  • The purpose of this study was to assess pain, discomfort, depression and coping patterns and the relations between these in chronic arthritis patients. The sampling method was a purposive sampling technique. 1) Who have been diagnosed as having chronic arthritis and. 2) Who were at the out patients clinic of rheumatoid arthritis departments of one University hospital in seoul between september, 11, 1993 to september, 18, 1993. The instruments used for this study were Graphic Rating Scales of pain, discomfort level of the activities of daily living(ADL) developed by Lee, Eun Ok and The Beck Depression inventory. The research used to measure coping patterns was a tool developed by the present study researcher. Analysis of data was done frequency, Pearson correlation coefficients, ANOVA, regression and ANCOVA. The results were summerlized as follows ; 1. Female exceed male patients in number and onset of joint pain were more prevalent in the age groups of the 40s and the 50s. The average duration of suffering from the pain were seven years six mounths. 2. The mean pain score : The mean sensory score was 119mm and the affective score was 109mm. 3. The discomfort level of ADL, the mean score was 2.95 out of a possible score of 5.0 and depression syndrome subjects were 62.2%. 4. The coping responses for each pattern were as follows : 1) "Active coping" mean score was 2.28. 2) "Wishful coping" mean score was 2.89. 3) "Receptive coping" mean score was 3.31. 4) "Negative coping" mean score was 1.82. 5. Significant differences were found in age, religion, marriage status and the coping patterns of patients. 1) In the coping pattern of "receptive coping", the score of the age groups of the 50s were higher than that of 20s, and in the coping pattern of "negative coping", the score of the age groups of the 20s were higher than other age groups. 2) In the coping pattern of "wishful coping", the score of the christian were higher than other religion groups. 3) In the coping pattern of "negative coping", the score of the marrieds were lower than other groups. 6. Patients who scored low on factor 2, wishful coping, were much more likely to report having pain sensory than patioets scoring high on this factor. 7. Patients who scored high on factor 4, negative coping, were significantly more likely to report having pain sensory than patients scoring high on this factor. Consider overall, chronic arthritis patients report using a wide varity of strategies, certain strategies such as receptive, wishful and active coping are used frequently, whereas other strategies such as negative coping are rarely used. One of the most important finding of present study is that the reported use of coping strategies is related to adjustment to a chronic pain problem. The present study suggests that negative coping is related to poor emotional adjustment as assessed by depression, but not pain ratings. Considered overall, this pattern of findings suggests that counseling patients to decreased their use of negative coping may be useful. The present study has a number of limitations. First, the sample is restricted to chronic arthritis patients. Weather chronic pain patients suffering from other types of pain syndromes use similar coping strategies needs to be determined in subsequent research. Second, the tool of coping pattern must be studied further to obtain reliability.

  • In this paper, we checked health problems of patients who suffered from arthritis. After assessing each patients condition, in order to alleviate their sufferings, we introduced several methods of aromatherapy as a holistic approach to these health problems. Aromatherapy is used of essential oils, which are obtained from aromatic plants, can be defined as a nature-friendly complementary/alternative method, has its great merit in its each way of application. If the arthritis condition is localized, massage, compress or poultices, and hand or foot baths are most effective. Many of essential oils in the recipe have dual benefits, as anti-inflammative and analgesic agents. The Juniper and carrot seed aid in eliminating fluid and toxin accumulation in the joint and surrounding tissue. There are many different courses available and health professional should choose one that is relevant to clinical practice. Considering the future of nursing as a major contribution in the consumer-centered structure of medical health service, aromatherapy as a complementary/alternative method is expected to contribute not only to establishing more effective health service but also to resolving chronic health problems of arthritis.

  • Purpose: This study aims to analyze the outcome of exercise programs for Rheumatoid Arthritis patients and its tendency with network analysis. Method: 30 articles from Medline search of foreign nursing journals(1966-2002) and 5 from three Korean nursing journals(1970-2002) were surveyed. The contents analyses were focused on outcome measures and network analysis of exercise programs. Results: Exercise programs result in improving muscle strength(91.7%), aerobic capacity(72.9%), and 50 feet walk time(57.3%). Among several exercises, water exercise, and Tai-chi are in the middle of weight-bearing exercises. In the experimental design, non-randomized control trials were 40%, and the functional status of subjects were not classified in 40% of the articles surveyed. Conclusion: Considering the results, muscle strength, aerobic capacity, and 50 feet walk time were tested as valid outcome measures. Tai-chi exercise could be recommended as proper exercise for Rheumatoid arthritis patients because of its low intensity of weight-bearing exercise. In terms of research methods, the randomized trials and functional classifications of rheumatoid arthritis should be done more strictly.

  • This study attempts to provide the rudimentary data that might help patients with Rheumatoid Arthritis manage an independent life by the analysis of their activities of daily living(ADL). The subjects were forty patients who had been diagnosed Rheumatoid Arthritis and under drug therapy from RA clinics of the two university hospitals in Pusan. Data were collected by performing an individual interview for each of them during June 15th through July 15th 1998. Questionaires were imployed to investigate the general characteristics of patients. the traits of Rheumatoid Arthritis, and actual conditions of their activities of daily living. The questionaires utilized especially for the study of ADL is the version of Lee Eun Ok's tool which modified and complemented Katz scale (Katz, Down, Cogh and Grots 1970) and Bathel index(Bathel. 1973). It's consisted of 31 questions that each question is measured by 0-3points. The high score in the questionaire signifies the high independency in the ADL. The data were analyzed by SPSS/Win in terms of frequency, percentage, mean, and standard deviation, close connection with the contents of the personal interview. Total mean of the ADL shows $1.55{\pm}0.68$, indicating the overall activities of daily living is generally dependent. It also indicates that patients, in the condition of dependent activity. tends either to have recourse to drugs or to take negative strategy, getting help from inside or outside family member. Hence, it is necessary that patients are exposed to the introduction and instruction of self-help devies that might help them manage a comparatively independent ADL, easing their paychological burden.

  • The etiology of rheumatic arthritis (RA) is associated with a number of genetic and environmental factors, but is not definitively elucidated. Recently, more attention has been paid to the possibility of microbial etiology in the pathogenesis of RA, because many different infectious agents have been reported to precede the onset or exacerbation of RA. Adenovirus (ADV) may be one cause of persistent or recurrent inflammatory arthritis. Varicella zoster virus (VZV) arthritis is detected frequently in RA patients treated with low dose methotrexate. The demonstration of simultaneous presence of both viral agents of specific viral nucleic acid in synovial fluids from synovitis patients would provide more direct evidence for arthritis etiological relationship, but there are no confirmed results. Therefore, we studied the ability of adenovirus and VZV to establish coinfection and persistent infection in synovial fluid from synovitis patients. The presence of viral agents in the synovial fluid demonstrated by isolation of cell culture, enzyme immunoassay and nested-PCR. The synovial fluids were also investgated for the presence of viral nucleic acid by nested-PCR using specific primer. ADV produced 220 bp and VZV produced 447 bp by each nested-PCR with specific primers. We detected 4/6 cases (66.7%) with persistent infection of ADV and 5/6 cases (83.3%) of VZV with 13 synovial fluids (between 7 to 52 day intervals) from synovitis patients by monoclonal ErA and nested-PCR. 21/28 cases (75%) with coinfection of adenovirus and VZV with synovial fluids from synovitis patients by nested-PCR. ADV and VZV coinfection and persistent infection of synovial fluids may provide a chronic antigenic stimuli to the immune system therefore provoking a continuing inflammatory response and caused the possibility of synovitis and arthritis.

  • The demand for total knee arthroplasty (TKA) is rapidly increasing worldwide. The most common indication for TKA is osteoarthritis (OA); however, some patients with rheumatoid arthritis (RA) also undergo TKA. This study aimed to investigate the effects of RA on TKA. Our findings revealed that patients with RA underwent TKA at a younger age than did patients with OA. However, contrary to the findings of pre-21st century studies, the average age of TKA among patients with RA was not significantly different from that of patients with OA. Additionally, patients with RA had a 1.5-fold higher risk of undergoing TKA. Although not statistically significant, patients with RA had a higher revision TKA rate, a shorter time until revision TKA, and underwent more revision TKAs due to infections than did patients with OA. An analysis of factors that affect revision TKA revealed that the risk of revision increased if the erythrocyte sedimentation rate and C-reactive protein levels were increased at the time of TKA. This study showed that patients with RA have a slightly higher risk of undergoing TKA than did patients with OA. Furthermore, the presence of inflammation at the time of TKA increases the risk of revision; therefore, inflammation should be adequately controlled before performing TKA.

  • Purpose: To examine the effects of the GEAP on pain, fatigue, self-esteem, perceived health status and self-efficacy in patients with chronic arthritis. Methods: One group Pre- & post-test design was used. Outcome domains included pain, number of painful joints, fatigue, self-esteem, perceived health status and self-efficacy. Thirty-four patients were recruited over four times from a rheumatic clinic affiliated to a university medical center in Seoul, Korea The GEAP is an exercise program modified by the investigators from PACE program which developed by Arthritis Foundation. The program duration is about 60 minutes, three times a week for 6 weeks. Participants were predominantly women (68%) ; on average 57.3 years old; and diagnossed with RA (58.8), osteoarthritis (29.4) or other (11.8). Results: After completing the GEAP, subjects showed significant improvement in pain (p=.02), number of painful joints (P= .04), fatigue (p= .0001), and perceived health status (P=.006). There were no improvements in self-esteem and self-efficacy score for adults with chronic rheumatic diseases. Conclusion: This study showed that the GEAP is an effective exercise intervention for improving outcomes for patients with chronic arthritis. More sample and research are needed to 1) accurately evaluate on self-esteem and self-efficacy; 2) understand and improve adherence. And also a longer follow up period is recommended to evaluate long-term effects of the PACE program.
