AUDIT척도에 의한 한국대학생의 알코올사용장애 실태 및 원인 분석 (Correlates of Problem Drinking by the Alcohol Use Disorders Identification Test on Korean College Campus)

  • Objectives : To survey college students with an Alcohol Use Disorder, and analyze the reasons for their disorder. Methods : The cross-sectional study was conducted at 60 four-year colleges within Seoul and 9 other provinces. The schools and students selected for the study provide a nationally representative sample, and the survey was conducted between May 15th and June 14th 2003. 2,385 cases were analyzed using questionnaires, which included a series of questions about students' alcohol use and associated problems, as well as an Alcohol Use Disorder Identification Test. Results : 42.3% of students were found to have an Alcohol Use Disorder. The probability of a student having an Alcohol Use Disorder was 1.30 times higher among male compared to female students. Those students not living with their parents or relatives were 1.40 times more likely to have an Alcohol Use Disorder. Those students where the father had a drinking problem and those who admitted that their parents drank heavily while they were growing up were 1.38 and 1.54 times more likely, respectively, to have an Alcohol Use Disorder. Those students attending a general university, joining a student club, attaining less than a B average credit score and those unsatisfied with their education were 1.60, 1.36, 1.41 and 1.27 times more likely, respectively, to have an Alcohol Use Disorder. Those students who had experience of drugs, smoking, binge drinking when they were in the last year of high school and the forceful consumption of mixed alcohol were 3.67, 1.95, 2.15 and 1.76 times more likely, respectively, to have an Alcohol Use Disorder. Conclusions : College students' with an Alcohol Use Disorder is a very severe and large problem within colleges. An Alcohol Use Disorder is determined by individual and family variables, the college environmental and life variables, as well as behavior variables.

재가노인의 알코올 사용장애에 영향을 미치는 요인 (A Study of the Factors Influencing Alcohol Use Disorder in Elders)

  • Purpose: This study was conducted in order to analyze the factors that influence alcohol use disorder in elders in KyongBuk province. The results of the study will provide basic information for the development of nursing interventions to promote proper alcohol use in elders. Methods: The subjects were 626 elderly individuals. Alcohol use disorder was defined as a score of more than 15 point on the AUDIT-K(Korean version of Alcohol Use Identification Test). Descriptive statistics, chi-square test, ANOVA and logistic regression were utilized to analyze the data. Results: Overall incidence of alcohol use disorder level; normal 73.2%, problem drinking 7.5% and alcohol use disorder 19.3%. Significant factors affecting alcohol use disorder were sex (OR=6.897), religious belief (OR=1.836), smoking (OR=2.948), liver disease (OR=4.753) and depression (OR=1.779). Conclusion: Community health care nurses perform a crucial function in the screening of elderly alcoholics. Early detection and treatment of depression in elders may help to prevent alcoholism.

의사결정나무분석을 이용한 성인의 알코올사용장애 위험요인 (Risk factors of alcohol use disorder in Korean adults based on the decision tree analysis)

  • Objectives: The aim of this study was to identify risk factors of alcohol use disorder among Korean adults. Methods: Cross-sectional exploratory study based on data collected from Data from the 6th Korea National Health and Nutrition Examination Survey in 2015 were performed in this study. There were 3,248 participants who were 2,558 normal drinkers while 690 had alcohol use disorder. Decision tree analysis were used to exam socio-demographic and health-related factors to predict alcohol use disorder. Results: As a result of decision tree analysis, the predictive model for factors related to alcohol use disorder in Korean adults presented with 8 pathways. The significant predictors of alcohol use disorder were age, gender, smoking, marital status, and house income. Male smokers whose household income is 'high' or 'low' are most vulnerable to alcohol use disorders. Conclusions: This study indicates that need to consider health behavior and house income when we practice prevention policies and health education of alcohol use disorder.

노인의 알코올 사용장애에 따른 우울, 삶의 질과의 관계 (The Relationship of Alcohol Use Disorders and Depression, Qualty of Life in the Eldery)

  • 본 연구는 농촌지역 제가노인의 알코올 사용장애 정도와 관련 요인을 파악하여 제가노인의 알코올 사용장애 개선프로그램 개발에 기초 자료를 제공하기 위함이다. 자료는 SPSS 19.0 Version으로 기술적 통계, chi-square test. t-test를 이용하여 분석하였다. 농촌지역 제가노인의 사용장애는 성별, 연령, 동거인 존재여부, 학력, 종교, 직업, 흡연 여부에 따라 통계적으로 유의한 차이가 있었다. 제가노인의 알코올 사용장애는 우울정도와 유의한 상관관계가 있는 것으로 나타났다. 제가노인의 알코올 사용장애를 보건사회문제로 인식하고, 이를 예방할 수 있는 프로그램 개발과 노인들의 참여에 지역사회와 정부가 노력해야 할 것이며, 이는 고령화사회를 맞이하는 기초작업이 될 것이다.

한국 여성의 알코올사용장애 영향요인 (The Factor Affecting Alcohol Use Disorder in Korean women)

  • 본 연구는 한국 여성의 알코올사용장애 영향요인을 파악하여 한국 여성의 알코올사용장애 중재를 위한 기초자료를 제시하고자 수행되었다. 본 연구는 제6기 국민건강영양조사 자료를 활용하였으며 만 19세 이상 여성을 대상으로 인구사회학적 특성, 건강관련 특성 및 알코올사용장애 여부를 파악하였다. 자료분석은 IBM SPSS 25.0 프로그램을 이용하여 복합표본설계를 적용하였다. 연구 결과 한국 여성의 알코올사용장애 유병률은 14.7%였으며 알코올사용장애군과 정상군은 연령, 결혼 여부, 교육수준, 경제활동 여부, 흡연 여부, 스트레스 인지 정도, 우울증상 여부에 따라 유의한 차이가 있었다. 알코올사용장애 영향요인을 분석한 결과 알코올사용장애 유병 위험은 연령이 낮을 경우 4.0배, 미혼일 경우 1.6배, 경제활동을 하는 경우 1.6배, 흡연을 하는 경우 4.3배, 우울증상이 있는 경우 1.9배 높은 것으로 나타났다. 본 연구의 결과에 따라 경제활동을 하는 미혼 여성의 음주와 흡연 여부, 우울증상을 파악하고 함께 접근하는 것이 필요하며, 추후 다양한 변수를 포함한 경제활동을 하는 미혼 여성의 알코올사용장애 위험요인에 대한 연구를 제언한다.

  • 알코올은 생물학적으로 피부부터 위장, 간장과 같은 장기와 말초 및 중추 신경계의 기능과 구조적 변화를 일으키며 그로 인하여 심리적, 생리적 의존과 함께 다양한 정신 장애를 일으킨다. 그리고 실직, 자살, 가정 내 폭력, 범죄 등의 사회적인 문제와 관련이 깊다. 이런 다양한 이유로 알코올 사용 장애는 타과로부터 흔하게 자문이 의뢰되어 조정되는 대표적인 정신신체 질환이다. 이에, 자문 조정 정신의학에서의 알코올 사용 장애의 유병률, 자문률 등의 현황을 살펴보고 알코올 상용장애 환자를 자문 조정하는 의사로서의 역할과 일반적인 원칙, 그리고 초기 치료 계획을 세울 때 고려해야 할 사항 등에 관하여 살펴보고자 한다.

알코올 사용장애의 한약치료 연구동향 (Current Research Trends of Traditional Herbal Medicine for Alcohol Use Disorder)

  • Objectives: To review clinical research trends of herbal medicine treatment for alcohol use disorder. Methods: Three domestic databases and eight foreign databases were used to search for published articles by November 1, 2023 in each database. A total of 9 studies were included. Results: There were eight randomized control studies and one non-randomized control study. All randomized control studies were designed with 2-arm paralleled. The non-randomized control study was designed with 2-arm crossover. The most commonly used prescriptions were Kudzu extract capsules and Seoganhaeul capsules. The most used herbs were Puerariae Radix, Hyperici Perforati Herba, and Acanthopanacis Senticosi Radix et Rhizoma seu Caulis. Conclusions: Traditional herbal medicine might be effective in alleviating drinking behavior, physical problems, and psychological problems of patients with alcohol use disorder. However, the limited number of included studies suggests that further methodologically rigorous research studies on herbal medicine treatment for alcohol use disorder are needed in the future.

알코올 사용장애 환자의 음주심각도, 일주기리듬과 수면의 질이 수면장애에 미치는 영향 (Influence of Severity of Problem Drinking, Circadian Rhythm and Sleep Quality on Sleep Disorder in Alcohol Use Disorder Patients)

  • Purpose: This study was done to identify the influence of severity of drinking problem, circadian rhythm and sleep quality in patients with alcohol use. Methods: A descriptive study design was utilized. Data were collected using self-report questionnaires from 139 patients with alcohol use disorder who were admitted to a psychiatric hospital in D city, Korea. The questionnaires included Alcohol Use Disorders Identification Test (AUDIT), Composite Scale of Morningness (CMS), Pittsburgh Sleep Quality Index (PSQI), and Korea sleep scale A. Data were analyzed using descriptive statistics, Pearson's correlation coefficients, and multiple regressions using the SPSS 20.0 program. Results: There was significant correlations among severity of problem drinking, circadian rhythm, sleep quality and sleep disorder. The significant factors influencing sleep disorder were severity of problem drinking(${\beta}=.12$, p= .042), circadian rhythm(${\beta}=-.14$, p= .039) and sleep quality(${\beta}=.63$, p= < .001). This model explained 56% of variance in sleep disorder(F = 57.34, p= < .001). Conclusion: The results of this study suggest that the development of sleep intervention programs for alcohol use disorder patients needs to consider severity of alcohol use, circadian rhythm and sleep quality, and sleep assessment and intervention are needed the early stage of the treatment and recovery process.

월남전 참전자에서 문제음주와 관련한 외상 후 스트레스 장애의 증상 차이 (Symptoms of Posttraumatic Stress Disorder in Relation to the Alcohol Problem in Korean Veterans of Vietnam War)

  • Objectives : The three symptom clusters of posttraumatic stress disorder (PTSD) are reexperience, avoidance, and hyperarousal. Alcohol use disorders frequently co-occur with PTSD, and possible functional correlations are suspected. Scholarly evaluation of the differences between the symptoms of PTSD and those of alcohol problems may be useful in understanding the pathophysiology of the comorbidity. Methods : We recruited Vietnam veterans with PTSD (n=97) and without PTSD (n=132). The alcohol use disorder identification test (AUDIT), and clinician-administered PTSD scale (CAPS) were administered to participants. The PTSD group was divided into two categories: those with PTSD only (n=57) and those with concurrent alcohol-related problems (n=40). Results : The PTSD group showed higher AUDIT scores compared to the control group. In the PTSD group, participants with alcohol problems had a severer symptoms of recurrent dream and sleep disturbance symptoms compared to the PTSD only group. No significant differences were found in the three major symptom clusters of PTSD. Conclusion : These findings support the proposed existence of a functional correlation between PTSD and alcohol use disorder. Clinicians should carefully evaluate and treat comorbid alcohol use disorder in patients with PTSD.

알코올 사용 장애 환자의 재발경험: Q 방법론적 접근 (Relapse Experience of Patients with Alcohol Use Disorder: Q Methodological Approach)

  • Purpose: The purpose of this study was to identify the subjective relapse experiences of patients with alcohol use disorder in one's life context. Methods: A Q methodology was used to analyze the subjectivity of relapse experiences among 55 participants with alcohol use disorder. Fifty-five Q-statement were derived from interviews and literature review. Q-statements were classified into normally distributed shapes using a 9-point scale. Data were analyzed using the QUANL program. Results: Four types of relapse experiences were identified: failure to self-regulation drinking cravings, fear of relapse and awareness of the need for treatment, drinking as a coping mechanism and defensive coping, and lack of motivation to change drinking behavior. Conclusion: The results of this study suggest that patients with alcohol use disorder need a differential approach based on four types of relapse experiences in the recovery process.
