OBSTETRICIAN'S VIEW OF TEENAGE PREGNANCY:PRESENT STATUS, PREVENTION AND PSYCHIATRIC CONSULTATION (산과 의사가 인지한 10대 임신의 현황, 예방, 정신과 자문)

  • Objectives:For the purpose of obtaining the more vivid present status and prevention program of teenage pregnancy, this survey was done by Obstetricians, as study subject, who manage the pregnant teenager in real clinical situation. Methods:Structured survey form about teenage pregnancy was sent to 2,800 obstetricians. That form contained frequency, characteristics, decision making processes, and psychiatric aspects of the teenage pregnancy. 349 obstetricians replied that survey form and we analysed these datas. Results:(1) The trend of teenage pregnancy was mildly increased. (2) The most common cases were unwanted pregnancy by continuing sexual relationship with boyfriends rather than by forced, accidental sexual relationship with multiple partners. (3) The most common reason of labor was loss the time of artificial abotion. (4) Problems of pregnant girls' were conduct behaviors and poor informations about contraception rather than sexual abuse or mental retardation. (5) Most obstetricians percepted the necessity of psychiatric consultation, however psychiatric consultation was rare due to parents refusal and abscense of available psychiatric facility. (6) For the prevention of teenage pregnancy, the most important thing was practical education about contraception. Conclusions:Based on the result of this study, further study using structured interview schedule with pregnant girl is needed for the detecting risk factor of teenage pregnancy and effective systematic approach to pregnant girl.

Differences in Sleep Patterns are Related to Behavior, Emotional Problems, Attention and Academic Performance in Elementary School Students of a South Korean Metropolitan City (일 도시의 초등학교 학생의 수면습관과 행동, 정서, 주의력, 학습과의 관계)

  • Objectives: The aim of this study was to investigate the sleep patterns of South Korean elementary school children and whether the differences in sleep patterns were related to behavior, emotional problems, attention and academic performance. Method: This study included a community sample of 268 boys and girls from fourth-, fifth- and sixth-grade classes in a South Korean metropolitan city from November to December 2010. The primary caregivers completed a questionnaire that included information on demographic characteristics, as well as the Child's Sleep Habit Questionnaire (CSHQ), the Korean version of Child Behavior Checklist (K-CBCL), the Korean version of the Learning Disability Evaluation Scale (K-LDES), the Korean version of ADHD Rating Scale (K-ARS) and the Disruptive Behavior Disorder Scale (DBDS). We conducted analyses on the CSHQ individual items, between the subscales, on the total scores and on the K-CBCL, the K-LEDS, the K-ARS and the DBDS. Results: Based on the findings from the CHSQ, the subjects had significantly higher scores for bedtime resistance ($9.18{\pm}2.17$), delayed sleep onset ($1.32{\pm}0.62$), the sleep duration ($4.19{\pm}1.52$) and daytime sleepiness ($14.10{\pm}3.55$) than the scores from the previous reports on children from western countries. The total CHSQ score showed positive correlations to all subscales of the K-CBCL : withdrawn (r=0.24, p<.005), somatic complaint (r=0.24, p<.005) and anxious/depressive (r=0.38, p<.005). Bedtime resistance was associated with oppositional defiant disorder (r=0.15, p<.05) and a positive correlation was demonstrated between sleep anxiety and the oppositional defiant disorder score (r=0.13, p<.05), night waking and the conduct disorder score (r=0.16, p<.05). Delayed sleep onset was related with low performance on the K-LDES with respect to thinking (r=-0.17, p<.05) and mathematical calculation (r=-0.17, p<.05). Conclusion: The results of this study reconfirm Korean children's problematic sleep patterns. Taken together the results provide that the reduced sleep duration and disruption of sleep pattern can have a significant impact on emotion, behavior, performance of learning in children. Further studies concerning more diverse psychosocial factors affecting sleep pattern will be helpful to understanding of the sleep health in Korean children.

DEVELOPMENT OF KOREAN FORM OF CHILDREN'S YALE-BROWN OBSESSIVE-COMPULSIVE SCALE(CY-BOCS):A RELIABILITY AND VALIDITY STUDY (한국판 아동용 예일-브라운 강박 척도 ; 신뢰도와 타당도 연구)

  • Objective:This study was carried out to evaluate the reliability and validity of the Korean form of the Children's Yale-Brown Obsessive Compulsive Scale(CY-BOCS). Method:Forty-two children with obsessive-compulsive disorder(OCD)(31 boys and 11 girls, aged 7 to 18 years) were assessed with CY-BOCS. Interrater agreement was assessed by two raters in subsample(n=20). discriminant and convergent validity were assessed by comparing CY-BOCS scores to Clinical Global Impression-OCD(CGI-OCD), Leyton Obsessional Inventory-Child Version (LOI-CV), State-Trait Anxiety Inventory for Children(STAI-C), and Child Depression Inventory(CDI). Results:Internal consistency was high, measuring 0.86 for the 10 items and total score. The intraclass correlations for the CY-BOCS total, obsession, and compulsion scores were 0.94, 0.94, 0.84, suggesting excellent interrater agreement for subscale and total scores. The CY-BOCS total score showed highest correlation with CGI-OCD(r=0.88), and significant correlation with LOI-CV(r=0.51), Trait Anxiety score(r=0.43), and CDI scores(r=0.49), but it was not correlated with State-Anxiety score(r=0.25). Conclusion:The results of this study indicate that the Korean form of CY-BOCS is a reliable and valid scale for rating obsessive-compulsive symptom severity.

THE STUDY OF RELATION TO VARIABLES RELATED TO CHILDREN'S DEPRESSION AND PARENTS' DEPRESSION (아동 우울에 관련된 요인 및 부모 우울과의 관계 연구)

  • Objectives:This study was conducted to investigate variables related to children's depression, and also the relationships between children's depression and parents' depression. Methods:258 5th and 6th grade elementary school students in Taegu, and their parents were asked to complete self-report questionnaires including the children's depression Inventory(CDI) and the Beck's depression Inventory(BDI). Correlation analysis, T-test, One-way ANOVA were used for the statistical analysis and the statistically significant level is p<0.05. Results:1) CDI mean for total children is $13.5{\pm}6.77$, BDI mean for their mother is $13.3{\pm}7.25$, and BDI mean for their father is $11.6{\pm}7.03$. 2) There are significant differences in CDI for children variables related to sex, health, school achievement, economic status. 3) Mother's health slightly affect children's depression, but there are no significant differences in CDI for parents' education, religion, father's health. 4) Children's school achievement and economic status among variables related to children affect mother's BDI scores and children's school achievement affect father's BDI scores. 5) For CDI item analysis, there are significant differences in 10 items for children's sex, in 8 items for health, and in 6 items for economic status. Especially, there are significant differences in 20 items among 27 items for children's school achievement. 6) There is significant correlation between parents' BDI scores and children's CDI scores, and especially high correlation to mother for both boys and girls. Conclusion:Children's depression are significantly affected by children's sex, health, school achievement, economic status. And especially school achievement affect grossly children's depression and their mother's depression, so we know that children and their mother have much burden about school achievement. There is high correlation between children's depression and parents'(especially mother) depression. Therefore, these result imply that in the therapy for children's depression, we must include not only children's problems but also parents' depression and parent-children relationship.

MENTAL REPRESENTATION OF PRESCHOOL CHILDREN:ASSOCIATION WITH PARENTAL MENTAL REPRESENTATION (학령전기 아동의 심적 표상 : 부모에 대한 심적 표상과의 관계를 중심으로)

  • The clinical assessment for preschool children who are known to have problems in selfreporting tends to be dependent on outsiders' reporting. Thus, the direct assessment of children's inner experience, thoughts and feelings is difficult. MacArthur Story-Stem Battery(MSSB) developed to learn more about preschool children's mental representation in play is used in this study to help assess clinical preschool children through developmental study of normal children's mental representation. Fifty five children(32boys and 23girls) who performed MSSB, IQ Test, Peabody Picture Vocabulary Test-Revised(PPVT) were videotaped and were analyzed. The results of this study were as follows:1) Children frequently displayed negative mental representation such as atypical negative response, reparation/guilt, punishment, personal injury and so on during emotionally laden play situation. 2) Mental representation of parent appeared positive, disciplinary, and negative in respective. 3) As a result of factor analysis of MSSB content themes, aggressive, prosocial, and oppositional content theme composites were generated. Aggressive content included atypical negative response, aggression, personal injury, and exclusion. Prosocial content included affection, affiliation, and reparation/guilt. Oppositional content included punishment and non-compliance. 4) Mental representation of parent and content themes showed significant correlation. Positive, negative, and disciplinary representation were significantly correlated for prosocial(r=0.40), aggressive (r=0.52), and oppositional(r=0.75) content theme respectively. 5) Among the correlations between parental mental representations and emotional responses, positive parental representation and anxiety showed significant negative correlation(r=-0.43). 6) Among the correlations between content themes and emotional responses, there were significant positive correlations between aggressive(r=0.28) and oppositional content themes(r=0.29) and distress, and were significant negative correlations between prosocial content theme and concern(r=-0.29) and anxiety(r=-0.43). According to the above results, preschool children frequently displayed negative mental representation in emotionally conflictual play situation. Children with more prosocial themes in their stories exhibit more positive parental mental representation. Also, children with more aggressive themes tend to display more negative parental representation and negative emotional responses.

Consumption and Preference of Seafood, and Desires for the Seafood Utilization in School Lunch Program among Middle and High School Students in Korea (중.고등학생의 수산물 이용 음식에 대한 이용실태와 선호도 및 학교급식에서의 요구도 조사)

  • A study on the seafood consumption was conducted from 1902 adolescent students (1110 boys and 792 girls) attending middle or high school to assess the attitude and degree of satisfaction for seafood being served in the school lunch program and ultimately to promote greater seafood consumption. A self-administered questionnaire was used to record the results. The results were as follows: Sixty percent of the respondents appeared to like or not to be reluctant to seafood. The main reasons for favoring seafood were its nutritious effect and taste. On the other hand, the reason for disliking seafood was due to its peculiar smell. In terms of cooking methods, the subjects preferred raw, fried, and grilled seafdod, whereas they disliked salt-fermented, seasoned and stewed dishes. The highly preferred seafood were crab, shrimp, tuna and squid. Only 6.6% of middle school students (MS) and 3.7% of high school students (HS) liked the seafood served in school lunch program, whereas the others responded 'so so'or disliked it. The reasons for disliking seafood being served in school lunch program were poor taste (14.7% of MS, 23.5% of HS), lack of freshness(6.3% of MS, 13.9% of HS) and unattractable cooking methods(3.4% of MS, 11.1% of HS). To promote seafood consumption in school lunch program, subjects anticipated the taste improvement (37.7% of MS, 48.5% of HS), use of a variety of seafood (19.8% of MS, 24.0% of HS) and cooking methods(12.6% of MS, 22.4% of HS). Above results suggested that further efforts are needed to provide preferred cooking methods and seafood items as a way of increasing seafood acceptability.

Effects of Stress Perception Level on Dietary Habits and Oral Health Behaviors in Adolescents (청소년의 스트레스 인지수준이 식습관 및 구강건강행태에 미치는 영향)

  • The purpose of this study was to assess the relationship that adolescent stress perception level has with dietary habits and oral health behaviors in high school students. A survey of high school students in some areas of the Jeollabuk-do province of South Korea was conducted. Five hundred fourteen survey responses were used in the final analysis. As a result of the questionnaire survey, the following conclusions were obtained. The highest stress type was indicated to be academic stress (mean${\pm}$standard deviation [SD], $3.09{\pm}0.89$). The next was shown to be home (family) stress (mean${\pm}$SD, $2.85{\pm}0.84$). The possibility of using a dental clinic was indicated to be less in girls than boys (p<0.001). Regarding subjective oral health behavior, the possibility of visiting a dental clinic was low in those who thought that their own oral health condition was not good or moderate (p<0.05). Also, it was shown that the higher stress led to the higher possibility of visiting a dental clinic (p<0.01). Students with higher grades had a in the upper ranks were indicated to have high possibility of having a regular meal (p<0.01). Higher stress led to the significantly higher possibility of eating cariogenic food (p<0.01). Students with median grades had a high possibility of eating cariogenic food (p<0.01), while students with higher grades had a low possibility of eating cariogenic food (p<0.05). These resultss show that stress perception level influences dietary habits and oral health behaviors. Thus, there is a need to develop a program in high scholls to promote the physical and mental health of students to relieve stress. Substantial and systematic oral health education is thought to be likely needed to develop desirable dietary habits.

Slipped Capital Femoral Epiphysis(SCFE) (대퇴골두 골단분리증의 치험례)

  • Slipped capital femoral epiphysis(SCFE) is a disorder in which there is a gradual or acute disruption through the capital physeal plate. The physiolysis is through a widened zone of hypertrophy, which is weakened due to altered chondrocytic maturation and endochondral ossification. The cause or causes of SCFE remain uncertain. The association of obesity and adolescent age with growth rate are predisposing factors. The possibility that most patients with subclinical hormonal abnormality were proved. The goal of treatment of slipped capital femoral epiphysis is to restore the function of the hip and delay the development of degenerative osteoarthrosis by prevention of additional displacement of the epiphysis. We report 10 patients(12hips) with SCFE who were treated by surgical means and followed along for more than one year, at Yeungnam University Hospital, from 1989 to 1996. There were six boys and four girls. The average age at operation was 11.8 years. Seven cases occurred in the left hip, one case in the right and 2 cases had bilateral involvement, five cases had a history of minor trauma on affected hip. Among hormonally studied six patients, panhypopituitarism patient was one case; decreased testosterone, two; decreased growth hormone, two; and decreased thyroid hormone, one. According to clinical stage, two cases were the acute type; five cases, acute on chronic type; and three cases, chronic type. On the radiological grades of slipping, mild slippage were nine hips; moderate, one; and severe, two. The eleven hips were treated by pin fixation in situ, and one, by cuneiform osteotomy. On the average follow-up of 2.6 years, ten hips were excellent or good functional results, two hips were failure.

Research Trends on Human Development and Family Studies in Journal of Korean Home Economic Education; A Review and Prospect of Research during the past 20 years (한국가정과교육학회지의 "인간발달.가족" 분야에 대한 20년 연구 동향분석; 성과와 과제)

  • The purpose of the study is to review the research trends of human development and family studies which published in the Journal of the Korean Home Economics Education for last 20 years. A total of 93 studies were analyzed in the area of nature and philosophy of Home Economic Education, curriculum, teaching-learning method and human development and family studies. Research topics mainly examined in the area of curriculum development, application of teaching methods, psychological and school adjustment of the adolescent and family relations. Reviews in methodological issues result in the heavy usage of survey method with self-administered questionnaires, convenient sampling and descriptive studies. Although research on human development and family studies in Home Economic Education has increased in quantity, there are greater needs to develop studios with representative subjects, various research methods, teaching - learning methods, and teaching materials to capture the rich complexity of individual development and family life. Limitations on previous studios and implications for future studies were also discussed.

Impact of Health Risk Factors on the Oral Health of Korean Adolescents: Korea Youth Risk Behavior Web-Based Survey, 2013 (우리나라 청소년의 건강위험요인이 구강건강에 미치는 영향)

  • The objective of this study was to investigate the relationship between health-risk factors and oral health in Korean adolescents. This cross-sectional study was based on the 9th Korea Youth Risk Behavior Web-Based Survey (2013). The final participation rate in the survey was 96.4%. of a Total of 72,435 adolescents (age, 12~18 years) who had participated in the survey, 66,951 adolescents (33,777 boys and 33,174 girls) were selected for analysis, after excluding those with missing data. The key variables were oral health factors (one or more of the six oral symptoms), general characteristics (five factors), and health-risk factors (five factors). After adjusting for the general characteristics, frequency analysis, ${\chi}^2-test$ using PASW Statistics ver. 18.0, and logistic regression analysis were performed to understand the effects of health risk-factors on the oral symptoms experienced by the study subjects. Subjects who answered 'Yes' for alcohol consumption had a 1.33 times higher risk of experiencing oral symptoms. Further, subjects who smoked were at a 1.2 times higher risk of experiencing oral symptoms. With regard to internet use, the risk of experiencing oral symptoms was 1.25 times higher for subjects who used the internet for 7 hours or more than for those who used it for less than 1 hour. Compared to those subjects who had not experienced violence in school, the odds ratio of subjects who had experienced it 3~4 times was 1.54-fold higher. The study found that health-risk factors were associated with oral symptom experience. Therefore, programs to understand health-risk factors and interventions should be developed for Korean adolescents and provided on a regular basis along with oral health education.
