  • One case of patient with adult polycystic kidney disease was reported in this clinical study. After the study, the results were as follows: The adult polycystic kidney disease(APCK) is inheritant disease in which cysts are distributed throught the cortex and medulla of both kidneys. It belongs to Hematuria in oriental medicine field. For this patient, it was effective to appliment acupuncture Hap-kok, Kok-chi, Oe-gwan, Chok-samni, Chok-imup, Sam-umgyo, Su-bun, Tae-chung and Hae-gye showed a great effect on decreasing the pain and increasing the volume of urine. For this patient, it was effective to dispense Gamiwuiryungtang(加味胃苓湯), Ohryungsan(五苓散) and Chungsimyeonjaeum(淸心蓮子飮), Dosubokryungtang(導水茯苓湯).

  • 1. Objectives: The purpose of this case study is to evaluate the effects of Sasang constitutional therapy and treatment of an adult atopic dermatitis patient, who was diagnosed Soyangin, Interior febrile disease induced from the stomach affected by heat(少陽人 胃受熱裏熱病). 2. Methods: The adult Atopic Dermatitis patient was diagnosed Soyangin, Interior febrile disease induced from the stomach affected by heat(少陽人 胃受熱裏熱病) which was based on his Nature & Emotion(性情), physical characteristics, symptoms. He was medicated with Prescriptions of Soyangin. 3. Results and Conclusions: The clinical evaluation was made by SCORAD index, Total eosinophil count and Total IgE. After treatment, his SCORAD results significantly decreased from 88.4 to 21.9, Total eosinophil count decreased from 1320 to 120 and Total IgE also decreased from 1406.6 to 897.3. And the patient's subjective symptom and gerneral condition considerably improved after treatment. This case study describe the effectiveness on adult Atopic Aermatitis symptom by using Prescriptions of Soyangin.

Orthodontic treatment and management of adult patient with cleft lip and palate (성인 구순구개열환자의 교정치료 및 관리)

  • Patients with cleft lip and palate require interdisciplinary treatment to achieve successful rehabilitation. However, there are special difficulties in orthodontic treatment of adult cleft lip and palate patients: 1. Lack of Tissue, Bone, and Soft tissue; 2. Heavy Scar Tissue, Vestibule, and Palate; 3. Severe Anteroposterior discrepancy and Impaired Maxilla; 4. Distortion of Alveolar Ridge; 5. Abnormal Eruption Path and Malalignment of Tooth. Solving these problems, orthodontist should have differential diagnosis on extent of cleft site and residual deformities of adult cleft lip and palate patient. The tooth missing area in cleft site was commonly treated with a removable or fixed prosthesis, but this method is not stable to retain maxillary arch shape. To establish the more stable arch shape in cleft lip and palate, endosseous implants in the alveolar clefts with bone graft is helpful for management of adult cleft lip and palate patient.

A Study on Manifest Needs of the Patient with Hyperthyroidism (갑상선 기능항진증 환자의 욕구구조에 관한 연구)

  • The purpose of this study was to compare the needs among the patients of hyperthyroid, malignant tumor and normal adult to clarify the level of the need for the patient of hyperthyroid. We analysed relationships between their personal characteristics and their needs to contribute comprehensive nursing care for the patient of hyperthyroid. The . subjects were chosen from 60 persons who visited for general physical examination at Ewha Woman's University Medical Center., 72 patients having diagnosis with hyperthyroidism, and 72 patients admitted with malignant tumors at atomic neuclear medical center. The data was collected from September 1 to December 31, 1980, and the study population was selected by means of convenience sampling. The data was analysed by means of the standard deviation and analysis of variance. The results were as follows: 1. In the patients having hyperthyroid, male to female ratio was 1 :13.5, and incidence was peak at the age groups of 20 to 40. 2. In compare with the needs among 3 groups, there are significant relationships: the patient of cancer has shown the highest score with Achievement, Aggression and Dominance, the patient of hyperthyroid shown higher and the normal adult shown low score. .In the need related with sex, there is significant relationship: the normal adult has shown highest score, the patient having hyperthyroid shown the higher score and the patient of cancer shown the low score. 3. Tn compare with Whang's Edward personal preference schedule, the Hyperthyroidism shown relatively higher needs for aggression, dominance, exhibitionism and sex.

  • Adult type of osteochondritis dissecans of elbow is very rare condition. We describe a case of a osteochondritis dissecans of capitellum in 51-year-old male patient. We performed arthroscopy procedure for removal of loose osteochondral fragments. After I year follow-up, the patient gained good result.

An Explanatory Model for Patient Adherence of Rehabilitation in patients with Spinal Cord Injury (척수손상 환자의 재활 치료 지속이행 설명모형)

  • Purpose: The purpose of this study was to identity factors affecting patient adherence and to develop an explanatory model for patient adherence in patients with spinal cord injury. 8 Variables that were based on the previous research and a review of literature were used to construct hypothetical model. Social support, economic status, perceived barrier, patient provider relationship and rehabilitation related knowledge were the exogenous variables, depression, self-efficacy and patient adherence were the endogenous variables. Methods: Data form 117 patients with SCI were analysed to test the hypothetical model, using SAS and LISREL 8.53 program. Results: The overall fitness of the model was good (GFI=.991, AGFI=.915, NNFI=1.299, NFI=.953, p=.632) Depression, powerlessness, economic status were the strong factors influencing patient adherence. Powerlessness was significant factors for self-efficacy. Conclusion: To improve of patient adherence should focus on nursing intervention for depression, powerlessness and economic status.

Patient Satisfaction as an Outcome Indicator (결과지표로서의 환자 만족도에 관한 연구)

  • The purpose of this study was to identify factors influencing patient satisfaction and to evaluate the utility of patient satisfaction as an outcome indicator. The study was conducted by mailed questionnaire. The subjects were 900 patients discharged from adult nursing units in a tertiary teaching hospital. On the discharge date, questionnaires were distributed by two trained research assistants. The questionnaire developed by the researchers was based on Larson(1996)'s study, and consisted of 71 items with the following components: overall satisfaction, domain-specific satisfaction(administration process, hospital facility and environment, nurses, and doctors), patients' loyalty(intention to use the health care service of the hospital in the future), recommendation to others, health benefits, and demographic characteristics. Each item was rated using a five point Likert scale ranging from '1=strongly disagree' to '5= strongly agree'. The response rate was 43%(387/900). The satisfaction level with the health care service was generally high. Perceived health status was the only significant factor influencing satisfaction level. Satisfaction with doctors contributed the most to explaining overall satisfaction. Overall satisfaction was significantly correlated with patient loyalty, recommendation, to other and perceived health benefit. It was found that the score of satisfaction was positively correlated with the score of loyalty, recommendation, and health benefit. Therefore, patient satisfaction seems to be a good outcome indicator.

Performance-based User Testing of a Patient Drug Leaflet in the Elderly (노인 인구에 있어 환자용 의약품 안전사용 설명서의 활용성 평가)

  • Objective: This study aimed to explore the readability and comprehensibility of the drug information on a patient leaflet for the senior by employing performance-based user-testing. Methods: We included 36 elderly (65 years old or older) as the senior group (intervention group) and 36 adults (40~59 years old) as the adult group (control). We developed a questionnaire to test if participants could access to drug information. After completing a questionnaire, the participant was interviewed about their understanding over the patient leaflet. We performed t-test, ${\chi}^2$-test or Fisher's exact test to examine differences between two groups in primary outcomes. Results: The senior were less likely able to find information (78%) than the adult (91%); they were much less likely able to understand information (42%) than the adult (69%). While we found differences between the ability of finding and understanding drug information in both groups, the senior group had greater difficulties in understanding all kinds of drug information. They had significant difficulties to remember information after reading the patient leaflet and frequently failed to find proper information even though they were allowed to access freely to the leaflet during interviewing. Conclusion: To secure safe and effective use of drugs for the senior, it is necessary to develop drug leaflets for the senior.

Predictors of Violent Behavior by Patient or Caregiver of Patient in the Emergency Department (응급실 환자 및 보호자의 폭력행위 예측요인)

  • Purpose: The purpose of this study was to investigate predictors of violent behavior by patient or caregiver of patient in the emergency department. Methods: Subjects of the study were 447 residents who have visited the emergency department in community P Metropolitan City during the past year. The data collecting period was from June 1, 2012 to August 31, 2012. Data was collected using self-reported questionnaires. Results: Predictors of violent behavior in the emergency department were divided into personal and institutional factors. Personal factors included relationship with patients, presence of alcohol, reports of discontent during and past treatment and responses to deterioration in patient's conditions, institution factors included perceived attitudes toward medical workers' explanations and proficiency of medical workers, and delay in medical treatment hours. Conclusion: Knowledge of personal and institutional factors may permit emergency staff to minimize or prevent potential violence in the emergency department.

The Effects of Smartphone Application to Educate Patient on Patient Safety in Hospitalized Surgical Patients (위해사건 예방을 위한 수술 환자 교육용 스마트폰 애플리케이션의 효과)

  • Purpose: This study was designed to evaluate a patient safety application in the prevention of adverse event among surgical patients. Seventy three surgical patients with thirty six of them using the patient safety application and the rest (N=37) were provided educational booklet. Further, the instrument would measure patients' right to know, knowledge about patient safety, and attitude toward patient safety. Methods: The patient safety application was developed by the ADDIE along with input from experts, patients and an extensive literature review. Data were collected from 7 September through 20 October 2015. Results: The experimental group had significantly higher scores in patients' right to know (t=2.01, p=.024), knowledge on patient safety (t=3.80, p<.001) and attitude toward patient safety (t=2.74, p=.004) than those of the control group. Conclusion: The patient safety application developed using Smartphone could be an effective tool enhancing patient involvement in preventing adverse events that may occur to patients. Further studies are recommended with diverse subjects with varying medical conditions.
