근위축성 측삭경화증의 한방치료에 관한 문헌적 고찰연구 (A Literature Review of Korean Medical Studies of Amyotrophic Lateral Sclerosis (ALS))

  • Objectives : There have been many reports that show that Korean Medical Treatment of ALS patients can maintain or improve their major and minor symptoms, but this has not been reviewed in literature. The aim of this study is to review studies of ALS patients treated with Korean Medicine. Methods : A search was completed of studies containing word ALS in English and in Korean from KIOM OASIS. Results : There were no cases of ALS patients that were fully cured. However with Korean Medical treatment, there were articles that reported improvement or maintenance of major symptoms, recovery of minor symptoms like pain, chest discomfort, insomnia, cough, sputum, etc, and success in the long term care of ALS patients. Conclusions : Although the current study has limitations, our findings support the potential use and effectiveness of Korean Medical Treatment for ALS patients.

기계학습 기반의 장애 음성 검출 시스템 (Machine Learning based Speech Disorder Detection System)

  • 본 논문에서는 기계학습 기반의 분류 방법을 이용하여 장애 음성을 검출하고자 한다. 음성 장애 중 마비말 장애는 뇌성마비, 파킨슨 질환, 뇌졸중 등 주로 뇌질환에 의해 발생하는 것으로 알려져 있다. 이러한 장애 음성을 검출함으로써 뇌졸중 등의 급성 뇌질환 발생에 대한 조기 처치가 가능하다. 장애 음성 검출은 입력 음성에 대한 특징벡터 추출과 기계학습을 이용한 분류과정을 통해 이루어질 수 있다. 실험을 위해서 장애 음성 DB인 TORGO 데이터를 사용하였으며, 10가지 기계학습 알고리즘과 다양한 특징벡터 스케일링 방법에 대해 장애 음성 검출 성능을 평가하였다.

사암침법 간정격과 간승격의 최근 연구 동향 분석 (Recent Study Trends of the Liver-tonification and Liver-sedation of Saam Acupuncture)

  • Objectives : This study was aimed to investigate current status researches and to provide source in setting development strategy on Liver-tonification acupuncture(LTA) and Liver-sedation acupuncture(LSA). Methods : We searched databases of Pubmed, KISS, OASIS, Google scholar and CNKI up to February 2017. We included articles that using LTA or LSA as main treatment, using the partial acupoints combination of LTA or LSA, or using each acupoint of LTA or LSA. We analyzed disease, details of acupuncture and control treatment and clinical outcomes from included studies. Results and Conclusions : Among the included clinical studies, case report was the most used methodology. LTA used to treat obesity, myopia, bartholinitis, tinnitus, hiccup and sleep disorder, prostate cancer and tremor. LSA used to treat panic disorder, gastroesophageal reflux disease and irritable bowl syndrome, herpes zoster and neuralgia. Obesity and tremor were the most studied diseases using LTA or LSA. Amyotrophic lateral sclerosis was the only disease which LTA or LSA have no therapeutic effect. Each acupoints was not used as a meaning of Saam acupuncture but as affiliated meridian acupoint. Since animal researches were not fully conducted, mechanism of LTA and LSA was hard to be proved. Thus, more clinical studies and basic animal research are needed.

한방치료 중 약인성 간손상으로 추정되는 환자에 대한 치료레이저 치험 1례 (A Case of Therapeutic Laser for Suggested Drug Induced Liver Injury Patient during Treatment of Korean Medicine)

  • A 84 year old male patient with amyotrophic lateral sclerosis (ALS) was treated by Jeungsonbaekchulsan and Gakbyeongyeonsu-tang for ten days. In the course of treatments, this patient was evaluated with Roussel Uclaf Casusality Assessment Method (RUCAM), showing results of drug induced liver injury. Herb medicine was immediately discontinued, followed by therapeutic laser treatments once a day for one week combined with 2 weeks of GODEX$^{(R)}$ administration. Transaminase results were lowered and changes in blood test results were time-effective changes. Therapeutic laser therapy could be considered effective on drug induced liver injury with further studies.

New insight into transglutaminase 2 and link to neurodegenerative diseases

  • Formation of toxic protein aggregates is a common feature and mainly contributes to the pathogenesis of neurodegenerative diseases (NDDs), which include amyotrophic lateral sclerosis (ALS), Alzheimer's, Parkinson's, Huntington's, and prion diseases. The transglutaminase 2 (TG2) gene encodes a multifunctional enzyme, displaying four types of activity, such as transamidation, GTPase, protein disulfide isomerase, and protein kinase activities. Many studies demonstrated that the calcium-dependent transamidation activity of TG2 affects the formation of insoluble and toxic amyloid aggregates that mainly consisted of NDD-related proteins. So far, many important and NDD-related substrates of TG2 have been identified, including $amlyoid-{\beta}$, tau, ${\alpha}-synuclein$, mutant huntingtin, and ALS-linked trans-activation response (TAR) DNA-binding protein 43. Recently, the formation of toxic inclusions mediated by several TG2 substrates were efficiently inhibited by TG2 inhibitors. Therefore, the development of highly specific TG2 inhibitors would be an important tool in alleviating the progression of TG2-related brain disorders. In this review, the authors discuss recent advances in TG2 biochemistry, several mechanisms of molecular regulation and pleotropic signaling functions, and the presumed role of TG2 in the progression of many NDDs.

  • The human gastrointestinal (GI) tract houses a diverse microbial community, known as the gut microbiome comprising bacteria, viruses, fungi, and protozoa. The gut microbiome plays a crucial role in maintaining the body's equilibrium and has recently been discovered to influence the functioning of the central nervous system (CNS). The communication between the nervous system and the GI tract occurs through a two-way network called the gut-brain axis. The nervous system and the GI tract can modulate each other through activated neuronal cells, the immune system, and metabolites produced by the gut microbiome. Extensive research both in preclinical and clinical realms, has highlighted the complex relationship between the gut and diseases associated with the CNS, such as Alzheimer's disease, Parkinson's disease, and amyotrophic lateral sclerosis. This review aims to delineate receptor and target enzymes linked with gut microbiota metabolites and explore their specific roles within the brain, particularly their impact on CNS-related diseases.

Neuroprotective Effects of Protein Tyrosine Phosphatase 1B Inhibition against ER Stress-Induced Toxicity

  • Several lines of evidence suggest that endoplasmic reticulum (ER) stress plays a critical role in the pathogenesis of many neurodegenerative diseases such as Alzheimer's disease, Parkinson's disease, and amyotrophic lateral sclerosis. Protein tyrosine phosphatase 1B (PTP1B) is known to regulate the ER stress signaling pathway, but its role in neuronal systems in terms of ER stress remains largely unknown. Here, we showed that rotenone-induced toxicity in human neuroblastoma cell lines and mouse primary cortical neurons was ameliorated by PTP1B inhibition. Moreover, the increase in the level of ER stress markers ($eIF2{\alpha}$ phosphorylation and PERK phosphorylation) induced by rotenone treatment was obviously suppressed by concomitant PTP1B inhibition. However, the rotenone-induced production of reactive oxygen species (ROS) was not affected by PTP1B inhibition, suggesting that the neuroprotective effect of the PTP1B inhibitor is not associated with ROS production. Moreover, we found that MG132-induced toxicity involving proteasome inhibition was also ameliorated by PTP1B inhibition in a human neuroblastoma cell line and mouse primary cortical neurons. Consistently, downregulation of the PTP1B homologue gene in Drosophila mitigated rotenone- and MG132-induced toxicity. Taken together, these findings indicate that PTP1B inhibition may represent a novel therapeutic approach for ER stress-mediated neurodegenerative diseases.

재가 가정용인공호흡기 적용 신경근육계 질환자의 호흡곤란, 수면장애, 불안 및 우울에 관한 연구 (Survey on Self Care, Respiratory Difficulty, Sleep Impediment, Anxiety and Depression among Patients with Neuromuscular Disease dependent on Home Mechanical Ventilator)

  • Purpose: This study aimed to investigate the influencing factors on self care, respiratory difficulty, sleep impediment, anxiety and depression among patients with neuromuscular disease who are dependent on Home Mechanical Ventilator (HMV). Methods: 157 patients were recruited through hospital based home nursing care departments and HMV rental centers. Data were collected by questionnaires. The analytic methods were n(%)/$M{\pm}SD$, $x^2$/t-test or ANOVA and multiple linear regression. Results: Patients with Amyotrophic lateral sclerosis were more likely to utilize HMV, had shorter periods of morbidity and the most extended daily use of HMV. As for medical equipment, they had ambu-bag (87.2%) and oxygenator (15.4%). Reports of respiratory difficulty and sleep impediment were low. There were no significant difference. In contrast, reported anxiety and depression were high and showed significant difference between non invasive HMV and invasive HMV. Anxiety contributes to respiratory difficulty (t=3.62, p=.002), sleep impediment (t=2.06, p=.042), and depression (t=7.24, p<.001). However, home nursing care exerts a positive influence in reducing anxiety (t=-2.73, p=.008). Conclusion: Anxiety contributes to respiratory difficulty, sleep impediment, and depression. However, those who use home nursing care reported less anxiety. Home nursing care positively impacts patients dependent on HMV as a practical service and available resource.

전자기파의 생체 위해성에 관한 소고 (Biological Hazard of Electromagnetic Field Exposure: A Review)

  • 현대인들은 가정생활과 직업환경 속에서 다양한 종류의 전자기파에 노출되며 전자기파 노출에 따른 생체위해성은 공중보건학적으로 중요한 이슈로 대두되었다. 현재까지 많은 체외 및 체내실험 결과에서 전자기파 노출이 세포대사, 내분비, 면역, 신경, 생식, 태아발달에 영향을 미치는 것으로 보고되었다. 세포나 개체 수준에서 시행된 실험연구에서는 전자기파 노출에 의해 세포내부 자유기의 증가, DNA 손상과 암발생, 발생기형, 생식기능 저하가 나타난다. 역학조사결과 전자기파 노출은 생명을 위협하는 질병인 백혈병, 뇌암, 근위축성 측삭경화증, 우울증, 자살, 알츠하이머와 상관성이 보고되었다. 이러한 생체기능 변화는 전자기파의 주파수, 노출기간, 강도 (에너지)에 따라 다르게 나타난다. 전자기파 노출은 곤충, 어류, 양서류, 파충류, 조류 등 야생동물에서도 동물행동, 번식, 생리기능의 변화를 초래한다. 본 소고에서는 인간보건학적 측면과 생태계에서 나타난 전자기파의 위해성을 전자기파의 종류, 노출시간에 따라 세포 및 개체 수준에서 보고된 위해성 자료를 정리하였다.

Ghrelin Protects Spinal Cord Motoneurons Against Chronic Glutamate Excitotoxicity by Inhibiting Microglial Activation

  • Glutamate excitotoxicity is emerging as a contributor to degeneration of spinal cord motoneurons in amyotrophic lateral sclerosis (ALS). Recently, we have reported that ghrelin protects motoneurons against chronic glutamate excitotoxicity through the activation of extracellular signal-regulated kinase 1/2 and phosphatidylinositol-3-kinase/Akt/glycogen synthase kinase-$3{\beta}$ pathways. Previous studies suggest that activated microglia actively participate in the pathogenesis of ALS motoneuron degeneration. However, it is still unknown whether ghrelin exerts its protective effect on motoneurons via inhibition of microglial activation. In this study, we investigate organotypic spinal cord cultures (OSCCs) exposed to threohydroxyaspartate (THA), as a model of excitotoxic motoneuron degeneration, to determine if ghrelin prevents microglial activation. Exposure of OSCCs to THA for 3 weeks produced typical motoneuron death, and treatment of ghrelin significantly attenuated THA-induced motoneuron loss, as previously reported. Ghrelin prevented THA-induced microglial activation in the spinal cord and the expression of pro-inflammatory cytokines tumor necrosis factor-${\alpha}$ and interleukin-$1{\beta}$. Our data indicate that ghrelin may act as a survival factor for motoneurons by functioning as a microglia-deactivating factor and suggest that ghrelin may have therapeutic potential for the treatment of ALS and other neurodegenerative disorders where inflammatory responses play a critical role.
