Clinical Experience with Vascular Surgery (혈관수술에 대한 임상적 고찰)

  • The vascular surgery is the field that has developed in early 20 century and is progressing nowadays. Recent advance in surgical technique accompanying with excellent medical diagnosis and treatment, prompt angiographic usage, development of variable prosthetic material, and concomitant use of anti-coagulant have made remarkable results of vascular surgery. 83 cases of vascular surgery have been performed at Thoracic and Cardiovascular Surgery Department of Pusan National Unversity Hosaital since 1971 till 1990, for 20 years and their results are followed. Patient ductus arteriosus and Buerger`s disease were omited in this study. 1. The age distribution shows that the fifth and sixth decades are most frequently affected and mean age was 56.1 years old. Male to female ratio is 1: 2.32. 2. Among the 83 cases of all, number of occlusive vascular disease is 46 and that of aneurysmal disease is 33. 3. In clinical manifestation, most common symptom of occlusive disease is pulselessness and pain was next. Mass sensation is most commonly complained by patients of aneurysmal disease. 4. CT scan was more important in diagnosis of aneurysmal diseases and angiogram was more commonly used in occlusive diseases. 5. The common site of arterial occlusion was common iliac artery, femoral artery, aortic bifurcation, and external iliac artery, as its frequency rate. The most commonly affecting portion of aortic aneurysm was abdminal aorta, and descending thoracic aorta and femoral artery were next 6. Preoperative associated diseases were atherosclerosis[41 cases], hypertension[21 cases], valvular heart disease[11 cases], and diabetes mellitus[9 cases], etc, 7. Operative methods in ocllusive diseases were thrombectomy[36.9%], endarterectomy [10.9%], and bypass graft insertion[52.7%]. Among the bypass graft, Y-graft was used in 7 case, straight graft was used in 17 cases, and saphenous venous graft was used in 2 cases. 8. Postoperative complications were developed in 17 cases, and morbidity rate was 36. 9. Eleven patient were died within 1 month after operation, so operative mortality rate was 13.3%. 10. Duration of patency was beteween 7 and 58 months[average 27.5 months] in occlusive diseases and their 5-year patency rate was 56.3%. Duration of patency of aneurysmal disease was 20 months in aveage and their 5-year patency rate was 51.3%. 11. Patients of eleven cases of occlusive disease and two cases of aneurysmal disease required reoperation for variable reason. 12. 35 cases of patient have used anticoagulants: coumadin, ticlid, and persanthin-ASA combination.

Numerical Study on the Effects of Pressure Wave Propagation for Tunnel Entrance Shape Change in High-Speed Railways (고속철도의 터널입구 형상변황에 따른 압력파동 현상에 관한 수치적 연구)

  • When a front head of train enters a tunnel at a high speed, compression wave is generated at tunnel entrance due to the confinement effect and propagated along the tunnel with sound of speed. The propagated compression wave is reflected at tunnel exit due to abrupt pressure change at passage. The reflected wave is expansion pressure wave. And when the rear head of train goes through the tunnel entrance, another expansion pressure wave is generated and propagated along the tunnel. The pressure drop occurs seriously around train when the two expansion pressure waves come cross on train in the tunnel. In order to reduce the pressure drop, the compression wave front must be controlled because the intensity and magnitude of pressure drop is nearly proportional to that of compression wave at tunnel entrance. This study relates to reduction of the pressure wave gradient with respect to tunnel entrance shape change with various kind of angle and rounding. The results show characteristics of wave propagation in tunnel, usefulness of characteristic curve to estimate proper time domain size in numerical study and measuring time in actual experiment. Also rounding is contributed to improve pressure wave front even if its radius is very small at tunnel entrance. In order to improve of pressure wave front at tunnel entrance, proper angle is prefered to rounding with big radius and an angle of around 14$^{\circ}$ is recommended according to this simulations, And it is expected to reduce additional pressure drop in tunnel when the location and the size of the internal space for attendant equipment are considered in advance.

  • The Purpose of this study are to investigate the characteristics of the flame behavior of gasoline-methanol blended fuels in spark ignition engine. Ionization probe were installed at the cylinder head and piston in order to measure flame speed. Other parameter such as engine performance, fuel consumption rate and exhaust gas were measured. The results were as follows. 1. In the case of increase methanol contents in blend fuel, flame propagation speed were increased, and thermal efficiency of the engine were increased due to decrease of energy consumption rate. 2. In the case of fixed equivalance ratio, NO sub(X) in exhaust gas were increased in accordance with increase of spark advance, and mean effective pressure were decreased in accordance with increase of methanol contents. 3. CO and HC concentration were decreased in accordance with increase of methanol contents.

A Study on Situations of Safety Education in Universities and University Students' Awareness of Coping Methods for Safety Accidents (대학의 안전교육 현황 및 대학생의 안전사고 대처방법 인식에 관한 연구)

  • This study researched methods for coping with safety accidents by university students, after considering the situation of safety education by universities in Korea. For this study, a survey of 411 students of universities in the cities of Gwangju and Daejeon was conducted. According to the results, the ratio was high for answers such as willingness to participate in safety education beginning with the departments of engineering, art and physical training, education, art and humanity, social sciences and natural sciences, and medical sciences. It was found that university students most preferred first aid among the contents of safety education. University students of all departments answered that they had experienced safety education, but were afraid to be embarrassed in the case of actual safety accidents, as was found through the results of investigations of their awareness of coping methods with safety accidents. It was verified that many times their suffered skin and other wound in an earlier safety accident that they had experienced. This study suggests that safety education needs to be accomplished in the university curriculum, predicting and practicing for causes and factors of danger in advance, in order to improve the level of safety awareness and safety competence among university students.

  • Objectives The purpose of this study is to understand the condition of the Ease-West Integrative Care system in one of Korean Medical hospital, to develop more effective system, and to collect advance information for future research. Methods We analyzed patient's status, patient's composition and the ranking of the major disease code. In addition, we investigated the operating system of how Ease-West Integrative Care in hospitals is operating in order to grasp the actual situation is being done. Results As a result of analyzing the status, there was a balanced cooperation between the Korean Medicine and Western Medicine with a ratio of 0.86:1. The disease status from Korean Medicine to Western Medicine were mostly occupied by stroke patients and from Western Medicine to Korean Medicine fragment were mostly by musculoskeletal pain patients. Conclusions The results of this study showed that the Ease-West Integrative Care system of surveyed Korean Medical hospital has more integrated medical characteristics than previous studies in terms of quality and quantitative. Future research based on detailed data collection and review for a longer period is expected in the further.

A Comparative Analysis on The Age and Gender of Player Character : Focusing on The USA and Japanese Video Games (미국과 일본 게임의 플레이어 캐릭터 나이와 성별 비교 분석)

  • A player character has a significant influence on immersion. In order to understand the consumers, it is necessary to compare age and gender of player characters, which is the most basic feature of the player character. We selected a prize winning work of 15 years of USA and japanese as research subjects(in RPG and Adventure), classified age and gender of player characters and analyzed the result. In the japanese games, the ratio of pre-set player characters was 94.3% for age and 88.6% for gender. In the USA games, It was the order of the pre-set character which did not specify the age and the customized type character. In this research, we would like to contribute guidelines for player character setting to successfully advance into the USA and Japanese game market.

  • The demand for subsea cables and pipelines that transfer marine energy resources onshore has recently increased. Laying these underground after trenching is one engineering method to stabilize exposed subsea cables and pipelines. This experimental study found the optimum conditions for operating two types of waterjet arms mounted on an ROV trencher. A waterjet arm for trenching the seabed was scaled down at a ratio of 1:6, and a comparative analysis was conducted using diverse parameters. The results of this research provide a practical fundamental database to assist in making decisions about the ROV trencher performance in advance.

  • Recently, Portable Internet being standardized provides fast movement with wider service range than wireless LAN does. If Portable Internet service starts, many people will use Portable Internet and thus wireless traffic is going to increase. In Portable Internet, it is important to reduce handover latency to provide user with satisfactory service when handover occurs. In IEEE 802.16e, MSS sends its own security context information to one Base Station which it will move to reduce handover latency. But this is not suitable in the situation that the BS doesn't know the security context. To reduce handover latency of proactive caching method that is to send security context information to adjacency Base Stations in advance has been proposed by[4]. In this paper we propose effective traffic management algorithm to reduce signaling network traffic caused by proactive caching method.

  • Recently, the biochip which is a prime representation of NT, IT, BT, as an example of convergence technology, has been frequently mentioned. With the recent rapid advance in biotechnology, these compact devices, such as lab-on-a-chip or u-TAS, have been developed, and more research is needed. These compact devices typically use the micro-channel in order to shed or detach and mix a variety of materials. Specially, in micro-fluidic systems, a mixer is necessary to produce a mixture because only laminar flow occurs at a low-Reynolds number. To solve this problem, HVM a micromixer that induces a horizontal and vertical multi-mixing flow motion, is proposed. The mixing performance was analyzed and verified by optimizing the shape through the CFD analysis and evaluating the structural analysis and the soundness with material properties that are obtained through the basic experiment.

  • It is considered in this paper that the main causes of tunnel collapse during the construction were the insufficiency of data of geotechnical investigations, or their limits due to special ground condition such as its heterogeneity and anisotropy It is thought that safety of ground can be affected by the geological conditions such as presences of discontinuities in good intact rocks, and considered to be necessary that awareness of the conditions of discontinuities in advance is important to apply adequate reinforcement measures. It is also shown that a serious accident had occurred because of the unawareness of the permeable alluvial deposits at the top of the tunnel. And it is shown that the example of application of the results of geotechnical investigation such as face-mapping, pilot boring etc. during tunnel construction, and a serious deformation of tunnel under special geological condition. Therefore, it is strongly recommended to perform an adequate geotechnical investigation to confirm the geotechnical conditons of ground before design, and supplimentary investigation is also needed depending on conditions for safe and econonic construction.
