Ondaron 주사제의 항 구토작용 (Anti-emetic Effect of Ondaron in Ferrets)

  • The anti-emetic effect of a 5-HT$_3$ receptor antagonist, Ondaron, was compared with that of the approved ondansetron agent, Zofran$\circledR$ in the ferrets. Emesis was induced by single intraperitoneal injection of cisplatin 10 mg/kg, and Ondaron or Zofran$\circledR$ was injected intraperitoneally in a dose of 1.0 mg/kg, respectively. Ondaron and Zofran$\circledR$ effectively antagonised the emetic response for 4 hours after injection. They significantly reduced the number of vomiting and retching, and prolonged the latency to the first episode. The anti-emetic effect of Ondaron was almost the equal to that of Zofran$\circledR$. These results suggest that Ondaron is an effective anti-emetic agent against cisplatin-induced emesis, and its anti-emetic potency is similar to that of 5-$HT_3$ receptor abtagonist, Zofran$\circledR$.

  • The tuber of Pinellia ternata Breitenbach(Araceae), which is distributed in Korea, China, and Japan, has been used in traditional Chinese medicine. The prescription containing Pinellia tuber shows anti-emetic, sedative, and anti-tussive effects. The purpose of this study was to investigate the effects of Pinellia ternata tuber on the xylazine-induced emetic and sedative responses in cats. The results were as follows ; 1. Intramuscular injection of xylazine hydrochloride(1.0mg/kg) reliably evoked vomiting with an incidence of 100% and sedated with a mean sedation time of 34.22 min. 2. The xylazine-induced emetic and sedative responses were not prevented by oral administration of powder (0.5g/head), decoction ($1.0m{\ell}/100g$), and methanol extract ($0.1m{\ell}/100g$) of the Pinellia ternata tuber. 3. The xylazine-induced emetic and sedative responses were inhibited by intravenous injection of decoction($0.3m{\ell}/100g$) of the Pinellia ternata tuber. 4. The xylazine-induced emetic and sedative responses were inhibited by intravenous injection of a combined mixture of yohimbine hydrochloride(0.125mg/kg) and 4-aminopyride(0.3mg/kg).

  • Bioassay-guided fractionation of anti-emetic constituents of Alpinia katsumadai Hayata was performed. Nine compounds including one novel compound, (3R,5S)-trans-3,5-dihydroxy-1,7-diphenyl-1-heptene (9) were isolated from it. Among these compounds, four diarylheptanoids, one sesquiterpenoid and one flavonoid showed anti-emetic activity on copper sulfate induced-emesis in young chicks.

  • The new assay method was developed by using young chicks instead of frogs for screening of anti-emetic compounds from natural sources. Comparing with our previous method using leopard and ranid frogs, the advantages of the new method were not only completely parallel results but also decreasing standard errors. Hinesol and eudesmol were isolated from MeOH extract of Atractylodes lancea rhizome as the anti-emetic principles.

비유혈(BL-20) 및 장문혈(LIV-13)에 대한 자침, 수침 및 뜸 요법이 Xylazine을 투여한 개에서의 구토 억제 효과 (The anti-emetic effect of needling acupuncture, aquapuncture and moxibustion at BL-20 and LIV-13 on the xylazine induced emesis in dogs)

  • The present experiment was performed in order to know the anti-emetic effect of acupuncture, aquapuncture with metoclopramide and moxibustion on the xylazine induced emesis in dogs. The animals were devided into a control group (non-acupoint) and two experimental groups (BL-20 and LIV-13), respectively. Acupuncture, aquapuncture with metoclopramide (1 mg/kg) and moxibustion were applied to animals for 20 minutes before xylazine injection (2.2 mg/kg, IM). In acupuncture group, the emetic rates in BL-20 (16.7%) and LIV-13 (16.7%) were lower than that of control group (50%), respectively. In aquapuncture group, the emetic rates in BL-20 (16.7%) and LIV-13 (0.0%) were lower than that of control group (50%), respectively. In moxibustion group, the emetic rates in BL-20 (50%) and LIV-13 (16.7%) were lower than that of control group (83.3%), respectively. Considering above the findings collectively, it is considered that acupuncture, aquapuncture with metoclopramide and moxibustion at BL-20 and LIV-13 are effective and especially aquapuncture with metoclopramide at LIV-13 is the most effective treatment to prevent the emesis induced by xylazine among groups.

Effects of Korean Red Ginseng Extract on Cisplatin-Induced Nausea and Vomiting

  • Ginseng, the root of Panax ginseng CA Meyer, is well known as a tonic medicine for restoring and enhancing human health. In traditional medicine, ginseng is utilized for the alleviation of emesis, which includes nausea and vomiting. However, it has not yet been demonstrated whether ginseng exhibits in vivo anti-nausea and anti-vomiting properties. In this study, we examined the anti-emetic effect of Korean red ginseng total extract (KRGE) on cisplatin-induced nausea and vomiting using ferrets. Intraperitoneal administration (i.p.) of cisplatin (7.5 mg/kg) induced both nausea and vomiting with one-hour latency. The episodes of nausea and vomiting reached a peak after 1.5 h and persisted for 3 h. Treatment with KRGE via oral route significantly reduced the cisplatin-induced nausea and vomiting in a dose-dependent manner. The anti-emetic effect was 12.7 $\pm$ 8.6, 31.8 $\pm$ 6.9, and 67.6 $\pm$ 4.0$\%$ with doses of 0.3, 1.0, and 3.0 g/kg of KRGE, respectively. Pretreatment with KRGE via oral route 1 and 2 h before cisplatin administration also significantly attenuated the cisplatin-induced nausea and vomiting. However this did not occur with a pretreatment 4 h before cisplatin administration. These results are supportive of KRGE being utilized as an anti-emetic agent against nausea and vomiting caused by chemotherapy (i.e. cisplatin).

내관(PC-6) 및 위유(BL-21)의 자침 및 수침이 xylazine을 투여한 개에 미치는 구토 억제효과 (The anti-emetic effect of acupuncture and aqua-acupuncture at PC-6 and BL-21 on the emesis by xylazine in dogs)

  • This study was performed to elucidate the effect of acupuncture and aqua-acupuncture in dogs. Each group was divided into control (18 dogs), PC-6 (6 dogs) and BL-21 (6 dogs) group for the antiemetic effect of acupuncture, respectively. And, the animals were divided into control (18 dogs), PC-6 (18 dogs) and BL-21 (18 dogs) group for the antiemetic effect of aqua-acupuncture with metoclopramide, respectively. 2% xylazine was injected to the all dogs to induce the emesis, and acupuncture and aqua-acupuncture treatment (0.5% metoclopramide) were applied to animals for 20 min. before xylazine injection, respectively. Emetic rates in PC-6 (33.3%) and BL-21 (16.7%) were lower than that of control group (44.4%) in acupuncture groups, respectively. Emetic rates in PC-6 (16.7%) and BL-21 (0.0%) were lower than that of control group (38.9%) in aqua-acupuncture groups, respectively. Based on these findings, it was considered that the antiemetic effects of acupuncture and aqua-puncture at BL-21 were more effective than those of PC-6 groups, and aqua-acupuncture with metoclopramide at BL-21 was the most effective among them.

Aprepitant Regimen의 고용량 Cisplatin 유발 오심 및 구토 예방 효과 (The Effect of Aprepitant Regimen on the Prevention of High-Dose Cisplatin-Induced Nausea and Vomiting)

  • Aprepitant is a substance P/neurokinin-1 (NK1)-receptor antagonist that was approved in 2003 for prevention of CINV. In addition, updated anti-emetic guidelines that include the aprepitant regimen have been published by NCCN and ASCO. However there is scarce clinical data in Korea. The prospective study was performed to evaluate the prevention of high dose cisplatin induced nausea and vomiting in all patients who started high-dose cisplatin-based chemotherapy at our hospital. We checked the nausea severity and vomiting episodes by calling patients within 4 to 5 days after chemotherapy. The retrospective study was performed to compare the prevention of CINV in solid tumor patients who switched their anti-emesis regimen from the standard regimen to the aprepitant regimen. In aprepitant regimen, aprepitant was added to the same anti-emetic regimen used during previous cycles. We checked the nausea, vomiting grades and adverse events in electronic medical records (EMR). In prospective study, 195 patients were included in the analysis. 88.2% of patients achieved a complete response (no emesis and no rescue therapy). In retrospective study, 54 patients were reviewed. With aprepitant regimen, nausea and vomiting grades were improved in 22 patients (40.7%) and in 9 patients (16.7%), respectively. Compared with standard regimen, addition of aprepitant provided superior prevention against CINV in Korean patients receiving highly emetogenic cisplatin-based chemotherapy. Moreover, aprepitant significantly prevented CINV in patients who received the standard regimen to prevent CINV in previous chemotherapy cycles.

복부 방사선치료를 받는 환자에서 발생하는 오심 및 구토에 대한 온단세트론과 메토클로프라미드의 효과 : 제 3상 전향적 무작위 비교임상시험 (A Prospective Randomized Comparative Clinical Trial Comparing the Efficacy between Ondansetron and Metoclopramide for Prevention of Nausea and Vomiting in Patients Undergoing Fractionated Radiotherapy to the Abdominal Region)

  • 목적 : 본 연구는 중등도 정도의 오심 및 구토의 발생 위험도를 가지는 복부 방사선치료를 받는 환자들을 대상으로 오심 및 구토에 대한 항구토제의 효과와 부작용 여부를 비교하는 전향적 무작위 임상연구로, 예방 목적으로 항구토제를 사용할 때 현재 통상적으로 많이 처방되는 항구토제인 메토클로프라미드(metoclopramide)에 비하여 온단세트론(Ondansetron; $Zofran^{\circledR}$)을 사용하는 것이 더 나은 효과를 가져오는지 알아보고자 하였다. 대상 및 방법 : 대상 환자의 포함 기준은 병리학적으로 암으로 확진되고 전이성 원인이 아닌 원발 부위에 치료를 받는 환자, 신체수행도가 ECOG 기준으로 2기 이하인 환자, 항암 화학요법이나 방사선치료를 받은 과거력이 없는 환자로 하였다. 1997년 3월부터 1998년 2월까지 총 60명의 환자가 덜 연구에 참여하였다. 내원한 순서에 따라 미리준비된 난수표에 의하여 각각 온단세트론 투약군(O군)과 메토클로프라미드 투약군(M 군)으로 할당하였다. O군의 온단세트론 용량은 8 mg, bid 로 하였고, M군의 메토클로프라미드 용량은 5 mg, tid 로 하였다. 평가 항목은 오심의 정도, 구토 회수, 식욕감소의 정도로 하였다. 환자에게 일일 기록카드를 교부하여 환자의 주관적인 증상인 오심의 정도, 구토의 횟수, 식욕감소의 정도를 기록하도록 하였다. 치료기간을 일주일 간격으로 나누어 각 주별 오심 및 식욕감소의 정도, 평균 구토횟수를 구하였다. 결과 : 본 연구에 참여한 60명의 대상 환자 중 55명의 환자에서 분석이 가능하였다. M군은 28명, O군은 27명이었다. 환자 특성 및 방사선치료에 관한 특성은 연령을 제외하면 차이가 없었다. 연령은 M 군이 $52.9{\pm}11.2$세, O군이 $46.5{\pm}9.6$세로 통계적으로 의미 있는 차이가 있었다. M군에 비해 O군에서 오심의 정도, 구토의 횟수, 식욕감소의 정도의 세 항목 모두 낮은 점수 분포를 보였다. M군의 경우 방사선치료를 시작한 후 5주째 각 항목별 점수가 가장 높은 경향을 보였다. 혼합모형을 이용한 반복측정 자료의 분석 결과 오심의 정도에는 원발암의 종류, 투약군이 의미 있게 영향을 미치는 요소였다. 구토의 횟수에는 성별, 연령, 총선량, 투약군이 의미 있게 영향을 미치는 요소였다. 식욕감소의 정도에는 연령, 투약군이 의미 있게 영향을 미치는 요소였다. 따라서, 투약군이 유일하게 세 가지 항목 모두에 통계적으로 의미 있는 영향을 미치는 요소임을 알 수 있었다. 전 치료기간 동안 양 투약군 모두 약제의 투여로 인한 부작용은 없었으며, 일반혈액검사 및 일반화학검사에서도 이상 소견을 보이지 않았다. 결론 : 연령이 적으면 오심 및 구토의 위험도가 증가하는 점을 고려하면, 양 투약군 간에 연령의 차이가 있었던 점이 통계적 변별력을 저하시켰을 가능성이 있다. 상기 결과를 토대로 본 연구자들은 중등도 정도의 오심 및 구토의 발생 위험도를 가지는 상복부 및 전복부조사를 전통적인 분할조사법을 이용하여 치료하는 경우 현재 흔히 사용되는 항구토제인 메토클로프라미드에 비해 온단세트론이 오심 및 구토, 식욕의 저하 등의 급성 부작용을 더 효과적으로 억제함을 알 수 있었다. 하지만, 온단세트론을 투여하는 경우에도 오심 및 구토가 호전되지 않는 경우도 많이 있으므로 이 부작용을 더욱 경감하기 위한 여러 가지 방안들이 강구되어야 한다고 생각된다.

  • 본 연구는 개에서 위수혈(BL-21) 및 중완혈(CV-12)에 대한 지침의 구토억제효과를 규명할 목적으로 수행하였다. 사용 혈위는 BL-21 및 CV-12이었다. 각 군에서 구토 유발을 위하여 $2{\%}$ xylazine(2.2 mg/kg)을 투여하였으며, metoclopramide 수침은 1 mg/kg의 비율로 해당 혈위에 각각 수침하였다 대조군 및 실험군에 있어서 xylazine 투여 20분전에 BL-21 및 CV-12에 자침, metoclopramide 수침 및 뜸 처치를 각각 실시하였다. 대조군 및 실험군에서 구토율 및 구토발현시간을 각각 조사한 결과는 다음과 같았다. BL-21자침군, CV-12 자침군 및 BL-21과 CV-12 병용 자침군의 구토율은 각각 $33.3{\%}\;50.0{\%}$$33.3{\%}$로서 대조군($66.7{\%}$)보다 낮았다. BL-21 자침군, CV-12 자침군 및 BL-21과 CV-12 병용 자침군에 있어서 구토 발현 시간은 대조군과 유사하였다. BL-21 수침군 및 CV-12 수침군의 구토 억제율은 각각 $100{\%}$$83.6{\%}$로서 수침군의 대조군($63.6{\%}$)보다 높았다. 구토발현시간은 수침군의 BL-21 수침군은 0분이었으며, CV-12 수침군은 5분으로서 수침군의 대조군과 유사하였다. BL-21 뜸 처치군 및 CV-12뜸 처치군의 구토율은 각각 $33.3{\%}$로서 뜸 처치군의 대조군($66.7{\%}$)보다 낮았다. 구토발현시간은 뜸 처치군의 대조군 $2{\~}4$분, BL-21 뜸 처치군이 $2{\~}4$분 및 CV-12 뜸 처치군이 $2{\~}3$분으로서 큰 차이를 나타내지 않았다. 이상의 결과를 종합해 볼 때, 개에서 $2{\%}$ xylazine 투여로 유발된 구토에 대한 BL-21 및 CV-12의 자침, metoclopramide 수침 및 뜸 처치가 각각 구토 억제에 효과적이었으며, 또한 이들 중 BL-21에 대한 metoclopramide 수침의 구토억제 효과가 가장 우수하였다.
