발등 굽힘 제한이 있는 사람에게 장딴지근 신장운동과 발바닥 자가근막이완이 발등 굽힘 각도와 장딴지근 긴장도에 미치는 즉각적인 효과 비교 (A Comparative Study on the Immediate Effect of Performing Gastrocnemius Stretching with and without Myofascial Release of the Sole on Ankle Dorsiflexion Angles and Gastrocnemius Muscle Tone in Subjects with Limited Ankle Dorsiflexion)

  • PURPOSE: This study compared the effect of performing gastrocnemius stretching with and without the self-myofascial release of the sole on the active and passive ankle dorsiflexion angles and muscle tone of the gastrocnemius muscle in subjects with short gastrocnemius muscle. METHOD: A total of 23 subjects with short gastrocnemius muscles were included in this study. The study participants were divided into two experimental groups. Group A performed gastrocnemius muscle self-stretching exercises only, while group B performed self-myofascial release of the sole using a massage ball after the gastrocnemius muscle self-stretching exercises. For both groups, the active and passive ankle dorsiflexion angles were measured using a goniometer, and the tone of the gastrocnemius muscle was assessed using the MyotonPRO®. RESULTS: Within-group comparison showed that the participants in both groups A and B had significantly increased active and passive ankle dorsiflexion angles and decreased gastrocnemius muscle tone (p < .05) after performing their respective exercises. However, no significant differences in the said criteria were observed between groups A and B (p>.05). CONCLUSION: The results of this study showed that both methods were effective in increasing active and passive dorsiflexion angles and decreasing muscle tone. Thus, it is recommended to tailor gastrocnemius stretching exercises according to the patient's condition. If the patient does not experience discomfort in the plantar fasciae, it is recommended to perform the gastrocnemius stretching exercise only without myofascial release and use a massage ball afterward.

지연발생근육통에 대한 테이핑 사례연구 (A Case Study of Taping Therapy Effects on Delayed Onset Muscle Soreness)

  • This case study was performed to assess the effectiveness of taping therapy for the patient who have delayed onset muscle soreness(DOMS). The results were as follows: 1. Pain degree of VAS was decreased at every taping therapy. 2. The range of motion of ankle joint was increased at every taping therapy. 3. Diameter of calf muscle was increased at every taping therapy. 4. Pattern of gait was normalized at every taping therapy. We found improve of pain degree, ROM of ankle joint, diameter of calf muscle, and pattern of gait. Therefore we can infer that taping therapy had effectiveness to those who had DOMS.

자가 골연골 이식술을 이용한 Freiberg 병의 치료(1예 보고) (Autogenous Osteochondral Graft for Freiberg's Disease (A Case Report))

  • Freiberg disease is a osteochondrosis of the lesser metatarsal heads. Various surgical treatment have been recommanded including joint debridement and metatarsal head reshaping, metatarsal dorsal wedge osteotomy, metatarsal head excision and joint arthroplasty. Autogenous osteochondral graft for the treatment of Freiberg disease is an effective restorative procedure that provides early range of motion exercise, weight bearing, and reduces other morbidity. We report a case of late stage Freiberg disease treated with arthrotomy, removal of loose body and autogenous osteochondral graft.

Lindholm 방법을 이용한 진구성 아킬레스 건 파열의 치료 (Treatment of the Chronic Achilles Tendon Rupture by Lindholm Method)

  • Purpose: This retrospective study was designed to evaluate the treatment results of chronic Achilles tendon rupture by Lindholm method. Materials and Methods: Between 2002 and 2006, we performed the reconstruction of the Achilles tendon by using of the gatrocnemius-sloeus fascia known as Lindholm method. Ten cases of ten patients were enrolled in this study (8 men and 2 women). The mean age of the patients at the time of operation was 49 years (range, 32${\sim}$66 years). The mean follow-up duration was 15.2 months (range, 12${\sim}$19 months). The retrospective review of the clinical history, physical examination, the American Orthopedic Foot and Ankle Society (AOFAS) score were conducted. Results: The mean AOFAS score before surgery was 74.10${\pm}$2.56 and that of the latest follow-up was 90.60${\pm}$5.72. The excellent results were six and good results were four patients. Eight patients were normal triceps power and the others were good. The average of heel to floor distance was 4.5 mm less in the operated legs than the contralateral ones in each patient, but there was no significant difference (p>0.05). The average of calf muscle circumference in the mid-leg was 7.5 mm less in the operated legs than the contralateral ones and there was significant difference (p<0.05). The active range of motion of the ankles, mean plantarflexion was 40 degrees and dorsiflexion was 16.8 degrees in operated side. The contralateral side was 43 degrees in plantarflexion and 19 degrees in dorsiflexion. No case showed rerupture of the reconstructed Achilles tendon. One patient had the superficial wound infection which was treated successfully by antibiotic therapy. Conclusion: It was suggested that the overall results of Lindholm method for the chronic Achilles tendon rupture indicated satisfactory outcomes.

관절 질환의 통증과 가동력에 파라핀 치료가 미치는 효과 (The Effect of Paraffin Therapy on Release of pain and Range of motion with Joint Disease Patients)

  • This study was conducted in order to determine the effect of Paraffin Therapy on release of pain, skin temperature and range of motion with joint diseases. This study was performed on 42 patients with general joint diseases and 21 patients with degenerative joint diseases. All the subjects were randomly assigned to three groups with 21 each. The experiment had been done from June, 1. 2002 to September, 30. 2002, in two general hospitals and a social welfare center using Digital Thermometer Kane-May KM330 and VAS(visual analogue scale). Each pint was measured in terms of skin temperature, pain scale and ROM(range of motion). The results of the experiment are as follows : 1. In application of three different heat modalities (Paraffin bath, Infra red, Hydro pack) on the Knee, Ankle, Elbow pints of the subjects for 30 minutes, the elevation of skin temperature and the reduction of pain scale were found to be significantly different. When checked right after the 30 minutes' application, the Infra red resulted in a higher degree elevation of skin temperature than two other modalities, while Paraffin bath resulted in the highest degree of the elevation of skin temperature when it was checked one hour after the application was over. It means that Paraffin bath lasted longer than the other two modalities(F=14.691, p<.000). 2. With Paraffin bath application, pint pain was reduced significantly both in 30 minutes and one hour checks(F=20.675, p<.001). 3. In terms of ROM(range of motion) increase, didn't seem to have any significant differences in 30 minutes and one hour. The results suggest that Paraffin therapy be helpful in maximizing th effects on pint diseases. I strongly suggest that further studies be done on other pints and muscle diseases using Paraffin bath in different methods of applying the therapy.

발등굽힘 관절가동범위 제한이 착지 시 움직임 전략에 미치는 영향 (Effects of Limited Dorsiflexion Range of Motion on Movement Strategies during Landing)

  • Objective: This study aimed 1) to compare the Landing Error Scoring System (LESS) score and movement patterns during landing of the lesser dorsiflexion range of motion (LDFROM) group to that with the greater dorsiflexion range of motion group, and 2) to identify the correlation between the weight-bearing dorsiflexion range of motion (WBDF ROM), LESS score, and movement patterns during landing. Method: Fifty health adults participated in this study. WBDF ROM was measured using the weight bearing lunge test while movement patterns during landing was assessed using the LESS. The joint angles of the ankle, knee and hip joints during landing were analyzed using the 2D video analysis. After mean value of WBDF ROM was calculated, participants were divided into two groups (GDFROM and LDFROM) based on the mean value. The Mann-Whiteny 𝒰 test was used to identify differences in movement strategies during landing between two groups and the Pearson's correlation analysis was performed to determine relationships between WBDF ROM and movement strategies. Results: The LDFROM group showed the poorer LESS score and stiffer landing kinematics during landing compared to the GDFROM group (p<0.05). In addition, DFROM was significantly related to the LESS score and landing kinematics (p<0.05) except for total hip excursion (p=0.228). Conclusion: Our main findings showed that the LDFROM group had poorer landing quality and stiffer landing movements compared to the GDFROM group. In addition, increase of WBDF ROM significantly improved landing quality and soft-landing movements. To reduce shock during landing such as ground reaction forces, individuals need to better utilize WBDF ROM and lower extremity movements based on our findings. Therefore, intervention programs for safer landings should include exercises that increase WBDF ROM and utilize eccentric contraction.

정상족과 편평족의 Arch Support 사용에 따른 운동역학적 분석 (Biomechanical Analysis of Arch Support Devices on Normal and Low Arch)

  • The purpose of this study was to the kinetic variables effects from the use of arch support inserts on low-arched people. We selected 10 people for the research and separated them into 2 groups, 5 people for the normal arched group and 5 people for the low arched group. Each group wear shoes which have a 3 step convertible arch support (level 0, level 2, level 5) and we measured their foot pressure and 3D motion analysis data. As a result, we found that the mean pressure at the heel of the low arched group was decreased when using the arch supports. The arch support induced the correct grounding area for the foot and dispersion of foot pressure. 3D motion analysis found that as the height of the arch support was increased, the movement of the Y-axis(inversion-eversion) was increased to relieve the shock to the heel. The arch support insert limited the range of motion(ROM) of the Z-axis(abduction-adduction) of the low arched person's ankle joint and prevented ankle injury caused by the excessive eversion when walking. Low arched people are seen to be easily tired due to the ineffective shock absorption of the knees and abnormal walking motion. In order to improve the problems, a 3 step convertible arch support(level 5) insert would improve the low-arched people's walking ability. In other words, the low arched people should be expected to walk as well as normal arched people when they wear shoes with the arch support insert.

제 2형 거골 경부 골절의 수술적 치료 (Surgical Treatment of Type II Talar Neck Fractures)

  • Purpose: To evaluate the clinical results and determine appropriate methods of surgical treatment about type II talar neck fracture. Materials and Methods: Among nineteen patients who received surgical treatment for type II talar neck fracture from May 2000 to May 2005. Fourteen patients with a follow-up period of more than 1 year were divided into two groups. Six patients reduced by closed reduction (Group A) with screw fixation and eight patients reduced by open reduction with screw fixation. We analyzed preoperative, postoperative and follow-up simple radiographs and reviewed patient hospital records retrospectively. Clinical results were evaluated by Hawkins scoring system. We analyzed pain, limp, range of motion of ankle and subtalar joint. Results: Five patients (83.3%) in group A and seven patients (87.7%) in group B had excellent and good clinical results. There were no complications including avascular necrosis, delayed union, nonunion. Conclusion: Closed reduction with screw fixation of talar neck fracture shows correct reduction and satisfactory results. But because of short term period of follow-up, we need long term results.

족부 골절수술 후 한방 재활치료의 효과: 증례보고 (Postoperative Rehabilitation of Foot Fracture in Korean Medicine Treatment: Clinical Case Report)

  • The objective of this study is to propose postoperative rehabilitation for foot fracture in Korean Medicine and to report its effectiveness. There were three patients who were got foot fracture surgery received korean medicine with acupuncture, herbal medicine, cupping and exercise treatment. Patients were evaluated numeric rating scale (NRS), range of motion (ROM), American Orthopaedic Foot and Ankle Society (AOFAS) ankle hindfoot score, foot function index (FFI) and EuroQol-5 dimension (EQ-5D) index. Case 1 and 2 were improved NRS from 6 to 3. Case 3 was improved NRS from 5 to 2. These cases were improved ROM in all directions, AOFAS score, FFI, EQ-5D and walking status. This study suggest that Korean medicine rehabilitation could be effective for patients who have received foot fracture operation.

심장이식 후 부신피질호르몬 투여로 인한 거골의 무혈성 괴사 - 1예 보고 - (Corticosteroid-induced Avascular Necrosis of Talus after Cardiac Transplantation)

  • A vascular necrosis of the talus has frequently been reported following trauma because talus has no muscle insertions, sixty percent of the surface of the talus is covered by hyaline cartilage, takes only a small area for entrance of a blood supply. Osteonecrosis is also associated with a variety of nontraumatic disorders. There are many indications for steroid usage, patient with rheumatoid arthritis, systemic lupus erythematosus, chronic obstructive pulmonary disease, and status- post renal or cardiac transplantation may be on long- term steroid usage, osteonecrosis may develop. A vascular necrosis of the talus secondary to chronic steroid usage is an unusual case. Delay in detection of osteonecrosis may lead to fragmentation and collapse of the talar body. When pain on range of motion is present and conservative treatment have been exhausted, surgical treatment is indicated, that is, fusion of the ankle joint. However it is important that conservative treatment may prevent its various sequelae with early diagnosis because steroid - treated patients have a more operative risk and increased risk for postoperative infection. We report a rare case of corticosteroid induced avascular necrosis of talus after cardiac transplantation.
