. Conclusion: Activating organized and accessible to support systems and community networks, such as community health centers, nurses can help pregnant couples to increase self-care through the performance of Taegyo.
